La medicina occidental és la imperant a tot el planeta. Malgrat tot, a la Xina la medicina tradicional hi té un pes important. La convivència entre totes dues visions de la salut i la malaltia -l'occidental i l'oriental- no és senzilla. En aquest treball es fa un recorregut pel sistema de salut xinès per a fer una aproximació a la situació de la medicina xinesa a la Xina contemporània.

The news in addiction medicine in 2013 are presented according to the new version of the DSM (DSM-5); new data on cannabinoid, highlight hypotheses on self-medication; a current status about treatment of the addiction via the internet is shown; and new therapeutic perspectives emerge from the knowledge on traumatic antecedents in addictive populations.

Three Plumbago spp have been tested for mosquito larvicidal activity. The crude extracts exhibiting the highest larvicidal activity against Anopheles gambiae were hexane (LC50 = 6.4 μg/mL) and chloroform (LC50 = 6.7 μg/mL) extracts from Plumbago zeylanica Linn, chloroform (LC50 = 6.7 ug/mL) extract from Plumbago stenophylla Bull and ethyl acetate (LC50 = 4.1 μg/mL) extract from Plumbago dawei Rolfe. These LC50 values were within 95% confidence limits. 5-hydroxy-2-methyl-1,4-naphthoquinone (plumbagin) 1 (LC50 = 1.9 μg/mL) and β-sitosterol 2 were characterised from ethyl acetate extract of root bark of P. dawei, a native medicinal plant growing in Kenya, based on spectral analysis and comparisons with data in literature.

BACKGROUND. Maternal genetics and feeding before and during pregnancy, different maternal metabolic pathologies, as well as nutrient intakes of newborns in their first months of life may be involved in the obesity aetiology and its long-term consequences. The possible role of these and others factors, the mechanisms and the effects on the metabolism, and the development of this disease need further research. OBJECTIVE. To acquire more knowledge about foetal adipose tissue development and the influence of genetic, dietetic and environmental factors on the risk to suffer from obesity. METHODOLOGY. Four study groups have been established with 30 pregnant women in each one: 1) control group; 2) mothers with glucose intolerance/gestational diabetes; 3) women with low weight gain during pregnancy, and 4) women with overweight/obesity at the beginning of the pregnancy. The magnitudes to be studied are: 1) dietary intake; 2) life-style habits; 3) physical activity; 4) anthropometry and body composition; 5) haematological study; 6) biochemical study (lipid and metabolic biomarkers); 7) immune function profile related to nutritional status; 8) psychological profile; 9) genetic biomarkers, and 10) microbiological markers; all of them in relation to the development of the foetal adipose tissue in the first stages of life and the risk of suffering from obesity in the future. CONCLUSION. This project, coordinated by the Department of Paediatrics of the School of Medicine in the University of Granada, and with the collaboration of well-known and expert research groups, tries to contribute to the knowledge about the obesity aetiology in infancy and its subsequent development in later periods of life.

També ofereix l’edició del text de tres d’aquests plecs solts, impresos a la segona meitat del segle XVI, inèdits modernament i poc coneguts. La tesis titulada “La práctica médica en el ejercicio físico en la Barcelona de principios del siglo XX” trata de examinar cómo se relacionaba la práctica médica con los ejercicios físicos como la gimnasia y el deporte, y saber si realmente se llegó a formar una especialidad médica llamada medicina del deporte en la Barcelona de los principios del siglo XX. La primera parte se titula “Perspectiva histórica del ejercicio físico y la medicina”, y trata de la relación general entre el ejercicio físico y la medicina desde el punto de macro-vista histórico y regional. La segunda parte se titula “Hacia la formación de la especialización de la medicina del deporte en Barcelona”, y analiza la práctica médica relacionada con el deporte en una ciudad concreta durante un tiempo determinado, es decir, desde un punto de micro-vista. En el fondo de la creación de las especialidades médicas a partir de mediados del siglo XIX existe la formación de una sociedad moderna, simbolizada por el sistema administrativo de la democracia nacional y el sistema económico del capitalismo. Considerando los elementos comprendidos dentro del sistema de la modernidad que provocaron la especialización médica tales como la urbanización, la industrialización, el aumento de la población, el interés estatal, el desarrollo de la prensa, el elevado interés público, el progreso intelectual y tecnológico de la ciencia y la medicina, la creación de un sistema de atención de la salud, la formación y participación en las instituciones internacionales, el cambio de la identidad de los médicos, y la reposición de la medicina holística, investigo integralmente la formación de la especialidad de la medicina del deporte en Catalunya, y lo caracterizo por las iniciativas privadas emprendidas por un sector experto y profano, y la ausencia de un interés estatal suficiente en la dicha especialidad.

The genetic characterization of unbalanced mixed stains remains an important area where improvement is imperative. In fact, with current methods for DNA analysis (Polymerase Chain Reaction with the SGM Plus™ multiplex kit), it is generally not possible to obtain a conventional autosomal DNA profile of the minor contributor if the ratio between the two contributors in a mixture is smaller than 1:10. This is a consequence of the fact that the major contributor's profile 'masks' that of the minor contributor. Besides known remedies to this problem, such as Y-STR analysis, a new compound genetic marker that consists of a Deletion/Insertion Polymorphism (DIP), linked to a Short Tandem Repeat (STR) polymorphism, has recently been developed and proposed elsewhere in literature [1]. The present paper reports on the derivation of an approach for the probabilistic evaluation of DIP-STR profiling results obtained from unbalanced DNA mixtures. The procedure is based on object-oriented Bayesian networks (OOBNs) and uses the likelihood ratio as an expression of the probative value. OOBNs are retained in this paper because they allow one to provide a clear description of the genotypic configuration observed for the mixed stain as well as for the various potential contributors (e.g., victim and suspect). These models also allow one to depict the assumed relevance relationships and perform the necessary probabilistic computations.

Sponsored by the Health Administrations of nine cantons, this study was conducted by the University Institute of Social and Preventive Medicine in Lausanne in order to assess how DRGs could be used within the Swiss context. A data base mainly provided by the Swiss VESKA statistics was used. The first step provided the transformation of Swiss diagnostic and intervention codes into US codes, allowing direct use of the Yale Grouper for DRG. The second step showed that the overall performance of DRG in terms of variability reduction of the length of stay was similar to the one observed in US; there are, however, problems when the homogeneity of medicotechnical procedures for DRG is considered. The third steps showed how DRG could be used as an account unit in hospital, and how costs per DRG could be estimated. Other examples of applications of DRG were examined, for example comparison of Casemix or length of stay between hospitals.

OBJECTIVE: Postmortem investigations are becoming more and more sophisticated. CT and MRI are already being used in pathology and forensic medicine. In this context, the impact of postmortem angiography increases because of the rapid evaluation of organ-specific vascular patterns, vascular alteration under pathologic and physiologic conditions, and tissue changes induced by artificial and unnatural causes. CONCLUSION: In this article, the advantages and disadvantages of former and current techniques and contrast agents are reviewed.

Pathogen inactivation of blood products represents a global and major paradigm shift in transfusion medicine. In the next near future, it is likely that most blood products will be inactivated by various physicochemical approaches. The concept of blood safety will be challenged as well as transfusion medicine practice, notably for donor selection or biological qualification. In this context, it seems mandatory to develop analytical economic approaches by assessing costs-benefits ratio of blood transfusion as well as to set up cohorts of patients based on hemovigilance networks allowing rigorous scientific analysis of the benefits and the risks of blood transfusion at short- and long-term.

Background: Systematic approaches for identifying proteins involved in different types of cancer are needed. Experimental techniques such as microarrays are being used to characterize cancer, but validating their results can be a laborious task. Computational approaches are used to prioritize between genes putatively involved in cancer, usually based on further analyzing experimental data. Results: We implemented a systematic method using the PIANA software that predicts cancer involvement of genes by integrating heterogeneous datasets. Specifically, we produced lists of genes likely to be involved in cancer by relying on: (i) protein-protein interactions; (ii) differential expression data; and (iii) structural and functional properties of cancer genes. The integrative approach that combines multiple sources of data obtained positive predictive values ranging from 23% (on a list of 811 genes) to 73% (on a list of 22 genes), outperforming the use of any of the data sources alone. We analyze a list of 20 cancer gene predictions, finding that most of them have been recently linked to cancer in literature. Conclusion: Our approach to identifying and prioritizing candidate cancer genes can be used to produce lists of genes likely to be involved in cancer. Our results suggest that differential expression studies yielding high numbers of candidate cancer genes can be filtered using protein interaction networks.

OBJECTIVE: The aim of this study was to determine whether V˙O(2) kinetics and specifically, the time constant of transitions from rest to heavy (τ(p)H) and severe (τ(p)S) exercise intensities, are related to middle distance swimming performance. DESIGN: Fourteen highly trained male swimmers (mean ± SD: 20.5 ± 3.0 yr; 75.4 ± 12.4 kg; 1.80 ± 0.07 m) performed an discontinuous incremental test, as well as square wave transitions for heavy and severe swimming intensities, to determine V˙O(2) kinetics parameters using two exponential functions. METHODS: All the tests involved front-crawl swimming with breath-by-breath analysis using the Aquatrainer swimming snorkel. Endurance performance was recorded as the time taken to complete a 400 m freestyle swim within an official competition (T400), one month from the date of the other tests. RESULTS: T400 (Mean ± SD) (251.4 ± 12.4 s) was significantly correlated with τ(p)H (15.8 ± 4.8s; r=0.62; p=0.02) and τ(p)S (15.8 ± 4.7s; r=0.61; p=0.02). The best single predictor of 400 m freestyle time, out of the variables that were assessed, was the velocity at V˙O(2max)vV˙O(2max), which accounted for 80% of the variation in performance between swimmers. However, τ(p)H and V˙O(2max) were also found to influence the prediction of T400 when they were included in a regression model that involved respiratory parameters only. CONCLUSIONS: Faster kinetics during the primary phase of the V˙O(2) response is associated with better performance during middle-distance swimming. However, vV˙O(2max) appears to be a better predictor of T400.

What's new in addiction medicine in 2012? The news are presented according three axes: first, in the field of neuroscience, the process of extinction of addiction memories. Then in the clinical field, a reflexion is reported on how to treat addiction in psychiatric hospitals. At last, in the area of teaching, an e-learning development with a virtual patient shows a great interest in addiction psychiatry.
