Vision suisse romande de la pratique infirmière avancée [Advanced nursing practice: vision in Switzerland].

To meet the challenges related to the development of health problems taking into account the development of knowledge, several innovations in care are being implemented. Among these, advanced nursing roles and increased interprofessional collaboration are considered as important features in Switzerland. Although the international literature provides benchmarks for advanced roles, it was considered essential to contextualize these in order to promote their application value in Switzerland. Thus, from 79 statements drawn from the literature, 172 participants involved in a two-sequential phases study only kept 29 statements because they considered they were relevant, important and applicable in daily practice. However, it is important to point out that statements which have not been selected at this stage to describe advanced practice cannot be considered irrelevant permanently. Indeed, given the emergence of advanced practice in western Switzerland, it is possible that a statement judged not so relevant at this moment of the development of advanced practice, will be considered as such later on. The master's program in nursing embedded at the University of Lausanne and the University of Applied Sciences Western Switzerland was also examined in the light of these statements. It was concluded that all the objectives of the program are aligned with the competencies statements that were kept.

Evaluation des campagnes de prévention contre le SIDA en Suisse sur mandat de l'Office fédéral de la santé publique : rapport de synthèse, décembre 1987

Les conclusions que l'on peut tirer après dix mois d'une campagne de prévention intensive, sont les suivantes: -> la campagne a atteint sa cible, la population générale et les groupes exposés, après une bonne diffusion et amplification par les médias; -> elle n'a pas donné lieu à l'expression de fortes résistances ou d'oppositions; -> des phénomènes multiplicateurs (actions locales, répercussions par les leaders de groupes, etc) sont apparus et contribuent à renforcer la campagne elle-même; -> la vente de préservatifs a augmenté; -> on observe des changements d'attitudes et de comportements dans le sens d'une meilleure protection dans la population générale aussi bien que dans les groupes observés, choisis spécifiquement pour leur probabilité plus grande d'adopter des comportements à risque (jeunes, homosexuels, toxicomanes...), ce qui permet de supposer que ces tendances se généraliseront, pour autant que les campagnes se poursuivent sans relâche et sans dérapage. Impact of prolonged antihypertensive duration of action on predicted clinical outcomes in imperfectly adherent patients: comparison of aliskiren, irbesartan and ramipril.

Background: Most patients miss occasional doses of antihypertensives. The use of 'forgiving' drugs (i.e. drugs with duration of action longer than the 24-h dosing interval) may allow an adequate blood pressure (BP) reduction to be maintained despite missed doses. Aim:To quantify the effects of adherence level and duration of action on estimated mean systolic BP (SBP) reduction and cardiovascular disease (CVD) risk. Method:For 1250 patients, we simulated 256-day dosing histories with realistically distributed drug holidays based on a study of electronically monitored dosing records. Adherence was set to the desired level by altering the proportion of doses missed. Mean office SBP-lowering effect (aliskiren 300 mg, -14.1 mmHg; irbesartan 300 mg, -13.3; ramipril 10 mg, -10.1 mmHg) and the rate of SBP increase after stopping treatment (off-rate; aliskiren, 1.0 mmHg/day; irbesartan, 3.6 mmHg/day; ramipril, 4.0 mmHg/day) were taken from the results of a randomised, double-blind trial. SBP was averaged over time and patient to estimate mean reductions in SBP and 10-year CVD risk (Framingham risk equation, baseline absolute 10-year CVD risk: 27%). Results:Predicted reductions in SBP and CVD risk with aliskiren were larger and less affected by imperfect adherence than the reductions with irbesartan or ramipril. For aliskiren, reducing adherence from 90% to 60% led to a predicted rise in SBP of 1.0 mmHg and three additional CVD events per 1000 treated patients; larger predicted differences were observed for irbesartan (2.5 mmHg; 7.5 events/1000 treated patients) and ramipril (2.2 mmHg; 6.7 events/1000 treated patients). Conclusion:To offset the effects of imperfect adherence, a common challenge with antihypertensives, for better BP management it may be prudent to prescribe 'forgiving' drugs.

The Rebuild Iowa Transition Strategy has been drafted to provide a comprehensive set of recommended action steps to help the state complete long-term recovery efforts while better preparing the state for future disasters. This report begins with a review of the 12 major Rebuild Iowa Advisory Commission (RIAC) recommendations which have guided RIO’s work, followed by a summary of the major accomplishments toward each recommendation. The identification of remaining needs and issues serves as the basis for the transition strategy. The following outlines the action steps necessary to achieve a successful transition and recovery.
