Palynologie und Sedimentologie der Interglazialprofile Döttingen, Bonstorf, Munster und Bilshausen Zusammenfassung In der vorliegenden Dissertation wurden vier dem Holstein-Interglazial zugehörige Bohrkerne sowie ein rhumezeitlicher Bohrkern palynologisch und sedimentologisch bearbeitet. Die holsteinzeitlichen Bohrkerne stammen aus Kieselgurlagerstätten der Lüneburger Heide (Bonstorf und Munster) und aus einem Trockenmaar (Döttingen) in der Eifel. Der rhumezeitliche Kern stammt aus der Typlokalität Bilshausen im Harzvorland. Neben Prozentwertdiagrammen werden Pollendichte- und wenn möglich Polleninfluxwerte vorgestellt, die insbesondere für die Lokalitäten Hetendorf/Bonstorf und Munster/Breloh bisher nicht verfügbar waren. Mit dem Profil Döttingen konnte erstmals eine sowohl vollständige als auch nicht innerhalb des klassischen Aufkommens holsteinzeitlicher Fundstellen im norddeutschen Tiefland positionierte Holsteinsequenz aus dem deutschen Mittelgebirge dokumentiert werden. Das erhaltene Pollendiagramm bestätigt die aus den norddeutschen Profilen bekannte holsteintypische Vegetationsabfolge, durch die das Holstein gegenüber anderen Interglazialen wie Holozän, Eem oder Rhume palynologisch definiert ist. Neben der grundsätzlichen Übereinstimmung der Pollensequenz unterscheidet sich das Profil Döttingen aber deutlich im prozentualen Aufkommen der beteiligten Taxa von den norddeutschen Profilen. So wird eine hohe Alnus-Präsenz als Merkmal deutscher Holsteinprofile bestätigt, jedoch ist die, in den norddeutschen Lokalitäten durchhaltend hohe oder dominante Beteiligung von Pinus im deutschen Mittelgebirge nicht vorhanden und muss daher auf die Standortbedingungen Norddeutschlands zurückgeführt werden. Abies dagegen ist im Holstein der Mittelgebirge wesentlich präsenter als im norddeutschen Flachland. Im Profil Döttingen wurden insgesamt 10 Tephralagen gefunden. Auf eine dieser Tephren folgt ein „Birken-Kiefern-Gräser Vorstoß“, der palynostratigraphisch dem älteren „Birken-Kiefern Vorstoß“ in Munster/Breloh entspricht. Als eine Typologie des Holstein kann das in den Profilen Döttingen und Munster bestätigte intraholsteinzeitliche Carpinus-Minimum verstanden werden. An Hand sedimentologischer und palynologischer Befunde aus dem Bohrkern MU 2 muss die Existenz zweier, in der Literatur postulierter, postholsteinzeitlicher, „Nachschwankungen“ in Munster/Breloh in Frage gestellt, wenn nicht abgelehnt werden. In Kern MU 2 fallen palynostratigraphische Grenzen häufig mit Sandeinschaltungen zusammen. Eine dieser Sandeinschaltungen, nämlich unmittelbar vor dem älteren „Birken-Kiefern-Vorstoß“, korreliert in ihrer stratigraphischen Position mit der den „Birken-Kiefern-Gräser-Vorstoß“ im Profil Döttingen einleitenden Tephralage. Es gelang die Dauer des intraholsteinzeitlichen Carpinus Minimums auf etwa 1500±100 Jahre zu bestimmen und eine interne Zweigliederung zu dokumentieren. Im rhumezeitlichen Kern von Bilshausen (BI 1) konnten zahlreiche Störungen nachgewiesen werden. Insbesondere im Teufenbereich des Bilshausener „Birken-Kiefern-Vorstoßes“ deuten diese auf eine möglicherweise verfälschte Überlieferung. Der palynologisch markante „Lindenfall“ von Bilshausen liegt im Bereich einer isoklinalen Schichtenverfaltung. Die in der Literatur im Horizont des „Lindenfalls“ beschriebene „Bilshausentephra“ wurde nicht gefunden. Warvenzählungen an den Kernen MU 1, MU 2 und BI 1 ermöglichten Pollenzonendauern in Holstein- und Rhume-Interglazial zu bestimmen. Dabei wurde mittels den Warvenzählungen, unter zu Hilfenahme von Literaturdaten und von Schätzwerten eine Dauer für das Holstein-Interglazial sensu stricto (Pollenzonen I-XIV) von 15400-17800 Jahren und für das Rhume-Interglazial von wahrscheinlich 22000 Jahren bis maximal 26000 Jahren ermittelt.

Human Papillomavirus (HPV) is the cause of cervical cancers (among these, adenocarcinoma, AdCa) and is associated to a subgroup of oropharyngeal carcinomas (OPSCCs). Even if the risk for cancer development is linked to the infection by some viral genotypes, mainly HPV16 and 18, viral DNA alone seems not to be sufficient for diagnosis. Moreover, the role of the virus in OPSCCs has not been totally clarified yet. In the first part of the thesis, the performances concerning viral genotyping in clinical cervical samples of a new pyrosequencing-based test and a well-known hybridization-based assay have been compared. Similar results between the methods have been obtained. However, the former showed advantages in detecting intratype variants, higher specificity and a broader spectrum of detectable HPV types. The second part deals with the evaluation of virological markers (genotyping, viral oncoproteins expression, viral load, physical state and CpG methylation of HPV16 genome) in the diagnosis/prognosis of cervical AdCa and HPV-associated OPSCCs. HPV16 has been confirmed the most prevalent genotype in both the populations. Interestingly, the mean methylation frequency of viral DNA at the early promoter showed the tendency to be associated to invasion for cervical AdCa and to a worse prognosis for OPSCCs, suggesting a promising role as diagnostic/prognostic biomarker. The experiments of the third part were performed at the DKFZ in Heidelberg (Germany) and dealt with the analysis of the response to IFN-k transfection in HPV16-positive cervical cancer and head&neck carcinoma cell lines to evaluate its potential role as new treatment. After 24h, we observed increased IFN-b expression which lead to the up-regulation of genes involved in the antigens presentation pathway (MHC class I and immunoproteasome) and antiviral response as well, in particular in cervical cancer cell lines. This fact suggested also the presence of different HPV-mediated carcinogenic pathways between the two anatomical districts.

Swiss ambulatory care is characterized by independent, and primarily practice-based, physicians, receiving fee for service reimbursement. This study analyses supply sensitive services using ambulatory care claims data from mandatory health insurance. A first research question was aimed at the hypothesis that physicians with large patient lists decrease their intensity of services and bill less per patient to health insurance, and vice versa: physicians with smaller patient lists compensate for the lack of patients with additional visits and services. A second research question relates to the fact that several cantons are allowing physicians to directly dispense drugs to patients ('self-dispensation') whereas other cantons restrict such direct sales to emergencies only. This second question was based on the assumption that patterns of rescheduling patients for consultations may differ across channels of dispensing prescription drugs and therefore the hypothesis of different consultation costs in this context was investigated.

Symptom development during the prodromal phase of psychosis was explored retrospectively in first-episode psychosis patients with special emphasis on the assumed time-related syndromic sequence of "unspecific symptoms (UN)-predictive basic symptoms (BS)-attenuated psychotic symptoms (APS)-(transient) psychotic symptoms (PS)." Onset of syndromes was defined by first occurrence of any of their respective symptoms. Group means were inspected for time differences between syndromes and influence of sociodemographic and clinical characteristics on the recalled sequence. The sequence of "UN-BS/APS-PS" was clearly supported, and both BS and, though slightly less, APS were highly sensitive. However, onset of BS and APS did not show significant time difference in the whole sample (N = 126; 90% schizophrenia), although when each symptom is considered independently, APS tended to occur later than first predictive BS. On descriptive level, about one-third each recalled an earlier, equal and later onset of BS compared with APS. Level of education showed the greatest impact on the recall of the hypothesized sequence. Thereby, those with a higher school-leaving certificate supported the assumed sequence, whereas those of low educational background retrospectively dated APS before BS. These findings rather point out recognition and recall bias inherent to the retrospective design than true group characteristics. Future long-term prospective studies will have to explore this conclusively. However, as regards the criteria, the results support the notion of BS as at least a complementary approach to the ultrahigh risk criteria, which may also allow for an earlier detection of psychosis.

We employ the case of USAID's Linking Agricultural Markets with Producers (LAMP) project to address opportunities and obstacles to development assistance. Framed within LAMP's identification of constraints to growth within Bosnia's agricultural market, we explore the complex interorganizational linkages required for success. We identify three distinct linkage types inherent to development situations. Relationships exist (1) within the international community, (2) within the local Bosnian community and (3) between international and local organizations.

Recent evidence on malnutrition and poverty raise important questions on the role of food assistance policies and programs. In this review article, we examine evidence on the economic and nutritional impacts of international food assistance programs (FAPs) and policies. The returns on investments in FAPs are, on average, high but depend considerably on the targeting and cost structures as well as on food quality and role of complementary activities. We disaggregate findings into four classes of recipients. Returns to FAPs are highest for children under two. But, FAPs oriented towards early childhood interventions are less well funded than are interventions aimed at school-age children or at the broader, largely adult population even though available evidence indicates that these latter classes of interventions offer considerably lower average returns in economic, health, and nutrition terms. Nonetheless, FAP effectiveness in achieving any of several objectives varies with a range of key factors, including targeting, additionality, seasonality, timeliness, incentive effects, social acceptability and political economy considerations. The rise of new food assistance instruments, including local and regional procurement, cash, and vouchers, has surpassed increase in understanding of the tradeoffs among and impacts of these options relative to traditional food aid. Response choices rarely appear to result from systematic response analyses. Further, impacts along multiple dimensions-timeliness, cost-effectiveness, local market effects, recipient satisfaction, food quality, impact on smallholder suppliers, etc.-may be competing or synergistic. No single food assistance tool is always and everywhere preferable. A growing body of evidence, including the papers in this special section, nonetheless demonstrates the clear value-added of new food assistance instruments. (C) 2013 Elsevier Ltd. The objective of this study was to develop a criteria catalogue serving as a guideline for authors to improve quality of reporting experiments in basic research in homeopathy. A Delphi Process was initiated including three rounds of adjusting and phrasing plus two consensus conferences. European researchers who published experimental work within the last 5 years were involved. A checklist for authors provide a catalogue with 23 criteria. The “Introduction” should focus on underlying hypotheses, the homeopathic principle investigated and state if experiments are exploratory or confirmatory. “Materials and methods” should comprise information on object of investigation, experimental setup, parameters, intervention and statistical methods. A more detailed description on the homeopathic substances, for example, manufacture, dilution method, starting point of dilution is required. A further result of the Delphi process is to raise scientists' awareness of reporting blinding, allocation, replication, quality control and system performance controls. The part “Results” should provide the exact number of treated units per setting which were included in each analysis and state missing samples and drop outs. Results presented in tables and figures are as important as appropriate measures of effect size, uncertainty and probability. “Discussion” in a report should depict more than a general interpretation of results in the context of current evidence but also limitations and an appraisal of aptitude for the chosen experimental model. Authors of homeopathic basic research publications are encouraged to apply our checklist when preparing their manuscripts. Feedback is encouraged on applicability, strength and limitations of the list to enable future revisions.

Today's malaria control efforts are limited by our incomplete understanding of the biology of Plasmodium and of the complex relationships between human populations and the multiple species of mosquito and parasite. Research priorities include the development of in vitro culture systems for the complete life cycle of P. falciparum and P. vivax and the development of an appropriate liver culture system to study hepatic stages. In addition, genetic technologies for the manipulation of Plasmodium need to be improved, the entire parasite metabolome needs to be characterized to identify new druggable targets, and improved information systems for monitoring the changes in epidemiology, pathology, and host-parasite-vector interactions as a result of intensified control need to be established to bridge the gap between bench, preclinical, clinical, and population-based sciences.
