Introduction : Cette thèse est constituée de trois articles liés les uns aux autres. Le premier s’attache à clarifier les perspectives théoriques et problèmes conceptuels entourant la notion de capacité/incapacité au travail, sa définition et son évolution au fil du temps. Les deuxième et troisième articles visent à évaluer les effets différentiels selon le genre de déterminants du retour au travail (RAT) et de la durée d’indemnisation ainsi que les coûts associés, dans une population de travailleurs indemnisés à long terme pour troubles musculosquelettiques (TMS). Méthodes : Dans le premier article, une revue systématique des définitions de l’(in)capacité au travail et une analyse comparative basée sur la théorisation ancrée débouchent sur une carte conceptuelle intégrative. Dans le second article, une cohorte de 455 adultes en incapacité à long terme pour TMS au dos/cou/membres supérieurs est suivie cinq ans au travers d’entretiens structurés et de données d’indemnisation. Des modèles de Cox stratifiés par genre ont été utilisés pour évaluer la durée jusqu’au premier RAT. Dans le troisième article, une cohorte populationnelle de 13,073 hommes et 9032 femmes en incapacité prolongée pour TMS au dos/cou/membres supérieurs a été suivie pendant trois ans à l’aide de données administratives. Des modèles de Cox stratifiés par genre ont été utilisés pour étudier la durée d’indemnisation et détecter les effets dépendants du temps. Les coûts ont également été examinés. Résultats : Les définitions analysées dans la première étude ne reflètent pas une vision intégrée et partagée de l’(in)capacité au travail. Cependant, un consensus relatif semble émerger qu’il s’agit d’un concept relationnel, résultant de l’interaction de multiples dimensions aux niveaux individuel, organisationnel et sociétal. La seconde étude montre que malgré des courbes de survie jusqu’au RAT similaires entre hommes et femmes (p =0.920), plusieurs déterminants diffèrent selon le genre. Les femmes plus âgées (HR=0.734, par tranches de 10 ans), d’un statut économique perçu comme pauvre (HR=0.625), travaillant ≥40 heures/semaine en ayant des personnes à charge (HR=0.508) et ne connaissant pas l’existence d’un programme de santé et sécurité sur leur lieu de travail (HR=0.598) retournent moins vite au travail, tandis qu’un revenu brut annuel plus élevé (par $10,000) est un facteur facilitant (HR=1.225). Les hommes de plus de 55 ans (HR=0.458), au statut économique perçu comme pauvre (HR=0.653), travaillant ≥40 heures/semaine avec une charge de travail physique perçue élevée (HR=0.720) et une plus grande précarité d’emploi (HR=0.825) retournent moins rapidement au travail. La troisième étude a révélé que trois ans après la lésion, 12.3% des hommes et 7.3% des femmes étaient encore indemnisés, avec un ratio de coûts homme-femme pour l’ensemble des réclamations de 2.1 :1. L’effet de certain prédicteurs (e.g. revenu, siège de lésion, industrie) varie selon le genre. De plus, l’effet de l’âge chez les hommes et l’effet de l’historique d’indemnisation chez les femmes varient dans le temps. Conclusion : La façon de définir l’(in)capacité au travail a des implications importantes pour la recherche, l’indemnisation et la réadaptation. Les résultats confirment également la pertinence d’investiguer les déterminants du RAT et de l’indemnisation selon le genre.

Introduction Provoked vestibulodynia (PVD) is the most frequent cause of genito-pelvic pain/penetration disorder (GPPPD) and is associated with negative psychological and sexual consequences for affected women and their partners. PVD is often misdiagnosed or ignored and many couples may experience a sense of injustice, due to the loss of their ability to have a normal sexual life. Perceiving injustice has been documented to have important consequences in individuals with chronic pain. However, no quantitative research has investigated the experience of injustice in this population. Aim The aim of this study was to investigate the associations between perceived injustice and pain, sexual satisfaction, sexual distress, and depression among women with PVD and their partners. Methods Women diagnosed with PVD (N = 50) and their partners completed questionnaires of perceived injustice, pain, sexual satisfaction, sexual distress, and depression. Main Outcome Measures (1) Global Measure of Sexual Satisfaction Scale; (2) Female Sexual Distress Scale; (3) Beck Depression Inventory-II; and (4) McGill-Melzack Pain Questionnaire. Results After controlling for partners' age, women's higher level of perceived injustice was associated with their own greater sexual distress, and the same pattern was found for partners. Women's higher level of perceived injustice was associated with their own greater depression, and the same pattern was found for partners. Women's higher perceived injustice was not associated with their own lower sexual satisfaction but partners' higher perceived injustice was associated with their own lower sexual satisfaction. Perceived injustice was not associated with women's pain intensity. Conclusion Results suggest that perceiving injustice may have negative consequences for the couple's sexual and psychological outcomes. However, the effects of perceived injustice appear to be intra-individual. Targeting perceived injustice could enhance the efficacy of psychological interventions for women with PVD and their partners.

Introduction Entry dyspareunia is a sexual health concern which affects about 21% of women in the general population. Characterized by pain provoked during vaginal penetration, introital dyspareunia has been shown by controlled studies to have a negative impact on the psychological well-being, sexual function, sexual satisfaction, and quality of life of afflicted women. Many cognitive and affective variables may influence the experience of pain and associated psychosexual problems. However, the role of the partner's cognitive responses has been studied very little. Aim The aim of the present study was to examine the associations between partners' catastrophizing and their perceptions of women's self-efficacy at managing pain on one side and women's pain intensity, sexual function, and sexual satisfaction on the other. Methods One hundred seventy-nine heterosexual couples (mean age for women = 31, SD = 10.0; mean age for men = 33, SD = 10.6) in which the woman suffered from entry dyspareunia participated in the study. Both partners completed quantitative measures. Women completed the Pain Catastrophizing Scale and the Painful Intercourse Self-Efficacy Scale. Men completed the significant-other versions of these measures. Main Outcome Measures Dependent measures were women's responses to (i) the Pain Numeric Visual Analog Scale; (ii) the Female Sexual Function Index; and (iii) the Global Measure of Sexual Satisfaction scale. Results Controlled for women's pain catastrophizing and self-efficacy, results indicate that higher levels of partner-perceived self-efficacy and lower levels of partner catastrophizing are associated with decreased pain intensity in women with entry dyspareunia, although only partner catastrophizing contributed unique variance. Partner-perceived self-efficacy and catastrophizing were not significantly associated with sexual function or satisfaction in women. Conclusions The findings suggest that partners' cognitive responses may influence the experience of entry dyspareunia for women, pointing toward the importance of considering the partner when treating this sexual health problem.

El ausentismo laboral es un fenómeno costoso. Cuando los trabajadores no asisten por enfermedad o accidentes laborales, el impacto que sufre la empresa es en el ámbito económico y/u operativo. Objetivo: Caracterizar los factores del ausentismo laboral en un centro médico de I nivel y su comportamiento durante el año 2010. Métodos: Estudio descriptivo de corte transversal, 118 trabajadores del centro médico de I nivel en Bogotá durante el año 2010.Criterios de inclusión: hombres y mujeres, con contratación directa, incapacidades médicas. Criterios de exclusión: personal de contratación indirecta, outsourcing, calamidades y licencias de maternidad. Información tomada base de datos incapacidades. Resultados: Se presentaron 332 incapacidades que representaron 1518 días perdidos causados por enfermedad general 99.7 %, sistemas respiratorio, digestivo y osteomuscular con mayor número de incapacidades (31,9%, 22,9% y 13% respectivamente). El mayor ausentismo en médicos fue 38,3%, auxiliares odontología 14,2 % y odontólogos 10,8%. El área asistencial tiene 78,3% incapacidades, contratos a término indefinido con un 74.6% y mujeres con 78,01% incapacidades. Conclusiones: La mayor causa del ausentismo es enfermedad general. El sistema respiratorio es el más afectado, seguido del digestivo y osteomuscular, los trabajadores con contrato indefinido, género femenino, área asistencial y por cargos los médicos seguido de auxiliares odontología y los odontólogos. Sin embargo en los resultados evidenciados se debe tener en cuenta la población general y su distribución para variables como género, área, cargos, por lo que se requiere posteriormente análisis estadísticos para establecer si hay diferencia significativa.

La esclerosis múltiple (EM) es una enfermedad desmielinizante del sistema nervioso central. Varias características de la EM son comunes a muchas enfermedades autoinmunes, como la mayor prevalencia en las mujeres (razón mujer:hombre 3:1), mecanismos fisiopatológicos comunes, heterogeneidad clínica, factores ambientales, herencia poligénica moderada, susceptibilidad génica, co-ocurrencia de diferentes enfermedades autoinmunes en un individuo y en los miembros de la familia, y respuesta a la terapia inmunosupresora. El objetivo de este estudio es evaluar la EM como una enfermedad parte de la tautología autoinmune. Metodología Estudio analítico de corte transversal con 201 pacientes del Centro de Estudio de Enfermedades Autoinmunes (CREA) pertenecientes a regiones de Antioquia y Cundiboyaca, con diagnóstico definido de Esclerosis múltiple por medio de los criterios de Mc Donald. Resultados La poliautoinmunidad (OR 13.445 (1.772–102.009), la autoinmunidad familiar (OR 2.164(1.085-4.316), la región ( OR 2,891 (1.31-6.40), los cursos progresivos (OR 4.695-5.984) de esclerosis múltiple se encontraron significativamente asociados a la discapacidad y el género. Discusión Las características comunes de la esclerosis múltiple con otras enfermedades autoiunmunes, soportan la teoría del origen comun de estas condiciones.

Background: Infection with multiple types of human papillomavirus (HPV) is one of the main risk factors associated with the development of cervical lesions. In this study, cervical samples collected from 1,810 women with diverse sociocultural backgrounds, who attended to their cervical screening program in different geographical regions of Colombia, were examined for the presence of cervical lesions and HPV by Papanicolau testing and DNA PCR detection, respectively. Principal Findings: The negative binomial distribution model used in this study showed differences between the observed and expected values within some risk factor categories analyzed. Particularly in the case of single infection and coinfection with more than 4 HPV types, observed frequencies were smaller than expected, while the number of women infected with 2 to 4 viral types were higher than expected. Data analysis according to a negative binomial regression showed an increase in the risk of acquiring more HPV types in women who were of indigenous ethnicity (+37.8%), while this risk decreased in women who had given birth more than 4 times (-31.1%), or were of mestizo (-24.6%) or black (-40.9%) ethnicity. Conclusions: According to a theoretical probability distribution, the observed number of women having either a single infection or more than 4 viral types was smaller than expected, while for those infected with 2-4 HPV types it was larger than expected. Taking into account that this study showed a higher HPV coinfection rate in the indigenous ethnicity, the role of underlying factors should be assessed in detail in future studies.

The present paper presents the results of a transversal descriptive study which intended to estimate the contribution of the project “Caring for those who take care of people with disabilities” in the areas of: strength of personal and group competences, self care, life project, dexterity in the care process of people with disabilities, and communitarian auto management; that was implemented in 20 urban areas with caregivers of the city of Bogota in the year 2007. The study allowed the nresearches to acknowledge the little change perception that caregivers had in terms of self care, however, the caregivers perceived change in the four areas, although this were not statistically significant in comparison with the general population. There were only significant changes in the communitarian auto management area in 30% of the population. As a result, it is proposed that more extensive, continuous, and sustainable processes are implemented and that this process arises from contention spaces which can be created with the caregivers, from which they can be motivated to participate in other ´processes of collective and individual changes. Also there’s a need to rely on facilitators (professionals and change agents) who have stronger competences on the how to be and the how to interact competences, because there’s a need to manage the psychosocial components in this group of people. Also, we must make organizational processes and the social networks stronger, this is: collective actions are required, because disability is a social fact, and so, the individual issues are just a moment in the process of inclusion of the person with disability, his family and caregiver.

Although violence against women has gain attention, there is little evidence of studies about phycological violence against a partner. This paper uses data from Encuesta Nacional de Demografíay Salud (ENDS) to assess empirically models of violence against a partner. One of the main findingsis that the higher the economic independence of the women, the lower the phycologicalviolence against a partner. Some other results show that women with higher education level, belongingto a violent or low income family and living in cities different from Bogotá is correlatedwith higher probability of phycological violence.

The World Bank Report 2012 starts with this statement: “Gender equality matters in itself andit matters for development because, in today’s globalized worlds, countries that use the skillsand talents of their women would have an advantage over those which do not use it.” With theframe that suggest that gender equality matters, this paper describes some policy alternativesoriented to overcome gender disadvantages in the formal labor market incorporation of theurban middle class women in Colombia. On balance, the final recommendation suggest that itis desirable to adopt policy alternatives as Community Centers, which are programs orientedto a social redistribution of the domestic work as a way to encourage women participationin the formal labor market with the social support of the members of their own community.The problem that the social policy needs to address is the segregation of women in the formallabor market in Colombia. Although the evidence shows that the women overcome theeducational gap by showing better performance in education that their male peers, womenare still segregated of the labor market. The persistence of high rates of unemployment on thefemale population, the prevalence of the informal labor market as a women labor market, andthe presence of the payment difference between men and women with similar professionaltrainings are circumstances that sustain the segregation statement. These circumstances areinefficient for the society because an economic analysis shows that the cost of maintain the statuquo is externalized in the social security system that includes health, pension and maternityleave regimens. Therefore, the women segregation involves a market failure.This paper evaluates five policy alternatives each directed to the progress of a different causaldimension of the problem: (i) Quotas in the private market, (ii) Flexible working hours,(iii) replace the maternity leave with a family leave, (iv) Increase the Community Centers forredistributing the care work, and (v) Equal payment enforcement. The first alternative looksto increase women’s participation in the formal labor market. The second, third, and fourthalternatives constitute a package addressed at redistributing care work by reducing women’sresponsibility for reproductive work in the household with the help of husbands and the localgovernment. The fifth alternative intervenes to resolve the equal payment problem.After a four criteria evaluation that measure effectiveness, robustness and improbability inimplementation, efficiency and political acceptability or social opposition, the strongest alternativeis the fostering of Community Centers that promote a redistribution of care work. Thispolicy performs well in the assessment process because it combines gender focus with importantindirect effects: child support and human capabilities. The policy also shows a bottomup implementation process that overcomes the main adoption difficulties in the gender focusprograms and is supported by strong evidence of success in the Colombian context; this evidenceis produced by both transnational actors as a World Bank and also in local accountabilityreporters executed by local institutions like Colombian Institute of Family Welfare (ICBF).

Historic environments, the basis for heritage tourism, are difficult to access for people with disabilities. Many countries have introduced legislation to promote equal rights for people with disabilities. Historic environments, however, enjoy protection under national planning systems which limit the physical access improvements that can be made. The significance of historic environments for tourism in the UK is outlined. Barriers restricting tourists with disabilities accessing historic sites are reviewed from the heritage tourism service provider's viewpoint. Interests of the major stakeholders are considered in terms of the apparent conflict between conservation and access issues as heritage tourism service providers seek to comply with disability discrimination legislation. From a study of access improvements made by major heritage tourism service providers, good practice is identified. However, physical access improvements to enable tourists with disabilities to visit historic environments are a compromise because of the strength of conservation interests. Questions remain as to whether this compromise is acceptable to the tourist with disabilities and whether intellectual access is an acceptable substitute for physical presence.

Flavonoid extracts derived from plant foods have been shown to benefit certain types of fluid retention. However, no studies have investigated these compounds for use in premenstrual fluid retention, a complaint common among women with otherwise normal menstrual cycles. Therefore, we conducted a double-blind, placebo-controlled, pilot study into the effect of a daily flavonoid extract (Colladeen(R), 320 mg oligomeric procyanidins) on premenstrual fluid retention. Fluid retention was assessed at baseline and throughout 4 menstrual cycles of the intervention using validated questionnaires. Leg girth was also measured at baseline and at the end of the study. Thirty subjects completed the study (n = 18 active treatment; n = 12 placebo). Although no significant changes in leg girth measurements were noted, there was a significant improvement in subjective "leg health" scores after flavonoid treatment compared to placebo (p = 0.013). Furthermore, this was accompanied by an improvement in reported premenstrual fluid retention nearing significance (p = 0.066). We conclude that flavonoids supplements may provide a new therapeutic direction to counter premenstrual fluid retention and improve leg health. A larger study is now warranted.

Objective: To investigate determinants of the acceptability of isoflavone products among postmenopausal women with regard to social and lifestyle factors, dietary habits, health concerns, food beliefs, menopausal symptoms and therapies, and to elucidate preferences for specific products. Methods: A consumer survey was conducted among postmenopausal women in four European countries (Germany, Denmark, Italy and the UK), including a total of 465 respondents. Results: The declared acceptability of isoflavones was highest in Germany (80%), followed by Italy (75%), the UK (59%) and Denmark (55%; p < 0.001). Among other factors, prior information about isoflavones (odds ratio (OR) 2.64, 95% confidence interval (CI) 1.54-4.51, p < 0.001) and belief that foods can be used to treat medical conditions (OR 2.18, 95% CI 1.17-4.05, p=0.014) were the strongest predictors of potential acceptability. The preferred forms in which isoflavones might be taken differed markedly between countries. Conclusions: Information about the health effects of isoflavones and health-diet links should be considered the most important factors in attempts to influence the potential acceptability of isoflavone products. Foods enriched with isoflavones need to be developed with regard to specific cultural demands in order to assure consumer acceptance on the marketplace.

Background: Obesity is increasing globally across all population groups. Limited data are available on how obesity patterns differ across countries. Objective: To document the prevalence of obesity and related health conditions for Europeans aged 50 years and older, and to estimate the association between obesity and health outcomes across 10 European countries. Methods: Data were obtained from the 2004 Survey of Health, Ageing and Retirement in Europe, a cross-national survey of 22 777 Continental Europeans over the age of 50 years. The health outcomes included self-reported health, disability, doctor-diagnosed chronic health conditions and depression. Multivariate regression analysis was used to predict health outcomes across weight classes (defined by body mass index [BMI] from self-reported weight and height) in the pooled sample and individually in each country. Results: The prevalence of obesity (BMI >= 30) ranged from 12.8% in Sweden to 20.2% in Spain for men and from 12.3% in Switzerland to 25.6% in Spain for women. Adjusting for compositional differences across countries changed little in the observed large heterogeneity in obesity rates throughout Europe. Compared with normal weight individuals, men and women with greater BMI had significantly higher risks for all chronic health conditions examined except heart disease in overweight men. Depression was linked to obesity in women only. Particularly pronounced risks of impaired health and chronic health conditions were found among severely obese people. The effects of obesity on health did not vary significantly across countries. Conclusions: Cross-country differences in the prevalence of obesity in older Europeans are substantial and exceed socio-demographic differentials in excessive body weight. Obesity is associated with significantly poorer health outcomes among Europeans aged 50 years and over, with effects similar across countries. Large heterogeneity in obesity throughout Europe should be investigated further to identify areas for effective public policy.
