La durée de psychose non traitée (Duration of Untreated Psychosis, DUP) est définie par le temps écoulé entre l'émergence d'un trouble psychotique et le début de son traitement. La réduction de la DUP est un des objectifs principaux des programmes spécialisés dans le traitement de la psychose émergente, de nombreux travaux de recherche suggérant qu'une DUP longue est associée à une évolution défavorable de la maladie. Ces résultats restent cependant controversés, certaines études ne démontrant pas une telle association. Cette contradiction dans les résultats pourrait être la conséquence d'un manque d'uniformité dans les définitions appliquées pour mesurer la DUP, plus particulièrement en ce qui concerne la définition de ce que l'on considère être « début » du traitement. En effet, si l'étude de la phase d'émergence de la pathologie psychotique a été le focus d'une attention considérable qui a conduit à un certain degré de consensus quant à sa définition, le concept de début du traitement n'est clairement pas défini de manière aussi homogène. Compte tenu de l'importance des enjeux relatifs à l'intervention précoce dans les troubles psychotiques, il nous a semblé utile d'explorer cette question de manière plus approfondie, considérant qu'un manque de consensus dans la définition de la DUP contribue certainement à troubler les résultats des études qui visent à évaluer son impact sur l'évolution de ces maladies. En conséquence, l'objectif premier de ce travail est d'explorer l'impact de l'application de diverses définitions de début de traitement sur l'estimation de la DUP. Dans un premier article, publié dans Acta Neuropsychiatrica en 2009 (Duration of untreated psychosis : What are we talking about ?), le focus a été placé sur une revue de littérature concernant les définitions utilisées pour caractériser la fin de la DUP ainsi que sur les conséquences possibles d'un manque de précision dans cette définition sur l'évaluation de l'impact d'un retard de traitement dans la psychose débutante. Ce travail nous a permis d'identifier trois groupes principaux de définition de fin de DUP (End of DUP ; E-DUP) parmi les multiples critères utilisés dans les études publiées. E-DUP-1 est définie par la mise en route d'un traitement antipsychotique, le plus souvent sans tenir compte ni du dosage prescrit, ni de l'adhérence au traitement. E-DUP-2 est définie par l'entrée dans un programme de traitement spécialisé, et E-DUP-3 enfin est définie par la conjonction de la prescription d'un traitement antipsychotique adapté, de l'adhérence à ce traitement, et de la mise en route d'une prise en charge dans un programme spécialisé. En conclusion, nous relevions que cette grande variété dans les définitions appliquées pour l'évaluation de la DUP avait probablement contribué à l'aspect contradictoire des résultats des études de son impact sur l'évolution des psychoses et qu'il était donc temps de proposer une définition de consensus. La deuxième étude a été conduite dans le cadre d'un suivi de cohorte mis en place dans le programme de Traitement et Intervention Précoce dans les troubles Psychotiques (TIPP) établi dans le Département de Psychiatrie du CHUV à Lausanne depuis 2004. Les objectifs de cette seconde étude étaient au nombre de trois: (1) Exploration des variations de la DUP en fonction de l'application de trois principales définitions de fin de DUP (E-DUP) identifiées dans la littérature ; (2) Evaluation de la proportion de patients remplissant au moins une fois au cours des 18 mois de traitement la définition de E-DUP la plus compatible avec les directives de traitement proposées par l'International Early Psychosis Association (patient est à la fois engagé dans le traitement et se montre compliant à la médication, E-DUP-3); (3) Enfin, identification desfacteurs qui caractérisent les patients qui ne remplissent jamais les critères de cette dernière définition. L'exploration de différentes durées de DUP en utilisant les trois définitions d'E-DUP a donné les résultats suivants : La DUP1 médiane (2.2 mois) était significativement plus courte que la DUP2 (7.4 mois), et la DUP3 (13.6 mois) était significativement la plus longue des trois. De plus, 19.7% des patients n'avaient jamais rempli les critères de E-DUP-3 ; on peut donc considérer que près de 20% des patients traités dans ce programme spécialisé ne recevaient pas un traitement adéquat selon les directives intrernationales actuellement reconnues. Sur la base de ces chiffres, il apparaît clairement que, dans les études de l'impact de la DUP sur l'évolution de la psychose débutante, bon nombre des patients pour lesquels on considère que la DUP est terminée ne sont en fait pas adéquatement traités. Il est en conséquence très probable que ceci ait faussé les résultats de ces études, et qu'une définition plus restrictive permettrait de répondre de manière plus précise à cette question. Les patients qui ne remplissaient pas les critères E-DUP3 au cours des 18 premiers mois de traitement étaient caractérisés par un moins bon niveau de fonctionnement au cours de leur vie (« lower lifetime SOFAS » ; p=0.017) et ils étaient plus susceptibles de consommer du cannabis à l'entrée du programme ???? (?2 (1, n=49)=4.241, p=0.039). Pour ceux qui avaient rempli les critères E-DUP-3 au cours des 18 mois, une longue DUP3 était associée avec un jeune âge au début des symptômes psychotiques (rs =-0.573, p<0.001), et avec un faible niveau de fonctionnement pré-morbide (score de PAS élevés (rs =0.373, p=0.001), niveau maximal au cours de la vie bas pour le GAF(rs =-0.367, p<0.001) et pour le SOFAS (rs =-0.314, p=0.003)). En conclusion, ce travail a permis de mettre en évidence une grande variabilité dans la définition de la fin de la DUP parmi les études publiées jusque à ce jour, et l'impact important que le choix d'une ou l'autre de ces définitions peut avoir sur l'estimation de la DUP. De plus, nous avons observé que malgré la mise en place d'un programme spécialisé, près de 20% des patients ne remplissent pas les critères d'exposition à un traitement adéquat au cours des 18 premiers mois de prise en charge. Il est donc probable que l'estimation de l'impact de la DUP ait été faussé par cette variabilité, et il semble important que la communauté scientifique s'accorde sur une définition plus rigoureuse de cette variable. Enfin, certaines caractéristiques permettent d'identifier les patients qui sont à risque de ne pas remplir les critères de traitement adéquat a cours des 18 premiers mois de prise en charge ; il est possible qu'une identification précoce de ceux-ci permette la mise en place de stratégies mieux adaptées pour les aider à s'engager dans les soins. Le futur développement de ce travail sera d'évaluer l'impact de la DUP sur l'évolution des patients au cours des 36 mois de traitement proposés dans le programme TIPP, en appliquant les divers critères E-DUP, afin de voir si notre hypothèse que la variation des définitions a effectivement faussé les résultats de telles études. Nous devons pour cela attendre qu'un nombre suffisant de patients ait complété les 36 mois de traitement, de manière à avoir une puissance statistique suffisante pour répondre clairement à cette question.

OBJECTIVE: This research explored medical students' use and perception of technical language in a practical training setting to enhance skills in breaking bad news in oncology. METHODS: Terms potentially confusing to laypeople were selected from 108 videotaped interviews conducted in an undergraduate Communication Skills Training. A subset of these terms was included in a questionnaire completed by students (N=111) with the aim of gaining insight into their perceptions of different speech registers and of patient understanding. Excerpts of interviews were analyzed qualitatively to investigate students' communication strategies with respect to these technical terms. RESULTS: Fewer than half of the terms were clarified. Students checked for simulated patients' understanding of the terms palliative and metastasis/to metastasize in 22-23% of the interviews. The term ambulatory was spontaneously explained in 75% of the interviews, hepatic and metastasis/to metastasize in 22-24%. Most provided explanations were in plain language; metastasis/to metastasize and ganglion/ganglionic were among terms most frequently explained in technical language. CONCLUSION: A significant number of terms potentially unfamiliar and confusing to patients remained unclarified in training interviews conducted by senior medical students, even when they perceived the terms as technical. PRACTICE IMPLICATIONS: This exploration may offer important insights for improving future physicians' skills.

OBJECTIVES: To evaluate the socio-demographic as well as the health and psychiatric profiles of adolescents hospitalised for suicide attempt or overwhelming suicide ideation and to assess repetition of suicide attempt over a period of 18 months. PATIENTS AND METHODS: Between April 2000 and September 2001, all patients aged 16 to 21 years admitted to the University Hospitals of Geneva and Lausanne for suicide attempt or ideation were included in the study. At this time (T0) semi-structured face to face interviews were conducted to identify socio-demographic data, mental health and antecedents regarding suicidal conducts. Current psychiatric status was assessed with the MINI (Mini International Neuropsychiatric Instrument). At T1 and T2, reassessments included psychiatric status (MINI) as well as lifestyles, socio-professional situation and suicidal behaviours. RESULTS: At T0, 269 subjects met the study criteria, among whom 83 subjects (56 girls and 27 boys) left the hospital too quickly to be involved or refused to participate in the study (final sample at T0: 149 girls; 37 boys). The participation rate at T1 and T2 was respectively 66% and 62% of the original sample. The percentage of adolescents meeting the criteria for psychiatric diagnoses (91%) was high: affective disorder (78%); anxiety disorder (64%); substance use disorder (39%); eating disorder (9%); psychotic disorder (11%); antisocial personality (7%) with most subjects (85%) having more than one disorder. Around 90% of the subjects interviewed at T1, and/or T2, had received follow-up care after their hospitalisation, either by a primary care physician or a psychotherapist or both. Two subjects died of violent death and 18% made a further suicide attempt. CONCLUSION: Most adolescents hospitalised for suicidal episodes suffer from psychiatric problems which should be addressed by a careful psychiatric assessment, followed up if needed by a structured after care plan.

Background Most research has focused on mothers¿ experiences of perinatal loss itself or on the subsequent pregnancy, whereas little attention has been paid to both parents¿ experiences of having a child following late perinatal loss and the experience of parenting this child. The current study therefore explored mothers¿ and fathers' experiences of becoming a parent to a child born after a recent stillbirth, covering the period of the second pregnancy and up to two years after the birth of the next baby.MethodIn depth interviews were conducted with 7 couples (14 participants). Couples were eligible if they previously had a stillbirth (after 24 weeks of gestation) and subsequently had another child (their first live baby) who was now under the age of 2 years. Couples who had more than one child after experiencing a stillbirth and those who were not fluent in English were excluded. Qualitative analysis of the interview data was conducted using Interpretive Phenomenological Analysis.ResultsFive superordinate themes emerged from the data: Living with uncertainty; Coping with uncertainty; Relationship with the next child; The continuing grief process; Identity as a parent. Overall, fathers' experiences were similar to those of mothers', including high levels of anxiety and guilt during the subsequent pregnancy and after the child was born. Coping strategies to address these were identified. Differences between mothers and fathers regarding the grief process during the subsequent pregnancy and after their second child was born were identified. Despite difficulties with bonding during pregnancy and at the time when the baby was born, parents' perceptions of their relationship with their subsequent child were positive.ConclusionsFindings highlight the importance of tailoring support systems not only according to mothers' but also to fathers' needs. Parents¿, and particularly fathers', reported lack of opportunities for grieving as well as the high level of anxiety of both parents about their baby's wellbeing during pregnancy and after birth implies a need for structured support. Difficulties experienced in bonding with the subsequent child during pregnancy and once the child is born need to be normalised.

Teaching community medicine represents a significant challenge for medical schools, which tend to struggle to promote interest in the issue among students. In 2009, the Lausanne medical school introduced a "community immersion" module specifically designed to address the issue. The new module requires students working in small groups under the supervision of a tutor to investigate a health question of their choice. The investigation involves conducting interviews with stakeholders (health professionals, patients, politicians, etc.), carrying out a survey, and presenting the results of the investigation in a "congress". An external evaluation showed that the objectives of the initiative had been largely achieved, with an increase of interest in community medicine for over 50% of students (based on a total cohort of 150 students) and a high level of satisfaction for over 90% of students and tutors. This paper presents the results of the initiative and its use for promoting community-oriented medicine.

BACKGROUND AND OBJECTIVE: Patient-specific quality of life indices show great potential, but certain conceptual and methodological concerns have yet to be fully addressed. The present study reviewed nine patient-specific instruments used in musculoskeletal disorders: the Canadian Occupational Performance Measure (COPM), Juvenile Arthritis Quality of life Questionnaire (JAQQ), McMaster-Toronto Arthritis questionnaire (MACTAR), Measure Yourself Medical Outcome Profile (MYMOP), Patient-Specific Index (PASI) for total hip arthroplasty, Problem Elicitation Technique (PET), Patient Generated Index (PGI) of quality of life, Patient-Specific Functional Scale (PSFS), and Schedule for the Evaluation of Individual Quality of Life (SEIQoL). STUDY DESIGN AND SETTING: Each tool was evaluated for purpose, content validity, face validity, feasibility, psychometric properties, and responsiveness. RESULTS: This critical appraisal revealed important differences in terms of the concept underlying these indices, the domains covered, the item-generation techniques and the scoring (response scale, methods) in each scale. The nine indices would generate different responses and likely scores for the same patient, despite the fact that they all include patient-generated items. CONCLUSION: Although the value of these indices in treatment planning and monitoring at an individual level is strong, more studies are needed to improve our understanding of how to interpret the numeric scores of patient-specific indices at both an individual and a group level.

Rheumatoid arthritis is the only secondary cause of osteoporosis that is considered independent of bone density in the FRAX(®) algorithm. Although input for rheumatoid arthritis in FRAX(®) is a dichotomous variable, intuitively, one would expect that more severe or active disease would be associated with a greater risk for fracture. We reviewed the literature to determine if specific disease parameters or medication use could be used to better characterize fracture risk in individuals with rheumatoid arthritis. Although many studies document a correlation between various parameters of disease activity or severity and decreased bone density, fewer have associated these variables with fracture risk. We reviewed these studies in detail and concluded that disability measures such as HAQ (Health Assessment Questionnaire) and functional class do correlate with clinical fractures but not morphometric vertebral fractures. One large study found a strong correlation with duration of disease and fracture risk but additional studies are needed to confirm this. There was little evidence to correlate other measures of disease such as DAS (disease activity score), VAS (visual analogue scale), acute phase reactants, use of non-glucocorticoid medications and increased fracture risk. We concluded that FRAX(®) calculations may underestimate fracture probability in patients with impaired functional status from rheumatoid arthritis but that this could not be quantified at this time. At this time, other disease measures cannot be used for fracture prediction. However only a few, mostly small studies addressed other disease parameters and further research is needed. Additional questions for future research are suggested.

A new device for the analyses of nurses' satisfaction has been developed and validated on two types of general and intensive treatments at the University Hospital in Vaudois, Switzerland. A questionnaire has been elaborated for identifying the variables linked with characteristics of the nurse's work, as well as personal variables of the employer which could have an influence on the level of satisfaction. In identifying the sources of satisfaction and dissatisfaction, it has been possible to propose recommendations and corrective measures in order to improve the level of global satisfaction of the nursing team.

The Zeman Barn (86-00028) is an early twentieth-century example of a gothic roofed barn and is part of the Zeman Farmstead located along U.S. Highway 30 in Otter Creek Township (Township 38N, Range 14W), Tama County, Iowa (Figures 1 and 2). The farmstead was initially evaluated in a reconnaissance architectural survey conducted in 1998 by The Louis Berger Group, Inc (Berger). An intensive architectural survey of the property by Berger’s Principal Architectural Historian, Martha H. Bowers, evaluated the farmstead as not being eligible for listing in the National Register of Historic Places (National Register) but noted that the barn appears to be eligible for listing in the National Register under Criterion C (Bowers 1998). At the request of the Iowa Department of Transportation, Berger completed the recordation project to provide a documentary record of the Zeman Barn in accordance with the guidelines set forth by the Iowa State Historic Preservation Office regarding historic property studies for barns. Background research for this project was conducted in September 2008 and April 2009. The property was inspected and photographed in May 2008. Information on the property was gathered through background research, interviews with Zeman family members, field investigation, and photo documentation. Historical maps of the project area were used to collect data necessary for developing regional and local historic contexts. The research for this report was conducted at the Tama County Courthouse and the Tama County Historical Museum Genealogical Library, both in Toledo. Much of the background research for the project was conducted by Camilla Deiber and Michael Dulle. Ms. Deiber also prepared the photographic documentation, plan drawings, and the graphics used in this report. Mr. Roger L. Ciuffo conducted interviews with Zeman family members and wrote this report.

PURPOSE: To better define the long-term prognosis in patients with a vasculopathic sixth nerve palsy (6NP), specifically addressing the degree of recovery and incidence of recurrent similar episodes. DESIGN: Observational case series. METHODS: Retrospective chart review. SETTING: An outpatient neuroophthalmic practice. STUDY POPULATION: Patients with one or more vascular risk factors and an acute, isolated 6NP that spontaneously recovered. OBSERVATION PROCEDURE: Information regarding resolution of the 6NP, subsequent vascular events and recurrent ocular motor nerve palsy was obtained from chart review of follow-up clinic visits, mailed questionnaires and telephone interviews. The duration of follow-up ranged from 2 to 13 years. MAIN OUTCOME MEASURES: Resolution of 6NP (complete or incomplete) and incidence of recurrent ocular motor nerve palsy. RESULTS: Fifty-nine patients were identified with a mean age of 65.3 years +/- 11.6 (range 34-90 years). Fifty-one patients (86%) experienced complete resolution of their first episode of vasculopathic 6NP and eight patients (14%) had incomplete resolution. A subsequent episode of ocular motor mononeuropathy occurred in 18 of 59 (31%) patients. The number of recurrences ranged from one (in 14 patients) to four (in one patient). There was no association between any risk factor and recurrence of ocular motor nerve palsy. Similarly, incomplete resolution of the vasculopathic 6NP was not associated with any risk factor. CONCLUSIONS: Patients with a vasculopathic 6NP usually have complete resolution of their ophthalmoplegia, but nearly one third of patients in our study later experienced at least one episode of recurrent vasculopathic ocular motor nerve palsy.

When deciding to resort to a PPP contract for the provision of a local public service, local governments have to consider the demand risk allocation between the contracting parties. In this article, I investigate the effects of demand risk allocation on the accountability of procuring authorities regarding consumers changing demand, as well as on the cost-reducing effort incentives of the private public-service provider. I show that contracts in which the private provider bears demand risk motivate more the public authority from responding to customer needs. This is due to the fact that consumers are empowered when the private provider bears demand risk, that is, they have the possibility to oust the private provider in case of non-satisfaction with the service provision, which provides procuring authorities with more credibility in side-trading and then more incentives to be responsive. As a consequence, I show that there is a lower matching with consumers' preferences over time when demand risk is on the public authority rather than on the private provider, and this is corroborated in the light of two famous case studies. However, contracts in which the private provider does not bear demand risk motivate more the private provider from investing in cost-reducing efforts. I highlight then a tradeoff in the allocation of demand risk between productive and allocative efficiency. The striking policy implication of this article for local governments would be that the current trend towards a greater resort to contracts where private providers bear little or no demand risk may not be optimal. Local governments should impose demand risk on private providers within PPP contracts when they expect that consumers' preferences over the service provision will change over time.
