Os autores relatam um caso de obstrução nasal unilateral, rinorréia purulenta e infecção crônica em um mulher de 57 anos causada por rinolito. A patologia mais freqüentemente encontrada em associação com rinolitíase é a sinusite crônica. A rinolitíase é uma concreção calcária estratificada visível ao exame radiológico. O rinolito pode ser um achado acidental em alguns pacientes. Rinolitos são raros e podem ter várias apresentações clínicas. O tratamento de escolha é remoção cirúrgica. Um alto índice de suspeição é necessário para o correto diagnóstico.

The exponential increase of home-bound persons who live alone and are in need of continuous monitoring requires new solutions to current problems. Most of these cases present illnesses such as motor or psychological disabilities that deprive of a normal living. Common events such as forgetfulness or falls are quite common and have to be prevented or dealt with. This paper introduces a platform to guide and assist these persons (mostly elderly people) by providing multisensory monitoring and intelligent assistance. The platform operates at three levels. The lower level, denominated ‘‘Data acquisition and processing’’performs the usual tasks of a monitoring system, collecting and processing data from the sensors for the purpose of detecting and tracking humans. The aim is to identify their activities in an intermediate level called ‘‘activity detection’’. The upper level, ‘‘Scheduling and decision-making’’, consists of a scheduler which provides warnings, schedules events in an intelligent manner and serves as an interface to the rest of the platform. The idea is to use mobile and static sensors performing constant monitoring of the user and his/her environment, providing a safe environment and an immediate response to severe problems. A case study on elderly fall detection in a nursery home bedroom demonstrates the usefulness of the proposal.

Elders lose independence and wellbeing, accompanied by decreased functions in terms of hearing, vision, strength and coordination abilities. These factors contribute to balance difficulties that eventually lead to falls. The injuries due to falls, at this age, are risky, since most of the times may cause a significant – and permanent – decrease of quality of life or, in extreme cases, death. In this context, a fall detection system can bring an added value to assist elderly people.This paper describes a system consisting of a wearable sensor unit, a smartphone and a website. When the sensor detects a fall it sends an alert using the smartphone via Bluetooth 4.0, to notify the family members or stakeholders. The sensor device includes an inertial unit, a barometer, and a temperature and humidity sensor. The website displays the log of previous falls and enables the configuration of emergency contact numbers. The proposed fall detection system is one of multiple components within a larger project under development that offers a holistic perspective on falls; the complete wearable solution will also feature, among others, physical protection (minimizing the impact of falls that occur).

BACKGROUND: When an organization performs an integrated analysis of risks through its Occupational Health and Safety Management System, several steps are suggested to address the implications of the identified risks. Namely, the organization should make a detailed analysis of the monetary impact for the organization of each of the preventive measures considered. However, it is also important to perform an analysis of the impact of each measure on society (externalities). OBJECTIVE: The aim of this paper is to present a case study related to the application of the proposed economic evaluation methodology. METHODS: An analysis of the work accidents in a hospital has been made. Three of the major types of accidents have been selected: needle stings, falls and excessive strain. Following the risk assessment, some preventive measures have been designed. Subsequently, the Benefit/Cost ratio (B/C) of these measures has been calculated, both in financial terms (from the organization’s perspective) and in economic terms (including the benefits for the worker and for the Society). RESULTS: While the financial ratio is only advantageous in some cases, when the externalities are taken into account, the B/C ratio increases significantly. CONCLUSIONS: It is important to consider external benefits to make decisions concerning the implementation of preventive measures in Occupational Health and Safety projects

Com o intuito de averiguar a importância de alguns roedores como possíveis reservatórios do S. mansoni, na ausência do homem parasitado, foi realizada pesquisa, visando contribuir para o esclarecimento de aspectos ligados à cadeia epidemiológica da esquistossomose, bem como conhecer alguns parâmetros da biologia de certos roedores, em seu habitat semi-natural. O experimento foi realizado num viveiro de 952 m², no município de Taubaté, São Paulo (Brasil), numa área endêmica de esquistossomose mansônica humana, e teve a duração de três anos e seis meses (agosto de 1973 a dezembro de 1976). Foram utilizados como hospedeiros definitivos, Holochilus brasiliensis leucogaster, Zygodontomys lasiurus, Oryzomys nigripes eliurus e Cavia aperea aperea; como hospeideiro intermediário, Biomphalaria tenagophila e posteriormente B. glabrata. Entre agosto de 1973 e janeiro de 1976, não houve encontro de B. tenagophila eliminando cercárias de S. mansoni; não se verificou, também, infecção natural de roedores. Em agosto de 1975, houve introdução acidental de desovas da B. glabrata, cujos adultos, em 1976, apresentaram infecção por S. mansoni em três ocasiões, com índices de 2,0; 1,6 e 0,8%. No mesmo ano de 1976, dois Holochilus, nascidos no Viveiro, eliminaram ovos viáveis de S. mansoni. Foi possível obter dados de 41 H. b. leucogaster, 28 introduzidos e 14 nascidos no local. O exemplar que sobreviveu mais tempo completou 346 dias. Os animais nascidos no Viveiro e capturados pela primeira vez pesavam, em média, 20 a 50 g. Notou-se que o peso corporal aumentou com o tempo e parece não estacionar até a morte do animal. Z. lasiurus e C. a aperea não procriaram e nem adquiriram infecção ao S. mansoni. O. n. eliurus, procriou e permaneceu vivo, em média, menos de 100 dias; não foi observada eliminação de ovos do parasita. É pouco provável que H. b. leucogaster e B. tenagophila mantenham o ciclo da esquistossomose na ausência da contaminação humana, na natureza. Porém, é possível que, futuramente, H. b. leucogaster na presença de B. glabrata, possa servir de reservatório da esquistossomose, na natureza, quando encontrados em abundância e desde que adaptados com cepas adequadas do parasita.

The invasive tendency of Psychodopygus intermedius in the home environment, observed initially by Forattini et al. (1976), has now been confirmed by the demonstration of its high endophilic ability and by the use of human residences for shelter. Populations such as Lutzomyia migonei and Pintomyia fischeri were also present in that environment, though their low densities registered during this investigation could be an indication of their poor ability to overcome the barriers raised by the artificial environment. An objective epidemiological analysis based on the variables here given showed that human infection takes place in the extraforest environment, and the principal vectorial function falls, without doubt, on P. intermedius.

Purpose: to investigate the relationship between knee muscle strength, balance and functional independence within the first month after stroke. Relevance: cerebral vascular disease is one of the main causes of morbidity, disability and mortality in developed countries. Problems with movement control are frequent after stroke. Lower limb weakness and impaired balance are common problems that are related with the risk of falls and are likely to interfere with the ability to perform daily life activities. Physiotherapy intervention usually starts early after stroke and addresses impairments related to movement and posture in order to improve motor recovery and restore function.

Cerebral vascular disease is the primary cause of permanent disability in Portugal. Impaired stability is considered an important feature after stroke as it is related with higher risk of falls and functional dependence. Physiotherapy intervention usually starts early after stroke in order to direct motor recovery and help patients to improve their ability to perform activities of daily living (ADL). Purpose: to investigate the relationship of balance to functionality in acute stroke patients. Methods: 16 subjects (8 women and 8 men), mean age 63,62 ± 2,16y, with unilateral ischemic stroke in the middle cerebral artery territory, who were admitted to physiotherapy department of Fernando Fonseca Hospital in Portugal, within the first month after stroke were recruited to participate in this study. All subjects have no cognitive impairment according to Mini Mental State, no history of lower extremity orthopedic problems and no other disease that could interfere with treatments. All patients gave their inform consent to participate in this study. Subjects were assessed with the Modified Barthel Index (MBI) and the Berg Balance Scale (BBS).

O presente relatório de estágio enquadra-se no âmbito do trabalho final de Mestrado do curso de Engenharia Civil, área de especialização em Hidráulica, do Instituto Superior de Engenharia de Lisboa, e incide na temática dos sistemas de distribuição de água. O estágio decorreu na empresa EPAL – Empresa Portuguesa das Águas Livres, S.A., e teve como objecto o desenvolvimento de um plano de manutenção preventiva para a rede de distribuição de água de Lisboa. Devido à crescente complexidade dos sistemas de abastecimento de água e à grande dispersão territorial dos diversos órgãos integrados na rede, surge a necessidade de desenvolver um sistema que permita simplificar o planeamento e a gestão das intervenções de inspecção e manutenção preventiva dos órgãos. Para esse efeito, recorreu-se a um software SIG (Sistema de Informação Geográfica) para proceder à sectorização da rede de distribuição de água em pequenos grupos (Polígonos), constituídos por um número determinado de órgãos de manobra e segurança. Esta metodologia permite organizar as intervenções em pequenos grupos e optimizar as rotinas de Inspecção diárias. No decorrer do estágio efectuou-se ainda um levantamento dos procedimentos de inspecção e manutenção dos diversos órgãos integrados na rede de distribuição de água de Lisboa, quer por acompanhamento de actividades com as equipas de inspecção, quer por recolha de recomendações junto dos fabricantes dos órgãos. Este levantamento permitiu elaborar fichas de procedimento para as actividades de inspecção e manutenção preventiva dos diversos órgãos, afim de serem usadas como referência pelos trabalhadores.

OBJETIVO: As populações de Aedes albopictus podem colonizar os mais variados tipos de recipientes, e sua presença tem sido registrada, inclusive, em vegetais Bromeliaceae. O objetivo do estudo foi avaliar o significado epidemiológico dessas plantas como criadouros potenciais de Ae. albopictus e registrar a entomofauna culicidiana associada nesse micro-habitat. MÉTODOS: Foram efetuadas quinzenalmente, durante os anos de 1998 e 1999, coletas de larvas de culicídeos em conteúdo aquático de bromélias, localizadas em áreas urbana e periurbana de Ilhabela e Ilha Comprida, no Estado de São Paulo. RESULTADOS: Coletaram-se 26.647 larvas de culicídeos nas duas localidades de estudo, 14.575 na área urbana e 10.987 na área periurbana de Ilhabela. As 1.085 restantes foram provenientes da área periurbana de Ilha Comprida. Não houve diferença estatística na quantidade de larvas capturadas em ambiente urbano e periurbano de Ilhabela. Com relação a Ae. albopictus, houve maior freqüência e quantidade de larvas no ambiente urbano de Ilhabela, seguido pelo periurbano na mesma localidade, enquanto que, no periurbano de Ilha Comprida, essa presença foi apenas acidental, sendo capturada apenas nos três primeiros meses. CONCLUSÕES: Os resultados revelam que as bromélias, atualmente muito utilizadas em paisagismo, podem contribuir para a dispersão de Aedes albopictus. O encontro freqüente dessa espécie, em bromélias domesticadas e domiciliadas, sugere que esse culicídeo participe da fauna regional, podendo promover condições propícias para o contato entre a população humana e os agentes causadores de doenças do ecossistema natural.

This paper summarizes a project that is contributing to a change in the way of teaching and learning Mathematics. Mathematics is a subject of the Accounting and Administration course. In this subject we teach: Functions and Algebra. The aim is that the student understand the basic concepts and is able to apply them in other issues, when possible, establishing a bridge between the issues that they have studied and their application in Accounting. As from this year, the Accounting course falls under in Bologna Process. The teacher and the student roles have changed. The time for theoretical and practical classes has been reduced, so it was necessary to modify the way of teaching and learning. In the theoretical classes we use systems of multimedia projection to present the concepts, and in the practical classes we solve exercises. We also use the Excel and the mathematical open source software wxMaxima. To supplement our theoretical and practical classes we have developed a project called MatActiva based on the Moodle platform offered by PAOL - Projecto de Apoio Online (Online Support Project). With the creation of this new project we wanted to take advantage already obtained results with the previous experiences, giving to the students opportunities to complement their study in Mathematics. One of the great objectives is to motivate students, encourage them to overcome theirs difficulties through an auto-study giving them more confidence. In the MatActiva project the students have a big collection of information about the way of the subject works, which includes the objectives, the program, recommended bibliography, evaluation method and summaries. It works as material support for the practical and theoretical classes, the slides of the theoretical classes are available, the sheets with exercises for the students to do in the classroom and complementary exercises, as well as the exams of previous years. Students can also do diagnostic tests and evaluation tests online. Our approach is a reflexive one, based on the professional experience of the teachers that explore and incorporate new tools of Moodle with their students and coordinate the project MatActiva.
