A tuberculose (Tb) é a principal causa de morte no mundo, por um agente infeccioso. O tratamento padrão é a quimioterapia: rifampicina (RMP), isoniazida (INH) e pirazinamida (PZA). O maior problema global da Tb é o aumento de cepas multirresistentes (resistência pelo menos à INH e à RMP) do Mycobacterium tuberculosis (MTb). A resistência à INH e RMP ocorre geralmente por mutação genética nos genes KatG e rpoB, respectivamente. Os objetivos deste trabalho foram: 1. Analisar os tipos e freqüências de mutações em duas regiões iniciais do gene katG do MTb. 2. Determinar os tipos e freqüências das mutações no gene rpoB. Duas regiões do gene katG e uma do gene rpoB foram amplificadas por PCR e seqüenciadas para o diagnóstico das mutações. Para a análise do gene katG foram utilizadas 101 cepas. Dentre estas, 4 eram sensíveis e não apresentaram mutação (controle). Das 97 cepas restantes, na primeira região seqüenciada do KatG, não ocorreram mutações em 67. Nas outras 30 cepas houve 33 deleções de nucleotídeos, sendo que 24 ocorreram no último nucleotídeo do códon 4 (24,7%), o que caracterizou um novo alelo. Na região 2, dentre as 97 cepas não houve mutação em 16 - sete estavam associadas a ausência de mutação na região 1. Ocorreram 83 mutações pontuais, sendo 75,3% no códon 315. Sete cepas resistentes a INH não apresentaram mutações em nenhuma das duas regiões analisadas. As mutações na região 2 permitiram o diagnóstico de resistência à INH em 79 cepas ou 81,4%. Nove cepas que não mostraram mutações na região 2 tiveram mutações na região 1. Logo, esta região permitiu o acréscimo do diagnóstico de resistência à INH para 88 cepas, aumentando a positividade em 9,3%. Em sete casos resistentes não houve mutação em ambas as regiões. Na análise do gene rpoB usamos 120 cepas de MTb. Nenhuma mutação foi encontrada em 13 isolados resistentes à RMP. O códon que apresentou maior freqüência de mutação foi o 531 (45.6%), seguido pelo 526 (26%) e 516 (12.5%). Em outros onze códons, foi encontrado um total de 18 mutações (15.2%), principalmente nos códons 511 (3.4%) e 513 (3.4%). Nenhum dos isolados sensíveis à RMP apresentou mutações. No Estado do Rio de Janeiro, as mutações mais freqüentes foram: 516 (5%), 526 (2.5 %) e 531 (21.2%). Dentre os outros estados, as mutações mais freqüentes foram: 516 (2.5 %), 526 (11%) e 531 (19.4%). A freqüência de mutações dos isolados do Rio de Janeiro foi comparada com a encontrada nos outros estados, mas quando o removemos da análise, a freqüência de mutações nos códons 531 e 526 para os outros 15 estados é semelhante. A análise estatística mostra que este dado é significativo (p=0.002). No entanto, quando todos os estados são analisados simultaneamente, o códon 531 é novamente o mais freqüentemente mutado. A análise do gene rpoB diagnosticou a resistência à rifampicina em 89,17% das cepas. Nossos resultados confirmam que, no Brasil, mutações na região RRDR do gene rpoB podem predizer resistência a RMP.

O câncer colo-retal é a terceira neoplasia mais frequente em todo o mundo e a recorrência local e neoplasia refratária são desafios no tratamento do câncer colo-retal após a cirurgia convencional. Com o intuito de controlar a recorrência e aumentar a média de sobrevida dos pacientes, uma estratégia multidisciplinar que combina a radioterapia (RT) e a quimioterapia com o processo cirúrgico tem sido protocolo clínico de escolha. Embora esta combinação seja capaz de otimizar o tratamento, nem todos os pacientes são beneficiados com o protocolo quimio-rádio combinado, uma vez que existem os insucessos terapêuticos relacionados com a incidência de neoplasias secundárias tardias em pacientes que foram submetidos à RT para tratamento de neoplasias anteriores. Além da doença refratária, outro agravante da RT são os efeitos colaterais produzidos pela radiação ionizante (IR), em especial àqueles do trato gastrointestinal. Estes efeitos estão relacionados com alterações da homeostase do epitélio intestinal, através da desorganização dos complexos juncionais. Porém, os mecanismos que medeiam estes efeitos ainda não estão elucidados. Este estudo avaliou as vias de sinalização que medeiam os efeitos da IR em células Caco-2. Foi observado que a IR causa uma desorganização da junção aderente via Src, EGFR e MAPK, sendo estas alterações acompanhadas por desorganização do citoesqueleto de actina em todo o volume celular. Src, EGFR e MAPK participam de maneira diferenciada na modulação destes efeitos. Observamos também que a radiação aumenta a motilidade dessas células via Src e MAPK e não induz alteração na proliferação celular até 48 horas após o tratamento. Este é o primeiro trabalho que correlaciona vias de sobrevivência celular como Src, EGFR e MAPK com alterações nas proteínas de junção aderente, alterações do citoesqueleto e migração celular. Estes eventos são relacionados aos efeitos colaterais primários e tardios induzidos pela IR, e podem favorecer à aquisição de um fenótipo maligno herdável durante o fracionamento de doses na RT, favorecendo a progressão tumoral do câncer colo-retal. Logo, além da correlação das vias de sinalização envolvidas nos eventos induzidos pela IR mostrados neste estudo, os resultados também corroboram para um melhor entendimento da atividade farmacológica dos inibidores químicos utilizados, uma vez que muitos deles encontram-se em fase de ensaios pré-clínicos e clínicos.

Toxicity of four therapeutic compounds: sodium chloride, potassium permanganate, potassium dichromate and acriflavine to fry of Catla calla, Labeo rohita and Cirrhinus mrigala was tested. Bioassays were conducted in 5 and 10 liters glass jars in the laboratory for periods of 24-48 hrs. at temperatures of 26° and 32°C. LC 50 values of the chemicals were in the ranges of 5500 to 7500 ppm, 37.5 to 48 ppm, 92.5 to 125 ppm and 47.5 to 80 ppm respectively. The results obtained are of value in evolving cheap control methods against some common fish diseases. Sodium chloride and acriflavine can be widely used for chemotherapy and potassium dichromate is preferable to potassium permanganate for prophylaxis.

The recent re-emergence of tuberculosis, especially the multidrug-resistant cases, has highlighted the importance of screening effective novel drugs against Mycobacterium tuberculosis. In this study, the in vitro activities of small peptides isolated from snake venom were investigated against multidrug-resistant M. tuberculosis. Minimum inhibitory concentrations (MICs) were determined by the Bactec TB-460 radiometric method. A small peptide with the amino acid sequence ECYRKSDIVTCEPWQKFCYREVTFFPNHPVYLSGCASECTETNSKWCCTTDKCNRARGG (designated as vgf-1) from Naja atra (isolated from Yunnan province of China) venom had in vitro activity against clinically isolated multidrug-resistant strains of M. tuberculosis. The androgen role in the maintenance of prostate epithelium is subject to conflicting opinions. While androgen ablation drives the regression of normal and cancerous prostate, testosterone may cause both proliferation and apoptosis. Several investigators note decreased proliferation and stronger response to chemotherapy of the prostate cancer cells stably expressing androgen receptor (AR), however no mechanistic explanation was offered. In this paper we demonstrate in vivo anti-tumor effect of the AR on prostate cancer growth and identify its molecular mediators. We analyzed the effect of AR on the tumorigenicity of prostate cancer cells. Unexpectedly, the AR-expressing cells formed tumors in male mice at a much lower rate than the AR-negative controls. Moreover, the AR-expressing tumors showed decreased vascularity and massive apoptosis. AR expression lowered the angiogenic potential of cancer cells, by increasing secretion of an anti-angiogenic protein, thrombospondin-1. AR activation caused a decrease in RelA, a subunit of the pro-survival transcription factor NF kappa B, reduced its nuclear localization and transcriptional activity. This, in turn, diminished the expression of its anti-apoptotic targets, Bcl-2 and IL-6. Increased apoptosis within AR-expressing tumors was likely due to the NF kappa B suppression, since it was restricted to the cells lacking nuclear (active) NF kappa B. Thus we for the first time identified combined decrease of NF kappa B and increased TSP1 as molecular events underlying the AR anti-tumor activity in vivo. Our data indicate that intermittent androgen ablation is preferable to continuous withdrawal, a standard treatment for early-stage prostate cancer. (C) 2007 Wiley-Liss, Inc.

Further to the previous finding of the rainbow trout rtCATH_1 gene, this paper describes three more cathelicidin genes found in salmonids: two in Atlantic salmon, named asCATH_1 and asCATH_2, and one in rainbow trout, named rtCATH_2. All the three new salmonid cathelicidin genes share the common characteristics of mammalian cathelicidin genes, such as consisting of four exons and possessing a highly conserved preproregion and four invariant cysteines clustered in the C-terminal region of the cathelin-like domain. The asCATH_1 gene is homologous to the rainbow trout rtCATH_1 gene, in that it possesses three repeat motifs of TGGGGGTGGC in exon IV and two cysteine residues in the predicted mature peptide, while the asCATH_2 gene and rtCATH_2 gene are homologues of each other, with 96% nucleotide identity. Salmonid cathelicidins possess the same elastase-sensitive residue, threonine, as hagfish cathelicidins and the rabbit CAP18 molecule. The cleavage site of the four salmonid cathelicidins is within a conserved amino acid motif of QKIRTRR, which is at the beginning of the sequence encoded by exon W. Two 36-residue peptides corresponding to the core part of rtCATH_1 and rtCATH_2 were chemically synthesized and shown to exhibit potent antimicrobial activity. rtCATH_2 was expressed constitutively in gill, head kidney, intestine, skin and spleen, while the expression of rtCATH_1 was inducible in gill, head kidney, and spleen after bacterial challenge. Four cathelicidin genes have now been characterized in salmonids and two were identified in hagfish, confirming that cathelicidin genes evolved early and are likely present in all vertebrates.

A triblock poly(lactic acid)-b-poly(ethylene glycol)-b-poly(lactic acid) (PLA-PEG-PLA)/paclitaxel (PTX) conjugate was synthesized by the reaction of carboxyl-terminated copolymer PLA-PEG-PLA with PTX in the presence of dicyclohexylcarbodiimide and dimethylaminopyridine. Carboxyl-terminated copolymer PLA-PEG-PLA was prepared by the reaction of the hydroxyl end groups in copolymer PLA-PEG-PLA with succinic anhydride. Its structure was confirmed by NMR and gel permeation chromatography. The PLA-PEG-PLA/PTX conjugates could self-assemble into micelles in aqueous solutions with a low critical micelle concentration. Dynamic light scattering and environmental scanning electron microscopy analyses of the PLA-PEG-PLA/PTX micelles revealed their spherical structure and size of 220 nm. The antitumor activity of the conjugate against woman Hela cancer cells, evaluated by the 3-(4,5-dimethylthiazol2-yl)-2,5-diphenyl tetrazolium bromide method, showed that the conjugates had an antitumor activity similar to that of pure PTX. The obtained PLA-PEG-PLA/PTX conjugates are expected to be used in clinical practice.

The purpose of the present study was to develop implantable BCNU-toaded poly(ethylene glycol)poly(L-lactic acid) (PEG-PLLA) diblock copolymer fibers for the controlled release of 1,3-bis(2-chloroethyl)-1-nitrosourea (BCNU). BCNU was well incorporated and dispersed uniformly in biodegradable PEG-PLLA fibers by using electrospinning method. Environmental Scanning Electron Microscope (ESEM) images indicated that the BCNU-loaded PEG-PLLA fibers looked uniform and their surfaces were reasonably smooth. Their average diameters were below 1500 nm. The release rate of BCNU from the fiber mats increased with the increase of BCNU loading amount. In vitro cytotoxicity assay showed that the PEG-PLLA fibers themselves did not affect the growth of rat Glioma C6 cells. Antitumor activity of the BCNU-loaded fibers against the cells was kept over the whole experiment process, while that of pristine BCNU disappeared within 48 h. These results strongly suggest that the BCNU/PEG-PLLA fibers have an effect of controlled release of BCNU and are suitable for postoperative chemotherapy of cancers.

胸苷酸合成酶(thymidylate synthase,简称TS)和二氢叶酸还原酶(dihydrofolate reductase, 简称DHFR)都是叶酸依赖性酶,在维持DNA合成和DNA修复上发挥关键作用,并且多年来一直是肿瘤研究和化疗的重要靶点。我们前期的研究发现,TS和DHFR在翻译水平上存在负反馈调控机制。人TS和DHFR可以与其自身的mRNA结合,从而抑制mRNA的表达,化疗药物可以与TS或者DHFR相互作用,形成的复合物不能与TS mRNA结合, 使负反馈机制丧失。因此深入研究TS和DHFR的翻译调控机理,对阐明肿瘤抗药性机制,对发现新的抗肿瘤药物和肿瘤的治疗都具有十分重要的意义。 本论文利用mRNA体外展示技术,构建多肽库(约10万亿种多肽分子),利用多种实验手段将mRNA体外展示技术进行优化,提高了多肽库融合肽的产量,提高了mRNA体外展示技术筛选的特异性。将TS mRNA分子上的顺式因子TS30 RNA固定于磁珠上,将融合肽库与顺式因子作用,经过6轮循环,由多肽库中获得了与TS mRNA高度亲和的多肽序列,体外结合实验证明亲和肽可以与TS全长mRNA结合,体外翻译实验证明多肽可以抑制TS mRNA的翻译。并且利用phage display技术由噬菌体肽库(12个氨基酸随机肽库)经过四轮筛选,分别筛选到TS和DHFR的亲和肽,凝胶阻滞实验证明它们分别能与TS和DHFR mRNA结合。 本论文利用的展示技术可以广泛应用于特异靶点的蛋白质筛选,并且本论文筛选到的TS和DHFR亲和肽可以作为TS和DHFR的抑制剂,从而为获得新型的抗肿瘤药物奠定基础。

A set of tools of clinical psychological intervention for cancer patients had been composed for the first time in the form of a manual with three tapes and some pictures. The cancer patients during chemotherapy were treated by psychological intervention via the tools in group and individual therapy. QLQ-C30, POMS-SF and DWI-CI were first recommended and used as indicators for measuring the results of QoL, emotion and coping method in cancer patients in Chinese mainland. NK cell activity and IgG、IgM, IgA were used to measure the immune function. 120 cancer chemotherapy patients were randomly assigned to one of four conditions formed by a 2 (pre-chemotherapy and post-chemotherapy) *2 (experimental and control group) factorial design. The effect of psychological intervention on quality of life and immune function for cancer patients was investigated as well as the mediate role of coping method and personality and their factors. The efficiency of the tools was verified. The major conclusive results drawn out from the study were: 1. After treated by psychological intervention for three months, the status of quality of life and the symptoms of the patients during the chemotherapy was improved accompanying with the increase of immune parameters, especially the NK cell activity. The influential way of the factors, which effected on the effect of psychological intervention on the quality of life, immune parameters and coping methods, was synthetic. There was obvious mutual effects among the factors. 2. The patients of experimental group used more the active behavior, more the active cognition and less the avoidance coping method than those in control group. 3. The results of measurement showed that the personality of the caner patients was of obvious concealing and hiding. 4. Quality of Life Questionnaire-Core 30(QLQ-C30) was suitable for the cancer patients with great efficiency in Chinese mainland although the scale of SF needed to be revised in the light of Chinese culture.

Background: Ototoxicity is a known side effect of combined radiation therapy and cisplatin chemotherapy for the treatment of medulloblastoma. the delivery of an involved field boost by intensity modulated radiation therapy (IMRT) may reduce the dose to the inner ear when compared with conventional radiotherapy. the dose of cisplatin may also affect the risk of ototoxicity. A retrospective study was performed to evaluate the impact of involved field boost using IMRT and cisplatin dose on the rate of ototoxicity.Methods: Data from 41 medulloblastoma patients treated with IMRT were collected. Overall and disease-free survival rates were calculated by Kaplan-Meier method Hearing function was graded according to toxicity criteria of Pediatric Oncology Group (POG). Doses to inner ear and total cisplatin dose were correlated with hearing function by univariate and multivariate data analysis.Results: After a mean follow-up of 44 months (range: 14 to 72 months), 37 patients remained alive, with two recurrences, both in spine with CSF involvement, resulting in a disease free-survival and overall survival of 85.2% and 90.2%, respectively. Seven patients (17%) experienced POG Grade 3 or 4 toxicity. Cisplatin dose was a significant factor for hearing loss in univariate analysis (p < 0.03). in multivariate analysis, median dose to inner ear was significantly associated with hearing loss (p < 0.01). POG grade 3 and 4 toxicity were uncommon with median doses to the inner ear bellow 42 Gy (p < 0.05) and total cisplatin dose of less than 375 mg/m(2) (p < 0.01).Conclusions: IMRT leads to a low rate of severe ototoxicity. Median radiation dose to auditory apparatus should be kept below 42 Gy. Cisplatin doses should not exceed 375 mg/m(2).
