BACKGROUND The purpose of the present study is to evaluate the 10-year results following treatment of intrabony defects treated with an enamel matrix protein derivative (EMD) combined with either a natural bone mineral (NBM) or β-tricalcium phosphate (β-TCP). METHODS Twenty-two patients with advanced chronic periodontitis and displaying one deep intrabony defect were randomly treated with a combination of either EMD + NBM or EMD + β-TCP. Clinical evaluations were performed at baseline and at 1 and 10 years. The following parameters were evaluated: plaque index, bleeding on probing, probing depth, gingival recession, and clinical attachment level (CAL). The primary outcome variable was CAL. RESULTS The defects treated with EMD + NBM demonstrated a mean CAL change from 8.9 ± 1.5 mm to 5.3 ± 0.9 mm (P <0.001) and to 5.8 ± 1.1 mm (P <0.001) at 1 and 10 years, respectively. The sites treated with EMD + β-TCP showed a mean CAL change from 9.1 ± 1.6 mm to 5.4 ± 1.1 mm (P <0.001) at 1 year and 6.1 ± 1.4 mm (P <0.001) at 10 years. At 10 years two defects in the EMD + NBM group had lost 2 mm, whereas two other defects had lost 1 mm of the CAL gained at 1 year. In the EMD + β-TCP group three defects had lost 2 mm, whereas two other defects had lost 1 mm of the CAL gained at 1 year. Compared with baseline, at 10 years, a CAL gain of ≥3 mm was measured in 64% (i.e., seven of 11) of the defects in the EMD + NBM group and in 82% (i.e., nine of 11) of the defects in the EMD + β-TCP group. No statistically significant differences were found between the 1- and 10-year values in either of the two groups. Between the treatment groups, no statistically significant differences in any of the investigated parameters were observed at 1 and 10 years. CONCLUSION Within their limitations, the present findings indicate that the clinical improvements obtained with regenerative surgery using EMD + NBM or EMD + β-TCP can be maintained over a period of 10 years.

Future clinical trials investigating the natural history and treatment of femoroacetabular impingement (FAI) will require multimodal staging systems for hip osteoarthritis because the optimal system will differ based on the size of the study population, the specific objective in question, and the time frame in which the investigator expects to see the specified end point. Plain radiographs are readily available, low in cost, and of unquestioned validity, but they are relatively insensitive to early joint damage. MRI allows assessment of both bony and soft-tissue pathology within the joint, and it is much more sensitive for early joint damage because cartilage is visualized directly. Biochemical imaging techniques such as delayed gadolinium-enhanced MRI of cartilage, T2 mapping, and T1rho offer the potential to identify biochemical damage to cartilage before the onset of irreversible tissue loss. In the future, biomarkers may allow earlier detection of osteoarthritis before the development of radiographic evidence of disease.

Purpose Skeletal-related events represent a substantial burden for patients with advanced cancer. Randomized, controlled studies suggested superiority of denosumab over zoledronic acid in the prevention of skeletal-related events in metastatic cancer patients, with a favorable safety profile. Experts gathered at the 2012 Skeletal Care Academy in Istanbul to bring forward practical recommendations, based on current evidence, for the use of denosumab in patients with bone metastases of lung cancer. Recommendations Based on current evidence, use of denosumab in lung cancer patients with confirmed bone metastases is recommended. It is important to note that clinical judgment should take into consideration the patient’s general performance status, overall prognosis, and live expectancy. Currently, the adverse event profile reported for denosumab includes hypocalcemia and infrequent occurrence of osteonecrosis of the jaw. Therefore, routine calcium and vitamin D supplementation, along with dental examination prior to denosumab initiation are recommended. There is no evidence for renal function impairment due to denosumab administration. At present, there is no rationale to discourage concomitant use of denosumab and surgery or radiotherapy.

America’s low-income families struggle to protect their children from multiple threats to their health and growth. Many research and advocacy groups explore the health and educational effects of food insecurity, but less is known about these effects on very young children. Children’s HealthWatch, a group of pediatric clinicians and public health researchers, has continuously collected data on the effects of food insecurity alone and in conjunction with other household hardships since 1998. The group’s peer reviewed research has shown that a number of economic risks at the household level, including food, housing and energy insecurity, tend to be correlated. These insecurities alone or in conjunction increase the risk that a young child will suffer various negative health consequences, including increases in lifetime hospitalizations, parental report of fair or poor health,1 or risk for developmental delays.2 Child food insecurity is an incremental risk indicator above and beyond the risk imposed by household-level food insecurity. The Children’sHealthwatch research also suggests public benefits programs modify some of these effects for families experiencing hardships. This empirical evidence is presented in a variety of public venues outside the usual scientific settings, such as congressional hearings, to support the needs of America’s most vulnerable population through policy change. Children’s HealthWatch research supports legislative solutions to food insecurity, including sustained funding for public programs and re-evaluation of the use of the Thrifty Food Plan as the basis of SNAP benefits calculations. Children’s HealthWatch is one of many models to support the American Academy of Pediatrics’ call to “stand up, speak up, and step up for children.”3 No isolated group or single intervention will solve child poverty or multiple hardships. Aids steht für die letzte grosse Krise der westlichen Welt im ausgehenden 20. Jahrhundert. Heute hat sich die Situa­tion normalisiert: Aus der verheerenden Seuche ist eine zwar ernste, doch einschätzbare Krankheit geworden. Im Rückblick zeigen sich die dreissig Jahre des gesellschaftlichen Umgangs mit Aids als dicht gedrängte Zeit, in der der Umgang mit der neuen, höchst bedrohlich erscheinenden Krankheit ausgehandelt wurde. Der Band zeichnet die Entwicklung des Aids-Diskurses im deutschsprachigen Raum von den Anfängen in den 1980er Jahren bis zur Gegenwart nach. In der Rückschau werden die dominanten Strömungen und Gegenströmungen charakterisiert und die entscheidenden Drehpunkte des Diskurses akzentuiert. Besonderes Augenmerk gilt dem Beitrag von Literatur, Theater und Film zur gesellschaftlichen Verarbeitung von Aids. Die systematische Analyse macht die komplexen Wechselverhältnisse zwischen den Massen­medien, den fiktionalen Gattungen sowie der medizinischen Kommunikation sichtbar. Die regelmässige Wiederkehr von epidemischen Szenarien – BSE, SARS, Vogel- und Schweinegrippe und jüngst EHEC – zeigt, dass ein prüfender Blick auf die sich wiederholenden dramaturgischen Muster der Auseinandersetzung mit ansteckenden Krankheiten nottut. In diesem Zusammenhang trägt das gleichermassen auf wissenschaftliche Genauigkeit wie auf Anschaulichkeit und Allgemeinverständlichkeit angelegte Buch zur kritischen Reflexion der jüngsten Zeitgeschichte bei.

Gegenstand / Untersuchungskorpus Die massenmediale Aufbereitung von Aids ist bereits seit den Anfängen der geisteswissenschaftlichen Beschäftigung mit diesem Thema ein zentraler Gegenstand kritischer Betrachtung. Demgegenüber stellt sich die systematische Erforschung des Beitrages von eher fiktionalen Gattungen zur gesellschaftlichen Verarbeitung von Aids ein Forschungsdesiderat dar. Die Dissertation „Dramaturgie der Seuche“ schliesst mit der Fokussierung auf Literatur, Theater und Film diese Lücke. Die dezidiert interdisziplinäre Auswahl des Untersuchungskorpus’ liefert eine Zusammenschau der Funktionen, die diese Gattungen im Laufe der Zeit innerhalb des Aids-Diskurses übernommen haben. Die Arbeit zeigt die komplexen Wechselverhältnisse zwischen den Massenmedien, den fiktionalen Gattungen sowie der medizinischen Kommunikation auf. Fragedesign auf der Höhe der aktuellen Forschung Gegenwärtig ist erneut ein Interesse kultur- und sozialwissenschaftlicher Disziplinen am Thema Aids zu beobachten. Eine junge Forschergeneration nimmt sich des Themas an und beleuchtet die Auseinandersetzung mit Aids an Hand neuer Fragestellungen und mit neuen Methoden. Im Mittelpunkt dieser wissenschaftlichen Auseinandersetzung stehen die reflektierte Historisierung und Kontextualisierung des Aids-Diskurses seit den 1980er-Jahren. Die Dissertation „Dramaturgie der Seuche“ positioniert sich mit ihrer Frage nach der Struktur und der Entwicklung der gesellschaftlichen Auseinandersetzung mit Aids seit den Anfängen bis zur Gegenwart innerhalb dieses aktuellen Forschungsfeldes. Die Herangehensweise unterscheidet sich damit deutlich von den Forschungen der 1990er-Jahre, die eher von Betroffenheit und/oder unmittelbarer Kritik am Aids-Diskurs in den Massenmedien geprägt war. Zugleich verschafft die kritische Re-Lektüre der zentralen Publikationen zum Thema Aids, etwa von Susan Sontag oder Sander L. Gilman, diesen eine kritische Aktualisierung. Innovatives Methodendesign Um dem interdiszplinären Korpus und der kulturwissenschaftlich inspirierten Fragestellung gerecht zu werden, entwirft die Dissertation ein innovatives Methodendesign, das diskursanalytische und systemtheoretische Ansätze mit theater-, literatur- und filmwissenschaftlichen Analyseinstrumenten synthetisiert. Dieses leistet in der Anwendung sowohl eine präzise und adäquate Tiefenanalyse der untersuchten Texte, Bilder und Filme als auch eine Auswertung dieser Ergebnisse auf einer abstrakteren Ebene, die die komplexe Struktur der Entwicklung des Aids-Diskurses seit den 1980er-Jahren bis heute überzeugend und anschaulich darlegt. Das entworfene Methodendesign lässt sich auf andere Gegenstände anwenden und verspricht ebenso präzise wie innovative Ergebnisse. Ergebnisse: Nutzen für die Öffentlichkeit Die analytische Auseinandersetzung mit der letzten grossen Seuche innerhalb der westlichen Welt birgt nicht nur in der Rückschau auf die letzten Dekaden einen Mehrwert für die Öffentlichkeit. Die regelmässige Wiederkehr von epidemischen Szenarien – BSE, SARS, Vogel- und Schweinegrippe und jüngst EHEC – zeigt, dass ein kritischer Blick auf die sich wiederholenden dramaturgischen Muster des Redens über ansteckende Krankheiten nottut. Die Dissertation „Dramaturgie der Seuche“ trägt dazu bei, die Muster des Seuchendiskurses zu erkennen und reflektiert und kritisch mit der Berichterstattung in den Medien wie auch mit den kursierenden Urban Legends umzugehen. Der Aufbau der Argumentation und der sprachliche Stil verbinden wissenschaftliche Genauigkeit mit Allgemeinverständlichkeit. Dadurch wird die Arbeit breit rezipierbar.

PURPOSE The purpose of this study was to investigate whether there are microbiological differences in bacterial samples collected from labial piercings made of different materials. METHODS Sterile piercings of 4 materials were randomly allocated to 80 pierced subjects. After 2 weeks, microbiologic samples were collected and processed by checkerboard DNA-DNA hybridization methods. Wilcoxon signed ranks and Mann-Whitney tests were used for statistical analysis (adjustment for multiple comparisons). RESULTS There were no statistically significant differences between material groups in relation to baseline data. In samples from stainless steel piercings, the total microbial load was significantly higher than the other materials (P<.05). Ten (mainly periopathogenic) species were found at significantly higher levels (P<.001) on steel than on polypropylene and/or polytetrafluoroethylene piercings. CONCLUSIONS Labial piercings made of stainless steel could promote the development of a pathogenic biofilm.

STUDY OBJECTIVES: To describe the time structure of leg movements (LM) in obstructive sleep apnea (OSA) syndrome, in order to advance understanding of their clinical significance. LOCATION: Sleep Research Centre, Oasi Institute (IRCCS), Troina, Italy. SETTING: Sleep laboratory. PATIENTS: Eighty-four patients (16 females, 68 males, mean age 55.1 y, range 29-74 y). METHODS: Respiratory-related leg movements (RRLM) and those unrelated to respiratory events (NRLM) were examined within diagnostic polysomnograms alone and together for their distributions within the sleep period and for their periodicity. MEASUREMENTS AND RESULTS: Patients with OSA and RRLM exhibited more periodic leg movements in sleep (PLMS), particularly in NREM sleep. A gradual decrease in number of NRLM across the sleep period was observed in patients with RRLM. This pattern was less clear for RRLM. Frequency histograms of intermovement intervals of all LMs in patients with RRLM showed a prominent first peak at 4 sec, and a second peak at approximately 24 sec coincident with that of PLMS occurring in the absence of OSA. A third peak of lowest amplitude was the broadest with a maximum at approximately 42 sec. In patients lacking RRLM, NRLM were evident with a single peak at 2-4 sec. A stepwise linear regression analysis showed that, after controlling for a diagnosis of restless legs syndrome and apnea-hypopnea index, PLMS remained significantly associated with RRLM. CONCLUSION: The time structure of leg movements occurring in conjunction with respiratory events exhibit features of periodic leg movements in sleep occurring alone, only with a different and longer period. This brings into question the validity, both biologic and clinical, of scoring conventions with their a priori exclusion from consideration as periodic leg movements in sleep.

The discussion on the New Philology triggered by French and North American scholars in the last decade of the 20th century emphasized the material character of textual transmission inside and outside the written evidences of medieval manuscripts by downgrading the active role of the historical author. However, the reception of the ideas propagated by the New Philology adherents was rather divided. Some researchers questioned its innovative status (K. Stackmann: “Neue Philologie?”), others saw a new era of the “powers of philology” evoked (H.-U. Gumbrecht). Besides the debates on the New Philology another concept of textual materiality strengthened in the last decade, maintaining that textual alterations somewhat relate to biogenetic mutations. In a matter of fact, phenomena such as genetic and textual variation, gene recombination and ‘contamination’ (the mixing of different exemplars in one manuscript text) share common features. The paper discusses to what extent the biogenetic concepts can be used for evaluating manifestations of textual production (as the approach of ‘critique génétique’ does) and of textual transmission (as the phylogenetic analysis of manuscript variation does). In this context yet the genealogical concept of stemmatology – the treelike representation of textual development abhorred by the New Philology adepts – might prove to be useful for describing the history of texts. The textual material to be analyzed will be drawn from the Parzival Project, which is currently preparing a new electronic edition of Wolfram von Eschenbach’s Parzival novel written shortly after 1200 and transmitted in numerous manuscripts up to the age of printing (www.parzival.unibe.ch). Researches of the project have actually resulted in suggesting that the advanced knowledge of the manuscript transmission yields a more precise idea on the author’s own writing process.

BACKGROUND The purpose of this study is to compare clinical outcomes in the treatment of deep non-contained intrabony defects (i.e., with ≥70% 1-wall component and a residual 2- to 3-wall component in the most apical part) using deproteinized bovine bone mineral (DBBM) combined with either enamel matrix protein derivative (EMD) or collagen membrane (CM). METHODS Forty patients with multiple intrabony defects were enrolled. Only one non-contained defect per patient with an intrabony depth ≥3 mm located in the interproximal area of single- and multirooted teeth was randomly assigned to the treatment with either EMD + DBBM (test: n = 20) or CM + DBBM (control: n = 20). At baseline and after 12 months, clinical parameters including probing depth (PD) and clinical attachment level (CAL) were recorded. The primary outcome variable was the change in CAL between baseline and 12 months. RESULTS At baseline, the intrabony component of the defects amounted to 6.1 ± 1.9 mm for EMD + DBBM and 6.0 ± 1.9 mm for CM + DBBM sites (P = 0.81). The mean CAL gain at sites treated with EMD + DBBM was not statistically significantly different (P = 0.82) compared with CM + DBBM (3.8 ± 1.5 versus 3.7 ± 1.2 mm). No statistically significant difference (P = 0.62) was observed comparing the frequency of CAL gain ≥4 mm between EMD + DBBM (60%) and CM + DBBM (50%) or comparing the frequency of residual PD ≥6 mm between EMD + DBBM (5%) and CM + DBBM (15%) (P = 0.21). CONCLUSION Within the limitations of the present study, regenerative therapy using either EMD + DBBM or CM + DBBM yielded comparable clinical outcomes in deep non-contained intrabony defects after 12 months.
