Örebro Studies in Sport Sciences 6, 141 pp. Physical education (PE) at school may play an important role in the process of becoming physically active in the adolescence and in developing a physically active lifestyle. The opportunities for teachers to provide positive physical activity experiences to the student population extend regularly over the school terms. For some students, PE can be the only opportunity they have to be physically active. Therefore it is important that the students become motivated to practice physical activity and know the purpose with physical activity. The overall purpose of this dissertation was to investigate the possibilities to motivate youth, 12-16 years old, to practice physical activity and hopefully to stimulate them to continue to be physically active in young adulthood. The sample consisted of 122 Swedish compulsory school students (12-16 years old), 51 girls and 71 boys, who were either physically inactive, physically active but not formal members of sport clubs, or physically active and members of a sport club. They used a self-monitoring instrument to describe their physical activity and we compared their self-reported physical activity with their actual VO2 capacity and physical status. The self-monitoring instrument was further validated against an activity monitor, RT3. The intervention lasted one and a half years, and four to five years later a follow-up study was done. Using a quasi-experimental design, the three groups of participants were compared with youths who did not use the self-monitoring instrument. The main findings were that the associations between the accelerometer counts and the activities the students recorded in the self-monitoring instrument were high. The participants  in Group 1 (physically inactive) continued exercising nearly to the same extent as during the intervention while youths in a matched control group did not develop regular physical activity habits. Concerning Group 2 (exercisers), participants in the intervention group were more physically active both during the intervention and at follow-up, compared with a matched control group of exercisers who did not use the self-monitoring instrument. In Group 3 (sport team members), there was no difference between the intervention group and a matched control group after the intervention or at follow-up.  The conclusion of these main findings was that when someone motivates students to continue being physically active and to change their physical activity patterns in a positive direction, the self-monitoring instrument can be a door-opener for youths who are physically inactive or regularly active outside sports clubs.

The re-unification of a family of nations: usages of the family metaphor in the EU This article analyses usages of the family metaphor in the EU. It starts up with a scrutiny of feminist theories of the nation-as-family metaphor. Introducing the concept of domopolitics, the author infers that the family, on the one hand, connotes to feelings of security and homelyness and, on the other hand, fears of the well-known, of immanent threats to in-group cohesion. The significance of the family metaphor in the EU rhetoric connects to a renewed emphasis on distinct values, principles and norms that balance the otherwise technocratic image of the EU. He further applies the nation-as-family metaphor to contemporary EU rhetoric. In the analysis, he infers that all three dimensions of the metaphor (ethnic, civil and hierarchic) are manifest in the EU political language making its use an enterprise that strives at moving beyond, but not completely away from the nation-state paradigm.

Mobile computing has enabled users to seamlessly access databases even when they are on the move. Mobile computing environments require data management approaches that are able to provide complete and highly available access to shared data at any time from any where. In this paper, we propose a novel replicated data protocol for achieving such goal. The proposed scheme replicates data synchronously over stationary sites based on three dimensional grid structure while objects in mobile sites are asynchronously replicated based on commonly visited sites for each user. This combination allows the proposed protocol to operate with less than full connectivity, to easily adapt to changes in group membership and not require all sites to agree to update data objects at any given time, thus giving the technique flexibility in mobile environments. The proposed replication technique is compared with a baseline replication technique and shown to exhibit high availability, fault tolerance and minimal access times of the data and services, which are very important in an environment with low-quality communication links.

Recent research indicates that 3,4-methylene-dioxymethamphetamine (MDMA), also known as ‘ecstasy’, is becoming increasingly popular as an illicit drug among young people. This study investigated risk and harm reduction practices among recreational ecstasy users. A semi-structured interview with 40 participants was designed to investigate how ecstasy users identify and manage the harms associated with their drug use, and the underlying decision-making process. Overall, the participants identified both positive and negative effects. The reported positive effects predominantly centred around enhanced psychological, physiological and social experiences. However, there were a number of factors that contributed to regulating ecstasy use. These included specific in-group and out-group practices executed within the peer group, preventative harm-reducing practices, shared decision making, and shared responsibility for harm prevention. Recommendations for promoting harm reduction strategies and suggestions for future research are discussed.

Objective: To examine the relative effectiveness of ice therapy and/or pulsed electromagnetic field in reducing pain and swelling after the immobilization period following a distal radius fracture.Methods: A total of 83 subjects were randomly allocated to receive 30 minutes of either ice plus pulsed electromagnetic field (group A); ice plus sham pulsed electromagnetic field (group B); pulsed electromagnetic field alone (group C), or sham pulsed electromagnetic field treatment for 5 consecutive days (group D). All subjects received a standard home exercise programme. A visual analogue scale was used for recording pain; volumetric displacement for measuring the swelling of the forearm; and a hand-held goniometer for measuring the range of wrist motions before treatment on days 1, 3 and 5.Results: At day 5, a significantly greater cumulative reduction in the visual analogue scores as well as ulnar deviation range of motion was found in group A than the other 3 groups. For volumetric measurement and pronation, participants in group A performed better than subjects in group D but not those in group B.Conclusion: The addition of pulsed electromagnetic field to ice therapy produces better overall treatment outcomes than ice alone, or pulsed electromagnetic field alone in pain reduction and range of joint motion in ulnar deviation and flexion for a distal radius fracture after an immobilization period of 6 weeks.

There is a growing need to develop an understanding of the positive pathways that strengthen the relationships of Australian couples and families. The couples and families in contemporary society are faced with many challenges and pressures that can mitigate against maintaining satisfying and enduring couple and family relationships. For example, increasing need for dual income families, longer working hours and demographic shifts that see older people living longer and children staying at home longer mean that couples are often required to provide familial care across two generations – frail ageing parents and children. These highlight just some of the pressures faced which can lead to couples not taking the time to cultivate their relationships. The aim of the workshop was to deal with many of these issues by linking research, policy and practice in ways that would help families meet such challenges. The two-day workshop, held 1-2 November 2008 in Melbourne, brought together leading and emerging Australian and international relationship researchers with practitioners, educators, policy makers and service-delivery organisations. Alongside the Academy, the event was sponsored by Deakin University, the University of Queensland, the Department of Families, Housing and Community Services and Indigenous Affairs (FaCHSIA), the Attorney General's Department (AG Department), the Australian Institute of Family Studies (AIFS) and Lifeworks. The workshop comprised seven sessions and two break-out discussions. A total of 28 participants attended the workshop. Of these participants, there were 19 speakers and 7 delegates from the various sponsor organisations that took part in group discussions.

The use of rigid materials with high oxygen permeability (Dk) is on the increase, their major benefit being the reduction of hypoxia. However, there is a reluctance to use these materials possibly due to increased surface scratching, reduced wettability, increased deposition, reduced life expectancy and parameter instability. Considering parameter stability, various studies have demonstrated contradictory results or used high Dk materials based on the silicone acrylate polymer. This study was designed to investigate whether the parameters of a high Dk rigid fluorocarbon contact lens material changed during daily wear and extended wear schedules. Thirty five subjects, divided into group groups, Group I wore the lens on a daily wear basis, whereas those in Group II wore the lens on a monthly extended wear basis. The parameters and integrity of the lenses were monitored in both groups every 3 months. For lens integrity a statistically significant increase in surface scratching was demonstrated for the lenses worn by the subjects of both groups over the time of the study (Group I, F=7.990, P <0.0001 [ANCOVA]; Group II, F=6.241, P=0.003 [ANCOVA]). The only parameter to show a statistically significant variation over the study period was that of centre thickness for the lenses worn by the subjects in Group I (F=3.976, P=0.0063 [ANCOVA]), with a mean reduction in centre thickness of 0.022 mm at the 12 month visit.

No change was noted for either group or between groups for the other parameters measured. This study demonstrates that the parameters of rigid contact lenses manufactured from high Dk fluorocarbons are stable with only a non-clinically significant reduction in centre thickness for the contact lenses worn by the subjects in Group I.

The early years of a child’s life are especially important, and the quality of those experiences set the scene for the future. Children’s first and most enduring bond is within their own family. However, a high proportion of young children in the Australia spend time as the responsibility of non-family adults within the many different types of early years settings.

The NQS outlines through Quality Area 5: Relationships with children, the need for educators to develop and maintain respectful and equitable relationships with each child. These relationships are brought about by educators and children forming trusting and reciprocal bonds where each child feels secure and included.

Consumer directed care (CDC) is increasing in community aged care. However, limited information is available to successfully transition social workers and other case managers to their new role. This paper reports on a case study of six senior case managers who supervised staff in three Australian community-aged care agencies as they transitioned from agency directed care to consumer directed care. A change management framework was used to analyse the qualitative data collected in 12 semistructured interviews. A key finding is that changes in values, attitudes, and organisational culture are needed before staff can fully implement CDC principles of service user self-determination, empowerment, and choice. Process changes needed to assist staff transition to CDC are: using a change management strategy that maximises certainty; monitoring and responding to feelings of anxiety through ongoing consultations; and providing ongoing education and support in group sessions.

Collectivism is one of the well-researched dimensions of culture that pertains to an individual’s relationship to an in-group. Organisational climate, on the other hand, is predominantly defined as the shared perceptions of employees about their working environment. In spite of the long tradition of both constructs in the literature, the conceptual relationship between collectivism and climate has oftentimes been neglected. This paper explores this relationship by presenting (1) the conceptual overlap between culture and climate; (2) the congruence between collectivism and climate in terms of levels of conceptualisation and analysis; (3) the apparent influence of collectivism on organisational processes and practices that have been the domain of climate studies; and (4) the apparent influence of collectivism on climate outcomes. This paper also offers some recommendations to guide future studies including suggestions to have more empirical investigation to strongly establish the relationship between collectivism and climate, to investigate facets of climate simultaneously, to extend the link between climate and other work outcomes, to engage in multi-level research, and to explore how collectivism influences climate formation and change.

Consensus measures can be useful in group decision making problems both to guide users toward more reasonable judgments and to give an overall indication of the support for the final decision. The level of consensus between decision makers can be measured in contexts where preferences over alternatives are expressed either as evaluations or scores, pairwise preferences, and weak orders, however these different representations often call for different approaches to consensus measurements. In this paper, we look at the distance metrics used to construct consensus measures in each of these settings and how consistent these are for preference profiles when they are converted from one representation to another. We develop some methods for consistent approaches across decision making settings and provide an example to help investigate differences between some of the commonly used distances.

In group decision-making problems it is common to elicit preferences from human experts in the form of pairwise preference relations. When this is extended to a fuzzy setting, entries in the pairwise preference matrix are interpreted to denote strength of preference, however once logical properties such as consistency and transitivity are enforced, the resulting preference relation requires almost as much information as providing raw scores or a complete order over the alternatives. Here we instead interpret fuzzy degrees of preference to only apply where the preference over two alternatives is genuinely fuzzy and then suggest an aggregation procedure that minimizes a generalized Kemeny distance to the nearest complete or partial order. By focusing on the fuzzy partial order, the method is less affected by differences in the natural scale over which an expert expresses their preference, and can also limit the influence of extreme scores.

O espessamento intimal e a reestenose que ocorrem após os procedimentos de angioplastia transluminal percutânea e/ou o implante de stents representam uma causa freqüente de falência destes procedimentos. O principal achado patológico responsável pela reestenose parece ser a hiperplasia intimal, já que o dispositivo intravascular é resistente ao remodelamento arterial geométrico. O propósito deste estudo é avaliar, através da morfometria digital, o espessamento intimal presente nas regiões da parede arterial imediatamente proximal e distal ao implante de um stent metálico em configuração em “Z “ não recoberto ou recoberto com PTFE. Vinte e cinco suínos de raça mista, com seis a dez semanas de idade, pesando em média 20 kg foram divididos em três grupos. No grupo I, cinco animais foram submetidos à exposição cirúrgica retroperitoneal da aorta abdominal, aortotomia e manipulação com uma bainha introdutora de 12 F. O grupo II incluiu dez animais que foram submetidos ao implante de um stent metálico auto-expansível não recoberto. No grupo III, incluindo também dez animais, foram implantados stents recobertos com PTFE. Após quatro semanas, todos os animais foram sacrificados e o segmento aorto-ilíaco foi removido. Quatro animais foram excluídos do estudo por trombose da aorta (um animal do grupo II e três animais do grupo III). Para a análise morfométrica foram utilizados os testes não paramétricos de Wilcoxon e de Kruskal-Wallis, para as comparações, respectivamente, no mesmo grupo e entre os grupos. Foi adotado o nível de significância de 5% ( p< 0,05). Quando os espécimes da parede arterial, imediatamente proximal e distal aos stents foram comparados, nenhuma diferença estatisticamente significativa foi encontrada entre as áreas luminal, intimal, média ou índice intimal em cada grupo. Na comparação entre os grupos, as áreas intimal, média e o índice intimal não demonstraram variação estatisticamente significativa. Foram identificadas diferenças entre os grupos quanto às áreas luminais proximais (p = 0,036) e distais (p=0,044). Pelo teste de comparações múltiplas para Kruskal-Wallis (Teste de Dunn) identificou-se diferença significativa entre os grupos I e II. Entretanto, quando estas variáveis foram controladas pelo fator peso (relação área luminal/peso) a diferença não foi mais observada. Concluímos que, após quatro semanas, stents recobertos com PTFE induzem um espessamento intimal justa-stent similar ao observado com stents não recobertos ou com a simples manipulação arterial com uma bainha introdutora. Neste modelo experimental suíno, de curto seguimento, o revestimento com PTFE não foi responsável por adicional espessamento intimal.

A artrose do quadril é uma doença articular degenerativa que atinge principalmente idosos, faixa etária que vem gradativamente aumentando nos últimos tempos. Assim, medidas de racionalização do uso de leitos e recursos hospitalares são necessárias para melhor viabilizar os procedimentos cirúrgicos e reduzir gastos para o sistema de saúde, uma vez que quase dois terços destes gastos decorrem do período de internação. Com esse objetivo, foi desenvolvido um protocolo assistencial de artroplastia total de quadril (PAATQ), multidisciplinar, com consultas ambulatoriais e visitas domiciliares programadas para pacientes a serem submetidos à artroplastia total de quadril (ATQ), visando uma melhor abordagem por parte da equipe médica e de enfermagem e uma melhor orientação dos pacientes e familiares quanto ao procedimento cirúrgico. Este estudo transversal (piloto), com controle histórico aninhado a um ensaio clínico randomizado, tem por objetivo avaliar a factibilidade de implantação e a adesão, por parte das equipes médica e de enfermagem, à utilização deste protocolo. Secundariamente, visa determinar o impacto desta rotina no tempo de internação, na independência funcional do paciente e nos eventos clínicos da fase hospitalar, bem como avaliar a factibilidade das visitas domiciliares de enfermagem. Um total de 22 pacientes (9 homens e 13 mulheres) submetidos à ATQ no Hospital de Clínicas de Porto Alegre (Brasil), com média (dp) de idade de 58,86 (16,87), variando de 21 a 86 anos, foram incluídos no estudo. Os pacientes foram divididos em dois grupos  grupo 1 (n=12) e grupo 2 (n=10)  de acordo com aplicação ou não do PAATQ. No grupo 1, as principais comorbidades foram a hipertensão arterial sistêmica (HAS) (dois casos), o diabete mellitus (um caso); e o alcoolismo (um caso).No grupo 2, houve um caso de HAS. O tempo de permanência médio (dp) pós-operatório (DPO) foi de 5,2 e 7,5 dias para os grupos 1 e 2, respectivamente (p=0.0055). A adesão ao protocolo foi de 90% e 100% por parte das equipes médica e de enfermagem, respectivamente. Sete pacientes do grupo 1 conseguiram sentar fora do leito no 2o dia pós-operatório (PO), dois sentaram no 3o e nenhum foi incapaz de sair do leito. Todos deambularam com muletas um dia após sentarem. No grupo 2, todos os pacientes saíram do leito no 4o DPO. Em conclusão, o presente protocolo mostrou-se factível, tendo obtido ótima adesão por parte da equipe e propiciado redução do tempo de internação. O seguimento do programa determinará mais detalhadamente a sua eficácia e a factibilidade das visitas domiciliares.
