Background. In Finland, the incidence of type 1 diabetes mellitus (T1DM) is the highest in the world, and it continues to increase steadily. No effective preventative interventions exist either for individuals at high risk or for the population as a whole. In addition to problems with daily lifelong insulin replacement therapy, T1DM patients with long-lasting disease suffer from various diabetes related complications. The complications can lead to severe impairments and reductions in functional capacity and quality of life and in the worst case they can be fatal. Longitudinal studies on the costs of T1DM are extremely rare, especially in Finland. Typically, in these studies, distinctions between the various types of diabetes have not been made, and costs have not been calculated separately for the sexes. Aims. The aim of this study was to describe inpatient hospital care and costs of inpatient care in a cohort of 5,166 T1DM patients by sex during 1973-1998 in Finland. Inpatient care and costs of care due to T1DM without complications, due to T1DM with complications and due to other causes were calculated separately. Material and Methods. The study population consisted of all Finnish T1DM patients diagnosed before the age of 18 years between January 1st in 1965 and December 31st in 1979 and derived from the Finnish population based T1DM register (N=5,120 in 1979 and N=4,701 in 1997). Data on hospitalisations were obtained from the Finnish Hospital Discharge Register. Results. In the early stages of T1DM, the majority of the use of inpatient care was due to the treatment of T1DM without complications. There were enormous increases in the use of inpatient care for certain complications when T1DM lasted longer (from 9.5 years to 16.5 years). For women, the yearly number of bed-days for renal complications increased 4.8-fold, for peripheral vascular disease 4.3-fold and for ophthalmic complications 2.5-fold. For men, the corresponding increases were as follows: 5-fold, 6.9-fold and 2.5-fold. The yearly bed-days for glaucoma increased 8-fold, nephropathy 7-fold and microangiopathy 6-fold in the total population. During these 7 years, the yearly numbers of bed-days for T1DM without complications dropped dramatically. The length of stay in inpatient care decreased notably, but hospital visits became more frequent when the length of duration of T1DM increased from 9.5 years to 16.5 years. The costs of treatments due to complications increased when T1DM lasted longer. Costs due to inpatient care of complications in the cohort 2.5-folded as duration of T1DM increased from 9.5 years to 16.5 years, while the total costs of inpatient care in the cohort dropped by 22% due to an 80% decrease in the costs of care of T1DM without complications. Treating complications of female patients was more expensive than treating complications of men when T1DM had lasted 9.5 years; the mean annual costs for inpatient care of a female diabetic (any cause) were 1,642 , and the yearly costs of care of complications were 237 . The corresponding yearly mean costs for a male patient were 1,198 and 167 . Treating complications of female patients was more expensive than that of male patients also when the duration of diabetes was 16.5 years, although the difference in average annual costs between sexes was somewhat smaller. Conclusions. In the early phases of T1DM, the treatment of T1DM without complications causes a considerable amount of hospital bed-days. The use of inpatient care due to complications of T1DM strongly increases with ageing of patients. The economic burden of inpatient care of T1DM is substantial.

Hereditary leiomyomatosis and renal cell cancer (HLRCC) is a recently characterized cancer syndrome which predisposes to cutaneous and uterine leiomyomas as well as renal cell carcinoma (RCC). Uterine leiomyosarcoma (ULMS) has also been observed in certain Finnish HLRCC families. The predisposing gene for this syndrome, fumarate hydratase (FH), was identified in 2002. The well-known function of FH is in the tricarboxylic acid cycle (TCAC) in the energy metabolism of cells. As FH is a novel cancer gene, the role of FH mutations in tumours is in general unknown. Similarly, the mechanisms through which defective FH is associated with tumourigenesis are unclear. The loss of a wild type allele has been observed in virtually all HLRCC patients tumours and the FH enzyme activities are either totally lost or remarkably reduced in the tissues of mutation carrier patients. Therefore, FH is assumed to function as a tumour suppressor. Mutations in genes encoding subunits of other TCAC enzyme SDH have also been reported recently in tumours: mutations in SDHB, SDHC, and SDHD genes predispose to paraganglioma and pheochromocytoma. In the present study, mutations in the SDHB gene were observed to predispose to RCC. This was the first time that mutations in SDHB have been detected in extra-paraganglial tumours. Two different SDHB mutations were observed in two unrelated families. In the first family, the index patient was diagnosed with RCC at the age of 24 years. Additionally, his mother with a paraganglioma (PGL) of the heart and his maternal uncle with lung cancer were both carriers of the mutation. The RCC of the index patient and the PGL of his mother showed LOH. In the other family, an SDHB mutation was detected in two siblings who were both diagnosed with RCC at the ages of 24 and 26 years. Both of the siblings also suffered PGL. All these tumours showed LOH. Therefore, we concluded that mutations in SDHB predispose also for RCC in certain families. Several tumour types were analysed for FH mutations to define the role of FH mutations in these tumour types. In addition, patients with a putative cancer phenotype were analysed to identify new HLRCC families. Three FH variants were detected, of which two were novel. One of the variants was observed in a patient diagnosed with ULMS at the age of 41 years. However, LOH was not detected in the tumour tissue. The FH enzyme activity of the mutated protein was clearly reduced, being 43% of the activity of the normal protein. Together with the results from an earlier study we calculated that the prevalence of FH mutations in Finnish non-syndromic ULMS is around 2.4%. Therefore, FH mutations seem to have a minor role in the pathogenesis on non-syndromic ULMS. Two other germline variants were detected in a novel tumour type, ovarian mucinous cystadenoma. However, tumour tissues of the patients were not available for LOH studies and therefore LOH status remained unclear. Therefore, it is possible that FH mutations predispose also for ovarian tumours but further studies are needed to verify this result. A novel variant form of the FH gene (FHv) was identified and characterized in more detail. FHv contains an alternative first exon (1b), which appeared to function as 5 UTR sequence. The translation of FHv is initiated in vitro from exons two and three. The localization of FHv is both cytosolic and nuclear, in contrast to the localization of FH in mitochondria. FHv is expressed at low levels in all human tissues. Interestingly, the expression was induced after heat shock treatment and in chronic hypoxia. Therefore, FHv might have a role e.g. in the adaptation to unfavourable growth conditions. However, this remains to be elucidated.

Focus on opportunities is a cognitive-motivational facet of occupational future time perspective that describes how many new goals, options, and possibilities individuals expect to have in their personal work-related futures. This study examined focus on opportunities as a mediator of the relationships between age and work performance and between job complexity and work performance. In addition, it was expected that job complexity buffers the negative relationship between age and focus on opportunities and weakens the negative indirect effect of age on work performance. Results of mediation, moderation, and moderated mediation analyses with data collected from 168 employees in 41 organizations (mean age = 40.22 years, SD = 10.43, range = 19-64 years) as well as 168 peers providing work performance ratings supported the assumptions. The findings suggest that future studies on the role of age for work design and performance should take employees' focus on opportunities into account.

Research on career adaptability and its relationships with work outcomes has so far primarily focused on the cohort of younger workers and largely neglected older workers. We investigated the relationship between career adaptability and job satisfaction in a sample of 577 older workers from Australia (M age = 59.6 years, SD = 2.4, range 54–66 years), who participated in a 4-wave substudy of the 45 and Up Study. Based on socioemotional selectivity theory, we examined older workers’ chronological age (as a proxy for retirement proximity) and motivation to continue working after traditional retirement age as moderators of the relationship between career adaptability and job satisfaction. We hypothesized that the positive relationship between career adaptability and job satisfaction is stronger among relatively younger workers and workers with a high motivation to continue working compared to relatively older workers and workers with a low motivation to continue working. Results showed that older workers’ age, but not their motivation to continue working, moderated the relationship between career adaptability and job satisfaction consistent with the expected pattern. Implications for future research on age and career adaptability as well as ideas on how to maintain and improve older workers’ career adaptability and job satisfaction are discussed.

BACKGROUND Physical therapy for youth with cerebral palsy (CP) who are ambulatory includes interventions to increase functional mobility and participation in physical activity (PA). Thus, reliable and valid measures are needed to document PA in youth with CP. OBJECTIVE The purpose of this study was to evaluate the inter-instrument reliability and concurrent validity of 3 accelerometer-based motion sensors with indirect calorimetry as the criterion for measuring PA intensity in youth with CP. METHODS Fifty-seven youth with CP (mean age=12.5 years, SD=3.3; 51% female; 49.1% with spastic hemiplegia) participated. Inclusion criteria were: aged 6 to 20 years, ambulatory, Gross Motor Function Classification System (GMFCS) levels I through III, able to follow directions, and able to complete the full PA protocol. Protocol activities included standardized activity trials with increasing PA intensity (resting, writing, household chores, active video games, and walking at 3 self-selected speeds), as measured by weight-relative oxygen uptake (in mL/kg/min). During each trial, participants wore bilateral accelerometers on the upper arms, waist/hip, and ankle and a portable indirect calorimeter. Intraclass coefficient correlations (ICCs) were calculated to evaluate inter-instrument reliability (left-to-right accelerometer placement). Spearman correlations were used to examine concurrent validity between accelerometer output (activity and step counts) and indirect calorimetry. Friedman analyses of variance with post hoc pair-wise analyses were conducted to examine the validity of accelerometers to discriminate PA intensity across activity trials. RESULTS All accelerometers exhibited excellent inter-instrument reliability (ICC=.94-.99) and good concurrent validity (rho=.70-.85). All accelerometers discriminated PA intensity across most activity trials. LIMITATIONS This PA protocol consisted of controlled activity trials. CONCLUSIONS Accelerometers provide valid and reliable measures of PA intensity among youth with CP.

Summary: This research represents the first age-based demographic assessment of pearl perch, Glaucosoma scapulare (Ramsay, 1881), a highly valued species endemic to coastal waters off central eastern Australia. The study was conducted across the species' distribution that encompasses two state jurisdictions (Queensland in the north and New South Wales in the south) using data collected approximately 10 years apart in each state. Estimates of age were made by counting annuli (validated using marginal increment ratios) in sectioned sagittal otoliths. The maximum estimated age was 19 years. Pearl perch attained approx. 12 cm fork length (FL) after one year, 21 cm FL after 2 years and 29 cm FL after 3 years. Fish from the southern end of the species' distribution grew significantly more slowly than those from the northern part of its range. Commercial landings in the north were characterized by greater proportions of larger (>40 cm FL) and older (>6 years) fish than those in the south, with landings mainly of fish between 3 and 6 years of age. The observed variations in age-based demographics of pearl perch highlight the need for a better understanding of patterns of movement and reproduction in developing a model of population dynamics and life-history for this important species. There is a clear need for further, concurrent, age-based studies on pearl perch in the northern and southern parts of its distribution to support the conclusions of the present study based on data collected a decade apart. © 2013 Blackwell Verlag GmbH.

Objective To examine mean level differences, and longitudinal and reciprocal relations among behavioral sleep problems, emotional dysregulation, and attentional regulation across early childhood for children with and without ADHD at 8-9 years. Method This study used data from Growing Up in Australia: The Longitudinal Study of Australian Children (LSAC) – Infant Cohort (n = 4109 analyzed). Children with and without ADHD were identified at age 8-9 years via parent-report of ADHD diagnosis and the 5-item Inattention-Hyperactivity subscale from the Strengths and Difficulties Questionnaire. Maternal report of child sleep problems and self-regulation was collected at 0-1, 2-3, 4-5 and 6-7 years of age. ANOVA was used to compare mean level differences in sleep problems, emotional and attentional regulation by ADHD group. Longitudinal structural equation modeling examined the relations among sleep and self-regulation across time in children with and without ADHD. Results Children with ADHD had persistently elevated levels of sleep problems (from infancy) and emotional and attentional dysregulation compared to controls (from 2-3 years of age). Sleep problems, emotional dysregulation, and attentional regulation were stable over time for both groups. Sleep problems were associated with greater emotional dysregulation two years later from 2-3 years of age for both groups, which in turn was associated with poorer attentional regulation. There was no direct relationship between sleep problems and later attentional regulation. Conclusion Sleep problems in children with and without ADHD are associated with emotional dysregulation, which in turn contributes to poorer attentional functioning. This study highlights the importance of assessing and managing sleep problems in young children.

Objectives To examine the effects of overall level and timing of physical activity (PA) on changes from a healthy body mass index (BMI) category over 12 years in young adult women. Patients and Methods Participants in the Australian Longitudinal Study on Women's Health (younger cohort, born 1973-1978) completed surveys between 2000 (age 22-27 years) and 2012 (age 34-39 years). Physical activity was measured in 2000, 2003, 2006, and 2009 and was categorized as very low, low, active, or very active at each survey, and a cumulative PA score for this 9-year period was created. Logistic regression was used to examine relationships between PA accumulated across all surveys (cumulative PA model) and PA at each survey (critical periods PA model), with change in BMI category (from healthy to overweight or healthy to obese) from 2000 to 2012. Results In women with a healthy BMI in 2000, there were clear dose-response relationships between accumulated PA and transition to overweight (P=.03) and obesity (P<.01) between 2000 and 2012. The critical periods analysis indicated that very active levels of PA at the 2006 survey (when the women were 28-33 years old) and active or very active PA at the 2009 survey (age 31-36 years) were most protective against transitioning to overweight and obesity. Conclusion These findings confirm that maintenance of very high PA levels throughout young adulthood will significantly reduce the risk of becoming overweight or obese. There seems to be a critical period for maintaining high levels of activity at the life stage when many women face competing demands of caring for infants and young children.

Brachial plexus birth injury (BPBI) is caused by stretching, tearing or avulsion of the C5-C8 or Th1 nerve roots during delivery. Foetal-maternal disproportion is the main reason for BPBI. The goal of this study was to find out the incidence of posterior subluxation of the humeral head during first year of life in BPBI and optimal timing of the ultrasonographic screening of the glenohumeral joint. The glenohumeral congruity and posterior subluxation of the humeral head associated to muscle atrophy were assessed and surgical treatment of the shoulder girdle as well as muscle changes in elbow flexion contracture were evaluated. The prospective, population based part of the study included all neonates born in Helsinki area during years 2003-2006. Patients with BPBI sent to the Hospital for Children and Adolescents because of decreased external rotation, internal rotation contracture or deformation of the glenohumeral joint as well as patients with elbow flexion contracture were also included in this prospective study. The incidence of BPBI was calculated to be 3.1/1000 newborns in Helsinki area. About 80% of the patients with BPBI recover totally during the follow-up within the first year of life. Permanent plexus injury at the age of one year was noted in 20% of the patients (0.64/1000 newborns). Muscle imbalance resulted in sonographically detected posterior subluxation in one third of the patients with permanent BPBI. If muscle imbalance and posterior subluxation are left untreated bony deformities will develop. All patients with internal rotation contracture of the glenohumeral joint presented muscle atrophy of the rotator cuff muscles. Especially subscapular and infraspinous muscles were affected. A correlation was found particularly between greatest thickness of subscapular muscle and subluxation of the humeral head, degree of glenoid retroversion, as well as amount of internal rotation contracture. Supinator muscle atrophy was evident among all the studied patients with elbow flexion contracture. Brachial muscle pathology seemed to be an important factor for elbow flexion contracture in BPBI. Residual dysfunction of the upper extremity may require operative treatment such as tendon lengthening, tendon transfers, relocation of the humeral head or osteotomy of the humerus. Relocation of the humeral head improved the glenohumeral congruency among patients under 5 years of age. Functional improvement without remodeling of the glenohumeral joint was achieved by other reconstructive procedures. In conclusion: Shoulder screening by US should be done to all patients with permanent BPBI at the age of 3 and 6 months. Especially atrophy of the subscapular muscle correlates with glenohumeral deformity and posterior subluxation of the humeral head, which has not been reported in previous studies. Permanent muscle changes are the main reason for diminished range of motion of the elbow and forearm. Relocation of the humeral head, when needed, should be performed under the age of 5 years.

ENGLISH: Year-class composition of catch, virtual population size and yearclass strength were determined from serial samples of size composition of catches and catch records. Murphy's Solution to the catch equation, which is free from the effects caused by changes in fishing pressure, was used to estimate year-class strength, i.e. the total population of fish age 3/4 years. The resultant estimates indicated that the X55, X56, X57, X62 and X63 year classes were above average and the X58, X59, X60, X61 and X64 year classes were below average. The year-class designation refers to the year of actual entry or presumed year of entry into the commercial fishery (at approximately 1 year of age). The strongest and poorest year classes were the X57 and X61 classes, respectively. The ratio of the strongest to the weakest year class was 2.6. This amount of variation is small compared to that found for other species of fish. It was found that the relationship between stock size and yearclass strength is of no value in predicting year-class strength. As a by-product of the analysis, estimates of the catchability coefficients (qN) of the age groups in the fishery were obtained, These estimates were found to vary with age and time. Age-two fish apparently showed the greatest vulnerability to fishing gear, followed by ages three and one, respectively. The average estimate of the catchability coefficient in weight was calculated and found to compare favorably with Schaefer's estimate. The influence of sea-surface water temperature upon year-class strength was investigated to determine whether the latter can be predicted from a knowledge of sea-surface temperatures prevailing during and following spawning. No correlation was evident. SPANISH: La composición de la clase anual en la captura, el tamaño de la población virtual y la fuerza de clase anual, fueron determinados según una serie de muestras de la composición de tamaño de las capturas y de los registros de captura. La Solución de Murphy de la ecuación de captura, que está libre de los efectos causados por los cambios de la presión de pesca, fue usada para estimar la fuerza de la clase anual, i.e. la población total de peces de 3/4 años. Las estimaciones resultantes indican que las clases anuales X55, X56, X57, X62 y X63 fueron superiores al promedio y que las clases anuales X58, X59, X60, X61 y X64 fueron inferiores al promedio. La designación de la clase anual se refiere al año actual de entrada o al año supuesto de entrada en la pesca comercial (aproximadamente a la edad de 1 año). Las clases anuales más fuertes y más pobres fueron la X57 y X61 respectivamente. La razón de la clase anual más fuerte en relación a la más débil fue 2.6. Esta cantidad de variación es pequeña comparada con la encontrada para otras especies de peces. Se encontró que la relación entre en tamaño del stock y la fuerza de la clase anual no tiene valor en predecir la fuerza de la clase anual. Se obtuvieron estimaciones de los coeficientes de capturabilidad (qN) de los grupos de edad en la pesquería como un producto derivado del análisis. Se encontraron que estas estimaciones variaron con la edad y tiempo. Los peces de edad dos aparentemente presentaron la vulnerabilidad más grande en relación al arte pesquero, seguidos por las edades tres y una, respectivamente. La estimación promedio del coeficiente de capturabilidad en peso fue calculada y se encontró que podía compararse favorablemente con la estimación de Schaefer. La influencia de la temperatura del agua superficial del mar sobre la fuerza de la clase anual fue investigada para determinar si se podía predecir esta última según el conocimíento de las temperaturas superficiales del mar prevalecientes durante el desove y después de éste. A temática deste estudo está centrada na participação da enfermeira obstétrica no modelo humanizado de atenção ao parto e teve como objetivos: identificar as práticas de enfermeiras obstétricas atuantes no trabalho de parto e parto em uma maternidade municipal do Rio de Janeiro; avaliar a consonância das práticas desenvolvidas por enfermeiras obstétricas durante o trabalho de parto e parto com as recomendações do MS/OMS; analisar a implementação das práticas das enfermeiras obstétricas na assistência ao trabalho de parto e parto entre 2004 e 2008. Trata-se de um estudo descritivo, quantitativo, transversal. O período pesquisado foi de setembro de 2004 a setembro de 2008. A coleta dos dados foi através do livro de registro de partos onde foram registrados 4.510 partos assistidos por enfermeiras. Para a análise foram calculadas a média, mediana e proporção de cada variável estudada, conforme a indicação. As análises foram realizadas utilizando os programas Epi info versão 3.5.1 e Microsoft Word Excel 2007. Verificou-se que a maioria das parturientes assistidas foram jovens, sendo a mediana de idade de 23 anos, que já tinham engravidado duas vezes ou mais e que já tinham parido anteriormente pelo menos uma vez. Com relação à assistência pré-natal 92,8% realizaram o mesmo, contudo apenas 68,2% destas mulheres realizaram seis consultas ou mais. Identificou-se que o acompanhante esteve presente em 60,6% dos partos, porém esta presença foi maior nos partos das adolescentes (77,4%). A posição do parto predominante durante todo o período do estudo foi a vertical (77,6%). As lacerações perineais ocorreram em 52,3% dos partos e a mais incidente foi a laceração de primeiro grau (34,2%). As parturientes que não sofreram qualquer injúria perineal, seja esta espontânea ou cirúrgica, representaram 30,2% dos casos. As práticas obstétricas que não interferem na fisiologia do parto foram realizadas por 85,3% das parturientes, sendo que as mais utilizadas foram os exercícios respiratórios (73,6%), os movimentos pélvicos (42,1%) e a deambulação (29,8%). 67,9% das mulheres assistidas pelas enfermeiras receberam as práticas intervencionistas à fisiologia do parto, destas as mais registradas foram: a realização da amniotomia (25,1%), a administração endovenosa de ocitocina (54%), a realização da episiotomia (22,9%). A episiotomia foi mais utilizada entre as adolescentes (34,8%), nulíparas (44%) e naquelas que pariram na posição horizontal (28,8%). 40,2% das mulheres que não realizaram episiotomia não tiveram laceração perineal e a posição de parto com a menor incidência de laceração perineal foi a quatro apoios (25%), sendo esta classificada como primeiro grau. As adolescentes (55,1%) e as multíparas (71,8%) foram as que menos apresentaram lacerações de períneo. Conclui-se que tanto a prática apoiada no modelo tecnocrático, quanto àquela baseada no modelo humanizado de atenção ao parto, foram registradas na unidade pesquisada. Considera-se que tal fato pode ser investigado em outros estudos, com o objetivo de identificar os fatores que levam à realização de práticas que interferem na fisiologia do parto durante a assistência das enfermeiras obstétricas.

Nos últimos anos, a população de idosos vem crescendo em todo o mundo, e a pesquisa de marcadores de risco cardiovascular mais precoces é de fundamental importância. Um desses marcadores é a doença arterial periférica (DAP), cuja prevalência aumenta com a idade. A DAP pode ser avaliada de forma simples e não invasiva através do índice tornozelo-braquial (ITB) que, por sua vez, pode ser obtido por cálculos diferentes. Até o momento, apenas o método tradicional foi utilizado no cálculo do ITB na população idosa. O objetivo do presente estudo foi avaliar, em idosos hipertensos que se mostrassem independentes para as atividades diárias, os principais fatores relacionados à redução do ITB, considerando-se duas formas distintas de calcular o índice. Os pacientes (n=65) foram submetidos à avaliação clínica, geriátrica e laboratorial, e divididos nos grupos com ITB normal (> 0,9) e ITB reduzido (≤ 0,9). Inicialmente o ITB foi calculado a partir da divisão da maior pressão sistólica dos membros inferiores pela maior pressão das artérias braquiais. Em seguida, o cálculo do ITB foi realizado por um método alternativo, a partir da utilização da menor ao invés da maior média de pressão sistólica nos membros inferiores. A média de idade foi de 74 anos, sendo 76% do sexo feminino. A prevalência de ITB reduzido foi de 18% pelo método convencional e de 32% pelo método alternativo. Na avaliação pelo método convencional, o grupo com ITB baixo apresentou maior prevalência de doenças cardiovasculares (58 vs 9%, p<0,001), diabetes (83 vs 13%, p<0,01), síndrome metabólica (75 vs 41%, p<0,05), e valores significativamente maiores de pressão arterial sistólica (1699 vs 1523 mmHg, p<0,05) e pressão de pulso (877 vs 672 mmHg, p<0,01). A redução do ITB pelo método alternativo mostrou associação com as mesmas variáveis, mas adicionalmente com maior freqüência de tabagistas (29 vs 20%, p<0,05) e maiores níveis de LDL-colesterol (15413 vs 1245 mg/dl, p<0,05). Além disso, o método alternativo foi capaz de detectar pacientes sem alto risco pelo escore de Framingham, mas obesos e com síndrome metabólica. Esses dados apontariam para um valor adicional desta forma de estimar o risco ao escore de Framingham na estratificação de risco cardiovascular, sugerindo sua incorporação na rotina de avaliação de pacientes idosos

O câncer de próstata é a neoplasia mais incidente entre os homens brasileiros. Atualmente, grande parte destes tumores é confinada à próstata no momento do diagnóstico. No entanto, muitos tumores clinicamente classificados como localizados não o são de fato, levando a indicações terapêuticas curativas não efetivas. Por outro lado, muitos pacientes com câncer sem significância clínica são tratados desnecessariamente em função da limitação prognóstica do estadiamento clínicos (pré-tratamento) de pacientes com diagnóstico histológico de adenocarcinoma de próstata localizado (estágios I e II), em coorte hospitalar composta por pacientes tratados no Instituto Nacional de Câncer, Rio de Janeiro, matriculados entre 1990 a 1999. As funções de sobrevida foram calculadas empregando-se o estimados de Kaplan-Meier tomando-se como início a data do diagnóstico histológico e como eventos os óbitos cuja causa básica foi o câncer de próstata. Para avaliação dos fatores prognósticos clínicos foram calculadas as hazard ratios (HR), com intervalos de confiança de 95%, seguindo-se o modelo de riscos proporcionais de Cox. Foram analisadas como fatores prognósticos independentes as variáveis: idade, cor, grau de instrução, data do primeiro tratamento, grau de diferenciação celular d o tumor primário biopsiado (Gleason), estadiamento clínico e PSA total pré-tratamento. O pressuposto dos riscos proporcionais foi avaliado pela análise dos resíduos de Schoenfeld e a influência de valores aberrantes pelos resíduos martingale e escore. Foram selecionados 258 pacientes pelos critérios de elegibilidade do estudo, dos quais 46 foram a óbito durante o período de seguimento. A sobrevida global foi de 88% em 5 anos e de 71% em 10 anos. Idade maior que 80 anos, classificação de Gleason maior que 6, PSA maior que 40ng/ml, estádio B2 e cor branca foram marcadores independentes de pior prognóstico. Fatores prognósticos clássicos na literatura foram úteis na estimativa do prognóstico nesta coorte hospitalar. Os resultados mostram que para pacientes diagnosticados em fases iniciais, os fatores sócio-econômico analisados, não influenciaram o prognóstico. Outros estudos devem ser conduzidos no país para investigar as diferenças no prognóstico em relação à etnia.

The life history of the Atlantic sharpnose shark (Rhizoprionodon terraenovae) was described from 1093 specimens collected from Virginia to northern Florida between April 1997 and March 1999. Longitudinally sectioned vertebral centra were used to age each specimen, and the periodicity of circuli deposition was verified through marginal increment analysis and focus-to-increment frequency distributions. Rhizoprionodon terraenovae reached a maximum size of 828 mm precaudal length (PCL) and a maximum age of 11+ years. Mean back-calculated lengths-at-age ranged from 445 mm PCL at age one to 785 mm PCL at age ten for females, and 448 mm PCL at age one to 747 mm PCL at age nine for males. Observed lengthat-age data (estimated to 0.1 year) yielded the following von Bertalanffy parameters estimates: L∞= 749 mm PCL (SE=4.60), K = 0.49 (SE=0.020), and t0= –0.94 (SE=0.046) for females; and L∞= 745 mm PCL (SE = 5.93), K = 0.50 (SE=0.024), and t0= –0.91 (SE = 0.052) for males. Sexual maturity was reached at age three and 611 mm PCL for females, and age three and 615 mm PCL for males. Rhizoprionodon terraenovae reproduced annually and had a gestation period of approximately 11 months. Litter size ranged from one to eight (mean=3.85) embyros, and increased with female PCL.

Growth parameters were estimated for porbeagle shark (Lamna nasus) in the northwest Atlantic Ocean on the basis of vertebral annuli. A total of 578 vertebrae was analyzed. Annuli were validated up to an age of 11 years by using vertebrae from recaptured oxytetracycline-injected and known-age sharks. Males and females grew at similar rates until the size of male sexual maturity, after which the relative growth of the males declined. The growth rate of the females declined in a similar manner at the onset of maturity. Growth curves were consistent with those derived from tag-recapture analyses (GROTAG) of 76 recaptured fish and those based on length-frequency methods with measurements from 13,589 individuals. Von Bertalanffy growth curve parameters (combined sexes) were L∞ = 289.4 cm fork length, K = 0.07 and t0 = –6.06. Maximum age, based on vertebral band pair counts, was 25 and 24 years for males and females, respectively. Longevity calculations, however, indicated a maximum age of 45 to 46 years in an unfished population.
