Introdução: O câncer consiste em um problema de saúde publica mundial, com estimativa de 27 milhões de casos novos e 17 milhões de mortes por câncer no ano de 2030. No Brasil, as estimativas para o câncer no ano de 2014, apontam a ocorrência de aproximadamente 580 mil casos novos. As fluoropirimidinas são usadas nos principais esquemas quimioterápicos direcionados a tumores do trato gastrointestinal. Nos últimos anos muito se tem investigado sobre causas de respostas individuais diferenciadas em relação ao tratamento quimioterápico; dessa forma têm-se buscado uma terapia individualizada que possa maximizar a eficácia dos medicamentos e minimizar os efeitos adversos associados aos fármacos. Objetivamos buscar a associação do INDEL (rs16430) do gene TYMS com o padrão de resposta ao tratamento oncológico de fármacos com base em fluoropirimidinas, de maneira a contribuir para o desenvolvimento da medicina personalizada. Material: Estudadas 151 amostras de sangue periférico de pacientes oncológicos tratados com fluoropirimidinas, da população da região amazônica brasileira com elevado grau de mistura interétinica. Foi genotipado um polimorfismo INDEL (rs16430) no gene TYMS envolvido na resposta ao tratamento com uso de fluoropirimidinas. Resultados: A investigação relatou que a maioria dos pacientes tinha doença avançada no momento do diagnóstico; 32,7% receberam tratamento com intenção paliativa; e 22,8% tratamento neoadjuvante. Nossos resultados evidenciam que o polimorfismo INDEL no gene TYMS demonstrou ter um efeito de proteção à progressão tumoral (p=0,033). Pacientes tratados com fluoropirimidinas que eram homozigotos selvagens (INS/INS) apresentaram uma proteção à progressão tumoral de 24% comparado com outros genótipos desse polimorfismo. As estimativas de ancestralidade genômica global da amostra investigada foram: 62,4% europeia; 25,2% nativo americana e 12,4% africana. Foi possível estabelecer uma correlação inversa entre o aumento da ancestralidade ameríndia e a presença de metástase (p=0,024). Conclusão: Estudos farmacogenéticos podem proporcionar uma terapia personalizada reduzindo mortalidade por toxicidades e aumentando a eficácia terapêutica, desta forma proporcionando um tratamento oncológico com melhores resultados clínicos.

PURPOSE: To analyze the time between the first symptom and treatment in patients treated for breast cancer in public hospitals in the Federal District. METHODS: This was a cross-sectional analysis. We interviewed 250 women diagnosed with breast cancer treated in six hospitals of the State Department of Health of the Federal District from November 2009 to January 2011. The time intervals studied were the time between the detection of the symptoms and treatment subdivided into intervals until and after the first medical appointment. The variables were: age, menopausal status, color, educational level, average monthly household income, origin, reason for the initial consultation, staging, tumor size, laterality, metastasis to axillary lymph nodes, neoadjuvant chemotherapy, and type of surgery. The Mann-Whtney test was used to assess the association of these variables with the time intervals until treatment. RESULTS: The mean age was 52 years, with a predominance of white women (57.6%), from the Federal District (62.4%), with a family income of up to 2 minimum wages (78%), and up to four years of schooling (52.4%). The staging of the disease ranged from II to IV in 78.8% of the women. The time between the first symptom and treatment was 229 days (median). After detection of the first symptom, 52.9% of the women attended a consultation within 30 days and 88.8% took more than 90 days to start treatment. Women with elementary school education had a greater delay to the start of treatment (p=0.049). CONCLUSIONS: There was a significant delay to start treatment of women with breast cancer in public hospitals of the Federal District, suggesting that efforts should be made to reduce the time needed to schedule medical appointments and to diagnose and treat these patients.

Radiotherapy is a multidisciplinary speciality which uses complex equipment and radiation sources for delivery of treatment, using high-energy ionizing radiation to treat cancer at several stages of complexity. Since radiation therapy is a technique which involves a precalculated radiation dose, it shall be established quality assurance programs that provide an efficient and safety treatment. The International Commission on Radiation Units and Measurements (ICRU) report No. 50 has recommended dose uniformity between 5% of the prescribed dose throughout the region of interest. This is one of the most primordial points that justify the importance of a suitable attendance of the equipments quality and performance. For quality control, the medical physicist will be involved with establishing and running a Quality Control Program (QCP). He must adapt or develop the procedures of equipment acceptance and commissioning, besides verifying the use of principles and accepted protocols of national and international reports to assure the correct quality, quantity, and placement of radiation during the performance of a radiological procedure, establishing adequate protocols to ensure accurate patient dosimetry. This present work consists of a description of the activities carried through the Sectors of Radiation Therapy of the Hospital of Clinics of the Campinas State University (Unicamp), particularly in the implementation of the Quality Control Program

Purpose: The antitumor activity of Kielmeyera coriacea (Clusiaceae), a medicinal plant used in the treatment of parasitic, as well as fungal and bacterial infections by the Brazilian Cerrado population, was investigated. Methods: A chloroform extract (CE) of K. coriacea was tested in the murine melanoma cell line (B16F10-Nex2) and a panel of human tumor cell lines. Tumor cell migration was determined by the wound-healing assay and the in vivo antitumor activity of CE was investigated in a melanoma cell metastatic model. 1H NMR and GC/MS were used to determine CE chemical composition. Results: We found that CE exhibited strong cytotoxic activity against murine melanoma cells and a panel of human tumor cell lines in vitro. CE also inhibited growth of B16F10- Nex2 cells at sub lethal concentrations, inducing cell cycle arrest at S phase, and inhibition of tumor cell migration. Most importantly, administration of CE significantly reduced the number of melanoma metastatic nodules in vivo. Chemical analysis of CE indicated the presence of the long chain fatty compounds, 1-eicosanol, 1-docosanol, and 2-nonadecanone as main constituents. Conclusion: These results indicate that K. coriacea is a promising medicinal plant in cancer therapy exhibiting antitumor activity both in vitro and in vivo against different tumor cell lines.

In order to evaluate the quality of life of patients with head and neck cancer, this study analyzed data of 24 patients with squamous cell carcinoma, which indicated therapy was radiotherapy or not be combined with chemotherapy and surgery. The study was conducted in the Unit of Radiotherapy of Megavoltage located in the São José de Rio Preto-SP, in the period August 2007 to January 2008. Then, it was used the questionnaire of quality of life from University of Washington which enabled the identification of different quality of life patterns associated with the different stages of radiotherapy, indicating to be viable the prospect of recognition of prognostic factors of reduction in multiple domains of quality of life. From the data collected and analyzed, it was identified that the areas with the worst score in the begin of radiotherapy were appearance, speech and anxiety; during the treatment were taste, saliva and anxiety; and in the end were taste, saliva and swallowing. Throughout the treatment, it was observed the deterioration of patients' mood. In this regard, emphasizes the importance of dental and psychological follow-up, within the framework of a multidisciplinary care for patients with head and neck cancer during radiotherapy treatment.

Engineered nanomaterials have been extensively applied as active materials for technological applications. Since the impact of these nanomaterials on health and environment remains undefined, research on their possible toxic effects has attracted considerable attention. It is known that in humans, for example, the primary site of gold nanoparticles (AuNps) accumulation is the liver. The latter has motivated research regarding the use of AuNps for cancer therapy, since specific organs can be target upon appropriate functionalization of specific nanoparticles. In this study, we investigate the geno and cytotoxicity of two types of AuNps against human hepatocellular carcinoma cells (HepG2) and peripheral blood mononuclear cells (PBMC) from healthy human volunteers. The cells were incubated in the presence of different concentrations of AuNps capped with either sodium citrate or polyamidoamine dendrimers (PAMAM). Our results suggest that both types of AuNps interact with HepG2 cells and PBMC and may exhibit in vitro geno and cytotoxicity even at very low concentrations. In addition, the PBMC were less sensitive to DNA damage toxicity effects than cancer HepG2 cells upon exposure to AuNps. (C) 2012 Elsevier Ireland Ltd. All rights reserved.

OBJETIVO: Descrever uma série de pacientes portadores de obstrução do sistema lacrimal associado à radioiodoterapia para tratamento de carcinoma de tireoide, revisar os dados clínicos e a resposta ao tratamento cirúrgico desta rara complicação. MÉTODOS: Foi realizada uma análise retrospectiva dos achados oftalmológicos de pacientes com histórico de carcinoma de tireoide previamente submetidos à tireoidectomia e à RIT que foram encaminhados para cirurgia de vias lacrimais. RESULTADOS: Dezessete pacientes com carcinoma de tireoide tratados com tireoidectomia e RIT apresentaram obstrução do ducto nasolacrimal sintomática após período médio de 13,2 meses do tratamento do câncer. Onze pacientes tiveram epífora bilateral, 8 com mucocele de saco lacrimal. A idade dos pacientes variou entre 30 e 80 anos, sendo 10 com idade menor ou igual a 49 anos. A dose cumulativa média de radioiodo administrada foi de 571 mCi (variação entre 200-1200 mCi). Sintomas de obstrução nasal e aumento de glândulas salivares ocorreram em 53% dos pacientes. Todos os pacientes foram submetidos à dacriocistorrinostomia. Observou-se ainda que nos 3 pacientes mais jovens houve maior sangramento intraoperatótio e dilatação de saco lacrimal. A resolução completa da epífora e da dacriocistite ocorreu em 82,4%, e foi parcial em 17,6% (3 pacientes mantiveram queixa unilateral após a correção da obstrução bilateralmente). O seguimento médio foi de 6 meses (intervalo: 2-24 meses). CONCLUSÕES: Alta dose cumulativa de radioiodo, disfunção nasal e de glândulas salivares estão associadas à obstrução das vias lacrimais. Observa-se uma maior porcentagem de pacientes mais jovens apresentando quadro de dacriocistite quando comparado à dacrioestenose idiopática. A absorção de iodo radioativo pela mucosa do ducto nasolacrimal com subsequente inflamação, edema e fibrose parece ter relação direta com a obstrução do ducto nasolacrimal. O conhecimento desta complicação é importante para o estudo e abordagem correta desses pacientes.

Langerhans cell histiocytosis (LCH) is a rare disorder that can affect almost any organ, including bone. Treatment options include local corticosteroid infiltration in isolated bone lesions and oral corticosteroids and chemotherapy in multifocal bone lesions. Several studies show local corticosteroid injection in unifocal bone lesions heal in more than 75% of patients with minimal side effects. Therefore, it is unclear whether chemotherapy adds materially to the healing rate. We therefore compared overall survival, remission rate, and recurrence rate in patients with bone LCH treated with chemotherapy and corticosteroids or corticosteroids alone. We retrospectively reviewed the records of 198 patients with LCH since 1950. Median age at diagnosis was 5 years, male-to-female ratio was 1.33, and the most frequent symptom was local pain (95%). We recorded the disease presentation, demographics, treatment, and clinical evolution of each patient. Minimum followup was 4 months (median, 24 months; range, 4-360 months). The survival rate of the systemic disease group was 76.5% (65 of 85) while the survival rate in the unifocal and multifocal bone involvement groups was 100% at a median 5-year followup. All patients with unifocal bone involvement and 40 of 43 (93%) with multifocal bone involvement had complete remission. One of 30 patients with multifocal bone involvement treated with chemotherapy and oral corticosteroids did not achieve remission whereas two of six receiving only corticosteroids did not achieve remission. Our observations suggest intralesional corticosteroid injection without adjunctive chemotherapy achieves remission in unifocal bone LCH but may not do so in multifocal single-system bone involvement. Hematopoietic cell transplantation (HCT) is an emerging therapy for patients with severe autoimmune diseases (AID). We report data on 368 patients with AID who underwent HCT in 64 North and South American transplantation centers reported to the Center for International Blood and Marrow Transplant Research between 1996 and 2009. Most of the HCTs involved autologous grafts (n = 339); allogeneic HCT (n = 29) was done mostly in children. The most common indications for HCT were multiple sclerosis, systemic sclerosis, and systemic lupus erythematosus. The median age at transplantation was 38 years for autologous HCT and 25 years for allogeneic HCT. The corresponding times from diagnosis to HCT were 35 months and 24 months. Three-year overall survival after autologous HCT was 86% (95% confidence interval [CI], 81%-91%). Median follow-up of survivors was 31 months (range, 1-144 months). The most common causes of death were AID progression, infections, and organ failure. On multivariate analysis, the risk of death was higher in patients at centers that performed fewer than 5 autologous HCTs (relative risk, 3.5; 95% CI, 1.1-11.1; P = .03) and those that performed 5 to 15 autologous HCTs for AID during the study period (relative risk, 4.2; 95% CI, 1.5-11.7; P = .006) compared with patients at centers that performed more than 15 autologous HCTs for AID during the study period. AID is an emerging indication for HCT in the region. Collaboration of hematologists and other disease specialists with an outcomes database is important to promote optimal patient selection, analysis of the impact of prognostic variables and long-term outcomes, and development of clinical trials.
