This inventory of transit services in Iowa covers urban transit, intercity passenger bus carriers and charter operators, taxicab operations, rural transit services and special services, and includes the results of questionnaire and interview surveys, and the resulting recommendations. The recommendations urge a centralized source of data and expertise, a public information program, the utilization of federal aid, the continuance of existing transit services with no dimunition of service level, the establishment of new services in communities of over 20,000 population, the sponsorship of demonstrations in communities with populations of 10,000 to 20,000, and the development and improvement of rural transit service. Based on state and local community experience, recommendations are made concerning revenue sources to support transit in Iowa, and four alternative state transit assistance programs are presented.

Si dans certains domaines (ex. les services de l'emploi) la co-production est bien implantée, elle paraît plus ardue dans d'autres types de services publics, comme la police. La co-production de la sécurité est-elle faisable ? Quelles sont les volontés de part et d'autre pour développer ce partenariat ? À l'aune de la police de proximité en Suisse et sur la base de données quantitatives et qualitatives, cet article apporte des résultats innovants. If in some administrations (eg. employment services) it is clearly well established, the co-production seems to be more difficult in other services, like the police. Thus, is the co-production of security services feasible ? Are both parties (police forces and citizens) ready for such a process ? In the light of community policing in Switzerland and on the basis of quantitative and qualitative data, this article will provide innovative results and research avenues in the field.

Background: There may be a considerable gap between LDL cholesterol (LDL-C) and blood pressure (BP) goal values recommended by the guidelines and results achieved in daily practice. Design Prospective cross-sectional survey of cardiovascular disease risk profiles and management with focus on lipid lowering and BP lowering in clinical practice. Methods: In phase 1, the cardiovascular risk of patients with known lipid profile visiting their general practitioner was anonymously assessed in accordance to the PROCAM-score. In phase 2, high-risk patients who did not achieve LDL-C goal less than 2.6 mmol/l in phase 1 could be further documented. Results: Six hundred thirty-five general practitioners collected the data of 23 892 patients with known lipid profile. Forty percent were high-risk patients (diabetes mellitus or coronary heart disease or PROCAM-score >20%), compared with 27% estimated by the physicians. Goal attainment rate was almost double for BP than for LDL-C in high-risk patients (62 vs. 37%). Both goals were attained by 25%. LDL-C values in phase 1 and 2 were available for 3097 high-risk patients not at LDL-C goal in phase 1; 32% of patients achieved LDL-C goal of less than 2.6 mmol/l after a mean of 17 weeks. The most successful strategies for LDL-C reduction were implemented in only 22% of the high-risk patients. Conclusion: Although patients at high cardiovascular risk were treated more intensively than low or medium risk patients, the majority remained insufficiently controlled, which is an incentive for intensified medical education. Adequate implementation of Swiss and International guidelines would expectedly contribute to improved achievement of LDL-C and BP goal values in daily practice.

PURPOSE: Diisononyl phthalate (DiNP) is primarily used as a plasticizer in polyvinyl chloride (PVC) materials. While information is available on general population exposure to DiNP, occupational exposure data are lacking. We present DiNP metabolite urinary concentrations in PVC processing workers, estimate DiNP daily intake for these workers, and compare worker estimates to other populations. METHODS: We assessed DiNP exposure in participants from two companies that manufactured PVC materials, a PVC film manufacturer (n = 25) and a PVC custom compounder (n = 12). A mid-shift and end-shift urine sample was collected from each participant and analyzed for the DiNP metabolite mono(carboxy-isooctyl) phthalate (MCiOP). Mixed models were used to assess the effect on MCiOP concentrations of a worker being assigned to (1) a task using DiNP and (2) a shift where DiNP was used. A simple pharmacokinetic model was used to estimate DiNP daily intake from the MCiOP concentrations. RESULTS: Creatinine-adjusted MCiOP urinary concentrations ranged from 0.42-80 μg/g in PVC film and from 1.11-13.4 μg/g in PVC compounding. PVC film participants who worked on a task using DiNP (n = 7) had the highest MCiOP geometric mean (GM) end-shift concentration (25.2 μg/g), followed by participants who worked on a shift where DiNP was used (n = 11) (17.7 μg/g) as compared to participants with no task (2.92 μg/g) or shift (2.08 μg/g) exposure to DiNP. The GM end-shift MCiOP concentration in PVC compounding participants (4.80 μg/g) was comparable to PVC film participants with no task or shift exposure to DiNP. Because no PVC compounding participants were assigned to tasks using DINP on the day sampled, DiNP exposure in this company may be underestimated. The highest DiNP intake estimate was 26 μg/kg/day. CONCLUSION: Occupational exposure to DiNP associated with PVC film manufacturing tasks were substantially higher (sixfold to tenfold) than adult general population exposures; however, all daily intake estimates were less than 25% of current United States or European acceptable or tolerable daily intake estimates. Further characterization of DiNP occupational exposures in other industries is recommended.

Tutkimuksen tavoitteena oli selvittää Keskuskauppakamarin tilintarkastuslautakunnan ja Valtion tilintarkastuslautakunnan valvontaratkaisujen vaikutusta hyvään tilintarkastustapaan. Tavoitteeseen päästiin alatavoitteiden kautta: a) selvittämällä tilintarkastukselle asetetut vaatimukset ja tilintarkastajien valvonnan periaatteet, b) tarkastelemalla valvontaratkaisujen kautta tilintarkastajien velvollisuuksia: riippumattomuutta, salassapitovelvollisuutta sekä ammattitaitoa ja huolellisuutta, c) tutkimalla, millaisia seikkoja Keskuskauppakamarin tilintarkastuslautakunta ja Valtion tilintarkastuslautakunta korostavat ja nostavat esiin valvontaratkaisujen yhteydessä d) tutkimalla, mihin edellä mainitut valvontaorganisaatiot kiinnittävät huomiota valvontaratkaisujen käsittelyssä ja mihin päätökset perustuvat. Tutkimus on luonteeltaan kvalitatiivinen ja deskriptiivinen. Se on myös normiperustainen, koska tilintarkastuksen taustalla vaikuttavat erilaiset lait, asetukset ja suositukset. Tutkimuksen empiirinen aineisto koostuu Keskuskauppakamarin tilintarkas tuslautakunnan ja Valtion tilintarkastuslautakunnan valvontaratkaisuselosteista vuosilta 1995-2004. Tutkimuksen perusteella valvontaratkaisut muokkaavat hyvää tilintarkastustapaa. Tilintarkastuslautakunta julkaisee ratkaisujen johdosta kannanottoja, antaa tarvittaessa tilintarkastusalan säännöstöä täydentäviä suosituksia ja ohjeita sekä esittää oman kannanottonsa tulkinnanvaraisiin seikkoihin. Valvontaratkaisut perustuvat voimassa olevaan säädöstöön, tilintarkastusalan suosituksiin ja toimielinten aiempiin ratkaisuihin. Tilintarkastajien toiminnan arvioinnissa tilintarkastajan tekemän virheen olennaisuudella on keskeinen merkitys. Lisäksi arvioinnissa kiinnitetään erityisesti huomiota tilintarkastajien toimintaan kokonaisuutena ja siihen, onko ulkopuolinen voinut saada virheen johdosta väärän käsityksen. Ratkaisuissa korostuu säätiöiden, julkisen kaupankäynnin kohteena olevien yhtiöiden sekä erikoistarkastusten erityinen tarkastusvastuu.
