Traditional morphological examinations are not anymore sufficient for a complete evaluation of tumoral tissue and the use of neoplastic markers is of utmost importance. Neoplastic markers can be classified in: diagnostic, prognostic and predictive markers. Three markers were analyzed. 1) Insulin-like growth factor binding protein 2 (IGFBP2) was immunohistochemically examined in prostatic tissues: 40 radical prostatectomies from hormonally untreated patients with their preoperative biopsies, 10 radical prostatectomies from patients under complete androgen ablation before surgery and 10 simple prostatectomies from patients with bladder outlet obstruction. Results were compared with α-methylacyl-CoA racemase (AMACR). IGFBP2 was expressed in the cytoplasm of untreated adenocarcinomas and, to a lesser extent, in HG-PIN; the expression was markedly lower in patients after complete androgen ablation. AMACR was similarly expressed in both adenocarcinoma and HG-PIN, the level being similar in both lesions; the expression was slightly lower in patients after complete androgen ablation. IGFBP2 may be used a diagnostic marker of prostatic adenocarcinomas. 2) Heparan surface proteoglycan immunohistochemical expression was examined in 150 oral squamous cell carcinomas. Follow up information was available in 93 patients (range: 6-34 months, mean: 19±7). After surgery, chemotherapy was performed in 8 patients and radiotherapy in 61 patients. Multivariate and univariate overall survival analyses showed that high expression of syndecan-1 (SYN-1) was associated with a poor prognosis. In patients treated with radiotherapy, such association was higher. SYN-1 is a prognostic marker in oral squamous cell carcinomas; it may also represent a predictive factor for responsiveness to radiotherapy. 3) EGFR was studied in 33 pulmonary adenocarcinomas with traditional DNA sequencing methods and with two mutation-specific antibodies. Overall, the two antibodies had 61.1% sensitivity and 100% specificity in detecting EGFR mutations. EGFR mutation-specific antibodies may represent a predictive marker to identify patients candidate to tyrosine kinase inhibitors therapy.

Ovarialkarzinome stellen eine schwer zu therapierende onkologische Erkrankung mit im Durchschnitt sehr schlechter Prognose dar. Die Notwendigkeit einer weiteren Verbesserung der Therapie dieser Erkrankung ist sehr offensichtlich. Studien an anderen Tumorentitäten haben die große Bedeutung des Glukosestoffwechsels, speziell des Laktats, in der Erken- nung, Kategorisierung und Therapie von onkologischen Erkrankungen gezeigt. In der Kon- trolle des Glukosestoffwechsels, aber auch vieler anderer Funktionen, wie z. B. des Tumor- wachstums und des Zellüberlebens, hat sich der Hypoxia Inducible Factor (HIF) als beson- ders wichtig herausgestellt. In der vorliegenden Arbeit wurde daher der Glukosestoffwechsel in Ovarialkarzinomen und seine Beeinflussung durch eine Herunterregulierung von HIF-1α untersucht. Hierzu wurden die Ovarialkarzinomzelllinien OC 316 und IGROV1 (Wildtyp) und die Zelllinie OC 316 mit einem lentiviralen Vektor zur Herunterregulierung von HIF-1α ver- wendet. Das Wachstumsverhalten, die Laktatproduktion und der Glukoseverbrauch wurden bei diesen Zelllinien in vitro untersucht. Darüber hinaus wurden mithilfe der bildgebenden Biolumineszenz ATP, Laktat, Pyruvat und Glukose in Xenotransplantaten dieser Zelllinien gemessen. Diese in unserer Arbeitsgruppe entwickelte Methode erlaubt die quantitative Er- fassung von Metaboliten in selektiven Gewebsarealen, wie z. B. in vitalen Tumorregionen, in stomatösen Arealen oder im tumornahen Normalgewebe.rnIn dieser Arbeit kann gezeigt werden, dass die glykolytische Aktivität von Ovarialkarzinom- zelllinien mit dem Wachstumsverhalten positiv korreliert ist. Eine Herunterregulierung von HIF-1α führt zu einer deutlichen Verlangsamung des Zellwachstums, wobei allerdings alle HIF-Zielgene betroffen sein können. Des Weiteren wird mit den hier gezeigten Daten die prognostische Bedeutung des Laktats bestätigt. Hohe Laktatwerte in vitro waren mit schnel- lerem Wachstum korreliert. Zusätzlich zeigen die vorliegenden Daten, dass die gewonnenen Befunde in vitro nur näherungsweise auf die in vivo Situation übertragbar sind. Eine Herun- terregulierung von HIF-1α zeigt keine signifikant unterschiedlichen Laktatwerte in den Xe- notransplantaten. Allerdings spiegeln sich zelllinienspezifische Unterschiede in der metabo- lischen Aktivität in vitro im metabolischen Verhalten der entsprechenden Xenografttumoren recht gut wider.rnDie gewonnenen Ergebnisse weisen zum einen auf die prognostische Bedeutung einer Bestimmung von Laktatkonzentrationen aus Tumorbiopsien hin und bestätigen zum anderen die klinische Aussagekraft metabolischer Aktivitätsmessungen mittels PET. Solche Daten könnten dazu dienen Patienten einer individualisierten Therapie zuzuführen. Außerdem wur- de die Effektivität, aber auch die Komplexität einer gegen HIF-1α gerichteten Therapie auf Protein- und Genebene bestätigt. Somit zeigen die erzielten Resultate einerseits Möglichkei- ten einer individualisierten Therapie auf, andererseits unterstreichen sie die große Notwen- digkeit weiterer Grundlagenforschung auf diesem Gebiet.

Aim of this study is to describe the possible diagnostic value of sleep disturbances in the differential diagnosis of neurodegenerative diseases characterized by parkinsonism at onset. 42 consecutive patients with parkinsonian features and disease duration up to 3 years were included in the BO-ProPark study. Each patient was evaluated twice, at baseline (T0) and 16 months later (T1). Patients were diagnosed as Parkinson disease (PD, 27 patients), PD plus (PD with cognitive impairment/dementia or dysautonomia, 4 patients) and parkinsonian syndrome (PS, 11 patients). All patients underwent a full night video-polysomnography scored by a neurologist blinded to the clinical diagnosis. Sleep efficiency and total sleep time were reduced in all patients; wake after sleep onset was higher in patients with atypical parkinsonisms than in PD patients. No significant differences between groups of patients were detected in other sleep parameters. The mean percentage of epochs with enhanced tonic muscle EMG activity during REM sleep was higher in PD plus and PS than in PD. No difference in phasic muscle EMG activity during REM sleep was seen between the two groups. REM behaviour disorder was more frequent in PD plus and PS than in PD patients. Our data suggest that REM sleep motor control is more frequently impaired at disease onset in patients with PS and PD plus compared to PD patients. The presence of RBD or an enhanced tonic muscle EMG activity in a patient with recent onset parkinsonian features should suggest a diagnosis of atypical parkinsonism, rather than PD. More data are needed to establish the diagnostic value of these features in the differential diagnosis of parkinsonisms. The evaluation of sleep disorders may be a useful tool in the differential diagnosis of parkinsonism at onset.

Nel tumore combinato epatocolangiocellulare (CHC) le componenti epatocitarie e colangiocitarie sono entrambe presenti. Obiettivo: valutare gli aspetti diagnostici radiologici e caratteristiche clinico-demografiche del CHC su cirrosi. Raccolti pazienti con CHC su cirrosi afferenti a due centri del Nord Italia (Bologna, S. Orsola-Malpighi e Milano,IRCCS Ca’ Granda Maggiore Hospital) tra 2003-2013, con diagnosi istologica di CHC. FASE 1:confronto tra ecografia con mdc (CEUS), TC cmdc e RM cmdc nella diagnosi e caratterizzazione dei noduli di CHC. Casistica di 35 pazienti e 37 noduli (due recidive CHC incluse). Mediana delle dimensioni: 25 mm. Non si è identificato un pattern contrastografico patognomonico per CHC. Pattern di enhancement arterioso ad anello periferico, suggestivo per forma colangiocitaria, atipico per HCC, presente nel 26%,50%,29% dei noduli a CEUS,TC,RM. La CEUS avrebbe portato a una errata diagnosi di HCC tipico in un numero maggiore di casi (48%) vs TC(15%,p=0.005), e RM(18%,p=0.080).L’indicazione della malignità del nodulo (presenza di wash-out dopo enhancement arterioso), era fornita con maggiore accuratezza da parte della CEUS(78%), vs TC (24%,p<0.0001) e RM(29%,p=0.002). FASE 2:analisi degli aspetti clinico-laboratoristici e prognostici del CHC e confronto tramite match 1:2 con HCC su cirrosi (36 CHC,72 HCC). Nel CHC correlano positivamente con sopravvivenza le terapie “curative” (trapianto, resezione chirurgica, terapie ablative percutanee a radiofrequenza/ alcolizzazione), stadio precoce alla diagnosi, dimensioni e essere in sorveglianza per diagnosi precoce di HCC. Correlano indipendentemente con sopravvivenza stadio precoce di malattia (unifocale, ≤ 2 cm) e essere in programma di sorveglianza(multivariata). Sopravvivenze del CHC sovrapponibili al gruppo HCC a 1 anno, e lievemente inferiori a 3/5 anni (81%, 39%, 21% vs 83%, 59% e 40%,p=0.78,p=0.080 e p=0.14). Sopravvivenza mediana per CHC (2.36 anni) inferiore vs HCC (4.09 anni) pur senza significatività statistica.

BACKGROUND: Inflammatory lung diseases are a major morbidity factor in children. Therefore, novel strategies for early detection of inflammatory lung diseases are of high interest. Bacterial lipopolysaccharide (LPS) is recognized via Toll-like receptors and CD14. CD14 exists as a soluble (sCD14) and membrane-associated (mCD14) protein, present on the surface of leukocytes. Previous studies suggest sCD14 as potential marker for inflammatory diseases, but their potential role in pediatric lung diseases remained elusive. Therefore, we examined the expression, regulation and significance of sCD14 and mCD14 in pediatric lung diseases. METHODS: sCD14 levels were quantified in serum and bronchoalveolar lavage fluid (BALF) of children with infective (pneumonia, cystic fibrosis, CF) and non-infective (asthma) inflammatory lung diseases and healthy control subjects by ELISA. Membrane CD14 expression levels on monocytes in peripheral blood and on alveolar macrophages in BALF were quantified by flow cytometry. In vitro studies were performed to investigate which factors regulate sCD14 release and mCD14 expression. RESULTS: sCD14 serum levels were specifically increased in serum of children with pneumonia compared to CF, asthma and control subjects. In vitro, CpG induced the release of sCD14 levels in a protease-independent manner, whereas LPS-mediated mCD14 shedding was prevented by serine protease inhibition. CONCLUSIONS: This study demonstrates for the first time the expression, regulation and clinical significance of soluble and membrane CD14 receptors in pediatric inflammatory lung diseases and suggests sCD14 as potential marker for pneumonia in children.

Aim: To investigate the association of the Periodontal Risk Assessment (PRA) model categories with periodontitis recurrence and tooth loss during supportive periodontal therapy (SPT) and to explore the role of patient compliance. Material and Methods: In a retrospective cohort, PRA was performed for 160 patients after active periodontal therapy (APT) and after 9.5 ± 4.5 years of SPT. The recurrence of periodontitis and tooth loss were analysed according to the patient's risk profile (low, moderate or high) after APT and compliance with SPT. The association of risk factors with tooth loss and recurrence of periodontitis was investigated using logistic regression analysis. Results: In 18.2% of patients with a low-risk profile, in 42.2% of patients with a moderate-risk profile and in 49.2% of patients with a high-risk profile after APT, periodontitis recurred. During SPT, 1.61 ± 2.8 teeth/patient were lost. High-risk profile patients lost significantly more teeth (2.59 ± 3.9) than patients with moderate- (1.02 ± 1.8) or low-risk profiles (1.18 ± 1.9) (Kruskal–Wallis test, p=0.0229). Patients with erratic compliance lost significantly (Kruskal–Wallis test, p=0.0067) more teeth (3.11 ± 4.5) than patients compliant with SPT (1.07 ± 1.6). Conclusions: In multivariate logistic regression analysis, a high-risk patient profile according to the PRA model at the end of APT was associated with recurrence of periodontitis. Another significant factor for recurrence of periodontitis was an SPT duration of more than 10 years.

INTRODUCTION: Apical surgery has seen continuous development with regard to equipment and surgical technique. However, there is still a shortage of evidence-based information regarding healing determinants. The objective of this meta-analysis was to review clinical articles on apical surgery with root-end filling in order to assess potential prognostic factors. METHODS: An electronic search of PubMed and Cochrane databases was performed in 2008. Only studies with clearly defined healing criteria were included, and data for at least two categories per prognostic factor had to be reported. Prognostic factors were divided into patient-related, tooth-related, or treatment-related factors. The reported percentages of healed teeth ("the healed rate") were pooled per category. The statistical method of Mantel-Haenszel was applied to estimate the odds ratios and their 95% confidence intervals. RESULTS: With regard to tooth-related factors, the following categories were significantly associated with higher healed rates: cases without preoperative pain or signs, cases with good density of root canal filling, and cases with absence or size < or = 5 mm of periapical lesion. With regard to treatment-related factors, cases treated with the use of an endoscope tended to have higher healed rates than cases without the use of an endoscope. CONCLUSIONS: Although the clinician may be able to control treatment-related factors (by choosing a certain technique), patient- and tooth-related factors are usually beyond the surgeon's power. Nevertheless, patient- and tooth-related factors should be considered as important prognostic determinants when planning or weighing apical surgery against treatment alternatives.

Although associated with adverse outcomes in other cardiopulmonary diseases, limited evidence exists on the prognostic value of anaemia in patients with acute pulmonary embolism (PE). We sought to examine the associations between anaemia and mortality and length of hospital stay in patients with PE. We evaluated 14,276 patients with a primary diagnosis of PE from 186 hospitals in Pennsylvania, USA. We used random-intercept logistic regression to assess the association between anaemia at the time of presentation and 30-day mortality and discrete-time logistic hazard models to assess the association between anaemia and time to hospital discharge, adjusting for patient (age, gender, race, insurance type, clinical and laboratory variables) and hospital (region, size, teaching status) factors. Anaemia was present in 38.7% of patients at admission. Patients with anaemia had a higher 30-day mortality (13.7% vs. 6.3%; p <0.001) and a longer length of stay (geometric mean, 6.9 vs. 6.6 days; p <0.001) compared to patients without anaemia. In multivariable analyses, anaemia remained associated with an increased odds of death (OR 1.82, 95% CI: 1.60-2.06) and a decreased odds of discharge (OR 0.85, 95% CI: 0.82-0.89). Anaemia is very common in patients presenting with PE and is independently associated with an increased short-term mortality and length of stay.

In haemodynamically stable patients with acute symptomatic pulmonary embolism (PE), studies have not evaluated the usefulness of combining the measurement of cardiac troponin, transthoracic echocardiogram (TTE), and lower extremity complete compression ultrasound (CCUS) testing for predicting the risk of PE-related death.
