Background: Inflammation is associated with heart failure (HF) risk factors and also directly affects myocardial function. However, the association between inflammation and HF risk in older adults has not been adequately evaluated. Methods: The association of baseline serum concentrations of interleukin-6 (IL-6), tumor necrosis factor alpha (TNF- ), and C-reactive protein (CRP) with incident HF was assessed with Cox proportional hazards models among 2610 older persons without prevalent HF enrolled in the Health, Aging, and Body Composition (Health ABC) Study (age, 73.6±2.9 years; 48.3% men; 59.6% white). Results: Median (interquartile range) baseline concentrations of IL-6, TNF- , and CRP were 1.80 (1.23, 2.76) pg/mL, 3.14 (2.41, 4.06) pg/mL, and 1.64 (0.99, 3.04) µg/mL, respectively. On follow-up (median, 9.4 years), 311 participants (11.9%) developed HF. In models controlling for clinical predictors of HF and incident coronary heart disease, doubling of IL-6, TNF- , and CRP concentrations was associated with 34% (95% CI, 18 -52%; P<.001), 33% (95% CI, 9 - 63%; P=.006), and 13% (95% CI, 3-24%; P=.01) increase in HF risk, respectively. In models including all 3 markers, IL-6 and TNF- , but not CRP, remained significant. Findings were similar across sex and race. Post-HF ejection fraction (EF) was available in 239 (76.8%) cases. When only cases with preserved EF were considered (n=105), IL-6 (HR per doubling, 1.57; 95% CI, 1.28 -1.94; P<.001), TNF- (HR per doubling, 1.59; 95% CI, 1.12-2.26; P=.01), and CRP (HR per doubling, 1.23; 95% CI, 1.05-1.44; P=.01) were all associated with HF risk in adjusted models. In contrast, when only cases with reduced EF (n=134) were considered, only IL-6 attained marginal significance in adjusted models (HR per doubling, 1.20; 95% CI, 0.99 -1.46; P=.06). Participants with 2 or 3 markers above median had pronounced HF risk in adjusted models (HR, 1.66; 95% CI, 1.12-2.46; P=.01; and HR, 1.76; 95% CI, 1.16 -2.65; P=.007, respectively). Addition of IL-6 to the clinical Health ABC HF model improved discrimination (C index from 0.717 to 0.734; P=.001) and fit (decreased Bayes information criterion by 17.8; P<.001). Conclusions: Inflammatory markers are associated with HF risk among older adults and may improve HF risk stratification.

La clinique systémique est trop souvent identifiée à la thérapie de couple ou de famille au détriment de la psychothérapie individuelle. Cet article présente les résultats d'un groupe de travail de thérapeutes systémiciens qui ont longuement réfléchi à cette question. Après une revue bibliographique commentée sur le sujet, il propose un certain nombre de points critiques concernant la clinique de la psychothérapie individuelle d'orientation systémique. Systems therapy is too often identified with couple or family therapy to the detriment of individual psychotherapy. This paper presents the results of a group of systems oriented therapists who have reflected on this question in depth. After a review of literature on the subject, this group advances key points concerning the practice of systems oriented individual psychotherapy.

INTRODUCTION Human host immune response following infection with the new variant of A/H1N1 pandemic influenza virus (nvH1N1) is poorly understood. We utilize here systemic cytokine and antibody levels in evaluating differences in early immune response in both mild and severe patients infected with nvH1N1. METHODS We profiled 29 cytokines and chemokines and evaluated the haemagglutination inhibition activity as quantitative and qualitative measurements of host immune responses in serum obtained during the first five days after symptoms onset, in two cohorts of nvH1N1 infected patients. Severe patients required hospitalization (n = 20), due to respiratory insufficiency (10 of them were admitted to the intensive care unit), while mild patients had exclusively flu-like symptoms (n = 15). A group of healthy donors was included as control (n = 15). Differences in levels of mediators between groups were assessed by using the non parametric U-Mann Whitney test. Association between variables was determined by calculating the Spearman correlation coefficient. Viral load was performed in serum by using real-time PCR targeting the neuraminidase gene. RESULTS Increased levels of innate-immunity mediators (IP-10, MCP-1, MIP-1beta), and the absence of anti-nvH1N1 antibodies, characterized the early response to nvH1N1 infection in both hospitalized and mild patients. High systemic levels of type-II interferon (IFN-gamma) and also of a group of mediators involved in the development of T-helper 17 (IL-8, IL-9, IL-17, IL-6) and T-helper 1 (TNF-alpha, IL-15, IL-12p70) responses were exclusively found in hospitalized patients. IL-15, IL-12p70, IL-6 constituted a hallmark of critical illness in our study. A significant inverse association was found between IL-6, IL-8 and PaO2 in critical patients. CONCLUSIONS While infection with the nvH1N1 induces a typical innate response in both mild and severe patients, severe disease with respiratory involvement is characterized by early secretion of Th17 and Th1 cytokines usually associated with cell mediated immunity but also commonly linked to the pathogenesis of autoimmune/inflammatory diseases. The exact role of Th1 and Th17 mediators in the evolution of nvH1N1 mild and severe disease merits further investigation as to the detrimental or beneficial role these cytokines play in severe illness.

This study evaluated two vaccine candidates for their effectiveness in protecting BALB/c mice against Leishmania chagasiinfection. These immunogenic preparations were composed of Leishmania amazonensisor Leishmania braziliensisantigenic extracts in association with saponin adjuvant. Mice were given three subcutaneous doses of one of these vaccine candidates weekly for three weeks and four weeks later challenged with promastigotes of L. chagasiby intravenous injection. We observed that both vaccine candidates induced a significant reduction in the parasite load of the liver, while the L. amazonensisantigenic extract also stimulated a reduction in spleen parasite load. This protection was associated with a suppression of both interleukin (IL)-10 and IL-4 cytokines by spleen cells in response to L. chagasiantigen. No change was detected in the production of IFN-γ. Our data show that these immunogenic preparations reduce the type 2 immune response leading to the control of parasite replication.

INTRODUCTION Genetic variations may influence clinical outcomes in patients with sepsis. The present study was conducted to evaluate the impact on mortality of three polymorphisms after adjusting for confounding variables, and to assess the factors involved in progression of the inflammatory response in septic patients. METHOD The inception cohort study included all Caucasian adults admitted to the hospital with sepsis. Sepsis severity, microbiological information and clinical variables were recorded. Three polymorphisms were identified in all patients by PCR: the tumour necrosis factor (TNF)-alpha 308 promoter polymorphism; the polymorphism in the first intron of the TNF-beta gene; and the IL-10-1082 promoter polymorphism. Patients included in the study were followed up for 90 days after hospital admission. RESULTS A group of 224 patients was enrolled in the present study. We did not find a significant association among any of the three polymorphisms and mortality or worsening inflammatory response. By multivariate logistic regression analysis, only two factors were independently associated with mortality, namely Acute Physiology and Chronic Health Evaluation (APACHE) II score and delayed initiation of adequate antibiotic therapy. In septic shock patients (n = 114), the delay in initiation of adequate antibiotic therapy was the only independent predictor of mortality. Risk factors for impairment in inflammatory response were APACHE II score, positive blood culture and delayed initiation of adequate antibiotic therapy. CONCLUSION This study emphasizes that prompt and adequate antibiotic therapy is the cornerstone of therapy in sepsis. The three polymorphisms evaluated in the present study appear not to influence the outcome of patients admitted to the hospital with sepsis.

La première guerre mondiale a créé des déséquilibres fondamentaux au sein de l'économie mondiale. D'une part, elle a endetté les Etats à un point tel que ces derniers ont été poussés à pratiquer des politiques inflationnistes aux effets sociaux parfois désastreux. D'autre part, elle a détruit le système monétaire international et avec lui le système de régulation des échanges commerciaux. Enfin, elle a provoqué un gonflement de l'appareil productif, ce qui se traduira par des tendances à la surproduction une fois la paix revenue. Bien que la Suisse soit restée neutre durant le conflit, la situation de guerre ne la pas épargnée et les déséquilibres fondamentaux évoqués l'ont aussi touchée de plein fouet à partir de 1919. Fortement intégrée dans l'économie mondiale, que ce soit au niveau des échanges d'hommes, de marchandises et de capitaux, la Suisse a surtout souffert des dérèglements intervenus sur le plan des échanges internationaux. Menant une politique du franc fort, les autorités ont dû faire face aux conséquences négatives de cette stratégie monétaire sur les différentes branches productives de l'économie. Surdimensionnées à l'issue de quatre années de pénurie, marquées par la difficulté d'importer, les branches de l'industrie et de l'agriculture travaillant pour le marché intérieur doivent faire face à une importation massive de produits étrangers. Celle-ci est encore stimulée par la politique de dumping monétaire de certains partenaires commerciaux. Quant à l'industrie d'exportation, elle ne parvient plus à écouler ses produits sur les marchés à monnaie dépréciée. Le chômage grimpe rapidement au cours de l'année 1921, ravivant les tensions sociales exacerbées par les grèves générales de 1918 et 1919. C'est dans ce contexte social explosif que la question de l'assainissement des finances fédérales doit être résolue. Durant le conflit, l'accroissement des dépenses de l'Etat a été couvert, dans une large mesure, par un accroissement de la dette et un recours à la planche à billet. Il s'agit donc de déterminer quels impôts vont fournir les sommes nécessaires au service et à l'amortissement de la dette. Les nouvelles tâches embrassées par la Confédération au cours du conflit provoquent par ailleurs un déficit budgétaire structurel que les autorités veulent combler au plus vite pour ne pas mettre en danger le franc suisse et éviter de tirer les taux de l'argent à la hausse. Pour faire face aux défis commerciaux et financiers générés par la guerre, la Confédération dispose de plusieurs outils lui permettant de mener une politique volontariste. Parmi ceux-ci, la politique douanière occupe une place très importante, puisqu'elle est à la fois la principale source de revenus de la Confédération et un bras de levier efficace pour influencer les flux commerciaux. Certes, de par son importance dans la répartition de la richesse nationale, la politique douanière a toujours été l'objet de conflits politiques homériques. Entre 1880 et 1914, un débat continu a opposé les différents groupes socio-économiques cherchant à défendre des intérêts fiscaux et commerciaux bien compris. Cependant, à l'issue de la Première guerre mondiale, les enjeux du débat prennent un ampleur qualitativement différente, puisqu'il s'agit de répondre à la question que tout le monde se pose: qui va payer la guerre ? Ce mémoire de licence analyse pourquoi et comment la réponse à cette question a engendré des conflits politiques extrêmement violents qui ont contribué à maintenir un climat social tendu au cours de la première moitié des années 1920. Pour diriger la politique douanière suisse en conformité avec leurs intérêts, les grandes associations faîtières de l'économie n'ont pas hésité à prolonger le régime des pleins pouvoirs en vigueur durant la guerre. Le 18 février 1921, des pleins pouvoirs douaniers sont attribués par le Parlement au Conseil fédéral. En vertu de ceux-ci, le gouvernement mène une politique entièrement dévouée aux intérêts des partenaires du bloc bourgeois-paysan alors au pouvoir. Afin de réguler les flux commerciaux, des mesures de restriction de l'importation sont instaurées. En matière de fiscalité, il s'agit avant tout de ne pas recourir trop à l'imposition directe frappant le revenu et le capital, mais d'utiliser l'imposition indirecte, et en particulier la taxation douanière. Pour satisfaire la paysannerie, dont l'appui politique est nécessaire, les positions agricoles du nouveau tarif sont fortement augmentées, ce qui pousse le prix des denrées alimentaires à la hausse. Cette partie du programme douanier, qui est défavorable aux milieux de l'industrie d'exportation, engendre quelques tensions au sein même du bloc bourgeois. Grands perdants de la politique fiscale menée par le Conseil fédéral, les salariés tentent de s'y opposer par tous les moyens à disposition. La politique autoritaire instaurée dans le domaine douanier les empêche toutefois de recourir au référendum. Une large coalition d'associations et de partis de gauche décident alors de s'opposer à la politique du gouvernement par le biais d'une initiative. La politique douanière devient le lieu de rassemblement d'une clientèle politique très diverse susceptible de déboucher sur la constitution d'un cartel politique de centre-gauche. Parallèlement, le PSS lance une autre initiative demandant à ce que la dette de guerre soit payée par un prélèvement unique sur la fortune. Au cours des années 1921 à 1923, la politique fiscale devient un champ d'affrontement politique de première importance. Certes, la gauche socialiste ne se gêne pas d'instrumentaliser ce champ pour attiser la lutte des classes, mais c'est surtout la droite nationaliste qui jette de l'huile sur le feu. Le discours antisocialiste qui caractérise la campagne de votation est de la plus grande violence. Au-delà des enjeux financiers et économiques qui sous-tendent le débat douanier, l'initiative remet en question les fondements mêmes de l'organisation politique suisse. Scellée en 1902, l'alliance douanière est en effet la clef de voûte du bloc bourgeois-paysan des associations faîtières. Une victoire de l'initiative serait la porte ouverte à un cartel de centre-gauche ou, plus grave encore, à une alliance rouge-verte. En stigmatisant les partisans bourgeois de l'initiative, qui sont accusés de faire un pacte avec le diable, l'USCI et l'USP parviennent à isoler le mouvement ouvrier. La défaite de 1923 est ainsi sans appel. Elle marque un jalon important de la «ghettoïsation» que le mouvement ouvrier aura à subir tout au long de l'Entre-deux-guerres. En plébiscitant les pleins pouvoirs douaniers, la votation donne aussi décharge aux autorités politiques pour leur gestion autoritaire de l'économie. Impensable avant la guerre, la soustraction d'arrêtés au référendum se systématisera dans l'Entre-deux-guerres, participant à une remise en question plus large des principes démocratiques. Au centre de la gestion des conséquences économiques et financières de la Première guerre mondiale, la politique douanière permet donc d'expliciter les conséquences sociales et politiques que le conflit a provoquées sur le plus long terme par le biais de l'explosion de la dette de l'Etat.

In America, there are two species of Trypanosoma that can infect humans: Trypanosoma cruzi, which is responsible for Chagas disease and Trypanosoma rangeli, which is not pathogenic. We have developed a model of vaccination in mice with T. rangeli epimastigotes that protects against T. cruzi infection. The goal of this work was to study the pattern of specific immunoglobulins in the peritoneum (the site of infection) and in the sera of mice immunized with T. rangeli before and after challenge with T. cruzi. Additionally, we studied the effects triggered by antigen-antibodies binding and the levels of key cytokines involved in the humoral response, such as IL-4, IL-5 and IL-6. The immunization triggered the production of antibodies reactive with T. cruzi in peritoneal fluid (PF) and in serum, mainly IgG1 and, to a lesser magnitude, IgG2. Only immunized mice developed specific IgG3 antibodies in their peritoneal cavities. Antibodies were able to bind to the surface of the parasites and agglutinate them. Among the cytokines studied, IL-6 was elevated in PF during early infection, with higher levels in non-immunized-infected mice. The results indicate that T. rangeli vaccination against T. cruzi infection triggers a high production of specific IgG isotypes in PF and sera before infection and modulates the levels of IL-6 in PF in the early periods of infection.

Résumé La ricerca del dibattito sul restauro nella Firenze del Novecento copre un arco temporale piuttosto lungo, che va dall'istituzione del Gabinetto dei restauri fiorentino, avvenuta tra il 1932 e il 1934 sino al 1966, anno della drammatica alluvione che travolse il patrimonio storico e artistico della città di Firenze. Sono più di trent'anni densi di storia, in cui grazie alla tradizionale vocazione artigiana messa in atto di volta in volta attraverso metodiche sperimentali, il Gabinetto dei restauri fiorentino, organizzato da Ugo Procacci affrontò dapprima l'emergenza bellica e, grazie alle esperienze maturate in quella circostanza, fronteggiò i danni provocati alle opere d'arte alluvionate. Lo scopo della ricerca è stato proprio quello di individuare gli aspetti ancora attuali del dibattito sul restauro e sulla conservazione. Filo conduttore è stato il dibattito sull'unificazione dei princìpi del restauro in Italia e quindi, dei suoi riflessi a Firenze. Nella prima parte della ricerca, trattando degli inizi dell'attività del Gabinetto dei restauri di Firenze era inevitabile studiare i riflessi che la creazione dell'Istituto Centrale del Restauro ha avuto sull'ambiente fiorentino. L'incombere della seconda guerra mondiale ebbe un peso determinante nell'accelerare i tempi di attuazione di un simile progetto: si temeva fortemente per le sorti del patrimonio artistico italiano e alle Soprintendenze sarebbe spettato il compito di mettere in salvo il maggior numero possibile di opere d'arte nei rifugi antiaerei e, successivamente, provvedere al restauro delle opere danneggiate: la questione dell'unificazione dei metodi da seguire nel campo del restauro e della conservazione delle opere d'arte era divenuta argomento di urgente attualità a guerra conclusa, soprattutto in vista del recupero delle opere danneggiate, Nella seconda parte del lavoro, trattando gli aspetti più attuali e quindi problematici della storia del restauro fiorentino, in particolare riferiti all'arco cronologico che va dalla metà degli anni Cinquanta sino alla fine degli anni Sessanta, è risultato di estremo interesse analizzare le cause e gli effetti della nota "stagione degli stacchi" e quindi l'avvio del dibattito sulla conservazione preventiva delle pitture murali esposte all'aperto. La questione relativa alla conservazione delle pitture murali esposte all'aperto, nei chiostri e nei tabernacoli, rappresentò il caso paradigmatico attorno al quale l'interesse e le soluzioni adottate per la salvaguardia dei cicli pittorici trovarono gli studiosi e i teorici del restauro italiani e stranieri per un'unica volta tutti concordi nell'avvalersi della prassi sistematica preventiva dello strappo delle pitture murali e del distacco delle sottostanti sinopie. Fu dunque questa l'unica occasione in cui si assistette ad una vera unificazione di intenti a livello nazionale.

The presence of intestinal helminths can down-regulate the immune response required to control mycobacterial infection. BALB/c mice infected with Mycobacterium bovis following an infection with the intestinal helminth Strongyloides venezuelensis showed reduced interleukin-17A production by lung cells and increased bacterial burden. Also, small granulomas and a high accumulation of cells expressing the inhibitory molecule CTLA-4 were observed in the lung. These data suggest that intestinal helminth infection could have a detrimental effect on the control of tuberculosis (TB) and render coinfected individuals more susceptible to the development of TB.

This study was designed to assess the effect of GB virus (GBV)-C on the immune response to human immunodeficiency virus (HIV) in chronically HIV-infected and HIV- hepatitis C virus (HCV)-co-infected patients undergoing antiretroviral therapy. A cohort of 159 HIV-seropositive patients, of whom 52 were HCV-co-infected, was included. Epidemiological data were collected and virological and immunological markers, including the production of interferon gamma (IFN-γ) and interleukin (IL)-2 by CD4, CD8 and Tγδ cells and the expression of the activation marker, CD38, were assessed. A total of 65 patients (40.8%) presented markers of GBV-C infection. The presence of GBV-C did not influence HIV and HCV replication or TCD4 and TCD8 cell counts. Immune responses, defined by IFN-γ and IL-2 production and CD38 expression did not differ among the groups. Our results suggest that neither GBV-C viremia nor the presence of E2 antibodies influence HIV and HCV viral replication or CD4 T cell counts in chronically infected patients. Furthermore, GBV-C did not influence cytokine production or CD38-driven immune activation among these patients. Although our results do not exclude a protective effect of GBV-C in early HIV disease, they demonstrate that this effect may not be present in chronically infected patients, who represent the majority of patients in outpatient clinics.

Multinucleated giant cells (MGC) are cells present in characteristic granulomatous inflammation induced by intracellular infectious agents or foreign materials. The present study evaluated the modulatory effect of granulocyte macrophage colony-stimulating factor (GM-CSF) in association with other cytokines such as interferon-gamma (IFN-γ), tumour necrosis factor-alpha, interleukin (IL)-10 or transforming growth factor beta (TGF-β1) on the formation of MGC from human peripheral blood monocytes stimulated with Paracoccidioides brasiliensis antigen (PbAg). The generation of MGC was determined by fusion index (FI) and the fungicidal activity of these cells was evaluated after 4 h of MGC co-cultured with viable yeast cells of P. brasiliensis strain 18 (Pb18). The results showed that monocytes incubated with PbAg and GM-CSF plus IFN-γ had a significantly higher FI than in all the other cultures, while the addition of IL-10 or TGF-β1 had a suppressive effect on MGC generation. Monocytes incubated with both pro and anti-inflammatory cytokines had a higher induction of foreign body-type MGC rather than Langhans-type MGC. MGC stimulated with PbAg and GM-CSF in association with the other cytokines had increased fungicidal activity and the presence of GM-CSF also partially inhibited the suppressive effects of IL-10 and TGF-β1. Together, these results suggest that GM-CSF is a positive modulator of PbAg-stimulated MGC generation and on the fungicidal activity against Pb18.
