BACKGROUND: Upper gastrointestinal bleeding is the severe complication of stress-related mucosal disease in hospitalized patients. In intensive care units (ICU), risk factors are well defined and only mechanical ventilation and coagulopathy proved to be relevant for significant bleeding. On the contrary, in non-ICU settings there is no consensus about this issue. Nevertheless, omeprazole is still widely used in prophylaxis of bleeding. The objective of our study was to evaluate the relevance of stress-related mucosal disease bleeding in patients admitted to an internal medicine ward, and the role of omeprazole in its prophylaxis. METHODS: We conducted a retrospective study in which we analysed consecutive patients who were admitted to our ward over a year. We recorded demographic characteristics of the patients, potential risk factors for stress-related mucosal disease (clinical data, laboratory, and medication), administration of prophylactic omeprazole, and total cost of this prophylaxis. Patients with active gastrointestinal bleeding on the admission were excluded. We recorded every upper gastrointestinal bleeding event with clinical relevance. RESULTS: Five hundred and thirty-five patients, mean age 70 years, mean length of stay 9.6+/-7.7 days; 140 (26.2%) patients were treated with 40 mg of omeprazole intravenously, 193 (36.1%) with 20mg of omeprazole orally, and 202 (37.8%) patients had no prophylaxis. There was only one episode (0.2%) of clinically relevant bleeding. CONCLUSION: In patients admitted to an internal medicine ward, incidence of upper gastrointestinal bleeding as a complication of stress-related mucosal disease is low. We found that there is no advantage in prophylaxis with omeprazole.

SUMMARY Chagas disease (CD) is an endemic anthropozoonosis from Latin America of which the main means of transmission is the contact of skin lesions or mucosa with the feces of triatomine bugs infected by Trypanosoma cruzi. In this article, we describe the first acute CD case acquired by vector transmission in the Rio de Janeiro State and confirmed by parasitological, serological and PCR tests. The patient presented acute cardiomyopathy and pericardial effusion without cardiac tamponade. Together with fever and malaise, a 3 cm wide erythematous, non-pruritic, papule compatible with a "chagoma" was found on his left wrist. This case report draws attention to the possible transmission of CD by non-domiciled native vectors in non-endemic areas. Therefore, acute CD should be included in the diagnostic workout of febrile diseases and acute myopericarditis in Rio de Janeiro.

SUMMARYConsidered to be an emerging endemic mycosis in Latin America, paracoccidioidomycosis is characterized by a chronic course and involvement of multiple organs in immunocompromised hosts. Infection sequelae are mainly related to pulmonary and adrenal insufficiency. The host-parasite interaction results in different expressions of the immune response depending on parasite pathogenicity, fungal load and genetic characteristics of the host. A few controlled and case series reports have shown that azoles and fast-acting sulfa derivatives are useful treatment alternatives in milder forms of the disease. For moderate/severe cases, more prolonged treatments or even parenteral routes are required especially when there is involvement of the digestive tract mucosa, resulting in poor drug absorption. Although comparative studies have reported that shorter treatment regimens with itraconazole are able to induce cure in chronically-infected patients, there are still treatment challenges such as the need for more controlled studies involving acute cases, the search for new drugs and combinations, and the search for compounds capable of modulating the immune response in severe cases as well as the paradoxical reactions.

Toxoplasmosis is frequently acquired through the oral route by the ingestion of cysts or oocysts of Toxoplasma gondii. Once ingested, the parasites penetrate the intestinal epithelial cells and rapidly disseminate to all organs in the host. During T. gondii infection, the intestinal microbiota plays an important role in stimulating a protective immune response against the parasite. In this sense the use of probiotics is worthy of note since they are live microorganisms that have beneficial effects on the host through stimulation of the immune response that can be important in the control of T. gondii proliferation and dissemination in the host. In the present study, the action of the probiotic Bifidobacterium animalis subsp. lactis was investigated in C57BL/6 mice infected with oocysts of ME49 strain of T. gondii. The probiotic had an immunomodulatory action, inducing CD19 lymphocyte proliferation and consequently increasing anti-T. gondii antibody level.Bifidobacterium animalis subsp. lactisprovided protection in supplemented mice, compared to the control group. In addition, supplemented animals had milder inflammatory process in the small intestine, indicating that the probiotic protects the intestinal mucosa during infection with T. gondii. It was concluded that the probioticB. animalis subsp. lactis induces humoral immune response capable of providing protection against T. gondii infection.

Tiflite, síndrome fleo-cecal e enterocolite neutropénica serão denominações diversas do que parece ser uma infecção localizada à mucosa cecal, provocada por Clostridia. Habitualmente descrita em doentes neutropénicos, com leucémias ou após terapêutica anti-neoplásica, também tem sido descrita em doentes infectados por vírus da imunodeficiência humana, como no caso que apresentamos. O seu diagnóstico nem sempre é claro, tornando-se ainda mais problemático em doentes imunodeprimidos, nos quais múltiplas situações e podem apresentar com sintomas semelhantes. Todavia a um diagnóstico precoce deve seguir-se uma terapêutica médica agressiva, e se nalguns casos uma intervenção cirúrgica se sorna imperiosa, noutros poderá ser desnecessária. O seu controle a médio e a longo prazo poderá ser particularmente difícil.

Os autores fazem uma apresentação sumária de alguns tipos de instrumentos utilizados na realização da biópsia duodeno-jejunal através intubação oral, ao mesmo tempo que comentam a importância do método no estudo da patologia intestinal. Em seguida apresentam os resultados de sua experiência empregando 6 diferentes tipos de sondas e mostram os resultados histopatológicos observados em 150 casos de diversas parasitoses intestinais, submetidos a estudo. Entre os resultados chamam a atenção para a presença freqüente da Giardia lamblia, detectada em 16 dos 50 (32%) casos desta parasitose, contra 3 dos 27 (11,1%) de esquistossomose e apenas 2 dos 108 (1,8%) de estrongiloidíase. Relatam também o achado de numerosas leishmânias no córion da mucosa jejunal de um paciente parasitado pela Giardia lamblia, que era portador de Calazar. Apreciam, ainda, o significado de outras alterações, consideradas inespecíficas, sobre as quais aguardam estudos mais completos, para chegar a conclusões mais válidas. Entre estas, merece destaque o achado de atrofia das vilosidades intestinais em 27 pacientes infectados pelo Strongyloides stercoialis tendo que em 5 dêles havia parasitismo exclusivo pelo referido nematóide.

Quatro pacientes portadores de esquistossomose mansoni (três com a forma hepato-intestinal e um com a forma hepato-esplénica compensada) foram tratados com a substância K - 7505 ou Ronnel - 0,0 - dimetil - 0 - (2, 4, 5 - triclorofenil) - fosforotioato, avaliando-se o resultado do ensaio pela biopsia da mucosa retal (oograma) e o exame parasitológico das fezes pelo método de Hoffmann, Pons e Janer, quatorze dias após o término do tratamento. Conclui o autor que nas doses usadas o medicamento em aprêço é destituído de atividade esquistossomicida.

Helicobacter pylori (H. pylori) infection triggers a sequence of gastric alterations starting with an inflammation of the gastric mucosa that, in some cases, evolves to gastric cancer. Efficient vaccination has not been achieved, thus it is essential to find alternative therapies, particularly in the nutritional field. The current study evaluated whether curcumin could attenuate inflammation of the gastric mucosa due to H. pylori infection. Twenty-eight C57BL/6 mice, were inoculated with the H. pylori SS1 strain; ten non-infected mice were used as controls. H. pylori infection in live mice was followed-up using a modified 13C-Urea Breath Test (13C-UBT) and quantitative real-time polymerase chain reaction (PCR). Histologically confirmed, gastritis was observed in 42% of infected non-treated mice at both 6 and 18 weeks post-infection. These mice showed an up-regulation of the expression of inflammatory cytokines and chemokines, as well as of toll-like receptors (TLRs) and MyD88, at both time points. Treatment with curcumin decreased the expression of all these mediators. No inflammation was observed by histology in this group. Curcumin treatment exerted a significant anti-inflammatory effect in H. pylori-infected mucosa, pointing to the promising role of a nutritional approach in the prevention of H. pylori induced deleterious inflammation while the eradication or prevention of colonization by effective vaccine is not available.

A 57-year-old male presented with a 6-month history of blisters and painful erosions on the right buccal mucosa. No skin or other mucosal involvement was seen. The findings of histopathological and direct immunofluorescence examinations were sufficient for the diagnosis of oral mucous membrane pemphigoid in the context of adequate clinical correlation. No response was seen after topical therapies and oral corticosteroids or dapsone. Intravenous immunoglobulin was started and repeated every three weeks. Complete remission was achieved after three cycles and no recurrence was seen after two years of follow-up. The authors report a rare unilateral presentation of oral mucous membrane pemphigoid on the right buccal and hard palate mucosa, without additional involvement during a period of five years. Local trauma or autoimmune factors are possible etiologic factors for this rare disorder, here with unique presentation.

The literature on the thermosensitive properties of strains or species of Leishmania and of other miercorganisms is revised. Cutaneous or mucocutaneous strains that infect animais in the coldest areas of the skin or mucosa in general can not grow in tissue culture at 37°C or higher temperatures and their respiratory metabolism decreases at these temperatures. These facts suggest a thermosensitive event in some important metabolism phase of the organisme. The strains or species that are able to produce visceral leishmaniasis were probably originated from cutaneous strains after genetioally determined physiological adaptation, to warmer temperatures. These strains can not only visceralize in animais and man but will also grow in tissue culture at 36-37°C and the respiratory metabolism will be higher at such temperatures. There are reasons to believe that intermediate strains, i. e., with properties of both groupsí do exist. A thermosensitive physiological event is a more general phenomenon and examples of it can also be found in the fields of virology, bacteriology and mycology. It has practical applications since some of the diseases produced by these agents can be cured by treatments with heat or artificial fever. Experiments along these line were performed on hamsters with a Costa Rican strain of L. braziliensis as an experimental model. Even after intraperitoneal inoculation lesions appear in the nose, ears, paws and tail with a subcutaneous temperature bellow 33°C at 22-24°C. Healing of the lesión is accomplished by increasing room temperature. A good lesión is produced in the rump of the animal if the area is depilated (comercial cream depilatory) previously and the naked skin cooled artificially. Elevated temperature, or the growing back of the hair will tend to diminish or cure the lesion.

O A. faz um estudo panorâmico sobre a fase aguda da esquistossomose mansoni, abordando o quadro clínico e seu diagnóstico, os exames subsidiários, o diagnóstico diferencial, a terapêutica e os aspectos evolutivos. As manifestações clinicas dos períodos de incubação, de estado e de supressão são abordados. O diagnóstico da fase aguda é baseado em dado epidemiológico, 110 exame fisico e em exames subsidiários. O dado epidemiológico. em geral, é positivo, com menção a banho infectante, comumente 30 a 40 dias antes do início do quadro clínico e ao exame físico, encontramos hipertermia (38 - 4G°C), prostração, micropoliadenia hepatomegalia dolorosa em 95%, dos casos e esplenomegalia em 70% dos casos. Os exames prioritários para o diagnóstico são o exame parasiiológico de fezes seriado, que é positivo para ovos viáveis de S. mansoni e o leucograma seriado, que, geralmente, acusa leucocitose com eosinofilia. Em caso de dúvida ou para complementação diagnostica, podemos recorrer à endoscovia retal, ao oograma e á biópsia hepática. A endoscopia acusa, comumente, mucosa hiperêmica, edemaciada, friável, granulosa, com pontos hemorrágicos e o exame colhido por punção biópsia revela, entre outros achados, granulomas na fase necrótica-exsudativa. O diagnóstico diferencial deve ser feito com as seguintes entidades clínicas: gastroenterites. febre tifóide, disenteria bacilar, amebíase aguda, salmonelose septicêrnica prolongada, devendo, ainda, figurar a tuberculose miliar, abdome agudo, a G.N.D.A., a mononucleose infecciosa, a leptospirose, a hepatite e as poaneurites. A terapeutica é baseada nos cuidados gerais, na córticoterapia e na terapêutica específica. Observamos regressão dramática do quadro toxinfeccioso. nas primeiras 24 a 48 horas com a córticoterapia (prednisona) que tem duração aproximada de 7 a 10 dias. A terapêutica específica (derivado nitrotiazolico = ambilhar e derivado hidroximetílico do miracil D = hycanthone) é empregada após remissão do quadro toxiinfeccioso ou em plena fase aguda; nesta eventualidade os sintomas gerais e os distúrbios intestinais desaparecem a partir do 5.° dia de terapêutica. Quando empregamos ambilhar ou hycanthone, em apenas um esquema terapêutico, observamos 40% de cura; esta cifra atinge a 80-90%, quando repetimos a medicação específica, após verificarmos recidiva.

Os autores trataram 30 pacientes portadores de colite amebiana não disentérica, de ambos os sexos (17 homens e 13 mulheres), com idades compreendidas entre 15 (dois casos) a 55 anos (um doente), na Disciplina de Doenças Infecciosas. Parasitárias da Universidade Federal de Pernambuco. A posologia administrada foi de 1.500 mg em dose única e os resultados avaliados pelas técnicas de Hoffman, raspado da mucosa retaí, retossigmoidoscopia (em 16 enfermos) e exame direto com ou sem coloração pelo lugol, foram de 80% (24 doentes) de cura clínica e laboratorial. Nenhuma manifestação colateral foi relatada petos pacientas e concluem os autores que o Teclozan administrado em nova posologia, representa valioso subsídio no tratamento da amebíase intestinal.

Numa análise de 57 pacientes o acometimento da mucosa foi mais comumente observado em homens (77%) na terceira década de vida, embora fosse grande a variação das idades e ocorrendo mesmo o acometimento de duas crianças. Com a exceção de nove pacientes (16%) todos os outros tinham sinais de leishmaniose cutânea sendo que em somente oito (14%) de lesão era ativa. O acometimento do nariz foi observado em 100% de 19 pacientes que apresentavam lesões múltiplas e em 92% de 38 pacientes apresentando uma única lesão. A faringe, palato, laringe e lábio superior foram afetados nesta experiência. 42% dos pacientes com lesões múltiplas apresentavam acometimento da laringe sendo que em dois pacientes a única lesão existente apresentava-se neste ponto. Não foi observada qualquer diferença relacionada com a idade no que se referia à existencia de lesões únicas ou múltiplas. A duração do acometimento da mucosa variou de menos de 4 até 264 meses. Somente 7% desenvolveram o acometimento da mucosa após mais de dez anos o desenvolvimento da lesão cutânea. Os pacientes usualmente responderam ao tra-tamento adequado por antimonial embora em algumas exceções fosse usada amphotericina B. Morreram três pacientes que se recusaram a colaborar no tratamento. Dois anos após o tratamento observou-se positividade de anticorpos fluorescentes em somente 18% dos pacientes entre aqueles acompanhados.
