Therapeutic goal of vitamin D: optimal serum level and dose requirements Results of randomized controlled trials and meta-analyses investigating the effect of vitamin D supplementation on falls and fractures are inconsistent. The optimal serum level 25(OH) vitamin D for musculoskeletal and global health is > or = 30 ng/ml (75 nmol/l) for some experts and 20 ng/ml (50 nmol/l) for some others. A daily dose of vitamin D is better than high intermittent doses to reach this goal. High dose once-yearly vitamin D therapy may increase the incidence of fractures and falls. High serum level of vitamin D is probably harmful for the musculoskeletal system and health at large. The optimal benefits for musculoskeletal health are obtained with an 800 UI daily dose and a serum level of near 30 ng/ml (75 nmol/l).

Larvae and pupae of the cryptic firefly Photuris fulvipes (Blanchard,1837) are described. They were reared to adult stage from eggs laid by a female collected in São Luiz do Paraitinga, São Paulo, Brazil. Additionally, adult diagnosis and some bionomic data observed in the laboratory are presented. The bionomical and morphological characteristics of P. fulvipes were compared to other unidentified species of the genus and to Bycellonycha sp. from literature. At moment, the data are insufficient to propose a diagnosis for the species, but some characteristics appear to define species group or, at least, they may have generic value.

An open prospective study was conducted among the patients visiting an urban medical policlinic for the first time without an appointment to assess whether the immigrants (who represent more than half of our patients) are aware of the health effects of smoking, whether the level of acculturation influences knowledge, and whether doctors give similar advice to Swiss and foreign smokers. 226 smokers, 105 Swiss (46.5%), and 121 foreign-born (53.5%), participated in the study. 32.2% (95% CI [24.4%; 41.1%]) of migrants and 9.6% [5.3%; 16.8%] of Swiss patients were not aware of negative effects of smoking. After adjustment for age, the multivariate model showed that the estimated odds of "ignorance of health effects of smoking" was higher for people lacking mastery of the local language compared with those mastering it (odds ratio (OR) = 7.5 [3.6; 15.8], p < 0.001), and higher for men (OR = 4.3 [1.9; 10.0], p < 0.001). Advice to stop smoking was given with similar frequency to immigrants (31.9% [24.2%; 40.8%] and Swiss patients (29.0% [21.0%; 38.5%]). Nonintegrated patients did not appear to receive less counselling than integrated patients (OR = 1.1 [0.6; 2.1], p = 0.812). We conclude that the level of knowledge among male immigrants not integrated or unable to speak the local language is lower than among integrated foreign-born and Swiss patients. Smoking cessation counselling by a doctor was only given to a minority of patients, but such counselling seemed irrespective of nationality.

Some species of drosophilids have been intensely studied, especially in the areas of genetics and molecular biology. However, little is known about their ecology and biogeography. Here, we revise the records of Drosophilidae species in the Cerrado domain, in order to organize the available information and to recognize patterns of geographic distribution and endemism. The 43 sampled sites in this domain reveal 100 species of drosophilids (10 of them exotic to the Neotropical region), distributed in eight genera. The collections were concentrated mainly in the States of Goiás (including the Federal District) and Minas Gerais. Serra do Cipó is the site with the highest richness of these insects. Of the 90 neotropical species found in the Cerrado, 77 also occur outside this domain, most of which are in the Atlantic Forest. Since extensive areas of this domain were poorly sampled for drosophilids and most studies used only baited traps to collect the flies, this absence of endemism should be carefully considered. It is recommended new inventories in poorly sampled regions, especially in the northeastern part of the domain. Also diversification of collection methods and improvement in the taxonomy of these flies are proposed. These directions should provide a significant increase in the list of species of drosophilids of the Cerrado; and thus, increase our knowledge about its biodiversity.

O trabalho que agora se apresenta é fruto, antes de mais, de uma relação especial com Timor. Não necessariamente pela dimensão histórica das várias presenças naquele território mas, e acima de tudo, pela relação pessoal e afectiva com alguns dos responsáveis políticos e religiosos daquele jovem país. Um convite, formulado em 2007 por Xanana Gusmão, levou-nos a calcorrear todo o território num curto espaço de três semanas, corria o Verão de 2008. O objectivo era a análise, no terreno, da viabilidade da instalação de um poder local num país recentemente tornado independente. Timor, fruto da sua história e, muito em especial, das características antropológicas do seu povo, é uma nação assumidamente multicultural. Das origens ancestrais das suas comunidades, dos reinos dispersos que pulverizam o pequeno território, das suas lideranças e dos vários dilectos, associados à longa e marcante presença portuguesa, bem como outras ocupações de países estrangeiros, com destaque para a Indonésia, resulta um caldo cultural, a todos os níveis peculiar. Aqui e acolá ouvimos o povo, entrevistámos e reunimos com os 432 chefes de suco existentes no país, entrevistámos chefes de aldeia e anciãos, reunimos com políticos, sacerdotes, professores e jornalistas. Interpretámos, ou tentámos interpretar, as condicionantes e os cuidados que se devem observar na preparação do quadro jurídico para reger a municipalidade em Timor: as suas vertentes electivas, funcionais, financeiras e fiscalizadoras. Revistámos a História de Timor e passámos por Cabo Verde, país de referência internacional ao nível da sua gestão pública, muito em especial a autárquica. Fizemos, também, óbvia referência à história do poder local em Portugal. Das consultas, do muito que observámos e estudámos, resulta um desinteressado contributo para a implementação do poder local em Timor, com as conclusões que incorporam alertas e sugestões.

This study, now on presentation, comes as a result of a special relationship established with East Timor. Not necessarily through the several historical presences in that territory but, above all, through the emotional and personal relationships with some of the political and religious leaders of this young country. An invitation, made in 2007 by Xanana Gusmão, led us throughout the whole territory in a short time of three weeks, running the summer of 2008. Its purpose was the analysis, on the ground, of the possibility of installing local administration in a country which had recently became independent. Timor, as a result of its history, but mostly of the anthropological characteristics of its people, is an openly multicultural nation. The ancestral origins of their communities, its scattered kingdoms that spray along such a small territory, its leaders and many dialects, associated with the long portuguese presence, as well as other occupations made by foreign countries, especially Indonesia, became a cultural melting pot truly unique. Here and there we heard the people and interviewed and met the 432 local elected leaders of the country. We interviewed village elders and meet with politicians, priests, teachers and journalists. We have interpreted, or tried to, the actual conditions and cautions that must be observed in preparing the legal framework to govern the municipality in East Timor: its elective, functional, financial and oversight aspects. We have reviewed Timor’s history and took Cape Verde’s example as an international reference on its public administration, mostly its local authorities. We made also the obvious reference to the portuguese local government’s history. From search, and much of the observed and studied, remains a selfless contribution to the implementation of local government in East Timor, with alerts that incorporate conclusions and suggestions.

The combination of multiple exostoses (EXT) and enlarged parietal foramina (foramina parietalia permagna, FPP) represent the main features of the proximal 11p deletion syndrome (P11pDS), a contiguous gene syndrome (MIM 601224) caused by an interstitial deletion on the short arm of chromosome 11. Here we present clinical aspects of two new P11pDS patients and the clinical follow-up of one patient reported in the original paper describing this syndrome. Recognised clinical signs include EXT, FPP, mental retardation, facial asymmetry, asymmetric calcification of coronary sutures, defective vision (severe myopia, nystagmus, strabismus), skeletal anomalies (small hands and feet, tapering fingers), heart defect, and anal stenosis. In addition fluorescence in situ hybridisation and molecular analysis were performed to gain further insight in potential candidate genes involved in P11pDS.

Purpose: After tobacco and alcohol, cannabis is the most used substance among adolescents in Switzerland. Our aim is to assess whether cannabis use has become an ordinary means of socialization. We hypothesize that cannabis consumption has become a normative, although still illegal, behavior. Methods: As part of a larger qualitative study aimed at assessing new ways [patterns] of cannabis consumption, 16 daily cannabis consumers (11 males) and 2 former heavy consumers (both females), aged 15 to 20 years, participated in interviews and focus groups. Data were transcribed verbatim and analyzed using Atlas.ti qualitative analysis software. Results: Most consumers define the beginning of their consumption as a moment when they made new friends. They commonly use cannabis in group settings, which encourages the belief that all adolescents use cannabis. Thus, cannabis is mainly identified as an everyday social act. Joints are smoked like cigarettes: at all times of the day, during or after school or work with peers, often starting at lunch break, and mostly in public places. Friends offer a joint in a group setting, much like beer in a bar, as a means of making contact. Consumption invariably increases while socializing on vacation: "During vacation, we smoke up to 10-15 joints a day; at the end we're just dead." Additionally, in order to obtain cannabis, consumers have to be part of the right networks; they generally have several dealers to assure their supply, buy and sell themselves, or practice group-buying. As a result, all friends or acquaintances of consumers are themselves cannabis users. For instance, 4 boys, who say they are best friends, always smoke together and that, in order to quit, "All four of us should say to ourselves, 'Okay, now, let's all stop smoking'. That would be the only solution. . .but it would be impossible!" The 2 former consumers state that when they started using cannabis, "I found myself little by little in a vicious circle where I saw only people who also smoked". When they quit, they separated from their group of friends: "Either you make new friends who don't smoke or you smoke." Conclusions: Discussions with consumers demonstrate a normative facet of cannabis consumption as part of teenage socialization. Consequently, cannabis consumers develop a significant dependency since a majority of their friends use cannabis and their consumption involves most of their daily social life. Our study highlights the need for clear messages about the harmful aspects of using this substance while also suggesting that cessation efforts should include helping users separate from their consumption milieu. Morphological aspects of the male and female genitalia and their associated sclerites, abdominal tergite and sternite 8, are described, illustrated and discussed for seven species of Heilus Kuschel 1955 of tribe and subtribe Hylobiini-Hylobiina. The seven species are H. faldermanni (Boheman, 1836); H. fasciculatus (Boheman, 1843); H. freyreissi (Boheman, 1836); H. myops (Boheman, 1836); H. pupillatus (Olivier, 1790); H. rufescens (Boheman, 1836) and H. tuberculosus (Perty, 1832).

As the prevalence of smoking has decreased to below 20%, health practitioners interest has shifted towards theprevalence of obesity, and reducing it is one of the major health challenges in decades to come. In this paper westudy the impact that the final product of the anti-smoking campaign, that is, smokers quitting the habit, had onaverage weight in the population. To these ends, we use data from the Behavioral Risk Factors Surveillance System,a large series of independent representative cross-sectional surveys. We construct a synthetic panel that allows us tocontrol for unobserved heterogeneity and we exploit the exogenous changes in taxes and regulations to instrumentthe endogenous decision to give up the habit of smoking. Our estimates, are very close to estimates issued in the 90sby the US Department of Health, and indicate that a 10% decrease in the incidence of smoking leads to an averageweight increase of 2.2 to 3 pounds, depending on choice of specification. In addition, we find evidence that the effectovershoots in the short run, although a significant part remains even after two years. However, when we split thesample between men and women, we only find a significant effect for men. Finally, the implicit elasticity of quittingsmoking to the probability of becoming obese is calculated at 0.58. This implies that the net benefit from reducingthe incidence of smoking by 1% is positive even though the cost to society is $0.6 billions.

The synanthropic index and other ecological aspects of the Muscidae family were evaluated through simultaneous monthly sampling in three different environments (urban, rural and forest) using van Someren-Rydon traps baited with human faeces, chicken viscera and decomposing fish and onion. Four traps were set up in each environment (one per bait item) for 48 hours per month, with samples taken every 12 hours. A total of 5726 specimens were collected, belonging to 19 species and 13 genera. Brontaea normata (+99,9), Brontaea quadristigma (+96,9), Synthesiomyia nudiseta (+96,5), Ophyra aenescens (+96,2), Musca domestica (+95,7) and Atherigona orientalis (+93,8) had the highest synanthropic indices, showing a marked preference for human environments. The most abundant species were B. normata (24,31%), Biopyrellia bipuncta (20,60%) and Pseudoptilolepis nigripoda (15,82%), the latter two showed a preference for uninhabited areas. A total of 11 new records for Colombia were found: Ophyra aenescens, Cyrtoneuropsis pararescita, Morellia basalis, Neomuscina dorsipuncta, Biopyrellia bipuncta, Pseudoptilolepis nigripoda, Neomuscina instabilis, Neomuscina currani, Polietina orbitalis, Neomuscina pictipennis and Cyrtoneuropsis maculipennis. Except for the first four species, the remainder presented negative synantrophy indexes (from minor to major), which would allow to use them as ecological indicators of the disturbance degree of dry forests in Colombia.

This paper analyses the effect of tobacco prices on the propensity tostart and quit smoking using a pool of the 1993, 1995 and 1997 editionsof the Spanish National Health Surveys. The estimates for severalparametric models of the hazard rate for starting and quitting suggestthat i) The public health measures applied as of 1992 have had asignificative effect on both reducing the hazard of starting andincreasing the hazard of quitting, ii) Prices have a very weak effect onthe hazard of starting in the male population and no significant effectin the female population, iii) The price floor of cigarrettes, proxiedby the average price of a pack of black cigarrettes, has a significanteffect on the quitting hazard which is robust across specifications andapplies to both men and women. The implied price elasticity of the timeup to quitting is situated around -1.4.
