Several studies point to the plurality of care systems to deal with illness. They can be organized into professional, popular and alternative systems (the latter includes the complementary and the traditional ones). What the particular setup is in each cultural system is the core question of both the empirical studies we report. The purpose of this article is to understand how lay people deal with mental illness, examining the therapeutic itineraries that are constructed between plural care systems, featuring in particular the use of traditional medicine. The analysis of the two studies (one carried out in the north region and the other in Lisbon) allowed us to interpret these practices and discuss the social and cultural factors that determine and explain the settings that were found. Both researches fit into a qualitative methodology. In-depth, semi-structured interviews were performed and were analyzed using discourse analysis to describe and interpret data. The results point to a plurality of therapeutic itineraries, built around public and private speeches, where the explanatory systems underlying the use of official medicine and/or traditional practices found plural meanings. People may use these systems in several forms, using one or combining more than one, simultaneously or sequentially, depending on the context and on the needs they feel to face both illness and mental suffering. It is in between the space of the impotence and ‘incompetence’ of the ‘wise’ medicine that other therapeutic systems develop. It is important to understand those systems because of their achievements and their heuristic power to explain society and culture.

Cape Verde is considered part of Sahelian Africa, where drought and desertification are common occurrences. The main activity of the rural population is rain-fed agriculture, which over time has been increasingly challenged by high temporal and spatial rainfall variability, lack of inputs, limited land area, fragmentation of land, steep slopes, pests, lack of mechanization and loss of top soil by water erosion. Human activities, largely through poor farming practices and deforestation (Gomez, 1989) have accelerated natural erosion processes, shifting the balance between soil erosion and soil formation (Norton, 1987). According to previous studies, vegetation cover is one of the most important factors in controlling soil loss (Cyr et al., 1995; Hupy, 2004; Zhang et al., 2004; Zhou et al., 2006). For this reason, reforestation is a touchstone of the Cape Verdean policy to combat desertification. After Independence in 1975, the Cape Verde government had pressing and closely entangled environmental and socio-economic issues to address, as long-term desertification had resulted in a lack of soil cover, severe soil erosion and a scarcity of water resources and fuel wood. Across the archipelago, desertification was resulting from a variety of processes including poor farming practices, soil erosion by water and wind, soil and water salinity in coastal areas due to over pumping and seawater intrusion, drought and unplanned urbanization (DGA-MAAP, 2004). All these issues directly affected socio-economic vulnerability in rural areas, where about 70% of people depended directly or indirectly on agriculture in 1975. By becoming part of the Inter- State Committee for the Fight against Drought in the Sahel in 1975, the government of Cape Verde gained structured support to address these issues more efficiently. The molecular diversity of viruses complicates the interpretation of viral genomic and proteomic data. To make sense of viral gene functions, investigators must be familiar with the virus host range, replication cycle and virion structure. Our aim is to provide a comprehensive resource bridging together textbook knowledge with genomic and proteomic sequences. ViralZone web resource (www.expasy.org/viralzone/) provides fact sheets on all known virus families/genera with easy access to sequence data. A selection of reference strains (RefStrain) provides annotated standards to circumvent the exponential increase of virus sequences. Moreover ViralZone offers a complete set of detailed and accurate virion pictures.

A national survey conducted in Switzerland aimed to evaluate the knowledge of physiotherapists regarding the legal requirements for record keeping and to collect their feedback about record keeping in general. Three physiotherapists from various professional practice groups and a lawyer specialised in health law developed a questionnaire that was sent to the 7,753 members of two existing national associations of physiotherapists. The questionnaire evaluated the participants' knowledge by calculating a score of legal knowledge, which had a maximum of 30 points. We included 825 questionnaires in the analysis. The large majority (83.4%) of participants confessed an ignorance of the legal requirements concerning record keeping prior to the survey. The average score of legal compatibility was 8 points. The younger age of the physiotherapists was a significant predictor of having knowledge of the legal requirements for record keeping (p <0.001). The participants had an appreciation of the value of records, but they did not have the relevant knowledge regarding the legal requirements for keeping records. The participants blamed a lack of time and remuneration for their failure to keep records according to known requirements. All practising allied health professionals should keep up-to-date and accurate records that conform to active legal requirements and existing international guidelines. In addition to the existing legal requirements, the emergence of e-health and the electronic era will trigger major changes in patient record management by physiotherapists.

A pesca teve sempre grande importância socioeconómica para as comunidades costeiras de Cabo Verde, oferecendo meios de subsistência e, devido à vocação marítima do povo Cabo-verdiano, possibilidades de emprego. O peixe aparece como componente importante na alimentação da população e, por ser fonte de proteína e um animal de baixo custo para a população, requer que a sua exploração seja feita em moldes sustentáveis, perpetuando no tempo a disponibilidade desse recurso para toda a sociedade. Este trabalho apresenta o estudo das perceções dos pescadores sobre a sustentabilidade da exploração dos recursos haliêuticos pesqueiros e a pesca artesanal dominante na ilha. A pesca é uma das principais atividades económicas da zona costeira da Ilha do Sal, além de ser uma importante atividade de subsistência para as três comunidades haliêuticas da ilha. Com o objetivo de discutir caminhos sustentáveis para a atividade, reflete-se sobre a sustentabilidade da pesca artesanal na comunidade da Palmeira, Ilha do Sal, Cabo Verde. Numa comunidade como a Palmeira, onde a pesca é tipicamente artesanal, encontramos diversos elementos que garantem a sustentabilidade da atividade. Conhecer e desenvolver novos mecanismos que visam educar e criar políticas sustentáveis para a atividade e gestão dos recursos é importante para a nova conjuntura em que se vive. A educação e a organização dos pescadores, bem como a descentralização e a gestão participativa dos recursos pesqueiros, são condições fundamentais para a sustentabilidade da pesca. Este trabalho tenta responder à escassez de estudos sobre as comunidades piscatórias em Cabo Verde de modo a favorecer o conhecimento ambiental que potenciará a criação de estratégias-chave para a sustentabilidade, a análise dos projetos criados até à data e a respetiva implementação, permitindo a identificação das causas do insucesso total ou parcial, bem como a identificação das causas para o fraco envolvimento da comunidade piscatória na implementação dos projetos. Fishery has always been of great social economic importance for the coastal communities of Cape Verde, offering means of subsistence and employment opportunities due to their maritime vocation. Fish is an important food component for the population. Since fishery is a source of low-cost animal protein, its exploration must be sustainable in order to be permanently available for the community. This paper presents a study on the perception of fishermen on the sustainable exploitation of fishery resources in a symbiosis with the dominant artesanal fishing practiced on the island. Fishing is one of the main economic activities of the coastal zone of Sal island, besides being an important subsistence activity for the three fishing communities of the island. Aiming to discuss ways for sustainable activity, we will reflect on the sustainability of the traditional fishing in the community of Palmeira, Sal Island, Cape Verde. In a community like Palmeira, where fishing is typically artesanal, we find many elements that ensure the sustainability of the activity, such as the predominant use of renewable natural resources and the diversity of species caught. in Sal Inland knowing and developing new mechanisms to educate, create sustainable policies for the activity and resource management are important to the environment. Education and organization of fishermen, as well as decentralization and participatory management of fishery resources, are fundamental to the sustainability of fisheries. This work tries to answer the scarcity of studies on fishing communities in order to promote environmental knowledge that will enhance the creation of key strategies for sustainability, the analysis of projects created to date and the respective implementation, allowing the identification of the causes of the total or partial failure, as well as the identification of the causes for the poor involvement of the fishing community in the implementation of projects.

This paper surveys asset allocation methods that extend the traditional approach. An important feature of the the traditional approach is that measures the risk and return tradeoff in terms of mean and variance of final wealth. However, there are also other important features that are not always made explicit in terms of investor s wealth, information, and horizon: The investor makes a single portfolio choice based only on the mean and variance of her final financial wealth and she knows the relevant parameters in that computation. First, the paper describes traditional portfolio choice based on four basic assumptions, while the rest of the sections extend those assumptions. Each section will describe the corresponding equilibrium implications in terms of portfolio advice and asset pricing.

Research on judgment and decision making presents a confusing picture of human abilities. For example, much research has emphasized the dysfunctional aspects of judgmental heuristics, and yet, other findings suggest that these can be highly effective. A further line of research has modeled judgment as resulting from as if linear models. This paper illuminates the distinctions in these approaches by providing a common analytical framework based on the central theoretical premise that understanding human performance requires specifying how characteristics of the decision rules people use interact with the demands of the tasks they face. Our work synthesizes the analytical tools of lens model research with novel methodology developed to specify the effectiveness of heuristics in different environments and allows direct comparisons between the different approaches. We illustrate with both theoretical analyses and simulations. We further link our results to the empirical literature by a meta-analysis of lens model studies and estimate both human andheuristic performance in the same tasks. Our results highlight the trade-off betweenlinear models and heuristics. Whereas the former are cognitively demanding, the latterare simple to use. However, they require knowledge and thus maps of when andwhich heuristic to employ.

Os simuladores representam uma forma de transmissão e difusão do conhecimento, de um modo inovador face ao ensino tradicional das matérias. Esta forma de ensino pretende aplicar os métodos desde há muito utilizados nas ciências experimentais, em outras áreas do saber, onde a experiência laboratorial e o ensaio prático controlado não são possíveis. As ciências económicas e empresariais constituem, pela sua própria natureza, um ambiente propício à utilização de simuladores de ensino, dado que não é possível, ou ainda que eventualmente possível, não desejável, efetuar experiências em ambiente real para treino dos futuros profissionais. Este artigo foca-se na problemática da construção e utilização de simuladores para treino dos profissionais de gestão de empresas e relata um caso de estudo ocorrido no Instituto Superior de Ciências Económicas e Empresariais de Cabo Verde no ano letivo de 2013/2014. Abstract Simulators are an innovative way of knowledge transmission and dissemination when comparing to the traditional subject teaching. This form of education intends to apply the methods that have long been used in experimental sciences to other disciplinessuch as those where the controlled laboratory experiment or practical tests are not possible. The economic and business sciences are, by their very nature, conducive to the use of simulators in teaching, since it is not possible, or not desirable, to make experiments in a real life environment for training purposes. This article focuses on the issue of simulators construction and use whenapplied to the training of business management professionals, and reports a case study occurred at the Institute of Economics and Management of Cape Verde in academic year of 2013/2014.

Given the existing challenges in accessing print-based publications in developing countries, digital libraries are seen as a good alternative. Thus, it is important to understand how such libraries are used in these contexts, especially when compared with the usage of traditional libraries. This paper analyzes and compares the usage of the digital and traditional libraries of the University Jean Piaget of Cape Verde, aiming at understanding the way they are used, and the relation between the access to the existing information resources in these two libraries.

BACKGROUND: "Virtual" autopsy by postmortem computed tomography (PMCT) can replace medical autopsy to a certain extent but has limitations for cardiovascular diseases. These limitations might be overcome by adding multiphase PMCT angiography. OBJECTIVE: To compare virtual autopsy by multiphase PMCT angiography with medical autopsy. DESIGN: Prospective cohort study. (ClinicalTrials.gov: NCT01541995) SETTING: Single-center study at the University Medical Center Hamburg-Eppendorf, Hamburg, Germany, between 1 April 2012 and 31 March 2013. PATIENTS: Hospitalized patients who died unexpectedly or within 48 hours of an event necessitating cardiopulmonary resuscitation. MEASUREMENTS: Diagnoses from clinical records were compared with findings from both types of autopsy. New diagnoses identified by autopsy were classified as major or minor, depending on whether they would have altered clinical management. RESULTS: Of 143 eligible patients, 50 (35%) had virtual and medical autopsy. Virtual autopsy confirmed 93% of all 336 diagnoses identified from antemortem medical records, and medical autopsy confirmed 80%. In addition, virtual and medical autopsy identified 16 new major and 238 new minor diagnoses. Seventy-three of the virtual autopsy diagnoses, including 32 cases of coronary artery stenosis, were identified solely by multiphase PMCT angiography. Of the 114 clinical diagnoses classified as cardiovascular, 110 were confirmed by virtual autopsy and 107 by medical autopsy. In 11 cases, multiphase PMCT angiography showed "unspecific filling defects," which were not reported by medical autopsy. LIMITATION: These results come from a single center with concerted interest and expertise in postmortem imaging; further studies are thus needed for generalization. CONCLUSION: In cases of unexpected death, the addition of multiphase PMCT angiography increases the value of virtual autopsy, making it a feasible alternative for quality control and identification of diagnoses traditionally made by medical autopsy. INTRODUCTION: Systematic literature reviews provide best evidence, but are underused by clinicians. Thus, integrating Cochrane reviews into continuing medical education (CME) is challenging. We designed a pilot CME program where summaries of Cochrane reviews (Courriels Cochrane) were disseminated by e-mail. Program participants automatically received CME credit for each Courriel Cochrane they rated. The feasibility of this program is reported (delivery, participation, and participant evaluation). METHOD: We recruited French-speaking physicians through the Canadian Medical Association. Program delivery and participation were documented. Participants rated the informational value of Courriels Cochrane using the Information Assessment Method (IAM), which documented their reflective learning (relevance, cognitive impact, use for a patient, expected health benefits). IAM responses were aggregated and analyzed. RESULTS: The program was delivered as planned. Thirty Courriels Cochrane were delivered to 985 physicians, and 127 (12.9%) completed at least one IAM questionnaire. Out of 1109 Courriels Cochrane ratings, 973 (87.7%) conta-ined 1 or more types of positive cognitive impact, while 835 (75.3%) were clinically relevant. Participants reported the use of information for a patient and expected health benefits in 595 (53.7%) and 569 (51.3%) ratings, respectively. DISCUSSION: Program delivery required partnering with 5 organizations. Participants valued Courriels Cochrane. IAM ratings documented their reflective learning. The aggregation of IAM ratings documented 3 levels of CME outcomes: participation, learning, and performance. This evaluation study demonstrates the feasibility of the Courriels Cochrane as an approach to further disseminate Cochrane systematic literature reviews to clinicians and document self-reported knowledge translation associated with Cochrane reviews.

Many classifiers achieve high levels of accuracy but have limited applicability in real world situations because they do not lead to a greater understanding or insight into the^way features influence the classification. In areas such as health informatics a classifier that clearly identifies the influences on classification can be used to direct research and formulate interventions. This research investigates the practical applications of Automated Weighted Sum, (AWSum), a classifier that provides accuracy comparable to other techniques whilst providing insight into the data. This is achieved by calculating a weight for each feature value that represents its influence on the class value. The merits of this approach in classification and insight are evaluated on a Cystic Fibrosis and Diabetes datasets with positive results.

In conducting genome-wide association studies (GWAS), analytical approaches leveraging biological information may further understanding of the pathophysiology of clinical traits. To discover novel associations with estimated glomerular filtration rate (eGFR), a measure of kidney function, we developed a strategy for integrating prior biological knowledge into the existing GWAS data for eGFR from the CKDGen Consortium. Our strategy focuses on single nucleotide polymorphism (SNPs) in genes that are connected by functional evidence, determined by literature mining and gene ontology (GO) hierarchies, to genes near previously validated eGFR associations. It then requires association thresholds consistent with multiple testing, and finally evaluates novel candidates by independent replication. Among the samples of European ancestry, we identified a genome-wide significant SNP in FBXL20 (P = 5.6 × 10(-9)) in meta-analysis of all available data, and additional SNPs at the INHBC, LRP2, PLEKHA1, SLC3A2 and SLC7A6 genes meeting multiple-testing corrected significance for replication and overall P-values of 4.5 × 10(-4)-2.2 × 10(-7). Neither the novel PLEKHA1 nor FBXL20 associations, both further supported by association with eGFR among African Americans and with transcript abundance, would have been implicated by eGFR candidate gene approaches. LRP2, encoding the megalin receptor, was identified through connection with the previously known eGFR gene DAB2 and extends understanding of the megalin system in kidney function. These findings highlight integration of existing genome-wide association data with independent biological knowledge to uncover novel candidate eGFR associations, including candidates lacking known connections to kidney-specific pathways. The strategy may also be applicable to other clinical phenotypes, although more testing will be needed to assess its potential for discovery in general.
