Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients. Conversely, if the ART strategy is similar or inferior to the current evidence-based strategy (ARDSNet), this should also change current practice as many institutions routinely employ recruitment maneuvers and set PEEP levels according to some titration method.

OBJECTIVES: To determine the frequency of medical adverse events in elderly patients admitted to an acute care geriatric unit, the predictive factors of occurrence, and the correlation between adverse events and hospital mortality rates. METHODS: This prospective study included 171 admissions of patients aged 60 years and older in the acute care geriatric unit in a teaching hospital in Brazil between 2007 and 2008. The following variables were assessed at admission: the patient age, gender, number of prescription drugs, geriatric syndromes (e. g., immobility, postural instability, dementia, depression, delirium, and incontinence), comorbidities, functional status (evaluated with the Katz Index of Independence in Activities of Daily Living), and severity of illness (evaluated with the Simplified Acute Physiology Score II). The incidence of delirium, infection, mortality, and the prescription of potentially inappropriate medications (based on the Beers criteria) were assessed during hospitalization. An observer who was uninvolved in patient care reported the adverse events. RESULTS: The mean age of the sample was 78.12 years. A total of 187 medical adverse events occurred in 94 admissions (55%). The predictors of medical adverse events were undetermined. Compared with the patients with no adverse events, the patients with medical adverse events had a significantly longer hospital stay (21.41 +/- 15.08 days versus 10.91 +/- 7.21 days) and a higher mortality rate (39 deaths [41.5%] versus 17 deaths [22.1%]). Mortality was significantly predicted by the Simplified Acute Physiology Score II score (odds ratio [OR] = 1.13, confidence interval [CI] 95%, 1.07 to 1.20), the Katz score (OR = 1.47, CI 95%, 1.18 to 1.83), and medical adverse events (OR = 3.59, CI 95%, 1.55 to 8.30). CONCLUSION: Medical adverse events should be monitored in every elderly hospitalized patient because there is no risk profile for susceptible patients, and the consequences of adverse events are serious, sometimes leading to longer hospital stays or even death.

Adequate polymerization plays an important role on the longevity of the composite resin restorations. Objectives: The aim of this study was to evaluate the effect of light-curing units, curing mode techniques and storage media on sorption, solubility and biaxial flexural strength (BFS) of a composite resin. Material and Methods: Two hundred and forty specimens were made of one composite resin (Esthet-X) in a stainless steel mold (2 mm x 8 mm 0), and divided into 24 groups (n=10) established according to the 4 study factors: light-curing units: quartz tungsten halogen (QTH) lamp and light-emitting diodes (LED); energy densities: 16 J/cm(2) and 20 J/cm(2); curing modes: conventional (CM) and pulse-delay (PD); and permeants: deionized water and 75% ethanol for 28 days. Sorption and solubility tests were performed according to ISO 4049:2000 specifications. All specimens were then tested for BFS according to ASTM F394-78 specification. Data were analyzed by three-way ANOVA followed by Tukey, Kruskal-Wallis and Mann-Whitney tests (alpha=0.05). Results: In general, no significant differences were found regarding sorption, solubility or BFS means for the light-curing units and curing modes (p>0.05). Only LED unit using 16 J/cm(2) and PD using 10 s produced higher sorption and solubility values than QTH. Otherwise, using CM (16 J/cm(2)), LED produced lower values of BFS than QTH (p<0.05). 75% ethanol permeant produced higher values of sorption and solubility and lower values of BFS than water (p<0.05). Conclusion: Ethanol storage media produced more damage on composite resin than water. In general the LED and QTH curing units using 16 and 20 J/cm(2) by CM and PD curing modes produced no influence on the sorption, solubility or BFS of the tested resin.

Abstract Background Obesity has been associated with a variety of disease such as type II diabetes mellitus, arterial hypertension and atherosclerosis. Evidences have shown that exercise training promotes beneficial effects on these disorders, but the underlying mechanisms are not fully understood. The aim of this study was to investigate whether physical preconditioning prevents the deleterious effect of high caloric diet in vascular reactivity of rat aortic and mesenteric rings. Methods Male Wistar rats were divided into sedentary (SD); trained (TR); sedentary diet (SDD) and trained diet (TRD) groups. Run training (RT) was performed in sessions of 60 min, 5 days/week for 12 weeks (70–80% VO2max). Triglycerides, glucose, insulin and nitrite/nitrate concentrations (NOx-) were measured. Concentration-response curves to acetylcholine (ACh) and sodium nitroprusside (SNP) were obtained. Expression of Cu/Zn superoxide dismutase (SOD-1) was assessed by Western blotting. Results High caloric diet increased triglycerides concentration (SDD: 216 ± 25 mg/dl) and exercise training restored to the baseline value (TRD: 89 ± 9 mg/dl). Physical preconditioning significantly reduced insulin levels in both groups (TR: 0.54 ± 0.1 and TRD: 1.24 ± 0.3 ng/ml) as compared to sedentary animals (SD: 0.87 ± 0.1 and SDD: 2.57 ± 0.3 ng/ml). On the other hand, glucose concentration was slightly increased by high caloric diet, and RT did not modify this parameter (SD: 126 ± 6; TR: 140 ± 8; SDD: 156 ± 8 and TRD 153 ± 9 mg/dl). Neither high caloric diet nor RT modified NOx- levels (SD: 27 ± 4; TR: 28 ± 6; SDD: 27 ± 3 and TRD: 30 ± 2 μM). Functional assays showed that high caloric diet impaired the relaxing response to ACh in mesenteric (about 13%), but not in aortic rings. RT improved the relaxing responses to ACh either in aortic (28%, for TR and 16%, to TRD groups) or mesenteric rings (10%, for TR and 17%, to TRD groups) that was accompanied by up-regulation of SOD-1 expression and reduction in triglycerides levels. Conclusion The improvement in endothelial function by physical preconditioning in mesenteric and aortic arteries from high caloric fed-rats was directly related to an increase in NO bioavailability to the smooth muscle mostly due to SOD-1 up regulation.

Objectives: The objective of this study is to compare subjective image quality and diagnostic validity of cone-beam CT (CBCT) panoramic reformatting with digital panoramic radiographs. Materials and methods: Four dry human skulls and two formalin-fixed human heads were scanned using nine different CBCTs, one multi-slice CT (MSCT) and one standard digital panoramic device. Panoramic views were generated from CBCTs in four slice thicknesses. Seven observers scored image quality and visibility of 14 anatomical structures. Four observers repeated the observation after 4 weeks. Results: Digital panoramic radiographs showed significantly better visualization of anatomical structures except for the condyle. Statistical analysis of image quality showed that the 3D imaging modalities (CBCTs and MSCT) were 7.3 times more likely to receive poor scores than the 2D modality. Yet, image quality from NewTom VGi® and 3D Accuitomo 170® was almost equivalent to that of digital panoramic radiographs with respective odds ratio estimates of 1.2 and 1.6 at 95% Wald confidence limits. A substantial overall agreement amongst observers was found. Intra-observer agreement was moderate to substantial. Conclusions: While 2D-panoramic images are significantly better for subjective diagnosis, 2/3 of the 3D-reformatted panoramic images are moderate or good for diagnostic purposes. Clinical relevance: Panoramic reformattings from particular CBCTs are comparable to digital panoramic images concerning the overall image quality and visualization of anatomical structures. This clinically implies that a 3D-derived panoramic view can be generated for diagnosis with a recommended 20-mm slice thickness, if CBCT data is a priori available for other purposes.

Background Human homeobox genes encode nuclear proteins that act as transcription factors involved in the control of differentiation and proliferation. Currently, the role of these genes in development and tumor progression has been extensively studied. Recently, increased expression of HOXB7 homeobox gene (HOXB7) in pancreatic ductal adenocarcinomas (PDAC) was shown to correlate with an invasive phenotype, lymph node metastasis and worse survival outcomes, but no influence on cell proliferation or viability was detected. In the present study, the effects arising from the knockdown of HOXB7 in PDAC cell lines was investigated. Methods Real time quantitative PCR (qRT-PCR) (Taqman) was employed to assess HOXB7 mRNA expression in 29 PDAC, 6 metastatic tissues, 24 peritumoral tissues and two PDAC cell lines. siRNA was used to knockdown HOXB7 mRNA in the cell lines and its consequences on apoptosis rate and cell proliferation were measured by flow cytometry and MTT assay respectively. Results Overexpression of HOXB7 mRNA was observed in the tumoral tissues and in the cell lines MIA PaCa-2 and Capan-1. HOXB7 knockdown elicited (1) an increase in the expression of the pro-apoptotic proteins BAX and BAD in both cell lines; (2) a decrease in the expression of the anti-apoptotic protein BCL-2 and in cyclin D1 and an increase in the number of apoptotic cells in the MIA PaCa-2 cell line; (3) accumulation of cell in sub-G1 phase in both cell lines; (4) the modulation of several biological processes, especially in MIA PaCa-2, such as proteasomal ubiquitin-dependent catabolic process and cell cycle. Conclusion The present study confirms the overexpression of HOXB7 mRNA expression in PDAC and demonstrates that decreasing its protein level by siRNA could significantly increase apoptosis and modulate several biological processes. HOXB7 might be a promising target for future therapies.

The Brazilian network for genotyping is composed of 21 laboratories that perform and analyze genotyping tests for all HIV-infected patients within the public system, performing approximately 25,000 tests per year. We assessed the interlaboratory and intralaboratory reproducibility of genotyping systems by creating and implementing a local external quality control evaluation. Plasma samples from HIV-1-infected individuals (with low and intermediate viral loads) or RNA viral constructs with specific mutations were used. This evaluation included analyses of sensitivity and specificity of the tests based on qualitative and quantitative criteria, which scored laboratory performance on a 100-point system. Five evaluations were performed from 2003 to 2008, with 64% of laboratories scoring over 80 points in 2003, 81% doing so in 2005, 56% in 2006, 91% in 2007, and 90% in 2008 (Kruskal-Wallis, p = 0.003). Increased performance was aided by retraining laboratories that had specific deficiencies. The results emphasize the importance of investing in laboratory training and interpretation of DNA sequencing results, especially in developing countries where public (or scarce) resources are used to manage the AIDS epidemic.

Este estudo descritivo, com abordagem qualitativa, teve como objetivo identificar o suporte materno, no domicílio, para o cuidado do recém-nascido prematuro egresso de Unidade Neonatal. O estudo, realizado em 2008, entrevistou doze mães residentes no município de Sumaré-SP. Os relatos, gravados, foram tratados pela técnica do Discurso do Sujeito Coletivo (DSC). Os resultados acrescem o conhecimento de enfermagem pela diversidade de recursos que emergiram, e que revelaram a importância da inserção materna na Unidade Neonatal; valorizaram a Cartilha de orientação oferecida pelo serviço; e destacaram a relevância do apoio social para o cuidado do bebê, no domicílio. Recomenda-se a inserção do familiar no plano de enfermagem para a alta do prematuro na Unidade Neonatal.

No artigo discute-se a articulação entre sistemas de informações epidemiológicas, produção científica e políticas de saúde de assistência à saúde do homem. Foram utilizadas três fontes secundárias: dados do Ministério da Saúde (Sistemas de Informação sobre Mortalidade e Hospitalar, Vigilância de Fatores de Risco e Proteção para Doenças Crônicas por Inquérito Telefônico), artigos publicados na SciELO e documentos do Ministério da Saúde referentes à saúde do homem. Os resultados apontam que, em termos de morbimortalidade, os homens estão mais expostos a riscos do que as mulheres. Na produção científica, predominam estudos que focalizam os agravos e doenças exclusivamente masculinos em detrimento de outros aspectos relacionados à saúde. Documentos legais destacam o panorama epidemiológico de morbimortalidade masculina e a metodologia de elaboração da política. É necessário que os pesquisadores ampliem a utilização dos dados dos sistemas de informações epidemiológicas do Ministério da Saúde e procedam à incorporação crítica da perspectiva relacional de gênero.

Os solos hidromórficos, comuns na Amazônia e no Pantanal, estão sujeitos à alternância natural de períodos de alagamento e secamento, que conduzem a uma formação e características diferenciadas. Estes solos guardam estreita relação com a natureza do material de origem e com os processos de deposição e sedimentação. O objetivo neste trabalho foi avaliar as características químicas, morfológicas e mineralógicas de três perfis de solos do Pantanal Norte Matogrossense (Planossolo, Plintossolo e Gleissolo), a fim de interpretar as relações entre suas propriedades e o ambiente em que foram formados. Os Planossolos e Gleissolos possuem maior fertilidade natural, evidenciada pelos valores expressivos de CTC (capacidade de troca de cátions) e saturação por bases. Os menores teores de Fe2O3 do Planossolo estão relacionados com a redução e remoção do Fe durante sua gênese A mineralogia da fração areia dos solos é constituída principalmente de quartzo, nódulos e concreções de Fe e de Mn, e em menor grau, biotita, muscovita e traços de turmalina, magnetita, ilmenita, epídoto, zircão e rutilo. Os solos apresentaram perfil mineralógico semelhante na fração argila, constituído por caulinita, esmectita, ilita e interestratificados do tipo ilita-esmectita. A mineralogia da fração argila dos solos foi compatível com as diferenças químicas constatadas entre eles, pois o Planossolo apresentou argila de maior atividade relativa às esmectitas e interestratificados ilita/esmectita, com maior soma de bases trocáveis e CTC, enquanto o Plintossolo e o Gleissolo, cujo mineral predominante foi a caulinita, apresentaram baixo teor de bases trocáveis e menor CTC.

FUNDAMENTO: A Contração Pós-Repouso (CPR) do músculo cardíaco fornece informações indiretas sobre a manipulação de cálcio intracelular. OBJETIVO: Nosso objetivo foi estudar o comportamento da CPR e seus mecanismos subjacentes em camundongos com infarto do miocárdio. MÉTODOS: Seis semanas após a oclusão coronariana, a contratilidade dos Músculos Papilares (MP) obtidos a partir de camundongos submetidos à cirurgia sham (C, n = 17), com infarto moderado (MMI, n = 10) e grande infarto (LMI, n = 14), foi avaliada após intervalos de repouso de 10 a 60 segundos antes e depois da incubação com cloreto de lítio (Li+) em substituição ao cloreto de sódio ou rianodina (Ry). A expressão proteica de SR Ca(2+)-ATPase (SERCA2), trocador Na+/Ca2+ (NCX), fosfolambam (PLB) e fosfo-Ser (16)-PLB foi analisada por Western blotting. RESULTADOS: Os camundongos MMI apresentaram potenciação de CPR reduzida em comparação aos camundongos C. Em oposição à potenciação normal para camundongos C, foram observadas degradações de força pós-repouso nos músculos de camundongos LMI. Além disso, a Ry bloqueou a degradação ou potenciação de PRC observada em camundongos LMI e C; o Li+ inibiu o NCX e converteu a degradação em potenciação de CPR em camundongos LMI. Embora os camundongos MMI e LMI tenham apresentado diminuição no SERCA2 (72 ± 7% e 47 ± 9% de camundongos controle, respectivamente) e expressão protéica de fosfo-Ser16-PLB (75 ± 5% e 46 ± 11%, respectivamente), a superexpressão do NCX (175 ± 20%) só foi observada nos músculos de camundongos LMI. CONCLUSÃO: Nossos resultados mostraram, pela primeira vez, que a remodelação miocárdica pós-IAM em camundongos pode mudar a potenciação regular para degradação pós-repouso, afetando as proteínas de manipulação de Ca(2+) em miócitos.

O trabalho relata, de maneira sistematizada e crítica, a experiência de um Projeto de Extensão no período de 2010 a 2011. Teve como foco ações de educação em saúde como estratégia para melhorar a adesão das pessoas com diabetes mellitus e insulinodependentes, de uma Unidade Básica de Saúde do município de São Paulo, ao Programa Automonitoramento Glicêmico. Além disso, pretendeu-se contribuir na reorganização do processo de trabalho em relação ao Programa na unidade. Foram utilizadas estratégias de educação em saúde em grupos educativos e visitas domiciliares, assim, possibilitando cuidados mais singulares. Dados dos usuários foram organizados em planilha e em pastas para as equipes de Saúde da Família, facilitando na identificação dos usuários, inclusive os faltosos, e auxiliando na descentralização do cuidado. Com as ações de educação em saúde, pretendeu-se contribuir para um cuidado mais integral e emancipatório aos usuários, para um contínuo refletir dos trabalhadores quanto a suas práticas.

A alta do paciente da recuperação pós-anestésica (RPA) depende, dentre outros fatores, do retorno à normotermia e do escore alcançado pelo Índice de Aldrete e Kroulik. Sendo assim, o objetivo deste estudo foi verificar a relação entre o Índice de Aldrete e Kroulik e a temperatura corporal dos pacientes. O local de pesquisa foi o Hospital Universitário da Universidade de São Paulo. O cálculo amostral foi determinado por conveniência e foi constituído por 60 pacientes, entre 18 e 60 anos, submetidos à anestesia geral. Foram verificados a temperatura corporal na região timpânica e o Índice de Aldrete e Kroulik do paciente na recepção e alta da recuperação pós-anestésica. Os dados obtidos foram processados pelo pacote estatístico SPSS, com um nível de 5% de significância, e aplicaram-se o teste de Spearman e o teste de Wilcoxon. Conclui-se que não houve correlação significativa entre os dois parâmetros indicativos de alta.

Este estudo faz uma análise sobre o uso de diferentes sistemas de classificação e o atendimento aos padrões estabelecidos pela ISO 18.104:2003 a partir de uma situação clínica fictícia. Foram elaborados diagnósticos e intervenções de enfermagem utilizando a NANDA-I, NIC e CIPE®, e analisou-se a correspondência terminológica destes sistemas de classificação ao modelo proposto pela norma ISO 18.104:2003. Para a construção de diagnósticos de enfermagem, a NANDA-I e a CIPE® adequando-se à norma ISO 18.104:2003. Para a construção das intervenções de enfermagem, a CIPE® atende ao modelo de referência terminológica proposto pela ISO 18104:2003. Por sua vez, a NIC não propõe um modelo de referência terminológica combinatória. A unificação das terminologias de enfermagem depende da revisão, padronização e teste dessas classificações para o estabelecimento de uma linguagem comum e sólida da profissão.
