O objetivo deste estudo foi o de analisar um projeto de prevenção de drogas e Aids desenvolvido nas escolas públicas estaduais de São Paulo. A análise foi desenvolvida considerando-se a diversidade e a complexidade do uso contemporâneo de drogas e o papel da escola, como uma agência de socialização, ambos historicamente determinados. As concepções analisadas são concepções sobre drogas; a relação entre drogas e Aids - como um dos eventos vinculados ao processo saúde-doença; e concepções e objetivos da prevenção. A análise é baseada em documentos e nos depoimentos dos supervisores do projeto, professores treinados pelo projeto e estudantes que participaram das atividades. O estudo encaminha conclusões que implicam o ordenamento das políticas públicas de prevenção relacionadas a droga e Aids.

The Bridges Decision Support Model is a geographic information system (GIS) that assembles existing data on archaeological sites, surveys, and their geologic contexts to assess the risk of bridge replacement projects encountering 13,000- to 150-year-old Native American sites. This project identifies critical variables for assessing prehistoric sites potential, examines the quality of available data about the variables, and applies the data to creating a decision support framework for use by the Iowa Department of Transportation (Iowa DOT) and others. An analysis of previous archaeological surveys indicates that subsurface testing to discover buried sites became increasingly common after 1980, but did not become routine until after the adoption of guidelines recommending such testing, in 1993. Even then, the average depth of testing has been relatively shallow. Alluvial deposits of sufficient age, deposited in depositional environments conducive to human habitation, are considerably thicker than archaeologists have routinely tested.

This research project combined various datasets, existing and created for this project, into an Interactive Mapping Service (IMS) for use by Iowa DOT personnel, county planning and zoning departments and the public in order to make more informed decisions regarding aggregate sources and future access to them. Iowa DOT Technical Advisory Committee meetings were held, along with public forum presentations, in order to understand better the social, ecological and economic limitations to extracting aggregate. The information needed by potential users was conveyed and integrated into a single informational source, the Aggregate Planning IMS.

In recent years, the Department of Corrections has made major strides in assessing offenders’ risk to reoffend, particularly in measuring changes in that risk over time. Earlier this year, the DOC worked with the Board of Parole to develop a risk assessment that focuses on assessing offenders’ risk to commit violent crimes.

In Switzerland there is a strong movement at a national policy level towards strengthening patient rights and patient involvement in health care decisions. Yet, there is no national programme promoting shared decision making. First decision support tools (prenatal diagnosis and screening) for the counselling process have been developed and implemented. Although Swiss doctors acknowledge that shared decision making is important, hierarchical structures and asymmetric physician-patient relationships are still prevailing. The last years have seen some promising activities regarding the training of medical students and the development of patient support programmes. Swiss direct democracy and the habit of consensual decision making and citizen involvement in general may provide a fertile ground for SDM development in the primary care setting.

The vaccinia virus (VACV) C6 protein has sequence similarities with the poxvirus family Pox_A46, involved in regulation of host immune responses, but its role is unknown. Here, we have characterized the C6 protein and its effects in virus replication, innate immune sensing and immunogenicity in vivo. C6 is a 18.2 kDa protein, which is expressed early during virus infection and localizes to the cytoplasm of infected cells. Deletion of the C6L gene from the poxvirus vector MVA-B expressing HIV-1 Env, Gag, Pol and Nef antigens from clade B (MVA-B ΔC6L) had no effect on virus growth kinetics; therefore C6 protein is not essential for virus replication. The innate immune signals elicited by MVA-B ΔC6L in human macrophages and monocyte-derived dendritic cells (moDCs) are characterized by the up-regulation of the expression of IFN-β and IFN-α/β-inducible genes. In a DNA prime/MVA boost immunization protocol in mice, flow cytometry analysis revealed that MVA-B ΔC6L enhanced the magnitude and polyfunctionality of the HIV-1-specific CD4(+) and CD8(+) T-cell memory immune responses, with most of the HIV-1 responses mediated by the CD8(+) T-cell compartment with an effector phenotype. Significantly, while MVA-B induced preferentially Env- and Gag-specific CD8(+) T-cell responses, MVA-B ΔC6L induced more Gag-Pol-Nef-specific CD8(+) T-cell responses. Furthermore, MVA-B ΔC6L enhanced the levels of antibodies against Env in comparison with MVA-B. These findings revealed that C6 can be considered as an immunomodulator and that deleting C6L gene in MVA-B confers an immunological benefit by enhancing IFN-β-dependent responses and increasing the magnitude and quality of the T-cell memory immune responses to HIV-1 antigens. Our observations are relevant for the improvement of MVA vectors as HIV-1 vaccines.

Mucosal candidiasis is frequent in immunocompromised HIV-infected highly active antiretroviral (HAART) naive patients or those who have failed therapy. Mucosal candidiasis is a marker of progressive immune deficiency. Because of the frequently marked and prompt immune reconstitution induced by HAART, there is no recommendation for primary antifungal prophylaxis of mucosal candidiasis in the HIV setting in Europe, although it has been evidenced as effective in the pre-HAART era. Fluconazole remains the first line of therapy for both oropharyngeal candidiasis and oesophageal candidiasis and should be preferred to itraconazole oral solution (or capsules when not available) due to fewer side effects. For patients who still present with fluconazole-refractory mucosal candidiasis, oral treatment with any other azole should be preferred based on precise Candida species identification and susceptibility testing results in addition to the optimization of HAART when feasible. For vaginal candidiasis, topical therapy is preferred.

Neste artigo discutimos os desafios colocados pelas novas tecnologias reprodutivas considerando o caso específico dos direitos reprodutivos das pessoas vivendo com HIV na perspectiva de docentes da área do Direito. Abordamos questões levantadas na literatura acadêmica analisadas com base em material bibliográfico e entrevistas. Os resultados apontam os aspectos presentes nos discursos sobre direitos à reprodução a partir de diferentes posicionamentos, o que inclui o biodireito como um novo campo do saber. Destaca-se, por um lado, uma compreensão restrita do conceito de autonomia no trato das questões reprodutivas na qual prevalece o argumento do melhor interesse da criança, e, por outro, uma visão moralizante sobre o desejo de filhos por indivíduos que vivem com o HIV/Aids.
