Introduction Chaque année, 289 000 femmes décèdent des complications reliées à la grossesse et à l’accouchement, et 2.9 millions de nouveau-nés décèdent avant d’atteindre 28 jours de vie. La quasi-totalité (99%) des décès maternels et néonataux ont cours dans les pays à revenu faible et intermédiaire (PRFI). L’utilisation des services obstétricaux essentiels, incluant l’assistance qualifiée à l’accouchement (AA) et les services postnataux, contribue largement à la réduction de la morbidité et de la mortalité maternelle et néonatale. Il est donc essentiel d’évaluer les déterminants et les inégalités de couverture de ces services, en vue d’informer l’élaboration de politiques et de programmes de santé dans les PRFI. Objectifs 1. Étudier systématiquement les déterminants et inégalités socioéconomiques, géographiques et démographiques dans l’utilisation des services de santé postnataux dans les PRFI. 2. Évaluer l’effet de la politique de subvention des frais aux usagers introduite au Burkina Faso en 2007 sur les taux d’utilisation de l’assistance qualifiée à l’accouchement, en fonction du statut socioéconomique (SSE). Méthodes 1. Nous avons réalisé une revue systématique sur l’utilisation des services postnataux dans les PRFI, en fonction des déterminants socioéconomiques, géographiques et démographiques. Notre étude incluait une méta-analyse de l’utilisation des services selon les quintiles de SSE et le milieu de vie (urbain vs. rural). 2. Nous avons utilisé un devis quasi-expérimental. Les sources de données consistaient en deux sondages représentatifs (n=1408 et n=1403), conduits respectivement en 2008 et 2010 auprès de femmes des districts sanitaires de Houndé et de Ziniaré au Burkina Faso, en plus d’une enquête sur la qualité structurelle des soins offerts dans les centres de santé primaire. Nous avons utilisé des modèles de régression de Poisson, multi-niveaux et segmentés, afin d’évaluer l’effet de la politique de subvention sur les taux d’AA. Nous avons estimé des ratios et différences de taux d’incidence ajustés, en fonction du SSE et du temps écoulé depuis l’introduction de la subvention. Résultats 1. Les estimés de ratio de cotes (RC) agrégés (IC 95%) pour les femmes de SSE élevé (5e quintile ou Q5), Q4, Q3 et Q2 (référence : quintile le plus pauvre, Q1) étaient respectivement : 2.27 (1.75 – 2.93); 1.60 (1.30-1.98); 1.32 (1.12-1.55); et 1.14 (0.96-1.34). La méta-analyse a aussi démontré un gradient d’utilisation des services postnataux entre les femmes urbaines et rurales : RC (IC 95%) = 1.36 (1.01-1.81). L’évaluation narrative a par ailleurs identifié une différence dans la couverture de services selon le niveau d’éducation. 2. Pour les femmes de faible SSE, le taux d’AA était 24% plus élevé (IC 95% : 4-46%) immédiatement après l’introduction de la subvention, en comparaison au taux attendu en l’absence de ladite subvention. L’ampleur de l’effet a diminué dans le temps, correspondant à des estimés (IC 95%) de 22% (3-45%) à 6 mois, 20% (1-43%) à 12 mois, et 17% (-4-42%) à 24 mois après l’introduction de la subvention. La force d’association variait selon les strates de SSE, l’effet le plus prononcé étant observé au sein du SSE le plus faible. Conclusions 1. L’utilisation des services postnataux demeure inéquitable selon le SSE et l’accessibilité géographique aux formations sanitaires dans les PRFI. 2. Notre étude suggère que l’introduction de la subvention des frais aux usagers au Burkina Faso résulte en une augmentation soutenue dans le taux d’assistance qualifiée à l’accouchement, particulièrement chez les femmes de faible SSE. Cette évidence scientifique devrait alimenter l’élaboration de programmes de santé materno-infantile, en plus de guider la planification de politiques et le renforcement des systèmes de santé des PRFI.

Depuis la fin des années 1990, les jeux de hasard et d’argent (JHA) sèment régulièrement la controverse dans l’espace public québécois. En effet, au cours des dernières années, la question du management et de la régulation des JHA a pris une place de plus en plus importante dans les débats publics. Dans le cadre de cette thèse, qui prend la forme d’une thèse par articles, nous nous intéresserons au processus de construction du problème public que représente maintenant le jeu excessif au Québec. Pour ce faire, nous tracerons, dans un premier temps, la genèse de ce problème public en remontant à ses origines. Ce détour historique nous permettra de mettre en relief les diverses constructions par lesquelles l’action de s’adonner aux JHA et ce, de manière excessive ou non, est passée au cours du dernier siècle, soit de vice, à crime, à loisir en passant par maladie et problème de santé publique. Cette genèse nous permettra d’apporter un nouvel éclairage sur la présente controverse relative aux JHA au Québec. Nous tenterons, dans un premier temps, de mettre en relief comment la santé publique a réussi à construire ce « nouveau risque » que représente maintenant l’action de s’adonner aux JHA. Par la suite, nous tenterons de tirer des leçons pour la compréhension, l’analyse et le management des politiques publiques et ce, via une analyse de la controverse portant sur les JHA (article 1), un travail de reconceptualisation théorique (article 2) ainsi qu’une réflexion épistémologique sur des fondements de l’analyse et du management des politiques publiques (article 3).

This paper provides an overview of work done in recent years by our research group to fuse multimodal images of the trunk of patients with Adolescent Idiopathic Scoliosis (AIS) treated at Sainte-Justine University Hospital Center (CHU). We first describe our surface acquisition system and introduce a set of clinical measurements (indices) based on the trunk's external shape, to quantify its degree of asymmetry. We then describe our 3D reconstruction system of the spine and rib cage from biplanar radiographs and present our methodology for multimodal fusion of MRI, X-ray and external surface images of the trunk We finally present a physical model of the human trunk including bone and soft tissue for the simulation of the surgical outcome on the external trunk shape in AIS.

In the twentieth century, as technology grew with it. This resulted in collective efforts and thinking in the direction of controlling work related hazards and accidents. Thus, safety management developed and became an important part of industrial management. While considerable research has been reported on the topic of safety management in industries from various parts of the world, there is scarcity of literature from India. It is logical to think that a clear understanding of the critical safety management practices and their relationships with accident rates and management system certifications would help in the development and implementation of safety management systems. In the first phase of research, a set of six critical safety management practices has been identified based on a thorough review of the prescriptive, practitioner, conceptual and empirical literature. An instrument for measuring the level of practice of these safety conduction a survey using questionnaire in chemical/process industry. The instrument has been empirically validated using Confirmatory Factor Analysis (CFA) approach. As the second step. Predictive validity of safety management practices and the relationship between safety management practices and self-reported accident rates and management system certifications have been investigated using ANOVA. Results of the ANOVA tests show that there is significant difference in the identified safety management practices and the determinants of safety performance have been investigated using Multiple Regression Analysis. The inter-relationships between safety management practices, determinants of safety performance and components of safety performance have been investigated with the help of structural equation modeling. Further investigations into engineering and construction industries reveal that safety climate factors are not stable across industries. However, some factors are found to be common in industries irrespective of the type of industry. This study identifies the critical safety management practices in major accident hazard chemical/process industry from the perspective of employees and the findings empirically support the necessity for obtaining safety specific management system certifications

For improving agricultural marketing, which has been discussed in the previous chapter, the Government has intervened in different ways. The direct regulatory role through the regulation of markets and market practices is one of the ways in which governmental intervention can improve agricultural marketing. This study is an enquiry of the direct regulatory role of the government through regulation of markets and market practices. By restructuring the operational methods and redesigning the existing physical markets, this system gives direct benefit to the cultivating class and protects them from the market manipulations of organised and powerful private traders. If traders do not continue their trade for the time being they will not be affected financially, because they are resourceful or financially solvent. On the other hand, Cultivators must sell their produce immediately after harvesting for the lack of additional facilities or to satisfy other needs for which finance is required. Another important reason is that Cultivators/farmers are not organised and because of lack of their organisation, they sell their produces individually. In this situation, a farmer is helpless when astute traders indulge in manipulations at the time of purchase of the produces. So it is the government's obligation to protect the interest of the farmers. Protection of the farmer/cultivator is necessary not only from the point of social justice but also from that of economic growth. If the farmers are assured of a remunerative or incentive price for their produce, they will get the inspiration to produce more and through more production, economy will be developed and the nation as a whole will be benefitted. This study will examine the management system of the markets through the direct regulatory role played by the governments to control markets and market practices in West Bengal and Bangladesh.

Production Planning and Control (PPC) systems have grown and changed because of the developments in planning tools and models as well as the use of computers and information systems in this area. Though so much is available in research journals, practice of PPC is lagging behind and does not use much from published research. The practices of PPC in SMEs lag behind because of many reasons, which need to be explored This research work deals with the effect of identified variables such as forecasting, planning and control methods adopted, demographics of the key person, standardization practices followed, effect of training, learning and IT usage on firm performance. A model and framework has been developed based on literature. Empirical testing of the model has been done after collecting data using a questionnaire schedule administered among the selected respondents from Small and Medium Enterprises (SMEs) in India. Final data included 382 responses. Hypotheses linking SME performance with the use of forecasting, planning and controlling were formed and tested. Exploratory factor analysis was used for data reduction and for identifying the factor structure. High and low performing firms were classified using a Logistic Regression model. A confirmatory factor analysis was used to study the structural relationship between firm performance and dependent variables.

Fish, a natural resource, has received great attention from all over the world. since it provides a cheap protein, employment and income to the millions of people for centuries. So fishermen, industrialist and multinationals are trying to exploit the marine resources to their maximum benefit by using modern craft, advance fishing equipments and efficient gear. Fishery resources in the open system particularly in oceans, were considered to be unlimited. However, recent developments in the innovation of efficient craft and gear using well tested material fitted with modern equipments that have greatly enhanced the mobility of craft, agility of gear and the ability of equipments to locate fishery resources have proved otherwise. Hence as the exploitation increases with more effort entering the fishery, the catch per unit of fishing effort starts to decline due to the limitness of the resources. The heavy fishing pressure in the recent past led to commercial extinction of a number of stocks such as. the North Sea herring, California sardine, Japanese Sardine and Peruvian anchovy (FAO. 1968: Gulland, 1974). In India, seer fish from Palk-Bay declined due to uncontrolled fishing (Devaraj 1983).

Facing growth in demand, dairy production in peri-urban areas of developing countries is changing rapidly. To characterise this development around Bamako (Mali), this study establishes a typology of dairy production systems with a special focus on animal genetic resources. The survey included 52 dairy cattle farms from six peri-urban sites. It was conducted in 2011 through two visits, in the dry and harvest seasons. The median cattle number per farm was 17 (range 5-118) and 42% of farmers owned cropland (8.3 +/- 7.3 ha, minimum 1 ha, maximum 25 ha). Feeding strategy was a crucial variable in farm characterisation, accounting for about 85% of total expenses. The use of artificial insemination and a regular veterinary follow-up were other important parameters. According to breeders’ answers, thirty genetic profiles were identified, from local purebreds to different levels of crossbreds. Purebred animals raised were Fulani Zebu (45.8%), Maure Zebu (9.2%), Holstein (3.0%), Azawak Zebu (1.3%), Mere Zebu (0.5%) and Kuri taurine (0.1%). Holstein crossbred represented 30.5% of the total number of animals (19.0% Fulani-Holstein, 11.2% Maure-Holstein and 0.3% Kuri-Holstein). Montbéliarde, Normande and Limousin crossbreds were also found (6.6%, 0.7% and 0.3%, respectively). A multivariate analysis helped disaggregate the diversity of management practices. The high diversity of situations shows the need for consideration of typological characteristics for an appropriate intervention. Although strongly anchored on local breeds, the peri-urban dairy systems included a diversity of exotic cattle, showing an uncoordinated quest of breeders for innovation. Without a public intervention, this dynamic will result in an irremediable erosion of indigenous animal genetic resources.

KAM is a computer program that can automatically plan, monitor, and interpret numerical experiments with Hamiltonian systems with two degrees of freedom. The program has recently helped solve an open problem in hydrodynamics. Unlike other approaches to qualitative reasoning about physical system dynamics, KAM embodies a significant amount of knowledge about nonlinear dynamics. KAM's ability to control numerical experiments arises from the fact that it not only produces pictures for us to see, but also looks at (sic---in its mind's eye) the pictures it draws to guide its own actions. KAM is organized in three semantic levels: orbit recognition, phase space searching, and parameter space searching. Within each level spatial properties and relationships that are not explicitly represented in the initial representation are extracted by applying three operations ---(1) aggregation, (2) partition, and (3) classification--- iteratively.

We address the problem of jointly determining shipment planning and scheduling decisions with the presence of multiple shipment modes. We consider long lead time, less expensive sea shipment mode, and short lead time but expensive air shipment modes. Existing research on multiple shipment modes largely address the short term scheduling decisions only. Motivated by an industrial problem where planning decisions are independent of the scheduling decisions, we investigate the benefits of integrating the two sets of decisions. We develop sequence of mathematical models to address the planning and scheduling decisions. Preliminary computational results indicate improved performance of the integrated approach over some of the existing policies used in real-life situations.
