Background/objectives: Therapy using bone marrow (BM) cells has been tested experimentally and clinically due to the potential ability to restore cardiac function by regenerating lost myocytes or increasing the survival of tissues at risk after myocardial infarction (MI). In this study we aimed to evaluate whether BM-derived mononuclear cell (MNC) implantation can positively influence the post-MI structural remodeling, contractility and Ca(2 +)-handling proteins of the remote non-infarcted tissue in rats. Methods and results: After 48 h of MI induction, saline or BM-MNC were injected. Six weeks later, MI scars were slightly smaller and thicker, and cardiac dilatation was just partially prevented by cell therapy. However, the cardiac performance under hemodynamic stress was totally preserved in the BM-MNC treated group if compared to the untreated group, associated with normal contractility of remote myocardium as analyzed in vitro. The impaired post-rest potentiation of contractile force, associated with decreased protein expression of the sarcoplasmic reticulum Ca2 +-ATPase and phosphorylated-phospholamban and overexpression of Na(+)/Ca(2 +) exchanger, were prevented by BM-MNC, indicating preservation of the Ca(2 +) handling. Finally, pathological changes on remodeled remote tissue such as myocyte hypertrophy, interstitial fibrosis and capillary rarefaction were also mitigated by cell therapy. Conclusions: BM-MNC therapy was able to prevent cardiac structural and molecular remodeling after MI, avoiding pathological changes on Ca(2 +)-handling proteins and preserving contractile behavior of the viable myocardium, which could be the major contributor to the improvements of global cardiac performance after cell transplantation despite that scar tissue still exists.

Apolipoprotein J (ApoJ) ist ein sezerniertes heterodimeres 80kDa Glykoprotein mit zytoprotektiven und antiinflammatorischen Eigenschaften, das ein nahezu ubiquitäres Expressionsmuster aufweist. Eine stark erhöhte ApoJ-Expression ist mit neurodegenerativen Erkrankungen, Atherosklerose, myokardialem Infarkt sowie einer Vielzahl anderer pathophysiologischer Bedingungen assoziiert. Die potentielle Bedeutung von ApoJ umfasst eine Funktion als extrazelluläres Chaperon, Komplementinhibitor, NF-kB-Inhibitor sowie eine Beteiligung an der Endozytose von nekrotischen Zellfragmenten. Unter Bedingungen, die zu einer massiven Akkumulation von absterbenden Zellen führen, ist eine vermehrte Expression von ApoJ auf die überlebenden Nachbarzellen in den betroffenen Geweben beschränkt. Die molekularen Mechanismen, die dieser gesteigerten ApoJ-Genexpression zugrunde liegen, sind jedoch unbekannt. Untersuchungen unserer Arbeitsgruppe konnten zeigen, dass eine Inkubation mit nekrotischem Zellmaterial in vitro eine Akkumulation von ApoJ-mRNA in Fibroblasten der Zelllinie Rat1 induziert, was darauf hindeutet, dass unter pathophysiologischen Bedingungen von nekrotischen Zellen exponierte bzw. freigesetzte Faktoren zu einer gesteigerten ApoJ-Genexpression in umliegenden vitalen Zellen beitragen können. Die im Rahmen der vorliegenden Arbeit durchgeführten Untersuchungen zeigen eine Korrelation zwischen der Expression von Toll-like Rezeptoren (TLRs) in Fibroblasten (Rat1), glatten Gefäßmuskelzellen (CRL2018) sowie embryonalen Dottersackzellen (10A) und einer durch nekrotische Zellen induzierten ApoJ-mRNA-Expression in diesen Zelllinien. Es wird angenommen, dass TLRs neben pathogenassoziierten Strukturen (PAMPs) auch durch körpereigene Agonisten wie Hitzeschockproteine und Nukleinsäuren aktiviert werden. In weiterführenden Experimenten stellte sich unter anderem heraus, dass neben nekrotischen Zellen auch der TLR3-spezifische Agonist Poly(I:C), eine synthetische doppelsträngige RNA, ausschließlich in den beiden TLR3-exprimierenden Zelllinien CRL2018 und Rat1, nicht jedoch in TLR3-defizienten 10A-Zellen, die ApoJ-mRNA-Expression induziert. Darüber hinaus führt auch die Inkubation mit eukaryotischer RNA (Gesamt-RNA, t-RNA) zu einer Akkumulation von ApoJ-mRNA in CRL2018-Zellen. Die Ergebnisse dieser Arbeit zeigen erstmals, dass die Expression von ApoJ-mRNA durch extrazelluläre Ribonukleinsäuren in TLR3-abhängiger Weise induziert wird, was darauf hindeutet, dass in verletzten Geweben aus post-apoptotischen oder nekrotischen Zellen freigesetzte Ribonukleinsäuren zu einer vermehrten ApoJ-Genexpression in vitalen Nachbarzellen beitragen.

Auf der Suche nach dem „vulnerablen Plaque“, der ein besonders hohes Risiko für Schlaganfall und Herzinfarkt besitzt, findet momentan ein Paradigmenwechsel statt. Anstelle des klassischen Stenosegrades gewinnt die Darstellung der Plaquemorphologie zunehmend an Bedeutung. Fragestellung: Ziel dieser Arbeit ist es, die Fähigkeiten eines modernen 16-Kanal-CT hinsichtlich der Auflösung des Plaqueinneren bei Atherosklerose der Karotiden zu untersuchen und den Halo-Effekt in vivo zu erforschen. Methoden: Für die Studie wurden von 28 Patienten mit bekannter, symptomatischer Karotisstenose vor der gefäßchirurgischen Intervention CT-Bilder angefertigt, die nachfolgend mit der Histologie der Gefäßpräparate korreliert wurden. Auf diese Weise konnten die mikroskopisch identifizierten Lipidkerne im CT-Bild eingezeichnet und hinsichtlich ihrer Fläche und Dichtewerte evaluiert werden. In einem weiteren Schritt führten 2 Radiologen in Unkenntnis der histologischen Ergebnisse unabhängig voneinander eine Befundung durch und markierten mutmaßliche Lipidkerne. Zudem wurden sowohl in der verblindeten als auch in der histologiekontrollierten Auswertung die Plaquetypen anhand der AHA-Klassifikation bestimmt. Ein dritter Befundungsdurchgang geschah unter Zuhilfenahme einer von uns entwickelten Software, die CT-Bilder farbkodiert um die Detektion der Lipidkerne zu verbessern. Anhand der Farbkodierung wurde zudem ein Indexwert errechnet, der eine objektive Zuordnung zur AHA-Klassifikation ermöglichen sollte. Von 6 Patienten wurde zusätzlich noch eine native CT-Aufnahme angefertigt, die durch MPR exakt an die Kontrastmittelserie angeglichen wurde. Auf diese Weise konnte der Halo-Effekt, der die Plaqueanteile im lumennahen Bereich überstrahlt, quantifiziert und charakterisiert werden. Ergebnisse: Während die Einstufung in die AHA-Klassifikation sowohl durch den Befunder als auch durch den Softwarealgorithmus eine hohe Korrelation mit der Histologie aufweist (Typ IV/Va: 89 %, Typ Vb: 70 %, Typ Vc: 89 %, Typ VI: 55 %), ist die Detektion der Lipidkerne in beiden Fällen nicht ausreichend gut und die Befunderabhängigkeit zu groß (Cohens Kappa: 18 %). Eine Objektivierung der AHA-Klassifikation der Plaques durch Indexberechnung nach Farbkodierung scheint möglich, wenn auch dem Befunder nicht überlegen. Die fibröse Kappe kann nicht abgegrenzt werden, da Überstrahlungseffekte des Kontrastmittels dessen HU-Werte verfälschen. Dieser Halo-Effekt zeigte sich im Median 1,1 mm breit mit einer Standardabweichung von 0,38 mm. Eine Abhängigkeit von der Kontrastmitteldichte im Gefäßlumen konnte dabei nicht nachgewiesen werden. Der Halo-Effekt fiel im Median um -106 HU/mm ab, bei einer Standardabweichung von 33 HU/mm. Schlussfolgerung: Die CT-Technologie zeigt sich, was die Darstellung von einzelnen Plaquekomponenten angeht, den bekannten Fähigkeiten der MRT noch unterlegen, insbesondere in Bezug auf die fibröse Kappe. Ihre Fähigkeiten liegen bisher eher in der Einstufung von Plaques in eine grobe Klassifikation, angelehnt an die der AHA. Die klinische Relevanz dessen jedoch gilt es in Zukunft in größeren Studien weiter zu untersuchen. Auch lässt die Weiterentwicklung der Computertomographie auf eine zukünftig höhere Auflösung der Plaquemorphologie hoffen.

L’aumento dei costi in sanità, l’aumentata prevalenza delle patologie croniche, le disuguaglianze attuali, evidenziano la domanda crescente di una popolazione fragile che richiede una risposta globale ai bisogni della persona nel suo insieme, attraverso la costruzione di un sistema sanitario integrato che ne garantisca una presa in carico efficace. Riuscire a gestire le patologie croniche in modo appropriato è la sfida a cui sono chiamati i professionisti socio-sanitari; ma quali sono gli elementi per riuscirci? Le evidenze scientifiche dimostrano che è fondamentale l’integrazione tra i professionisti e lo sviluppo dei Percorsi Diagnostici Terapeutici Assistenziali (PDTA). In quest’ottica, in Italia e in particolare in Emilia-Romagna e nell’Azienda USL di Bologna si sono succeduti, e ancora si stanno evolvendo, diversi modelli di organizzazione per migliorare la gestione appropriata delle patologie croniche e l’aderenza alle linee guida e/o ai PDTA. Il ruolo del medico di medicina generale (MMG) è ancora fondamentale e il suo contributo integrato a quello degli gli altri professionisti coinvolti sono imprescindibili per una buona gestione e presa in carico del paziente cronico. Per questo motivo, l’Azienda USL di Bologna ha sviluppato e implementato una politica strategica aziendale volta a disegnare i PDTA e incoraggiato la medicina generale a lavorare sempre di più in gruppo, rispetto al modello del singolo medico. Lo studio ha individuato nelle malattie cardiovascolari, che rimangono la causa principale di morte e morbilità, il suo focus prendendo in esame, in particolare,lo scompenso cardiaco e il post-IMA. L’obiettivo è verificare se e quanto il modello organizzativo, le caratteristiche del medico e del paziente influiscono sul buon management delle patologie croniche in esame valutando la buona adesione alla terapia farmacologica raccomandata dalle linee guida e/o PDTA dello scompenso cardiaco e post-IMA.

Le cardiomiopatie che insorgono a seguito di infarto miocardico sono causa di elevata morbilità e mortalità dalle importanti ricadute cliniche, dovute alle patologie insorgenti a seguito dell’ischemia e della cicatrice post-infatuale. Il ventricolo sinistro danneggiato va incontro a un rimodellamento progressivo, con perdita di cardiomiociti e proliferazione dei fibroblasti, risultante in un’architettura e in una funzionalità dell’organo distorta. I fibroblasti cardiaci sono i principali responsabili della fibrosi, il processo di cicatrizzazione caratterizzato da un’eccessiva deposizione di matrice extracellulare (ECM). Negli ultimi anni gli sforzi del nostro laboratorio sono stati volti a cercare di risolvere questo problema, attraverso l’uso di una molecola da noi sintetizzata, un estere misto degli acidi butirrico, retinoico e ialuronico, HBR, capace di commissionare le cellule staminali in senso cardio-vascolare. Studi in vivo mostrano come l’iniezione diretta di HBR in cuori di animali sottoposti a infarto sperimentale, sia in grado, tra le atre cose, di diminuire la fibrosi cardiaca. Sulla base di questa evidenza abbiamo cercato di capire come e se HBR agisse direttamente sui fibroblasti, indagando i meccanismi coinvolti nella riduzione della fibrosi in vivo.. In questa tesi abbiamo dimostrato come HBR abbia un’azione diretta su fibroblasti, inibendone la proliferazione, senza effetti citotossici. Inoltre HBR induce una significativa riduzione della deposizione di collagene.. HBR agisce sull’espressione genica e sulla sintesi proteica, sopprimendo la trascrizione dei geni del collagene, così come dell’a-sma, inibendo la trasizione fibroblasti-miofibroblasti, e promuovendo la vasculogenesi (attraverso VEGF), la chemoattrazione di cellule staminali (attraverso SDF) e un’attività antifibrotica (inibendo CTGF). HBR sembra modulare l’espressione genica agendo direttamente sulle HDAC, probabilmente grazie alla subunità BU. L’abilità di HBR di ridurre la fibrosi post-infartuale, come dimostrato dai nostri studi in vivo ed in vitro, apre la strada a importanti prospettive terapeutiche.

Background: Acute coronary syndromes (ACS) in very young patients have been poorly described. We therefore evaluate ACS in patients aged 35 years and younger. Methods: In this prospective cohort study, 76 hospitals treating ACS in Switzerland enrolled 28,778 patients with ACS between January 1, 1997, and October 1, 2008. ACS definition included ST-segment elevation myocardial infarction (STEMI), non-ST-segment elevation myocardial infarction (NSTEMI), and unstable angina (UA). Results: 195 patients (0.7%) were 35 years old or younger. Compared to patients N35 years, these patients were more likely to present with chest pain (91.6% vs. 83.7%; P=0.003) and less likely to have heart failure (Killip class II to IV in 5.2% vs. 23.0%; Pb0.001). STEMI was more prevalent in younger than in older patients (73.1% vs. 58.3%; Pb0.001). Smoking, family history of CAD, and/or dyslipidemia were important cardiovascular risk factors in young patients (prevalence 77.2%, 55.0%, and 44.0%). The prevalence of overweight among young patients with ACS was high (57.8%). Cocaine abuse was associated with ACS in some young patients. Compared to older patients, young patients were more likely to receive early percutaneous coronary interventions and had better outcome with fewer major adverse cardiac events. Conclusions: Young patients with ACS differed from older patients in that the younger often presented with STEMI, received early aggressive treatment, and had favourable outcomes. Primary prevention of smoking, dyslipidemia and overweight should be more aggressively promoted in adolescence.

BACKGROUND The study set out to identify clinical, laboratory and radiological predictors of early mortality after an acute ischaemic stroke (AIS) and to analyse medical and neurological complications that caused death. METHODS A total of 479 consecutive patients (mean age 63+/-14 years) with AIS underwent stroke examination and treatment. Examination included clinical evaluation, laboratory tests, and brain CT and/or MRI. Follow-up data at 30 days were available for 467 patients (93%) who were included in the present analysis. RESULTS The median National Institute of Health Stroke Study (NIHSS) score on admission was 6. A total of 62 patients (13%) died within 30 days. The cause of death was the initial event in 43 (69%), pneumonia in 12 (19%), intracerebral haemorrhage in 9 (15%), recurrent stroke in 6 (10%), myocardial infarction in 2 (3%), and cancer in 1 (2%) of the patients. In univariate comparisons, advanced age (p<0.001), hypertension (p=0.013), coronary disease (p=0.001), NIHSS score (p<0.001), undetermined stroke etiology (p=0.031), relevant co-morbidities (p=0.008), hyperglycemia (p<0.001), atrial fibrillation (p<0.001), early CT signs of ischemia (p<0.001), dense artery sign (p<0.001), proximal vessel occlusion (p<0.001), and thrombolysis (p=0.008) were associated with early mortality. In multivariate analysis, advanced age (HR=1.12; 95% CI 1.05-1.19; p<0.001) and high NIHSS score on admission (HR=1.15, 95% CI 1.05-1.25; p=0.002) were independent predictors of early mortality. CONCLUSIONS We report 13% mortality at 30 days after AIS. More than two thirds of the deaths are related to the initial stroke. Advanced age and high NIHSS score are the only independent predictors of early mortality in this series.

BACKGROUND: Atrial fibrillation (AF) is a significant risk factor for cardiovascular (CV) mortality. This study aims to evaluate the prognostic implication of AF in patients with peripheral arterial disease (PAD). METHODS: The International Reduction of Atherothrombosis for Continued Health (REACH) Registry included 23,542 outpatients in Europe with established coronary artery disease, cerebrovascular disease (CVD), PAD and/or >/=3 risk factors. Of these, 3753 patients had symptomatic PAD. CV risk factors were determined at baseline. Study end point was a combination of cardiac death, non-fatal myocardial infarction (MI) and stroke (CV events) during 2 years of follow-up. Cox regression analysis adjusted for age, gender and other risk factors (i.e., congestive heart failure, coronary artery re-vascularisation, coronary artery bypass grafting (CABG), MI, hypertension, stroke, current smoking and diabetes) was used. RESULTS: Of 3753 PAD patients, 392 (10%) were known to have AF. Patients with AF were older and had a higher prevalence of CVD, diabetes and hypertension. Long-term CV mortality occurred in 5.6% of patients with AF and in 1.6% of those without AF (p<0.001). Multivariable analyses showed that AF was an independent predictor of late CV events (hazard ratio (HR): 1.5; 95% confidence interval (CI): 1.09-2.0). CONCLUSION: AF is common in European patients with symptomatic PAD and is independently associated with a worse 2-year CV outcome.

This prospective multicenter study assessed the efficacy and safety of electroanatomical mapping in combination with open-saline irrigated ablation technology for ablation of chronic recurrent mappable and unmappable VT in remote myocardial infarction.

Cardiac papillary fibroelastoma is a benign tumor that mainly affects cardiac valves. The tumor has the potential to cause angina and myocardial infarction due to embolization of tumor fragments. We describe a rare case of right coronary artery ostial obstruction by a 12 x 19 mm sized papillary fibroelastoma located in the sinus of Valsalva. The report underlies the importance of echocardiography in diagnosis and intraoperative treatment of this type of cardiac mass.

Objectives We compared the angiographic and long-term clinical outcomes of patients with and without overlap of drug-eluting stents (DES). Background DES overlap has been associated with delayed healing and increased inflammation in experimental studies, but its impact on clinical outcome is not well established. Methods We analyzed the angiographic and clinical outcomes of 1,012 patients treated with DES in the SIRTAX (Sirolimus-Eluting Versus Paclitaxel-Eluting Stents for Coronary Revascularization) trial according to the presence or absence of stent overlap and the number of stents per vessel: 134 (13.2%) patients with multiple DES in a vessel with overlap, 199 (19.7%) patients with multiple DES in a vessel without overlap, and 679 (67.1%) patients with 1 DES per vessel. Results Angiographic follow-up at 8 months showed an increased late loss in DES overlap patients (0.33 ± 0.61 mm) compared with the other groups (0.18 ± 0.43 mm and 0.15 ± 0.38 mm, p < 0.01). The smallest minimal lumen diameter was located at the zone of stent overlap in 17 (68%) of 25 patients with stent overlap who underwent target lesion revascularization. Major adverse cardiac events were more common in patients with DES overlap (34 events, 25.4%) than in the other groups (42 events, 21.1% and 95 events, 14.0%) at 3 years (p < 0.01). Both the risk of target lesion revascularization (20.2% vs. 16.1% vs. 9.7%, p < 0.01) and the composite of death or myocardial infarction (17.2% vs. 14.1% vs. 9.1%, p = 0.01) were increased in patients with DES overlap compared with the other groups. Conclusions DES overlap occurs in >10% of patients undergoing percutaneous coronary intervention in routine clinical practice and is associated with impaired angiographic and long-term clinical outcome, including death or myocardial infarction. The aim of this analysis was to assess the effect of body mass index (BMI) on 1-year outcomes in patients enrolled in a contemporary percutaneous coronary intervention trial comparing a sirolimus-eluting stent with a durable polymer to a biolimus-eluting stent with a biodegradable polymer. A total of 1,707 patients who underwent percutaneous coronary intervention were randomized to treatment with either biolimus-eluting stents (n = 857) or sirolimus-eluting stents (n = 850). Patients were assigned to 1 of 3 groups according to BMI: normal (<25 kg/m(2)), overweight (25 to 30 kg/m(2)), or obese (>30 kg/m(2)). At 1 year, the incidence of the composite of cardiac death, myocardial infarction, and clinically justified target vessel revascularization was assessed. In addition, rates of clinically justified target lesion revascularization and stent thrombosis were assessed. Cox proportional-hazards analysis, adjusted for clinical differences, was used to develop models for 1-year mortality. Forty-five percent of the patients (n = 770) were overweight, 26% (n = 434) were obese, and 29% (n = 497) had normal BMIs. At 1-year follow-up, the cumulative rate of cardiac death, myocardial infarction, and clinically justified target vessel revascularization was significantly higher in the obese group (8.7% in normal-weight, 11.3% in overweight, and 14.5% in obese patients, p = 0.01). BMI (hazard ratio 1.47, 95% confidence interval 1.02 to 2.14, p = 0.04) was an independent predictor of stent thrombosis. Stent type had no impact on the composite of cardiac death, myocardial infarction, and clinically justified target vessel revascularization at 1 year in the 3 BMI groups (hazard ratio 1.08, 95% confidence interval 0.63 to 1.83, p = 0.73). In conclusion, BMI was an independent predictor of major adverse cardiac events at 1-year clinical follow-up. The higher incidence of stent thrombosis in the obese group may suggest the need for a weight-adjusted dose of clopidogrel.

Reperfusion of an organ following prolonged ischemia instigates the pro-inflammatory and pro-coagulant response of ischemia / reperfusion (IR) injury. IR injury is a wide-spread pathology, observed in many clinically relevant situations, including myocardial infarction, stroke, organ transplantation, sepsis and shock, and cardiovascular surgery on cardiopulmonary bypass. Activation of the classical, alternative, and lectin complement pathways and the generation of the anaphylatoxins C3a and C5a lead to recruitment of polymorphonuclear leukocytes, generation of radical oxygen species, up-regulation of adhesion molecules on the endothelium and platelets, and induction of cytokine release. Generalized or pathway-specific complement inhibition using protein-based drugs or low-molecular-weight inhibitors has been shown to significantly reduce tissue injury and improve outcome in numerous in-vitro, ex-vivo, and in-vivo models. Despite the obvious benefits in experimental research, only few complement inhibitors, including C1-esterase inhibitor, anti-C5 antibody, and soluble complement receptor 1, have made it into clinical trials of IR injury. The results are mixed, and the next objectives should be to combine knowledge and experience obtained in the past from animal models and channel future work to translate this into clinical trials in surgical and interventional reperfusion therapy as well as organ transplantation.

We performed a pooled analysis of three trials comparing titanium-nitride-oxide-coated bioactive stents (BAS) with paclitaxel-eluting stents (PES) in 1,774 patients. All patients were followed for 12 months. The primary outcomes of interest were recurrent myocardial infarction (MI), death and target lesion revascularization (TLR). Secondary endpoints were stent thrombosis (ST) and major adverse cardiac events (MACE) including MI, death and TLR. There were 922 patients in the BAS group and 852 in the PES group. BAS significantly reduced the risk of recurrent MI (2.7% vs. 5.6%; risk ratio 0.50, 95% CI 0.31-0.81; p = 0.004) and MACE (8.9% vs. 12.6%; risk ratio 0.71, 95% CI 0.54-0.94; p = 0.02) during the 12 months of follow up. In contrast, the differences between BAS and PES were not statistically significant with respect to TLR (risk ratio 0.98, 95% CI 0.68-1.41), death (risk ratio 0.96, 95% CI 0.61-1.51) and definite ST (risk ratio 0.28, 95% CI 0.05-1.47). In conclusion, the results of this analysis suggest that BAS is effective in reducing TLR and improves clinical outcomes by reducing MI and MACE compared with PES.

In patients with ventricular tachycardia (VT) and a history of myocardial infarction, intervention with an implantable cardioverter defibrillator (ICD) can prevent sudden cardiac death and thereby reduce total mortality. However, ICD shocks are painful and do not provide complete protection against sudden cardiac death. We assessed the potential benefit of catheter ablation before implantation of a cardioverter defibrillator.
