Aminopeptidases are enzymes that selectively hydrolyze an amino acid residue from the N-terminus of proteins and peptides. They are important for the proper functioning of prokaryotic and eukaryotic cells, but very often are central players in the devastating human diseases like cancer, malaria and diabetes. The largest aminopeptidase group include enzymes containing metal ion(s) in their active centers, which often determines the type of inhibitors that are the most suitable for them. Effective ligands mostly bind in a non-covalent mode by forming complexes with the metal ion(s). Here, we present several approaches for the design of inhibitors for metallo-aminopeptidases. The optimized structures should be considered as potential leads in the drug discovery process against endogenous and infectious diseases. Crown Copyright (C) 2010 Published by Elsevier Masson SAS. Aim: To critically appraise recent research into associations between periodontal disease and systemic diseases and conditions specifically respiratory disease, chronic kidney disease, rheumatoid arthritis, cognitive impairment, obesity, metabolic syndrome and cancer.

Methods: A MEDLINE literature search of papers published between 2002 and April 2012 was conducted. Studies that included periodontitis as an exposure were identified. Cross-sectional epidemiological investigations on large samples, prospective studies and systematic reviews formed the basis of the narrative review. A threshold set for the identification of periodontitis was used to identify those studies that contributed to the conclusions of the review.

Results: Many of the investigations were cross-sectional secondary analyses of existing data sets in particular the NHANES studies. There were a small number of systematic reviews and prospective studies. There was substantial variability in the definitions of exposure to periodontitis. A small number of studies met the threshold set for periodontitis and supported associations; however, in some of the chronic diseases there were no such studies. There was strong evidence from randomized controlled trials that interventions, which improve oral hygiene have positive effects on the prevention of nosocomial pneumonias.

Conclusions: There was substantial heterogeneity in the definitions used to identify periodontitis and very few studies met a stringent threshold for periodontitis. Published evidence supports modest associations between periodontitis and some, although not all, of the diseases and conditions reviewed. There is a need to reach a consensus on what constitutes periodontitis for future studies of putative associations with systemic diseases.

Photodynamic therapy can be used in the treatment of pre-malignant and malignant diseases. It offers advantages over other therapies currently used in the treatment of skin lesions including avoidance of damage to surrounding tissue and minimal or no scarring. Unfortunately, systemic delivery of photosensitising agents can result in adverse effects, such as prolonged cutaneous photosensitivity; while topical administration lacks efficacy in the clearance of deeper skin lesions and those with a thick overlying keratotic layer. Therefore, enhancement of conventional photosensitiser delivery is desired. However, the physicochemical properties of photosensitising agents, such as extreme hydrophilicity or lipophilicity and large molecular weights make this challenging. This paper reviews the potential of microneedles as a viable method to overcome these delivery-limiting physicochemical characteristics and discusses the current benefits and limitations of solid, dissolving and hydrogel-forming microneedles. Clinical studies in which microneedles have successfully improved photodynamic therapy are also discussed, along with benefits which microneedles offer, such as precise photosensitiser localisation, painless application and reduction in waiting times between photosensitiser administration and irradiation highlighted.

Principal Findings: Over the period of 35 years, the risk of hospitalization for cardiovascular diseases and respiratory diseases decreased. Hospitalization for musculoskeletal diseases increased whereas mental and behavioral hospitalizations slightly decreased. The risk of cancer hospitalization decreased marginally in men, whereas in women an upward trend was observed.

Conclusions/Significance: A considerable health transition related to hospitalizations and a shift in the utilization of health care services of working-age men and women took place in Finland between 1976 and 2010.

Background: The health transition theory argues that societal changes produce proportional changes in causes of disability and death. The aim of this study was to identify long-term changes in main causes of hospitalization in working-age population within a nation that has experienced considerable societal change.

Methodology: National trends in all-cause hospitalization and hospitalizations for the five main diagnostic categories were investigated in the data obtained from the Finnish Hospital Discharge Register. The seven-cohort sample covered the period from 1976 to 2010 and consisted of 3,769,356 randomly selected Finnish residents, each cohort representing 25% sample of population aged 18 to 64 years.

The advent of microneedle (MN) technology has provided a revolutionary platform for the delivery of therapeutic agents, particularly in the field of gene therapy. For over 20 years, the area of gene therapy has undergone intense innovation and progression which has seen advancement of the technology from an experimental concept to a widely acknowledged strategy for the treatment and prevention of numerous disease states. However, the true potential of gene therapy has yet to be achieved due to limitations in formulation and delivery technologies beyond parenteral injection of the DNA. Microneedle-mediated delivery provides a unique platform for the delivery of DNA therapeutics clinically. It provides a means to overcome the skin barriers to gene delivery and deposit the DNA directly into the dermal layers, a key site for delivery of therapeutics to treat a wide range of skin and cutaneous diseases. Additionally, the skin is a tissue rich in immune sentinels, an ideal target for the delivery of a DNA vaccine directly to the desired target cell populations. This review details the advancement of MN-mediated DNA delivery from proof-of-concept to the delivery of DNA encoding clinically relevant proteins and antigens and examines the key considerations for the improvement of the technology and progress into a clinically applicable delivery system.

Mitochondrial complex I is a large, membrane-bound enzyme central to energy metabolism, and its dysfunction is implicated in cardiovascular and neurodegenerative diseases. An interesting feature of mammalian complex I is the so-called A/D transition, when the idle enzyme spontaneously converts from the active (A) to the de-active, dormant (D) form. The A/D transition plays an important role in tissue response to ischemia and rate of the conversion can be a crucial factor determining outcome of ischemia/reperfusion. Here, we describe the effects of alkali cations on the rate of the D-to-A transition to define whether A/D conversion may be regulated by sodium.At neutral pH (7–7.5) sodium resulted in a clear increase of rates of activation (D-to-A conversion) while other cations had minor effects. The stimulating effect of sodium in this pH range was not caused by an increase in ionic strength. EIPA, an inhibitor of Na+/H+antiporters, decreased the rate of D-to-A conversion and sodium partially eliminated this effect of EIPA. At higher pH (> 8.0), acceleration of the D-to-A conversion by sodium was abolished, and all tested cations decreased the rate of activation, probably due to the effect of ionic strength.The implications of this finding for the mechanism of complex I energy transduction and possible physiological importance of sodium stimulation of the D-to-A conversion at pathophysiological conditions in vivo are discussed.

Quantitative point-of-care (POC) devices are the next generation for serological disease diagnosis. Whilst pathogen serology is typically performed by centralized laboratories using Enzyme-Linked ImmunoSorbent Assay (ELISA), faster on-site diagnosis would infer improved disease management and treatment decisions. Using the model pathogen Bovine Herpes Virus-1 (BHV-1) this study employs an extended-gate field-effect transistor (FET) for direct potentiometric serological diagnosis. BHV-1 is a major viral pathogen of Bovine Respiratory Disease (BRD), the leading cause of economic loss ($2 billion annually in the US only) to the cattle and dairy industry. To demonstrate the sensor capabilities as a diagnostic tool, BHV-1 viral protein gE was expressed and immobilized on the sensor surface to serve as a capture antigen for a BHV-1-specific antibody (anti-gE), produced in cattle in response to viral infection. The gE-coated immunosensor was shown to be highly sensitive and selective to anti-gE present in commercially available anti-BHV-1 antiserum and in real serum samples from cattle with results being in excellent agreement with Surface Plasmon Resonance (SPR) and ELISA. The FET sensor is significantly faster than ELISA (<10 min), a crucial factor for successful disease intervention. This sensor technology is versatile, amenable to multiplexing, easily integrated to POC devices, and has the potential to impact a wide range of human and animal diseases.

Endothelial dysfunction is a central pathomechanism in diabetes-associated complications. We hypothesized a pathogenic role in this dysfunction of cathepsin S (Cat-S), a cysteine protease that degrades elastic fibers and activates the protease-activated receptor-2 (PAR2) on endothelial cells. We found that injection of mice with recombinant Cat-S induced albuminuria and glomerular endothelial cell injury in a PAR2-dependent manner. In vivo microscopy confirmed a role for intrinsic Cat-S/PAR2 in ischemia-induced microvascular permeability. In vitro transcriptome analysis and experiments using siRNA or specific Cat-S and PAR2 antagonists revealed that Cat-S specifically impaired the integrity and barrier function of glomerular endothelial cells selectively through PAR2. In human and mouse type 2 diabetic nephropathy, only CD68(+) intrarenal monocytes expressed Cat-S mRNA, whereas Cat-S protein was present along endothelial cells and inside proximal tubular epithelial cells also. In contrast, the cysteine protease inhibitor cystatin C was expressed only in tubules. Delayed treatment of type 2 diabetic db/db mice with Cat-S or PAR2 inhibitors attenuated albuminuria and glomerulosclerosis (indicators of diabetic nephropathy) and attenuated albumin leakage into the retina and other structural markers of diabetic retinopathy. These data identify Cat-S as a monocyte/macrophage-derived circulating PAR2 agonist and mediator of endothelial dysfunction-related microvascular diabetes complications. Thus, Cat-S or PAR2 inhibition might be a novel strategy to prevent microvascular disease in diabetes and other diseases.

Morphological changes in the retinal vascular network are associated with future risk of many systemic and vascular diseases. However, uncertainty over the presence and nature of some of these associations exists. Analysis of data from large population based studies will help to resolve these uncertainties. The QUARTZ (QUantitative Analysis of Retinal vessel Topology and siZe) retinal image analysis system allows automated processing of large numbers of retinal images. However, an image quality assessment module is needed to achieve full automation. In this paper, we propose such an algorithm, which uses the segmented vessel map to determine the suitability of retinal images for use in the creation of vessel morphometric data suitable for epidemiological studies. This includes an effective 3-dimensional feature set and support vector machine classification. A random subset of 800 retinal images from UK Biobank (a large prospective study of 500,000 middle aged adults; where 68,151 underwent retinal imaging) was used to examine the performance of the image quality algorithm. The algorithm achieved a sensitivity of 95.33% and a specificity of 91.13% for the detection of inadequate images. The strong performance of this image quality algorithm will make rapid automated analysis of vascular morphometry feasible on the entire UK Biobank dataset (and other large retinal datasets), with minimal operator involvement, and at low cost.

Objective: To determine the frequency of falls and identify risk factors in the homes of the elderly under the Home Care Service of a village in Alentejo (Portugal). Method: Exploratory, descriptive study. The target group were elderly persons under the Home Care Service (23). The questionnaire consists of open and closed questions, and was based on the Jefferson Area Board for Aging Safety in the Home Assessment; Instrument to Assess the Risk of Falls and Adaptations to Prevent Falls at Home. Results: Of the 23 seniors, 13 were men; the mean age was 85; 10 widowers; 11 live alone; 12 cannot read or write; 17 have experienced falls, loss of balance being the main cause. They report changes in vision (21), hearing (14) and rheumatic diseases (14); hypertension (19); they use 4 or more drugs on a daily basis (16). Conclusion: The physiological changes associated with ageing may increase the risk of falls. Due to the consequences, it is a priority field in community intervention.

O benzeno foi o primeiro poluente atmosférico carcinogénico a ser regulamentado a nível europeu. Vários trabalhos têm sido publicados demonstrando a relação deste poluente com diversos tipos de neoplasias nomeadamente decorrentes de exposições a nível ocupacional. Porém, o estudo deste poluente para concentrações atmosféricas em ambientes exteriores ainda é pouco conhecido e está em clara evolução. Neste sentido, este trabalho pretende ser um contributo para o conhecimento da relação entre o benzeno atmosférico e a incidência de patologias que afectam os tecidos linfáticos e órgãos hematopoiéticos nomeadamente linfomas de Hodgkin, linfomas de não-Hodgkin e leucemias na população residente na Área Metropolitana do Porto. Dado a quase ausência de dados de monitorização das concentrações de benzeno atmosférico actualmente em Portugal, estas foram estimadas com base na definição de uma relação entre o benzeno e o monóxido de carbono. O conhecimento das concentrações em todo o domínio de estudo baseou-se na análise dos dados da Rede Automática de Monitorização da Qualidade do Ar porém, de modo a aumentar o detalhe espacial e temporal recorreu-se à modelação atmosférica aplicando o modelo TAPM. Para perceber a evolução temporal das concentrações, a modelação foi efectuada para os anos de 1991, 2001 e 2006 com base no ano meteorológico de 2006 e emissões para os respectivos anos ao nível da freguesia. O modelo foi previamente validado de acordo com uma metodologia proposta para este tipo de modelos. Contudo, mais do que perceber qual a variação da qualidade do ar a nível exterior, é importante conhecer o impacte de fontes interiores e o seu efeito na população. Assim, desenvolveu-se um modelo de exposição e dose que permitem conhecer os valores médios populacionais. A modelação da exposição populacional é efectuada com base nos perfis de actividade-tempo, nos movimentos pendulares inter-concelhos e nas concentrações de benzeno em ambientes exteriores e interiores. Na modelação da dose é ainda possível variações por sexo e idade. Por outro lado, para o estudo das patologias em análise efectuou-se uma análise epidemiológica espacial nomeadamente no que respeita à elaboração de mapas de incidência padronizados pela idade, e estudo da associação com a exposição ao benzeno atmosférico. Os resultados indicam associação entre o benzeno e as doenças em estudo. Esta evidência é mais notória quando a análise é realizada junto às principais fontes de emissão deste poluente, vias de tráfego e postos de abastecimento de combustível. Porém, a ausência de informação limita o estudo não permitindo o controlo de potenciais variáveis de confundimento como a exposição ao fumo do tabaco. A metodologia permite efectuar uma gestão integrada da qualidade do ar exterior e interior, funcionando como uma ferramenta do apoio à decisão para elaboração de planos de prevenção de longo prazo de potenciais efeitos na saúde das populações nomeadamente para outro tipo de patologias.

In Portugal, there is an old tradition in using clayey materials for therapeutic purposes. They are applied in pelotherapy, at several beaches of the Atlantic coast in the form of clay-sea water mixtures (peloids) to treat skin and rheumatic diseases. During many generations, peloids have been applied without scienti c studies that prove their therapeutic validity. In the last decade, the Portuguese scienti c community has become increasingly more interested in assessing the properties that make clayey materials suitable for therapeutic purposes. The abundance of clayey formations and the established practices of medical hydrology in our country turned this interest into a new perspective of application. The studied materials include di erent clays (in age and origin) mainly collected from well-known Mesozoic-Cenozoic formations, in some cases outcropping at beaches where empirical applications occur. This thesis focus in the study of silt-clay fraction (< 63 m).To determine their suitability for therapy, compositional, physicochemical, technological, thermal and rheological properties were assessed. Conventional techniques (XRD, XRF and Sedigraph) were used to assess compositional features of silt-clay fraction. Electron microscopy (SEM, VPSEM, HREM) was used to study the micromorphology and composition of clay fraction (< 2 m). Physicochemical properties (cation exchange and speci c surface) were assessed using the Ammonium Acetate and BET methods. Technological properties (plasticity and abrasivity indices) were assessed using the Atterberg limits and Einlehner abrasion tests. Thermal properties (speci c heat and cooling kinetics) were estimated by DSC analysis and cooling tests. Pharmacotechnical tests (compressibility index, sediment volume and Brook eld viscosity) were used to assess the powder owability as well as the physical stability and viscosity of clay-water dispersions. We selected as suitable Portuguese clays for health applications the samples A-Pe, A-Be2, A-Sd, J-Fr , M-To, C-Lu1, C-Lu2, Pl-Ba, M-Ga and J-Ab because they represent safe materials, with an adequate composition, good technological, physicochemical and thermal properties for application, also presenting an adequate rheology when dispersed in water. Their most relevant characteristics are the high clay minerals content, abundant smectite, illite and kaolinite, and safe hazardous concentrations. They also showed moderate capacity to exchange Ca 2+, high plasticity, low abrasivity, high speci c heat and slow cooling kinetics. They evidenced fair powder owability and good potential to formulate viscous dispersions when stabilized. Because the majority of the assessed characteristics are in accordance with those presented by clays applied in European spas for pelotherapy, we considered this group of clays also suitable for medical hydrology treatments in Portuguese spas.
