Foram estudados 17 casos de ancilostomose e determinados alguns parâmetros hematológicos como: dosagem de hemoglobina, do ferro sérico e da transferrina, contagem de hemácias, hematócrito, volume corpuscular médio (VCM) e hemoglobina corpuscular média (HCM). O estudo incluiu também a obtenção de alguns dados eritrocinéticos, como a determinação da velocidade de decaimento do ferro plasmático (T1V2 do 59Fe) e da incorporação do ferro à hemoglobina. O estudo radioisotópico permitiu ainda determinar o volume de sangue e a quantidade de hemoglobina perdida nas fezes, bem como o teor de ferro reabsorvido dessa hemoglobina. Estabeleceram também o grau de infecção através da contagem de ovos e de vermes nas fezes. Os doentes não apresentaram evidente alteração nutricional. A carência de ferro foi o fator comum a todos os casos que exibiram anemia, constituindo a base fisiopatológica da anemia ancilostomótica. O verme fixado à mucosa duodenal suga o sangue do hospedeiro e esta espoliação de sangue a longo prazo provoca a anemia. O volume de sangue perdido é geralmente proporcional ao grau de infecção, mas a quantidade de hemoglobina perdida mostrou ser independente do volume de sangue espoliado. A reabsorção de grande parte do ferro da hemoglobina perdida na luz intestinal concorre para que a anemia se estabeleça mais tardiamente que em outras hemorragias como a vaginal. Os indivíduos anêmicos foram submetidos a transfusões de sangue e com isso apresentaram melhora clínica e laboratorial, imediata mas temporária. A cura clínica foi estabelecida somente após tratamento adequado com vermífugos.

Foram sensibilizadas hemácias humanas 0 Rh negativo com a fração semipurificada (Fp) da proteinase do Trypanosoma cruzi, e testadas quanto a antigenicidade com soros de pacientes portadores de tripanossomíase americana crônica e de outras doenças parasitárias não relacionadas. Reações de hemaglutinação positivas foram observadas com os soros de pacientes chagásicos e com alguns soros de indivíduos portadores de leishmaniose cutaneo-mucosa. Não foram observadas reações cruzadas com os soros de pacientes portadores de leishmaniose visceral, malária, toxoplasmose, sífilis, esquistossomose e mononucleose. Os resultados obtidos são favoráveis ao emprego desta fração antigênica em testes de imunodiagnóstico da tripanossomíase americana.

Desafios do estudo: realizar e optimizar a técnica para demonstração de MN em linfócitos de sangue periférico e células esfoliadas da mucosa bucal; screening de MN; estudo de polimorfismos. Objectivos do estudo: identificar efeitos para a saúde; comparar a frequência de MN em linfócitos do sangue periférico e em células esfoliadas da mucosa bucal dos trabalhadores expostos a FA nos laboratórios de AP –patologistas, técnicos e auxiliares com controlos.

O formaldeído é um gás incolor, solúvel na água e que reage rapidamente com o local de contacto. É utilizado nos laboratórios de Anatomia Patológica (AP) como fixador de células e tecidos - importante local de exposição ocupacional ao formaldeído, nomeadamente por médicos patologistas, técnicos de AP e auxiliares de acção médica. Objectivos do estudo: conhecer a exposição a formaldeído nos laboratórios hospitalares de Anatomia Patológica em Portugal; comparar a frequência de MN em linfócitos do sangue periférico e em células esfoliadas da mucosa bucal dos trabalhadores expostos a formaldeído nos laboratórios de AP – patologistas, técnicos de AP e auxiliares com controlos.

Foram identificadas associações entre conforto ambiental e sinais e sintomas entre trabalhadores expostos a ambientes fechados, ou seja, ambientes com ventilação e climatização artificiais. A população de estudo compreendeu 312 trabalhadores bancários distribuídos em dois edifícios localizados na Cidade de São Paulo. O delineamento é transversal com questionários autodirigidos, e a técnica de análise foi a regressão logística. A Síndrome dos Edifícios Doentes definida pela Organização Mundial da Saúde foi dividida em uma síndrome de sintomas gerais e em uma síndrome de sintomas de irritação de membrana mucosa. Foram relevantes para sintomas gerais, o sexo feminino, a organização do trabalho e a temperatura inadequada. Para sintomas de irritação de membrana mucosa, foram relevantes o sexo feminino e a temperatura inadequada. A organização do trabalho foi relevante para absenteísmo.
Occupational exposure to formaldehyde: effects of years of exposure in the frequency of micronucleus

Formaldehyde: an important industrial compound used in the manufacture of synthetic resins and chemical compounds such as lubricants and adhesives; also applied as a disinfectant, preservative and in cosmetics productions; relevant workplace exposure to FA also occurs in anatomy, pathology and in mortuaries; classified by IARC as carcinogenic to humans (Group 1), based on sufficient evidence in humans and experimental animals; manifold in vitro studies indicated that FA can induce genotoxic effects in proliferating cultured mammalian cells. Aim of the study: to evaluate if years of exposure induced a genotoxic biomarkers increase, namely MN in lymphocytes and buccal cells, in workers occupationally exposed to FA (factory and pathology anatomy laboratory).

Globalmente, o vírus do papiloma humano (HPV) é a segunda causa mais comum de cancro associado a mortalidade e morbilidade entre as mulheres devido ao seu relacionamento com o cancro do colo do útero (CCU) e caracteriza-se também como uma das doenças sexualmente transmissíveis (DST) mais prevalentes. Entre 2000 e 2006, foram diagnosticados cerca de 1.401.400 casos de CCU e estima-se que seja responsável por cerca de 260.000 mortes anuais. Aproximadamente 80% destas mortes ocorrem nos países em desenvolvimento. Um estudo Americano indicou que cerca de 30% a 40% das mulheres entre os 14 e 40 anos encontram-se infectadas pelo HPV e cerca de 75% foi infectada em alguma altura da sua vida. Estudos mostram que este vírus é também responsável por um razoável numero de casos de cancro da vulva, vagina, ânus, pénis, orofaríngeos e pulmonares. Com um número estimado de mais de 200 diferentes estirpes, este vírus é maioritariamente assintomático. Existem 13 a 18 tipos diferentes de HPV considerados de elevado risco oncogénico (HPV-ERO) responsáveis pela maioria dos casos de cancro. As estirpes consideradas de baixo risco oncogénico (HPV-BRO) estão intimamente relacionadas com riscos clínicos menores como lesões (e.g., verrugas) na mucosa oral, genital, anal e laríngea. Recentemente, o avançado conhecimento molecular da estrutura do HPV permitiu aos investigadores o desenvolvimento de medidas preventivas e de tratamento face ao vírus, incluindo a introdução de uma vacina no mercado aprovada pela FDA muito eficiente especialmente nas mulheres mais jovens.

Environmental tobacco smoke (ETS) is recognized as an occupational hazard in the hospitality industry. Although Portuguese legislation banned smoking in most indoor public spaces, it is still allowed in some restaurants/bars, representing a potential risk to the workers’ health, particularly for chronic respiratory diseases. The aims of this work were to characterize biomarkers of early genetic effects and to disclose proteomic signatures associated to occupational exposure to ETS and with potential to predict respiratory diseases development. A detailed lifestyle survey and clinical evaluation (including spirometry) were performed in 81 workers from Lisbon restaurants. ETS exposure was assessed through the level of PM 2.5 in indoor air and the urinary level of cotinine. The plasma samples were immunodepleted and analysed by 2D-SDSPAGE followed by in-gel digestion and LC-MS/MS. DNA lesions and chromosome damage were analysed innlymphocytes and in exfoliated buccal cells from 19 cigarette smokers, 29 involuntary smokers, and 33 non-smokers not exposed to tobacco smoke. Also, the DNA repair capacity was evaluated using an ex vivo challenge comet assay with an alkylating agent (EMS). All workers were considered healthy and recorded normal lung function. Interestingly, following 2D-DIGE-MS (MALDI-TOF/TOF), 61 plasma proteins were found differentially expressed in ETS-exposed subjects, including 38 involved in metabolism, acute-phase respiratory inflammation, and immune or vascular functions. On the other hand, the involuntary smokers showed neither an increased level of DNA/chromosome damage on lymphocytes nor an increased number of micronuclei in buccal cells, when compared to non-exposed non-smokers. Noteworthy, lymphocytes challenge with EMS resulted in a significantly lower level of DNA breaks in ETS-exposed as compared to non-exposed workers (P<0.0001) suggestive of an adaptive response elicited by the previous exposure to low levels of ETS. Overall, changes in proteome may be promising early biomarkers of exposure to ETS. Likewise, alterations of the DNA repair competence observed upon ETS exposure deserves to be further understood. OBJETIVO: Para evaluar la infección y obtener el estado adulto del cestodos, se buscó reproducir la equinococosis en perros a partir de quiste hidatídico de origen porcino. MÉTODOS: Se formaron 2 grupos, uno de 5 y otro de 3 perros, a cada animal del grupo experimental se le dió 2 g de membrana germinativa de quíste hidatídico fértil por vía oral, el segundo grupo fue testigo. Ambos grupos fueron evaluados clínica, serológica y parasitológicamente, en el grupo experimental se sacrificó un animal el día 35 de la infección y los siguientes cada 5 dias hasta el 55, en el segundo grupo todos se sacrificaron el día 55. Se observaron huevos del cestodos en heces a partir del dia 51 postinfección. La evaluación morfológica se realizó mediante observación microscópica del raspado de mucosa intestinal. RESULTADOS: De 50 cestodos analizados, 10 de cada uno de los perros infectados, 49 (98%) presentaron 3 proglótidos y 1 (2%) tenía 4; 18 (36%) de los cestodos presentaban un proglótido grávido. La longitud de los estróbilos varió de 1,6 a 2,6 mm. El número promedio de los ganchos largos y cortos fue de 31 y 34 respectivamente. La longitud de los ganchos largos varió entre 0,081 y 0,09 mm, los ganchos cortos fluctuaron entre 0,034 y 0,041 mm. En los perros evaluados clínicamente, el número de leucocitos y la cantidad de proteínas plasmáticas fue significativamente mayor en el grupo testigo (P < 0,05); la cantidad de alfa globulinas fue mayor en el grupo infectado (P < 0,05). CONCLUSIONES: Los resultados permiten confirmar el ciclo perro-cerdo y una infección subclínica en los huéspedes definitivos, lo que dificulta su diagnóstico y control en una especie intimamente relacionada con el hombre.

OBJETIVO: Apresentar levantamento estatístico descritivo da leishmaniose tegumentar americana (LTA) no Estado do Acre. MÉTODOS: Os dados obtidos foram extraídos dos formulários da "Campanha Contra a Leishmaniose", no período de janeiro de 1992 a dezembro de 1997. As variáveis estudadas foram submetidas a análise estatística descritiva. RESULTADOS: O total de casos registrados foi de 2.557. Foi adotada a divisão do Acre em meso e microrregiões para apresentação dos resultados. A maior prevalência foi na microrregião de Brasiléia (231,8 casos/10.000 hab.). A forma clínica predominante foi cutânea (84,05%). A maior ocorrência foi no sexo masculino (71,02%). Portadores com idade de até 24 anos corresponderam a 50% dos casos. Há uma predominância nas ocupações rurais. O exame clínico foi usado para diagnosticar 83,97% dos casos. A maior média de tempo de espera para procurar tratamento médico foi registrada na mesorregião do Vale do Juruá (10,37 meses). CONCLUSÃO: Os altos índices de LTA cutâneo e cutâneo-mucosa encontrados sugerem a necessidade de serem feitas pesquisas sobre os reflexos psicossociais e para identificar fatores que influem na demora do tratamento dos casos.

Although vaccination is still the most cost-effective strategy for tuberculosis control, there is an urgent need for an improved vaccine. Current BCG vaccine lacks efficacy in preventing adult pulmonary tuberculosis, the most prevalent form of the disease. Targeting nasal mucosa, Mycobacterium tuberculosis infection site, will allow a simpler, less prone to risk of infection and more effective immunization against disease. Due to its biodegradable, immunogenic and mucoadhesive properties, chitosan particulate delivery systems can act both as carrier and as adjuvant, improving the elicited immune response. In this study, BCG was encapsulated in alginate and chitosan microparticles, via a mild ionotropic gelation procedure with sodium tripolyphosphate as a counterion. The particulate system developed shows effective modulation of BCG surface physicochemical properties, suitable for mucosal immunization. Intracellular uptake was confirmed by effective transfection of human macrophage cell lines.

Immunisation against M. tuberculosis with current available BCG vaccine lacks efficacy in preventing adult pulmonary tuberculosis. Targeting nasal mucosa is an attractive option for a more effective immunization. The delivery of BCG via the intranasal route involves overcoming barriers such as crossing the physical barrier imposed by the mucus layer and ciliar remotion, cellular uptake and intracellular trafficking by antigen presenting cells. Due to its biodegradable, immunogenic and mucoadhesive properties, chitosan particulate delivery systems can act both as vaccine carrier and adjuvant, improving the elicited immune response. In this study, different combinations of Chitosan/Alginate/TPP microparticles with BCG were produced as vaccine systems. The developed microparticle system successfully modulates BCG surface physicochemical properties and promotes effective intracellular uptake by human macrophage cell lines Preliminary immune responses were evaluated after s.c. and intranasal immunisation of BALB/c mice. BCG vaccination successfully stimulated the segregation of IgG2a and IgG1, where intranasal immunisation with chitosan/alginate particulate system efficiently elicited a more equilibrated cellular/humoral immune response.

The International Agency for Research on Cancer classified formaldehyde as carcinogenic to humans because there is “sufficient epidemiological evidence that it causes nasopharyngeal cancer in humans”. Genes involved in DNA repair and maintenance of genome integrity are critically involved in protecting against mutations that lead to cancer and/or inherited genetic disease. Association studies have recently provided evidence for a link between DNA repair polymorphisms and micronucleus (MN) induction. We used the cytokinesis-block micronucleus (CBMN assay) in peripheral lymphocytes and MN test in buccal cells to investigate the effects of XRCC3 Thr241Met, ADH5 Val309Ile, and Asp353Glu polymorphisms on the frequency of genotoxicity biomarkers in individuals occupationally exposed to formaldehyde (n = 54) and unexposed workers (n = 82). XRCC3 participates in DNA double-strand break/recombination repair, while ADH5 is an important component of cellular metabolism for the elimination of formaldehyde. Exposed workers had significantly higher frequencies (P < 0.01) than controls for all genotoxicity biomarkers evaluated in this study. Moreover, there were significant associations between XRCC3 genotypes and nuclear buds, namely XRCC3 Met/Met (OR = 3.975, CI 1.053–14.998, P = 0.042) and XRCC3 Thr/Met (OR = 5.632, CI 1.673–18.961, P = 0.005) in comparison with XRCC3 Thr/Thr. ADH5 polymorphisms did not show significant effects. This study highlights the importance of integrating genotoxicity biomarkers and genetic polymorphisms in human biomonitoring studies.

Asthma is a chronic inflammatory disorder of the respiratory airways affecting people of all ages, and constitutes a serious public health problem worldwide (6). Such a chronic inflammation is invariably associated with injury and repair of the bronchial epithelium known as remodelling (11). Inflammation, remodelling, and altered neural control of the airways are responsible for both recurrent exacerbations of asthma and increasingly permanent airflow obstruction (11, 29, 34). Excessive airway narrowing is caused by altered smooth muscle behaviour, in close interaction with swelling of the airway walls, parenchyma retractile forces, and enhanced intraluminal secretions (29, 38). All these functional and structural changes are associated with the characteristic symptoms of asthma – cough, chest tightness, and wheezing –and have a significant impact on patients’ daily lives, on their families and also on society (1, 24, 29). Recent epidemiological studies show an increase in the prevalence of asthma, mainly in industrial countries (12, 25, 37). The reasons for this increase may depend on host factors (e.g., genetic disposition) or on environmental factors like air pollution or contact with allergens (6, 22, 29). Physical exercise is probably the most common trigger for brief episodes of symptoms, and is assumed to induce airflow limitations in most asthmatic children and young adults (16, 24, 29, 33). Exercise-induced asthma (EIA) is defined as an intermittent narrowing of the airways, generally associated with respiratory symptoms (chest tightness, cough, wheezing and dyspnoea), occurring after 3 to 10 minutes of vigorous exercise with a maximal severity during 5 to 15 minutes after the end of the exercise (9, 14, 16, 24, 33). The definitive diagnosis of EIA is confirmed by the measurement of pre- and post-exercise expiratory flows documenting either a 15% fall in the forced expiratory volume in 1 second (FEV1), or a ≥15 to 20% fall in peak expiratory flow (PEF) (9, 24, 29). Some types of physical exercise have been associated with the occurrence of bronchial symptoms and asthma (5, 15, 17). For instance, demanding activities such as basketball or soccer could cause more severe attacks than less vigorous ones such as baseball or jogging (33). The mechanisms of exercise-induced airflow limitations seem to be related to changes in the respiratory mucosa induced by hyperventilation (9, 29). The heat loss from the airways during exercise, and possibly its post-exercise rewarming may contribute to the exercise-induced bronchoconstriction (EIB) (27). Additionally, the concomitant dehydration from the respiratory mucosa during exercise leads to an increased interstitial osmolarity, which may also contribute to bronchoconstriction (4, 36). So, the risk of EIB in asthmatically predisposed subjects seems to be higher with greater ventilation rates and the cooler and drier the inspired air is (23). The incidence of EIA in physically demanding coldweather sports like competitive figure skating and ice hockey has been found to occur in up to 30 to 35% of the participants (32). In contrast, swimming is often recommended to asthmatic individuals, because it improves the functionality of respiratory muscles and, moreover, it seems to have a concomitant beneficial effect on the prevalence of asthma exacerbations (14, 26), supporting the idea that the risk of EIB would be smaller in warm and humid environments. This topic, however, remains controversial since the chlorified water of swimming pools has been suspected as a potential trigger factor for some asthmatic patients (7, 8, 20, 21). In fact, the higher asthma incidence observed in industrialised countries has recently been linked to the exposition to chloride (7, 8, 30). Although clinical and epidemiological data suggest an influence of humidity and temperature of the inspired air on the bronchial response of asthmatic subjects during exercise, some of those studies did not accurately control the intensity of the exercise (2, 13), raising speculation of whether the experienced exercise overload was comparable for all subjects. Additionally, most of the studies did not include a control group (2, 10, 19, 39), which may lead to doubts about whether asthma per se has conditioned the observed results. Moreover, since the main targeted age group of these studies has been adults (10, 19, 39), any extrapolation to childhood/adolescence might be questionable regarding the different lung maturation. Considering the higher incidence of asthma in youngsters (30) and the fact that only the works of Amirav and coworkers (2, 3) have focused on this age group, a scarcity of scientific data can be identified. Additionally, since the main environmental trigger factors, i.e., temperature and humidity, were tested separately (10, 28, 39) it would be useful to analyse these two variables simultaneously because of their synergic effect on water and heat loss by the airways (31, 33). It also appears important to estimate the airway responsiveness to exercise within moderate environmental ranges of temperature and humidity, trying to avoid extreme temperatures and humidity conditions used by others (2, 3). So, the aim of this study was to analyse the influence of moderate changes in air temperature and humidity simultaneously on the acute ventilatory response to exercise in asthmatic children. To overcome the above referred to methodological limitations, we used a 15 minute progressive exercise trial on a cycle ergometer at 3 different workload intensities, and we collected data related to heart rate, respiratory quotient, minute ventilation and oxygen uptake in order to ensure that physiological exercise repercussions were the same in both environments. The tests were done in a “normal” climatic environment (in a gymnasium) and in a hot and humid environment (swimming pool); for the latter, direct chloride exposition was avoided.

Formaldehyde (FA) ranks 25th in the overall U.S. chemical production, with more than 5 million tons produced each year. Given its economic importance and widespread use, many people are exposed to FA occupationally. Recently, based on the correlation with nasopharyngeal cancer in humans, the International Agency for Research on Cancer (IARC) confirmed the classification of FA as a Group I substance. Considering the epidemiological evidence of a potential association with leukemia, the IARC has concluded that FA can cause this lymphoproliferative disorder. Our group has developed a method to assess the exposure and genotoxicity effects of FA in two different occupational settings, namely FAbased resins production and pathology and anatomy laboratories. For exposure assessment we applied simultaneously two different techniques of air monitoring: NIOSH Method 2541 and Photo Ionization Detection Equipment with simultaneously video recording. Genotoxicity effects were measured by cytokinesis-blocked micronucleus assay in peripheral blood lymphocytes and by micronucleus test in exfoliated oral cavity epithelial cells, both considered target cells. The two exposure assessment techniques show that in the two occupational settings peak exposures are still occurring. There was a statistical significant increase in the micronucleus mean of epithelial cells and peripheral lymphocytes of exposed individuals compared with controls. In conclusion, the exposure and genotoxicity effects assessment methodologies developed by us allowed to determine that these two occupational settings promote exposure to high peak FA concentrations and an increase in the micronucleus mean of exposed workers. Moreover, the developed techniques showed promising results and could be used to confirm and extend the results obtained by the analytical techniques currently available.
