A 34-year-old obese male (96.8 kg; BMI, 30.2 kg m⁻¹) volitionally undertook a 50-day fast with the stated goal of losing body mass. During this time, only tea, coffee, water, and a daily multivitamin were consumed. Severe and linear loss of body mass is recorded during these 50 days (final 75.4 kg; BMI, 23.5 kg m⁻¹). A surprising resilience to effects of fasting on activity levels and physical function is noted. Plasma samples are suggestive of early impairment of liver function, and perturbations to cardiovascular dynamics are also noted. One month following resumption of feeding behavior, body weight was maintained (75.0 kg; BMI, 23.4 kg m⁻¹). Evidence-based decision-making with the fasting or hunger striking patient is limited by a lack of evidence. This case report suggests that total body mass, not mass lost, may be a key observation in clinical decision-making during fasting and starvation.

BACKGROUND: The role of statin therapy in heart failure (HF) is unclear. The amino-terminal propeptide of procollagen type III (PIIINP) predicts outcome in HF, and yet there are conflicting reports of statin therapy effects on PIIINP.

OBJECTIVES: This study determined whether there was an increase in serum markers of inflammation, fibrosis (including PIIINP), and B-type natriuretic peptide (BNP) in patients with systolic HF and normal total cholesterol and determined the effects of long-term treatment with atorvastatin on these markers.

METHODS: Fifty-six white patients with systolic HF and normal cholesterol levels (age 72 [13] years; 68% male; body mass index 27.0 [7.3] kg/m(2); ejection fraction 35 [13]%; 46% with history of smoking) were randomly allocated to atorvastatin treatment for 6 months, titrated to 40 mg/d (A group) or not (C group). Age- and/or sex-matched subjects without HF (N group) were also recruited. Biomarkers were measured at baseline (all groups) and 6 months (A and C groups).

RESULTS: Serum markers of collagen turnover, inflammation, and BNP were significantly elevated in HF patients compared with normal participants (all P < 0.05). There were correlations between these markers in HF patients but not in normal subjects. Atorvastatin treatment for 6 months caused a significant reduction in the following biomarkers compared with baseline: BNP, from median (interquartile range) 268 (190-441) pg/mL to 185 (144-344) pg/mL; high-sensitivity C-reactive protein (hs-CRP), from 5.26 (1.95 -9.29) mg/L to 3.70 (2.34-6.81) mg/L; and PIIINP, from 4.65 (1.86) to 4.09 (1.25) pg/mL (all P < 0.05 baseline vs 6 months). Between-group differences were significant for PIIINP only (P = 0.027). There was a positive interaction between atorvastatin effects and baseline hs-CRP and PIIINP (P < 0.01).

CONCLUSIONS: Long-term statin therapy reduced PIIINP in this small, selected HF population with elevated baseline levels. Further evaluation of statin therapy in the management of HF patients with elevated PIIINP is warranted.

OBJECTIVES: This study was designed to evaluate the impact of eplerenone on collagen turnover in preserved systolic function heart failure (HFPSF).

BACKGROUND: Despite growing interest in abnormal collagen metabolism as a feature of HFPSF with diastolic dysfunction, the natural history of markers of collagen turnover and the impact of selective aldosterone antagonism on this natural history remains unknown.

METHODS: We evaluated 44 patients with HFPSF, randomly assigned to control (n = 20) or eplerenone 25 mg daily (n = 24) for 6 months, increased to 50 mg daily from 6 to 12 months. Serum markers of collagen turnover and inflammation were analyzed at baseline and at 6 and 12 months and included pro-collagen type-I and -III aminoterminal peptides, matrix metalloproteinase type-2, interleukin-6 and -8, and tumor necrosis factor-alpha. Doppler-echocardiographic assessment of diastolic filling indexes and tissue Doppler analyses were also obtained.

RESULTS: The mean age of the patients was 80 +/- 7.8 years; 46% were male; 64% were receiving an angiotensin-converting enzyme inhibitor, 34% an angiotensin-II receptor blocker, and 68% were receiving beta-blocker therapy. Pro-collagen type-III and -I aminoterminal peptides, matrix metalloproteinase type-2, interleukin-6 and -8, and tumor necrosis factor-alpha increased with time in the control group. Eplerenone treatment had no significant impact on any biomarker at 6 months but attenuated the increase in pro-collagen type-III aminoterminal peptide at 12 months (p = 0.006). Eplerenone therapy was associated with modest effects on diastolic function without any impact on clinical variables or brain natriuretic peptide.

CONCLUSIONS: This study demonstrates progressive increases in markers of collagen turnover and inflammation in HFPSF with diastolic dysfunction. Despite high background utilization of renin-angiotensin-aldosterone modulators, eplerenone therapy prevents a progressive increase in pro-collagen type-III aminoterminal peptide and may have a role in management of this disease. The possibility of the EU member states to adapt copyright legislation to new circumstances and to address unforeseen issues is limited by the list of exceptions and restrictions of the InfoSoc Directive. In spite of this constraint, the EU copyright framework provides for a possibility of introduction of non-voluntary forms of collective rights management that can help to tackle some of the contemporary problems with remuneration and access. This article is an attempt to deepen the understanding of non-voluntary collective management and its possible use. First, it provides a detailed description of the French mechanism adopted for facilitating mass digitization and making out-of-commerce books available, which was implemented through a new form of collective management of copyright. Then, it examines the mechanism’s compatibility with the InfoSoc Directive through comparison with the extended collective licensing.

Two-third of the terrestrial C is stored in soils, and more than 50% of soil organic C (SOC) is stored in subsoils from 30 – 100 cm. Hence, subsoil is important as a source or sink for CO2 in the global carbon cycle. Especially the stable organic carbon (OC) is stored in subsoil, as several studies have shown that subsoil OC is of a higher average age than topsoil OC. However, there is still a lack of knowledge regarding the mechanisms of C sequestration and C turnover in subsoil. Three main factors are discussed, which possibly reduce carbon turnover rates in subsoil: Resource limitation, changes in the microbial community, and changes in gas conditions. The experiments conducted in this study, which aimed to elucidate the importance of the mentioned factors, focused on two neighbouring arable sites, with depth profiles differing in SOC stocks: One Colluvic Cambisol (Cam) with high SOC contents (8-12 g kg-1) throughout the profile and one Haplic Luvisol (Luv) with low SOC contents (3-4 g kg-1) below 30 cm depth. The first experiment was designed to gain more knowledge regarding the microbial community and its influence on carbon sequestration in subsoil. Soil samples were taken at four different depths on the two sites. Microbial biomass C (MBC) was determined to identify depth gradients in relation to the natural C availability. Bacterial and fungal residues as well as ergosterol were determined to quantify changes in the in the microbial community composition. Multi-substrate-induced-respiration (MSIR) was used to identify shifts in functional diversity of the microbial community. The MSIR revealed that substrate use in subsoil differed significantly from that in topsoil and also differed highly between the two subsoils, indicating a strong influence of resource limitations on microbial substrate use. Amino sugar analysis and the ratio of ergosterol to microbial biomass C showed that fungal dominance decreased with depth. The results clearly demonstrated that microbial parameters changed with depth according to substrate availability. The second experiment was an incubation experiment using subsoil gas conditions with and without the addition of C4 plant residues. Soil samples were taken from topsoil and subsoil of the two sites. SOC losses during the incubation, were not influenced by the subsoil gas conditions. Plant-derived C losses were generally stronger in the Cam (7.5 mg g-1), especially at subsoil gas conditions, than in the Luv (7.0 mg g-1). Subsoil gas conditions had no general effects on microbial measures with and without plant residue addition. However, the contribution of plant-derived MBC to total MBC was significantly reduced at subsoil gas conditions. This lead to the conclusion that subsoil gas conditions alter the metabolism of microorganisms but not the degradation of added plant residues is general. The third experiment was a field experiment carried out for two years. Mesh bags containing original soil material and maize root residues (C4 plant) were buried at three different depths at the two sites. The recovery of the soilbags took place 12, 18, and 24 months after burial. We determined the effects of these treatments on SOC, density fractions, and MBC. The mean residence time for maize-derived C was similar at all depths and both sites (403 d). MBC increased to a similar extent (2.5 fold) from the initial value to maximum value. This increase relied largely on the added maize root residues. However, there were clear differences visible in terms of the substrate use efficiency, which decreased with depth and was lower in the Luv than in the Cam. Hence freshly added plant material is highly accessible to microorganisms in subsoil and therefore equally degraded at both sites and depths, but its metabolic use was determined by the legacy of soil properties. These findings provide strong evidence that resource availability from autochthonous SOM as well as from added plant residues have a strong influence on the microbial community and its use of different substrates. However, under all of the applied conditions there was no evidence that complex substrates, i.e. plant residues, were less degraded in subsoil than in topsoil.

This report was commissioned by the Department of Health, Ireland. Obesity is a public health problem in Ireland which is largely responsible for the increasing prevalence of diet-related diseases and growing financial burden on our healthcare system. Although overweight and obesity rates may have reached a plateau in Irish adults and children, they remain at an extremely high level as 1 in 4 children areoverweight or obese and an estimated 61% of adults are overweight or obese. Urgent public health action is required to reduce the levels of obesity among our children and adults. A sustainable national intervention strategy that combines government and community-led interventions is required. These interventions need to incorporate both nutrition education and environmental modification strategies to reduce levels of obesity. International literature suggests that calorie posting has the potential to have a positive effect on the obesity crisis by encouraging people to make healthier food choices through informed consumer decisions. This evaluation focuses on the uptake of voluntary calorie posting from a national representative sample of food service businesses in Ireland and explores the attitudes of food service businesses that do and do not display calories. This evaluation will explore the most effective and efficient way of implementing mandatory calorie posting on menus in Ireland.

Introdução: São poucos os estudos que em Portugal se dedicaram a explorar constructos como o autocriticismo, autocompaixão, saúde mental e o comprometimento organizacional. São nossos objetivos analisar estas associações na Polícia de Segurança Pública, considerando igualmente a influência de variáveis sociodemográficas e profissionais, bem como realizar análises preditivas que permitam verificar quais são as variáveis preditores do comprometimento organizacional. Metodologia: A amostra é constituída por 188 polícias com idades compreendidas entre os 30 e 56 anos (M = 43,75, DP = 5,66). Os participantes preencheram um protocolo integrante do projeto Organizar Positivamente do Instituto Superior Miguel Torga, composto por um questionário sociodemográfico, a SELFCS (Escala da Autocompaixão), a FSCRS (Escala das Formas do Autocriticismo e Autotranquilização), o MHI-5 (Mental Health Inventory) e o QCO (Questionário do Comprometimento Organizacional). Resultados: Os polícias parecem regular a sua relação com a organização através do comprometimento calculativo. A autocompaixão associa-se positivamente ao comprometimento afetivo, sobressaindo a capacidade de mindfulness. O autocriticismo, na forma eu inadequado - menos corrosivo que a forma eu detestado -, revela uma relação positiva com o comprometimento calculativo, pelo que a permanência na organização pode ser emocionalmente negativa, originar ataques ao eu e diminuir a capacidade afetiva. Porém, a capacidade autotranquilizadora (forma eu tranquilizador do autocriticismo) parece capacitar os polícias para o desenvolvimento de maiores níveis de comprometimento organizacional e afetivo. A idade, a antiguidade e menos habilitações literárias parecem favorecer o comprometimento calculativo. Discussão: A autocompaixão pode contribuir para uma atitude positiva e afetuosa do polícia para com a organização. Assim, através de uma intervenção baseada na autocompaixão e no mindfulness, com impacto positivo no comportamento afetivo e normativo, não só se poderá preservar e promover a saúde e o bem-estar dos polícias, mas também fomentar o empenho organizacional, atenuando os efeitos negativos da profissão policial. Esta abordagem poderá contribuir para os objetivos organizacionais, através da supressão das emoções aversivas, da diminuição da intenção de turnover e abandono da organização. / Introduction: There are few studies in Portugal that have been dedicated to explore constructs such as self-criticism, self-compassion, mental health and organizational commitment. Our goals are to analyze these associations at the Public Safety Police, considering also the influence of sociodemographic and professionals variables, as well as to conduct predictive analyses, in order to verify which variables may predict organizational commitment. Methodology: The sample consisted of 188 police with ages between 30 and 56 years (M = 43.75, SD = 5.66). The participants filled in a protocol from the project Organizar Positivamente, from Instituto Superior Miguel Torga, composed by a sociodemographic questionnaire, the SELFCS (Self-Compassion Scale), the FSCRS (Scale of Forms of Self-Criticism and Self-Reassurance, the MHI-5 (Mental Health Inventory) and the QCO (Organizational Commitment Questionnaire). Results: The police officers appear to regulate their relationship with the organization through calculative commitment. Self-compassion is associated positively to affective commitment, with mindfulness showing to be the most important competence in this professional group. Self-criticism, in its inadequate self form – which is less corrosive than the hated self form - reveals a positive relationship with calculative commitment, which might imply that staying in the organization may be emotionally negative, may provoke attacks against the self and diminish the affective capacity. However, the self reassurance capacity (reassure self form of self-criticism) may enable the police officers to develop higher levels of organizational commitment and affective commitment, as well. Age, seniority and fewer qualifications seem to support the calculative commitment. Discussion: Through self-compassion police officers may experience a positive and warmth attitude towards the organization. Thus, an intervention program based on self-compassion and mindfulness may stimulate affective and normative commitment, not only preserving and promoting health and the well-being of police officers, but also fostering organizational commitment and mitigating negative effects in the police profession. This approach may contribute to organizational goals’ through the suppression of aversive emotions, decreasing turnover and intention to abandon the organization.

Purpose – The purpose of this paper is to explore the causes of teachers' turnover intentions in selected Swedish schools. Research methodology – This research took the form of a qualitative research. Semi-structured interviews were used to explore the causes of teachers' turnover intentions within the schools selected for this study. The sample was composed by 9 teachers that work in Säter. In order to provide a different yet meaningful perspective, a further interview with the Säter school department has been conducted. Results – Several of the results were coherent with the literature: elements such as motivations to teach, administrative support, workload, class size and collaboration were found to be related to teachers’ turnover intentions and therefore confirmed the previous studies. However, factors like salary, mentoring, autonomy, physical conditions and orderly environment were not found to have a connection with the respondents’ turnover intentions, hence constituting a result that did not confirm the literature. Originality – This thesis extended the previous research related to the causes of teachers’ turnover intentions by focusing on Sweden, which had not been investigated yet to that matter. The Swedish context appears particularly worth researching because of both the particularity of its decentralized educational system and the widespread turnover intentions of teachers.

This study aimed to examine how students perceives the factors that may influence them to attend a training course offered in the distance virtual learning environment (VLE) of the National School of Public Administration (ENAP). Thus, as theoretical basis it was used the Unified Theory of Acceptance and Use of Technology (UTAUT), the result of an integration of eight previous models which aimed to explain the same phenomenon (acceptance/use of information technology). The research approach was a quantitative and qualitative. To achieve the study objectives were made five semi-structured interviews and an online questionnaire (websurvey) in a valid sample of 101 public employees scattered throughout the country. The technique used to the analysis of quantitative data was the structural equation modeling (SEM), by the method of Partial Least Square Path Modeling (PLS-PM). To qualitative data was the thematic content analysis. Among the results, it was found that, in the context of public service, the degree whose the individual believes that the use of an AVA will help its performance at work (performance expectancy) is a factor to its intended use and also influence its use. Among the results, it was found that the belief which the public employee has in the use of a VLE as a way to improve the performance of his work (performance expectation) was determinant for its intended use that, in turn, influenced their use. It was confirmed that, under the voluntary use of technology, the general opinion of the student s social circle (social influence) has no effect on their intention to use the VLE. The effort expectancy and facilitating conditions were not directly related to the intended use and use, respectively. However, emerged from the students speeches that the opinions of their coworkers, the ease of manipulate the VLE, the flexibility of time and place of the distance learning program and the presence of a tutor are important to their intentions to do a distance learning program. With the results, it is expected that the managers of the distance learning program of ENAP turn their efforts to reduce the impact of the causes of non-use by those unwilling to adopt voluntarily the e-learning, and enhance the potentialities of distance learning for those who are already users

Destruction de la couche d'ozone, effet de serre, encombrement des sites d'enfouissement, voilà des sujets d'actualité abordés quotidiennement dans les médias. La société de consommation qui a apporté le confort matériel au citoyen, aujourd'hui détruit son environnement et diminue sa qualité de vie. Peut-on rester de glace devant les eaux dans lesquelles il n'est plus permis de se baigner? Devant un soleil qui augmente les risques de cancer ? Devant la qualité de l'air qui se dégrade ? Le citoyen peut, de plus en plus, constater de fait la dégradation de son milieu de vie. Les préoccupations concernant l'environnement du citoyen, qui est aussi un consommateur, se traduisent-ils comportements d'achat ? Comme le mentionne Driscoll (1991), les responsables du marketing doivent déterminer les actions et les compromis que certains consommateurs sont prêts à faire lorsqu'ils achètent un produit. L'objectif de la présente étude est donc de clarifier l'influence du critère environnemental, par rapport au degré de connaissance et d'implication des sujets, sur la perception de la valeur du produit et sur l'intention d'achat du consommateur vis-à-vis les produits "verts". De plus, l'étude permettra d'évaluer l'efficacité de stratégie de prix, de marque, d'efficacité et d'écologie dans le cadre d'une planification de marketing pour les produits environnementaux. Pour réaliser ce travail trois produits sont évalués par un échantillon probabiliste de la ville de Sherbrooke, soit; piles AA, nettoyant tout usage et savon à lessive. Les répondants à l'étude ont été regroupés selon deux niveaux de connaissance et d'implication, soit faible ou élevé. Ce travail débute par une revue de la littérature, suivie de la description de la méthodologie utilisée. Pour sa part, le chapitre 5 se consacre à la présentation et à la discussion des résultats de l'étude. La conclusion présente les limites de l'étude et les implications pour les gens d'affaires.
