BACKGROUND: LEOPARD syndrome (LS) belongs to the family of neuro-cardio-facio-cutaneous syndromes, which include Neurofibromatosis-1 (NF1), Noonan syndrome, Costello Syndrome, cardio-facio-cutaneous syndrome, Noonan-like syndrome with loose anagen hair and Legius syndrome. These conditions are caused by mutations in genes encoding proteins involved in the RAS-MAPK cellular pathway. Clinical heterogeneity and phenotype overlaps across those different syndromes is already recognized. CASE PRESENTATION: We hereby report a heterozygous de novo mutation in the PTPN11 gene (c.1403C > T) manifesting with a clinical picture of LS during childhood, and later development of neuropathic pain with hypertrophic plexi, which are typically observed in NF1 but have not been reported in LS. CONCLUSION: LS caused by PTPN11 mutations may be associated with hypertrophic roots and plexi. Consequently, clinicians should be aware of the possible development of neuropathic pain and consider specific diagnostic work-up and management.

BACKGROUND: A few studies have suggested an association between prenatal exposure to methylmercury and decreased heart rate variability (HRV) related to autonomic heart function, but no study has examined this association using baroreflex sensitivity (BRS). In this study we assessed the distribution of BRS and immediate orthostatic hypotension (IOH) in young Seychellois adults and their associations with exposure to prenatal and recent postnatal methylmercury. METHODS: Subjects in the Seychelles Child Development Study (SCDS) main cohort were evaluated at age 19 years. Non-invasive beat-to-beat blood pressure (BP) monitoring (Finapres, Ohmeda) was performed at rest and during active standing in 95 consecutive subjects. Recent postnatal mercury exposure was measured in subjects' hair at the age of 19 years and prenatal exposure in maternal hair grown during pregnancy. BRS was estimated by sequence analysis to identify spontaneous ascending and descending BP ramps. HRV was estimated by the following markers: PNN50 (relative numbers of normal-to-normal intervals which are shorter by more than 50 ms than the immediately following normal-to-normal intervals); rMSSD (root mean of the squared sum of successive interval differences); LF/HF (low frequency/high frequency component ratio); ratio of the mean expiratory/inspiratory RR intervals (EI ratio); and the ratio between the longest RR interval 30 s after active standing and the shortest RR interval at 15 s (Max30/Min15). IOH was estimated by the deepest BP fall within the first 15 s after active standing up. RESULTS: Prenatal MeHg exposures were similar in boys and girls (6.7±4.3, 6.7±3.8 ng/g) but recent postnatal mercury levels were higher in males than females (11.2±5.8 vs 7.9±4.3 ng/g, p=0.003). Markers of autonomic heart rate control were within the normal range (BRS: 24.8±7 ms/mm Hg, PNN50: 24.9±6.8%, rMSSD: 68±22, LF/HF: 0.61±0.28) in both sexes. After standing, 51.4% of subjects had a transient systolic BP drop>40 mm Hg, but only 5.3% reported dizziness or had syncope. Prenatal and recent postnatal MeHg levels, overall, were not associated with BRS, E/I ratio, PNN50, rMSSD, LF/HF ratio, Max30/Min15 ratio, and IOH. CONCLUSIONS: This study provides no support for the hypothesis that prenatal or recent postnatal MeHg exposure from fish consumption is associated with impaired autonomic heart rate control.

Although neuroimaging research has evidenced specific responses to visual food stimuli based on their nutritional quality (e.g., energy density, fat content), brain processes underlying portion size selection remain largely unexplored. We identified spatio-temporal brain dynamics in response to meal images varying in portion size during a task of ideal portion selection for prospective lunch intake and expected satiety. Brain responses to meal portions judged by the participants as 'too small', 'ideal' and 'too big' were measured by means of electro-encephalographic (EEG) recordings in 21 normal-weight women. During an early stage of meal viewing (105-145ms), data showed an incremental increase of the head-surface global electric field strength (quantified via global field power; GFP) as portion judgments ranged from 'too small' to 'too big'. Estimations of neural source activity revealed that brain regions underlying this effect were located in the insula, middle frontal gyrus and middle temporal gyrus, and are similar to those reported in previous studies investigating responses to changes in food nutritional content. In contrast, during a later stage (230-270ms), GFP was maximal for the 'ideal' relative to the 'non-ideal' portion sizes. Greater neural source activity to 'ideal' vs. 'non-ideal' portion sizes was observed in the inferior parietal lobule, superior temporal gyrus and mid-posterior cingulate gyrus. Collectively, our results provide evidence that several brain regions involved in attention and adaptive behavior track 'ideal' meal portion sizes as early as 230ms during visual encounter. That is, responses do not show an increase paralleling the amount of food viewed (and, in extension, the amount of reward), but are shaped by regulatory mechanisms.

Electrically driven Er3+ doped Si slot waveguides emitting at 1530 nm are demonstrated. Two different Er3+ doped active layers were fabricated in the slot region: a pure SiO2 and a Si-rich oxide. Pulsed polarization driving of the waveguides was used to characterize the time response of the electroluminescence (EL) and of the signal probe transmission in 1 mm long waveguides. Injected carrier absorption losses modulate the EL signal and, since the carrier lifetime is much smaller than that of Er3+ ions, a sharp EL peak was observed when the polarization was switched off. A time-resolved electrical pump & probe measurement in combination with lock-in amplifier techniques allowed to quantify the injected carrier absorption losses. We found an extinction ratio of 6 dB, passive propagation losses of about 4 dB/mm, and a spectral bandwidth > 25 nm at an effective d.c. power consumption of 120 μW. All these performances suggest the usage of these devices as electro-optical modulators.

La stimulation cérébrale profonde (SCP) nécessite l'implantation chirurgicale d'un système comprenant électrodes cérébrales et boîtier(s) de stimulation. Les noyaux cérébraux visés par la méthodologie stéréotaxique d'implantation doivent être visualisés au mieux par une imagerie à haute résolution. La procédure chirurgicale d'implantation des électrodes se fait si possible en anesthésie locale pour faire des mesures électro-physiologiques et tester en peropératoire l'effet de la stimulation, afin d'optimiser la position de l'électrode définitive. Dans un deuxième temps, le ou les générateur(s) d'impulsions sont implantés en anesthésie générale. La SCP pour les mouvements anormaux a une très bonne efficacité et un risque de complications graves faible quoique non nul. Les complications liées au matériel sont les plus fréquentes. Deep brain stimulation (DBS) requires the surgical implantation of a system including brain electrodes and impulsion generator(s). The nuclei targeted by the stereotaxic implantation methodology have to be visualized at best by high resolution imaging. The surgical procedure for implanting the electrodes is performed if possible under local anaesthesia to make electro-physiological measurements and to test intra-operatively the effect of the stimulation, in order to optimize the position of the definitive electrode. In a second step, the impulsion generator(s) are implanted under general anaesthesia. DBS for movement disorders has a very good efficacy and a low albeit non-zero risk of serious complications. Complications related to the material are the most common.

Peroxisome proliferator-activated receptors (PPARs) are ligand activated transcription factors belonging to the nuclear hormone receptor superfamily. PPARγ is involved in many different activities in the epidermis, such as keratinocyte differentiation, permeability barrier recovery, dermal wound closure, sebaceous gland formation, sebocyte differentiation, and melanogenesis. Preclinical studies with PPARγ ligands on various skin diseases have been performed and they could represent a new strategy in the treatment of scarring alopecia. PPARγ deserves further studies as therapeutic target, likely not with the current drugs, but with future new classes of safer molecules and in combined therapies.

Taajuusmuuttajassa tehokytkinten, tässä tapauksessa IGBT, kytkentäviiveet vääristävät lähtöjännitettä varsinkin pienillä lähtöjännitteen arvoilla. Kun viiveiden pituudet tunnetaan voidaan niiden vaikutus kompensoida taajuusmuuttajan ohjauksella. Perinteisissä toteutuksissa viiveiden pituudet on arvioitu ennalta määritetyn taulukon mukaisesti, jossa parametrina on ollut muun muassa moottorivirta. Viiveet kuitenkin vaihtelevat yksilökohtaisesti, eikä kaikkia viiveisiin vaikuttavia tekijöitä voida ottaa huomioon, joten taulukko antaa vain suuntaa. Ratkaisuna tähän on viiveiden mittaus. Työssä tutustutaan jännitevälipiirillisen PWM-taajuusmuuttajan toimintaan yleisellä tasolla. IGBT:n toimintaan syvennytään tarkemmin sekä teoreettisella tasolla että käytännön mittauksilla. Työssä tarkastellaan kytkimien kytkentäviiveiden syntytapaa ja niiden vaikutuksia lähtöjännitteeseen. Viiveiden aiheuttamiin lähtöjännitteen virheiden korjaukseen esitetään erilaisia menetelmiä. Työssä rakennetaan mittalaite jolla tarkkaillaan taajuusmuuttajan lähtöjännitettä ja mitataan sen avulla kytkentäviiveiden suuruutta. Kytkennässä huomioidaan erityisesti häiriöisen ympäristön EMI-vaikutusten minimointi laitteen toiminnassa. Käytännön mittauksilla ja testauksilla kytkentä todetaan toimivaksi ja soveltuvaksi kytkentäviiveiden vaihevirheettömään mittaukseen.

Lasers in pediatric dermatology were developed as a result of the treatment of port-wine stains. Infantile hemangiomas may benefit, in some cases, from laser treatment as well as venous and lymphatic malformations. For certain pigmented lesions, as well as some hamartomas, laser treatments are a credible alternative to surgical resection. Bum scars are improved by lasers which stimulate collagen remodeling. Furthermore, hair removal of congenital and acquired hypertrichosis can relieve psychosocial discomfort and improve quality of life. The management of pain and fear of children undergoing laser treatment, using either topical or general anesthesia, remains of central importance.

Mountain regions worldwide are particularly sensitive to on-going climate change. Specifically in the Alps in Switzerland, the temperature has increased twice as fast than in the rest of the Northern hemisphere. Water temperature closely follows the annual air temperature cycle, severely impacting streams and freshwater ecosystems. In the last 20 years, brown trout (Salmo trutta L) catch has declined by approximately 40-50% in many rivers in Switzerland. Increasing water temperature has been suggested as one of the most likely cause of this decline. Temperature has a direct effect on trout population dynamics through developmental and disease control but can also indirectly impact dynamics via food-web interactions such as resource availability. We developed a spatially explicit modelling framework that allows spatial and temporal projections of trout biomass using the Aare river catchment as a model system, in order to assess the spatial and seasonal patterns of trout biomass variation. Given that biomass has a seasonal variation depending on trout life history stage, we developed seasonal biomass variation models for three periods of the year (Autumn-Winter, Spring and Summer). Because stream water temperature is a critical parameter for brown trout development, we first calibrated a model to predict water temperature as a function of air temperature to be able to further apply climate change scenarios. We then built a model of trout biomass variation by linking water temperature to trout biomass measurements collected by electro-fishing in 21 stations from 2009 to 2011. The different modelling components of our framework had overall a good predictive ability and we could show a seasonal effect of water temperature affecting trout biomass variation. Our statistical framework uses a minimum set of input variables that make it easily transferable to other study areas or fish species but could be improved by including effects of the biotic environment and the evolution of demographical parameters over time. Several dermatophytes producing numerous pyriform or round microconidia were called Trichophyton mentagrophytes. Among these dermatophytes are the teleomorph species Arthroderma benhamiae, Arthroderma vanbreuseghemii and Arthroderma simii, and other species such as Trichophyton interdigitale, Trichophyton erinacei and Trichophyton quinckeanum for which only the anamorph is known. Confusion exists about which fungus should be really called T. mentagrophytes and about the rational use of this name in practice. We report a case of beard ringworm (tinea barbae) with A. vanbreuseghemii. According to both clinical signs and the type of hair parasitism, this case was exactly compatible to the first description of a non-favic dermatophytosis by Gruby under the name of "mentagrophyte" from which was derived the dermatophyte epithet mentagrophytes. In addition, the phenotypic characters of the isolated fungus in cultures perfectly matched with those of the first description of a dermatophyte under T. mentagrophytes by Blanchard (Parasites animaux et parasites végétaux à l'exclusion des Bactéries, Masson, Paris, 1896). In conclusion, T. mentagrophytes corresponds to the fungus later named A. vanbreuseghemii. However, because the neotype of T. mentagrophytes was not adequately designated in regard to the ancient literature, we would privilege the use of A. vanbreuseghemii and abandon the name of T. mentagrophytes.

The late president of the Palestinian Authority, Yasser Arafat, died in November 2004 in Percy Hospital, one month after having experienced a sudden onset of symptoms that included severe nausea, vomiting, diarrhoea and abdominal pain and which were followed by multiple organ failure. In spite of numerous investigations performed in France, the pathophysiological mechanisms at the origin of the symptoms could not be identified. In 2011, we found abnormal levels of polonium-210 ((210)Po) in some of Arafat's belongings that were worn during his final hospital stay and which were stained with biological fluids. This finding led to the exhumation of Arafat's remains in 2012. Significantly higher (up to 20 times) activities of (210)Po and lead-210 ((210)Pb) were found in the ribs, iliac crest and sternum specimens compared to reference samples from the literature (p-value <1%). In all specimens from the tomb, (210)Po activity was supported by a similar activity of (210)Pb. Biokinetic calculations demonstrated that a (210)Pb impurity, as identified in a commercial source of 3MBq of (210)Po, may be responsible for the activities measured in Arafat's belongings and remains 8 years after his death. The absence of myelosuppression and hair loss in Mr Arafat's case compared to Mr Litvinenko's, the only known case of malicious poisoning with (210)Po, could be explained by differences in the time delivery-scheme of intake. In conclusion, statistical Bayesian analysis combining all the evidence gathered in our forensic expert report moderately supports the proposition that Mr Arafat was poisoned by (210)Po.

Results of two intercomparison exercises are reported. In the first one, the participants were asked to determine Cd, Cu, Pb and Zn, at the sub mg mL-1 level, in an acidified aqueous solution. In the second one, the participants were asked to determine mercury (total) in two homogeneous biological samples, hair and fish. The exercises were considered satisfactory and few reported results were discharged (3s criteria).

Uusiutuvan energian tuottamisen lisääminen on edellytys saavuttaaksemme EU:n asettamat tavoitteet. Tämän takia on syytä tutkia kaikki mahdollisuudet uusiutuvan energian tuottamisen lisäämiseksi jopa aivan pienimmistä laitoksista lähtien. Tämän työn tarkoituksena on tutkia pienvesivoiman taloudellisuutta ja kannattavuutta energiantuotannossa. Työssä suoritetaan myös kustannuslaskenta, jossa pohditaan käytöstä poistetun laitoksen palauttamista takaisin sähkön tuotantoon. Samalla käydään läpi teknisiä, taloudellisia ja lakisääteisiä kysymyksiä, joita pienvoiman verkkoonliittäminen aiheuttaa. Tämä työ on eräänlainen käsikirja pienvesivoimasta. Pienvesivoiman liittäminen jakeluverkkoon on sinällään suojausteknisesti yksinkertaista, mutta kaikkien eri taloudellisten ja lakiteknisten tekijöiden huomioonottaminen on puolestaan haastavaa. Laitosten käyttö ja kunnossapito eivät sinällään vaadi sähköteknistä osaamista, kunhan turvallisuusseikat otetaan visusti huomioon. Työssä havaittiin, että pienvesivoima ei pienissä laitoksissa riitä kattamaan suuria työvoimakustannuksia, joita syntyy, kun yritysten henkilökunta ottaa osaa laitoksen toimintaan. Toisaalta pienvesivoima on erinomainen tapa tuottaa sähköä yksityiselle ihmiselle, mikäli hän on itse valmis operoimaan laitosta eikä laske omalle työlleen suuria kustannuksia. Edellytyksenä on, että omalta maa‐alueelta löytyy pienikin puro suhteellisen läheltä jakeluverkkoa.

Aspects related to the quality of human hair analysis are presented, such as sample collection, sample preparation, determination of trace elements and application examples. Also the hair morphology, the importance of this analysis and its uses and meanings are discussed.

It has been carried out an investigation of ethanol electro-oxidation on Ti/IrO2 electrodes. The experimental results show a high selectivity towards acetaldehyde formation thus, offering potential advantages in cost and availability of raw material. It has been observed that the electrode is partially blocked by a film formed after the oxidation of the starting material which can be removed by pulse technique between RDO and RDH onset. The mechanism and the selectivity of the product formed is presented.
