Bipolar disorder is a chronic disease that causes abnormal shifts in mood. Although it may be infra diagnosed, a prevalence of 1-2.5% is estimated. As treatment, there is a wide range of medicines but the bests goals are achieved with a mix of psicotherapy and medicines.The following is the case of AF, Who was diagnosed of bipolar disorder 20 years ago. Along this time, he has been amitted to hospital several times. This case runs the normal course of bipolar disorder and shows that treating severe mental illness may be really complex.

BACKGROUND Drugs for inhalation are the cornerstone of therapy in obstructive lung disease. We have observed that up to 75 % of patients do not perform a correct inhalation technique. The inability of patients to correctly use their inhaler device may be a direct consequence of insufficient or poor inhaler technique instruction. The objective of this study is to test the efficacy of two educational interventions to improve the inhalation techniques in patients with Chronic Obstructive Pulmonary Disease (COPD). METHODS This study uses both a multicenter patients´ preference trial and a comprehensive cohort design with 495 COPD-diagnosed patients selected by a non-probabilistic method of sampling from seven Primary Care Centers. The participants will be divided into two groups and five arms. The two groups are: 1) the patients´ preference group with two arms and 2) the randomized group with three arms. In the preference group, the two arms correspond to the two educational interventions (Intervention A and Intervention B) designed for this study. In the randomized group the three arms comprise: intervention A, intervention B and a control arm. Intervention A is written information (a leaflet describing the correct inhalation techniques). Intervention B is written information about inhalation techniques plus training by an instructor. Every patient in each group will be visited six times during the year of the study at health care center. DISCUSSION Our hypothesis is that the application of two educational interventions in patients with COPD who are treated with inhaled therapy will increase the number of patients who perform a correct inhalation technique by at least 25 %. We will evaluate the effectiveness of these interventions on patient inhalation technique improvement, considering that it will be adequate and feasible within the context of clinical practice.

Résumé Introduction : Plusieurs études américaines et australiennes ont décrit des systèmes de tri téléphonique des urgences pédiatriques. En Europe, les services publics d'urgences pédiatriques ont peu de données épidémiologiques sur lesquelles s'appuyer pour répondre à la demande de soins. Depuis 1996, le département de pédiatrie de l'hôpital Pourtalès, Neuchâtel, offre, en dehors des heures ouvrables, mi tri téléphonique infirmier gratuit. Le présent travail analyse : 1) la situation suisse de l'offre en tri téléphonique infirmier pour les urgences pédiatriques ; 2) une partie des données épidémiologiques de l'expérience neuchâteloise. Méthode : 1) Un questionnaire a été envoyé aux 35 services d'urgences pédiatriques publics de Suisse pour Savoir si un tel tri était utilisé ; 2) une analyse rétrospective de tous les appels reçus, consignés sur fiches standardisées, en 1997 et 2000 a été menée. Résultats : 1) La majorité des services (27/35) ont effectivement un système de tri infirmier. Peu offrent une formation spécifique pour ce travail (14/27) ; 2) Au total, 7870 appels ont été analysés (3242 en 1997; 4628 en 2000, ± 43%). En semaine, la majorité ont été reçus entre 18h et 23h et le week-end en milieu de matinée. Septante-cinq % des appels ont concerné des enfants de 5 ans ou moins. La fièvre, les otalgies et la toux ont représenté 42% des plaintes. Vingt-sept % des appels ont été pris en charge uniquement par les conseils infirmiers, 15 % ont été transmis à l'interne de garde et 50% ont conduit à un rendez-vous dans le service le jour même. Conclusion : Nos données peuvent aider d'autres services d'urgences pédiatriques à planifier au mieux la mise en place d'un tel système de tri téléphonique. Abstract Delivery of paediatric primary care by call centres has emerged as a satisfactory system. It been reported in the literature in the United States and Australia. European public-funded paediatric emergency departments (ED) have little epidemiological data to rely on to match the demand in care. Since 1996, we have run a free nurse-led after-hours paediatric telephone triage and advice (TTA) system, To determine wether other Swiss public paediatric departments practiced formal TTA, we conducted a nation-wide postal survey. To delineate who used our call centre and for what reasons, we embarked on a retrospective study of ail the 1997/2000 calls. Most of the units run a TTA (27/35) but few specifically train their staff (14/27). A 43% increase in call numbers was seen between 1997 (3242) and 2000 (4628). During week-days, most of the calls were between 6 and 11 pm and at weekends, a mid morning activity peak was seen. Some 75% of calls were for children aged 5 years or less. Fever, earache and cough accounted for 42% of the main complaints. Of all calls, 27% were dealt by nurses' advice only. About 15% of the calls were transferred to the on-call resident. About 50% led to a same day ED appointment. Conclusion: Nurse-led paediatric telephone triage and advice is common in Switzerland where training seems to be irregular. Study on the likelihood and prevalence of patients with copd, over a year in a family medicine consultation, during 2012 and first two months of 2013. In a query of a health center about 15oo patients every 6 months probabilistic evolution was studied according to the theory of Laplace. Analyze both the COPD, its symptoms, etiology, clinical consultation and treatment in Family Medicine.

Introduction. The Andalusian Public Health System Virtual Library (Biblioteca Virtual del Sistema Sanitario Público de Andalucía, BV-SSPA) was created in June 2006, after being determined by the II Quality Plan in the key process Guarantee the Knowledge Exchange into the Health System which was established by the Strategy IV, Knowledge Management, 2005-2008. It is a government strategy with its own budget and management with the aim of rationalizing the subscriptions into the Andalusian Health System and democratizing the health professional access to qualify scientific information, regardless of the professional workplace. Andalusia is a wide region with more than 8 million inhabitants, more than 90,000 health professionals for 41 hospitals, 1,500 primary healthcare centres, and 12 centres for non-medical attention purposes, and the Virtual Library was created to cover all this Health Services. Before the creation of the BV-SSPA every centre had its own budget and management decisions concerning scientific resources, with the creation of the BV-SSPA both management and budget were centralized. Objectives. With this work we pretend to analyze if the results after these five years have reached the expectations from an economic point of view and determine if really we can offer a benefit to the Andalusian Professional and Society in general. We will demonstrate the following: - The BV-SSPA supposed a cost reduction. It meant cost-effectiveness. - It resulted in Economics of Scale, as we have every year more resources and services investing a minor proportional amount of money. - In terms of Efficiency it implemented more services than the System had before its creation, we a lower budget. Methods. The BV-SSPA was appointed the only intermediary for contracting electronic resources destined to the Andalusian Health System. This had some consequences which should be analyzed: - Hospitals were not allowed to subscribe any resources. - Services offered for the whole System. - A remote access system was created. - Tools to give more visibility to the Public Health System were developed. - Negotiations techniques changed as the BV-SSPA is stronger than individual hospitals. Results. - The amount of 2,431 electronic reviews, 8 data bases and other scientific information resources at the disposal of the Andalusian Health System Professionals and available worldwide requiring only an internet connection. Before the BV-SSPA, 5,267 titles were subscribed by hospital and 2,967 of them were subscribed repeatedly (by two or more hospitals), this represented more than 55%. The rationalization of the subscription investment has been reached. - The establishment of several important scientific services for the whole territory of Andalusia, not only big hospitals. - The use of appropriate tools through a Web 2.0 and Social Media to be acknowledged by most National Health Professionals. Conclusions. It has been demonstrated that the BV-SSPA has become the Central Unit for purchasing, offering librarian services and a reference for users in terms of knowledge management, but from the point of view of business it has also obtained the following results: - Cost-Effectiveness: Its budget for subscriptions is lower than the hospital former one in a 30% and now more electronic resources are available. - Economics of Scale: Near 95,000 health professionals can access this Virtual Library in 2010. Before its creation Professionals for small hospital and Primary Care centres were not able to access to scientific information subscribed by big hospitals. - Efficiency Besides the central electronic purchasing, services were created for the System, without increasing the expenses: - Remote access to all the library resources independent of the user’s location. The BV-SSPA usage increased in a 147% in 2008, when it was installed. - The Document Supply Service implemented in 2009. - The Institutional Repository which contains the whole intellectual, scientific production generated by the Andalusian Public Health Professionals as a result of their healthcare, research or managing activity. - The creation of an application developed by the BV-SSPA to study the Andalusian Health System Scientific Production. - The visibility of the Andalusian Health System reached thanks to the BV-SSPA, through the numerous events in which it participates and organizes such as the 2nd. European National Digital Libraries of Health Conferences and the National Conference of Health Science Information and Documentation held in Cadiz in 2010; and its profile in social media where it can be contacted by citizens and health professionals all over the world. - Negotiation with electronic resource suppliers is much more advantageous as the BV-SSPA is stronger to deal with them thanks to its consolidated budget, its managing independence and its visibility.
