Dacron and nitrocellulose were evaluated as matrices for the dot enzyme linked immunosorbent assay (dot-ELISA) for schistosomiasis and compared to indirect immunofluorescence (IMF). Titration of sera from 18 schistosomiasis patients against soluble worm antigen preparation (SWAP) was carried out and sera from healthy individuals from non-endemic areas were used as controls. The IMF was less sensitive than the dot-ELISAs, although the difference was not statistically significant (p > 0.05). The dot-ELISA based on nitrocellulose was as sensitive as that using dacron. Stability did not differ between nitrocellulose and dacron. Specificity was lower when dacron was used than when nitrocellulose was used, although the difference was not statistically significant (p > 0.05). In conclusion, this work showed that nitrocellulose and dacron performed similarly in dot-ELISA, suggesting that they may be used alternatively in population surveillance in endemic areas.

A combination of histological techniques applied to the study of Biomphalaria glabrata yielded some interesting new data about the histology of this snail, a major intermediate host of Schistosoma mansoni in Brazil. Three kinds of pigments were identified: a dark pigment which bleached following oxidation with potassium permanganate; a lipofuchsin-like, diastase-resistant PAS-positive pigment and an iron-containing pigment, probably related to hemosiderin. Calcium was detected in small deposits within the connective tissue and forming a dense core inside the chitinous radular teeth. The presence of fibrils, staining with sirius-red and birefringence under polarized light strongly suggest primitive collagen tissue. The radular apparatus appeared as a storing site for glycogen, while abundant Alcian-blue positive material (proteoglycans) was extremely concentrated in the radular sac.

A fibrose periportal esquistossomótica observada no modelo murino apareceu muito mais freqüentemente (69,2%) em camundongos submetidos à múltiplas infecções pelo Schistosoma mansoni do que naqueles animais com infecção única (11,1%). A contagem dos ovos depositados no fígado não diferiu significativamente nos dois grupos ao término dos experimentos. Embora não tenha ficado esclarecido o motivo pelo qual as infecções repetidas favorecem o desenvolvimento da fibrose periportal esquistossomótica, os dados observados fornecem apoio experimental às observações clínico-epidemiológicas que sugerem ter as reinfecções um papel na patogenia da forma hepato-esplênica da esquistossomose.

O objetivo desse trabalho foi determinar a prevalência das parasitoses em escolares de Bambuí, através de exames coprológicos (direto e Kato-Katz) e reavaliar os criadouros de moluscos descritos no município. Dos 2.901 escolares examinados, 20,1% estavam parasitados, sendo que Giardia lamblia, Entamoeba coli, Ascaris lumbricoides e ancilostomídeos foram os parasitas mais freqüentes, com prevalências de 6,2%, 6,2%, 4,8% e 1,4%, respectivamente. Os ancilostomídeos foram significativamente mais freqüentes na zona rural e nos alunos com mais de 14 anos, enquanto a prevalência da E. coli foi maior na zona urbana e a G. lamblia mais freqüente na faixa etária de 0-6 anos. Somente três crianças eliminavam ovos de Schistosoma mansoni. O único hospedeiro intermediário encontrado foi a Biomphalaria glabrata e nenhuma delas estava eliminando cercárias de S. mansoni . Comparando-se estes dados a de levantamentos realizados anteriormente no município, observou-se uma queda na prevalência de todos os parasitas. Algumas hipóteses para tentar explicar esta queda são discutidas tais como: processo intenso de urbanização e a melhoria das condições sócio-sanitárias do município.

Um levantamento da malacofauna límnina do Campus de Manguinhos, Fundação Oswaldo Cruz, Rio de Janeiro, foi realizado nos últimos dois anos visando comparar as espécies hoje existentes com aquelas encontradas no início deste século. Foram pesquisadas 18 coleções hídricas numa extensão de 880.000m², sendo encontradas em 13 delas as seguintes espécies: Antillorbis nordestensis, Biomphalaria glabrata, Biomphalaria straminea, Lymnaea columella, Melanoides tuberculatus, Physa cubensis, Pomacea glauca e Pomacea lineata. Destacam-se o desaparecimento de Biomphalaria tenagophila, registrada por Lutz em 1918, a introdução de B. straminea e da espécie asiática M. tuberculatus. Nenhum molusco apresentou infecção por Schistosoma mansoni.

Uma tentativa para se julgar da importância das pesquisas sobre esquistossomose, feitas no Brasil, desde 1908 até hoje, revela quão difícil e imprecisa é esta tarefa quando são postos em prática os critérios com que a produção científica é hoje em dia avaliada. Todavia, sem desmerecer a grande importância da contribuição internacional, os conhecimentos gerados sobre a esquistossomose no Brasil, neste quase um século de pesquisas, aparecem suficientes para atender a todas as nossas necessidades práticas e cientificas no setor.

Clinical involvement of the nervous system in visceral larva migrans due to Toxocara is rare, although in experimental animals the larvae frequently migrate to the brain. A review of the literature from the early 50's to date found 29 cases of brain involvement in toxocariasis. In 20 cases, various clinical and laboratory manifestations of eosinophilic meningitis, encephalitis, myelitis or radiculopathy were reported. We report two children with neurological manifestations, in which there was cerebrospinal fluid pleocytosis with marked eosinophilia and a positive serology for Toxocara both in serum and CSF. Serology for Schistosoma mansoni, Cysticercus cellulosae, Toxoplasma and cytomegalovirus were negative in CSF, that was sterile in both cases. Improvement of signs and symptoms after specific treatment (albendazole or thiabendazole) was observed in the two cases. A summary of data described in the 25 cases previously reported is presented and we conclude that in cases of encephalitis and myelitis with cerebrospinal fluid pleocytosis and eosinophilia, parasitic infection of the central nervous system should be suspected and serology should be performed to establish the correct diagnosis and treatment.

In order to evaluate Callithrix jacchus as an animal model for mansoni schistosomiasis, a group of 10 male animals were once percutaneously exposed to 250 cercariae of the Schistosoma mansoni SLM (São Lourenço da Mata) strain. Animals were periodically bled for measuring serum level of enzymes and proteins and for blood cell counting. When comparing pre-infection to post-infection values, a significant increase was found for alkaline phosphatase at 15 to 120 days p.i., differential counts of eosinophil at 45 and 60 days, and total protein and global eosinophil counts at 120 days. No Schistosoma mansoni eggs were found in stools. Adult worms of small size were recovered from five animals. At day 120, the number of Schistosoma mansoni eggs/g of tissue was 0-289.7 (liver), 0-30.1 (large intestine) and 0-171.4 (small intestine). These findings lead us to classify Callithrix jacchus as a non-permissive host to the SLM strain of Schistosoma mansoni.

A mielorradiculopatia esquistossomótica é a forma ectópica mais grave e incapacitante da infecção pelo Schistosoma mansoni. A sua prevalência em área endêmica tem sido subestimada. O diagnóstico baseia-se na presença de sintomas neurológicos decorrentes de lesões da medula espinhal em nível torácico baixo e/ou lombar alto, na demonstração da infecção esquistossomótica por técnicas microscópicas ou sorológicas e na exclusão de outras causas de mielite transversa. O tratamento precoce, com esquistossomicidas e corticoesteróides, mostra-se eficaz na maioria dos casos e os pacientes não tratados não se recuperam ou morrem. Não há consenso sobre doses e duração do tratamento, mas estudo recente sugere que os corticoesteróides devam ser usados por pelo menos seis meses. Como o diagnóstico é presuntivo e o tratamento essencialmente clínico, há que se manter alerta para a presença da doença, aperfeiçoar a propedêutica e, dessa forma, evitar-se a laminectomia rotineira. Com o advento da ressonância magnética da medula espinhal houve grande avanço no diagnóstico da esquistossomose medular. Como conseqüência, o número de casos de mielopatia esquistossomótica relatados tem aumentado rapidamente.

This study compared the efficiency of Kato-Katz thick smear and thick smear techniques for the diagnosis of intestinal helminths. The sensitivity of the thick smear technique was higher than that of the Kato-Katz method for the diagnosis of all helminths except Schistosoma mansoni.

Relatamos o caso de uma mulher de 38 anos, assintomática, que procurou atendimento médico devido à infertilidade. No exame ginecológico, observou-se pólipo endocervical de 1,0 x 0,8 x 0,5cm que foi biopsiado. O exame histológico evidenciou granulomas contendo ovos de Schistosoma mansoni.

Lesions involving the intra-hepatic biliary ducts in schistosomiasis have been reported in the literature, both in mice and man, but there are no data concerning their quantitative, evolutionary or post curative chemotherapeutic aspects on record. In order to obtain such data an investigation on this subject was attempted. Mice infected with 50 Schistosoma mansoni cercariae were submitted to a liver biopsy at the 9th week post-infection, and treated with 400mg/bw praziquantel immediately afterwards. Infected and non-infected controls were submitted to the same procedures. By 19 weeks from cercarial exposure all surviving animals were sacrificed. The biliary ducts were counted on histological sections and the results were expressed as biliary ducts/portal spaces. This quantitative evaluation was compared with that from normal controls and revealed hyperplasia as the main biliary duct change (p<0.007) in schistosomiasis. Hyperplastic changes underwent only mild partial and not statistically significant regression after specific chemotherapy (p>0.05). Infected and untreated animals presented ductal changes that did not differ from those of the treated group. Measurements of serum bilirrubin (total and direct), and gamma-glutamyl-transpeptidase (gamma-GT) did not reveal significant differences when animals from the several groups were compared. Thus, bile ducts exhibit a proliferative response in relation to neighboring S. mansoni injury to portal areas, but although these lesions are histopathologically impressive, they lack a functional or prognostic significance.

Na região do Subúrbio Ferroviário da cidade do Salvador (Bahia, Brasil), foram relatados casos de esquistossomose, e isso motivou este estudo de prevalência em 268 escolares residentes em um dos seus bairros (São Bartolomeu). Em 30,2% das crianças, o exame parasitológico (Kato-Katz) foi positivo, com carga parasitária entre 24 a 2.160 ovos de Schistosoma mansoni/g de fezes, predominando entre os meninos e nos maiores de 10 anos de idade. Nas crianças com residências mais próximas das coleções naturais de água, ocorreu maior freqüência de eliminadores de ovos e nela residiam os três únicos casos com a forma hepatosplênica da esquistossomose. Também, todas as 81 crianças com ovos de Schistosoma mansoninas fezes nasceram e sempre residiram na área de estudo. Como nesse bairro há todos os elementos da cadeia epidemiológica do Schistosoma mansoni, são fortes as evidências da transmissão urbana dessa infecção.

Abdominal ultrasound can be a useful tool for diagnosing periportal fibrosis related to Schistosoma mansoni infection, and also for planning and monitoring the evolution of hepatic morbidity following control measures. We evaluated the standardized ultrasound methodology proposed by the World Health Organization for detecting periportal fibrosis and portal hypertension, among patients from an endemic area in Venezuela, and the impact of praziquantel treatment 3-5 years later. After chemotherapy, complete reversal of periportal lesions was observed in 28.2% of the cases and progression of the disease in 5.1%. Improvement in the hepatic disease started with a reduction in the periportal thickening followed by a decrease in the size of the left hepatic lobe, spleen and mesenteric and spleen veins. Ultrasound confirmed the clinical findings after chemotherapy among the patients with reversal of the disease. However, in patients with more advanced disease, these findings were contradictory. There was no correlation between evolution of the disease seen on ultrasound and age, intensity of infection or serological findings.

Schistosomal myeloradiculopathy is the most severe and disabling ectopic form of Schistosoma mansoni infection. The prevalence of SMR in centres in Brazil and Africa that specialise in attending patients with non traumatic myelopathy is around 5%. The initial signs and symptoms of the disease include lumbar and/or lower limb pain, paraparesis, urinary and intestinal dysfunctions, and impotence in men. The cerebrospinal fluid of SMR patients shows an increase in protein concentration and in the number of mononuclear cells in 90% of cases; eosinophils have been reported in 40%. The use of magnetic resonance imaging is particularly valuable in the diagnosis of Schistosomal myeloradiculopathy. The exclusion of other myelopathies and systemic diseases remains mandatory. Early diagnosis and treatment with steroids and schistosomicides provide a cure for most patients, whilst delayed treatment can result in irreversible physical disabilities or death. To improve awareness concerning Schistosomal myeloradiculopathy amongst public health professionals, and to facilitate the control of the disease, the Brazilian Ministry of Health has launched a program of education and control of this ectopic form of schistosomiasis. The present paper reviews current methods for the diagnosis of SMR and outlines protocols for treatment of the disease.
