Resumo - O avanço tecnológico e científico no campo da Medicina tem favorecido a aplicação das radiações ionizantes nas áreas da Medicina Nuclear, Radiologia, Radioterapia e noutras especialidades relacionadas com a medicina de intervenção, como a Cardiologia. As Organizações de Saúde, com profissionais com risco de exposição a radiações ionizantes, são responsáveis, legislativamente, por assegurar a sua vigilância. Certificando a observação controlada das doses de exposição, os profissionais são monitorizados com dosímetros individuais e submetidos a vigilância médica. As leituras de dosímetria individual destes profissionais, originam por vezes, nos próprios, desconfianças e incertezas quanto aos valores medidos. Objectivo: avaliar o grau de confiança dos profissionais de saúde, expostos a radiações ionizantes, no sistema de monitorização individual. Metodologia: estudo observacional do tipo analítico- transversal, por questionário. Amostra de 190 Técnicos de Diagnóstico e Terapêutica de Cardiopneumologia, Medicina Nuclear, Radiologia, Radioterapia, expostos a radiações ionizantes, que exercem a sua actividade profissional em hospitais do concelho de Lisboa. Resultados: 51,1% dos Técnicos de Diagnóstico e Terapêutica não confiam nas leituras dos dosímetros, não se determinou uma relação estatisticamente significativa entre essa confiança, o tipo de dosímetro e a empresa que realiza as leituras. A confiança é maior nos que exercem em Organizações de Saúde públicas. 40,2% das Organizações de Saúde apresentam plano de vigilância médica, constatando-se uma falta de conformidade entre os Técnicos de Diagnóstico e Terapêutica, nalguns hospitais, relativamente à existência desse plano no respectivo hospital. A margem de erro associado ao estudo é de 5,09%, com um nível de confiança de 95%. -----------Abstract - The technological and scientific advances in the field of medicine has encouraged the application of ionizing radiation with a considerable positive contribution in the areas of Nuclear Medicine, Radiology, Radiotherapy and other specialties related to medical intervention like Cardiology. The Health Organizations that arise the risk of occupational exposure to ionizing radiation and is their responsibility to ensure monitoring, according the law. To ensure the monitoring of controlled doses of exposure, the exposed works use an individual dosimeter and undergo medical supervision. The individual dosimetry readings of health-care professionals, is sometimes questionable for themselves, arising distrust and uncertainty about the values measured. Objective: evaluate the degree of confidence of health professionals, exposed to radiation, on the system of individual monitoring. Methods: Observational study of cross-type analysis by questionnaire. Sample of 190 of technologists of Cardiopneumology, Nuclear Medicine, Radiology, Radiotherapy, exposed to ionizing radiation, which work in hospitals on the region of Lisbon. Results: 51.1% of technologists does not rely on the readings of their dosimeters, and wasn’t determined a statistically significant relationship between this trust, the type of dosimeter and the company that makes the readings(s) of their(s) dosimeter(s). RESUMO O controlo vectorial representa uma parte importante da estratégia global actual para a prevenção das principais doenças transmitidas por insectos, como a malária, a febre de dengue ou do West Nile. Uma das vertentes do controlo vectorial é aquela que se associa à protecção individual dos hospedeiros contra a picada de insectos. É no âmbito desta problemática que surge o estudo aqui apresentado. Este teve como um dos objectivos testar em laboratório a eficácia de três compostos diferentes (permetrina, DEET e citronela) contra a picada de mosquitos Anopheles stephensi Liston, 1901, quando aplicados a tecidos de algodão. Com base neste estudo laboratorial, seleccionaram-se os tecidos mais eficientes que foram testados no terreno, na área da Comporta, em ensaios simuladores de uma situação real. Os resultados foram complementados com alguns ensaios preliminares efectuados em laboratório com a espécie Culex theileri Teobald, 1903. Nos primeiros ensaios laboratoriais, tecidos impregnados com permetrina mostraram induzir uma repelência mais eficaz do que tecidos com DEET e citronela micro-encapsulados. O efeito de repelência dos tecidos com permetrina manteve-se, mesmo quando estes foram sujeitos a vários ciclos de lavagem. No entanto, os ensaios de repelência/protecção efectuados no campo demonstraram que a eficácia do tecido impregnado com permetrina é afectada pelo número de lavagens. Testes laboratoriais realizados com Cx. theileri, a espécie mais abundante da área da Comporta, apontam para que a discrepância observada entre os resultados das experiências laboratoriais e de campo possa estar associada a um comportamento diferencial das espécies envolvidas nos dois tipos de ensaio. Em conclusão, embora o uso de vestuário tratado com microcápsulas de repelentes seja um método promissor na protecção contra as picadas de insectos, este terá de beneficiar de algum investimento futuro para que possa vir a ser considerado uma estratégia válida no controlo vectorial a larga escala. Há que melhorar o modo de incorporação e apresentação do composto activo nos tecidos de modo a obter-se um efeito repelente mais efectivo e prolongado e a procura de repelentes naturais, indutores de menor toxicidade e mais repelência, deve ser continuada.

Na ausência de uma retaguarda familiar capaz de se constituir como uma rede de segurança e apoio ao desenvolvimento integral de adolescentes com medida de promoção e proteção, o Apartamento de Autonomização oferece um espaço no qual os jovens podem treinar competências que lhes assegurem um futuro autónomo e minimizem os riscos de exclusão social. Esta resposta social propõe-se preparar adolescentes, em transição para a adultez juvenil, para a conquista da responsabilidade de se autoprotegerem, de cuidarem de si próprios e de assumirem a sua identidade perante os outros. Todavia, um projeto de Autonomização de Vida revela-se um desafio, não apenas para os jovens, como também para as famílias e para os profissionais que com eles trabalham. Foram, precisamente, as dificuldades inerentes à Autonomização de Vida em contexto institucional que motivaram o desenvolvimento de um projeto em educação e intervenção social promotor da eficiência dessa resposta social em termos de promoção da autonomia e da transição bem-sucedida para a vida adulta. O presente relatório constitui, assim, um olhar retrospetivo sobre o Projeto “Tornar-se Adulto na Casa 5”, o qual, através da metodologia de Investigação-Ação Participativa, visou alcançar a finalidade proposta pelos participantes: “Promover uma autonomia plena dos jovens da Casa 5, com vista à transição bem-sucedida para a vida adulta após o término da medida de promoção e proteção”. Ainda que os resultados obtidos tenham sido moderadamente satisfatórios, o Projeto terá contribuído para o desenvolvimento de uma consciencialização mais crítica acerca das oportunidades e dos constrangimentos ao desenvolvimento da Autonomia, favorecendo, desejavelmente, a transição para vida adulta após a desinstitucionalização.

The future of health care delivery is becoming more citizen-centred, as today’s user is more active, better informed and more demanding. The European Commission is promoting online health services and, therefore, member states will need to boost deployment and use of online services. This makes e-health adoption an important field to be studied and understood. This study applied the extended unified theory of acceptance and usage technology (UTAUT2) to explain patients’ individual adoption of e-health. An online questionnaire was administrated Portugal using mostly the same instrument used in UTAUT2 adapted to e-health context. We collected 386 valid answers. Performance expectancy, effort expectancy, social influence, and habit had the most significant explanatory power over behavioural intention and habit and behavioural intention over technology use. The model explained 52% of the variance in behavioural intention and 32% of the variance in technology use. Our research helps to understand the desired technology characteristics of ehealth. By testing an information technology acceptance model, we are able to determine what is more valued by patients when it comes to deciding whether to adopt e-health systems or not.

Economics is a social science which, therefore, focuses on people and on the decisions they make, be it in an individual context, or in group situations. It studies human choices, in face of needs to be fulfilled, and a limited amount of resources to fulfill them. For a long time, there was a convergence between the normative and positive views of human behavior, in that the ideal and predicted decisions of agents in economic models were entangled in one single concept. That is, it was assumed that the best that could be done in each situation was exactly the choice that would prevail. Or, at least, that the facts that economics needed to explain could be understood in the light of models in which individual agents act as if they are able to make ideal decisions. However, in the last decades, the complexity of the environment in which economic decisions are made and the limits on the ability of agents to deal with it have been recognized, and incorporated into models of decision making in what came to be known as the bounded rationality paradigm. This was triggered by the incapacity of the unboundedly rationality paradigm to explain observed phenomena and behavior. This thesis contributes to the literature in three different ways. Chapter 1 is a survey on bounded rationality, which gathers and organizes the contributions to the field since Simon (1955) first recognized the necessity to account for the limits on human rationality. The focus of the survey is on theoretical work rather than the experimental literature which presents evidence of actual behavior that differs from what classic rationality predicts. The general framework is as follows. Given a set of exogenous variables, the economic agent needs to choose an element from the choice set that is avail- able to him, in order to optimize the expected value of an objective function (assuming his preferences are representable by such a function). If this problem is too complex for the agent to deal with, one or more of its elements is simplified. Each bounded rationality theory is categorized according to the most relevant element it simplifes. Chapter 2 proposes a novel theory of bounded rationality. Much in the same fashion as Conlisk (1980) and Gabaix (2014), we assume that thinking is costly in the sense that agents have to pay a cost for performing mental operations. In our model, if they choose not to think, such cost is avoided, but they are left with a single alternative, labeled the default choice. We exemplify the idea with a very simple model of consumer choice and identify the concept of isofin curves, i.e., sets of default choices which generate the same utility net of thinking cost. Then, we apply the idea to a linear symmetric Cournot duopoly, in which the default choice can be interpreted as the most natural quantity to be produced in the market. We find that, as the thinking cost increases, the number of firms thinking in equilibrium decreases. More interestingly, for intermediate levels of thinking cost, an equilibrium in which one of the firms chooses the default quantity and the other best responds to it exists, generating asymmetric choices in a symmetric model. Our model is able to explain well-known regularities identified in the Cournot experimental literature, such as the adoption of different strategies by players (Huck et al. , 1999), the inter temporal rigidity of choices (Bosch-Dom enech & Vriend, 2003) and the dispersion of quantities in the context of di cult decision making (Bosch-Dom enech & Vriend, 2003). Chapter 3 applies a model of bounded rationality in a game-theoretic set- ting to the well-known turnout paradox in large elections, pivotal probabilities vanish very quickly and no one should vote, in sharp contrast with the ob- served high levels of turnout. Inspired by the concept of rhizomatic thinking, introduced by Bravo-Furtado & Côrte-Real (2009a), we assume that each per- son is self-delusional in the sense that, when making a decision, she believes that a fraction of the people who support the same party decides alike, even if no communication is established between them. This kind of belief simplifies the decision of the agent, as it reduces the number of players he believes to be playing against { it is thus a bounded rationality approach. Studying a two-party first-past-the-post election with a continuum of self-delusional agents, we show that the turnout rate is positive in all the possible equilibria, and that it can be as high as 100%. The game displays multiple equilibria, at least one of which entails a victory of the bigger party. The smaller one may also win, provided its relative size is not too small; more self-delusional voters in the minority party decreases this threshold size. Our model is able to explain some empirical facts, such as the possibility that a close election leads to low turnout (Geys, 2006), a lower margin of victory when turnout is higher (Geys, 2006) and high turnout rates favoring the minority (Bernhagen & Marsh, 1997).

In the present thesis I analyse the roles of individual ability and structural embeddedness on entrepreneurial success. The results retrieved from a matched employer-employee longitudinal data set show prior worker productivities and environmental embeddedness to have a persistent positive impact on the size and growth rates of new firms. What is more, embeddedness facilitates the impact of ability on start-up performance with outsiders of comparable abilities starting smaller and slower growing firms. Those in higher ability categories are more likely to transfer and also, albeit to a lesser extent, close their ventures, an effect attributed to the higher opportunity costs associated with the group. Firms managed by embedded agents enjoy longer longevities and have better chances of finding a new owner after the departure of the previous one. Finally, higher ability types show evidence of specialisation in serial entrepreneurship.

OBJECTIVE-We studied whether manganese-enhanced high-field magnetic resonance (MR) imaging (MEHFMRI) could quantitatively detect individual islets in situ and in vivo and evaluate changes in a model of experimental diabetes.RESEARCH DESIGN AND METHODS-Whole pancreata from untreated (n = 3), MnCl(2) and glucose-injected mice (n = 6), and mice injected with either streptozotocin (STZ; n = 4) or citrate buffer (n = 4) were imaged ex vivo for unambiguous evaluation of islets. Exteriorized pancreata of MnCl(2) and glucose-injected mice (n = 6) were imaged in vivo to directly visualize the gland and minimize movements. In all cases, MR images were acquired in a 14.1 Testa scanner and correlated with the corresponding (immuno)histological sections.RESULTS-In ex vivo experiments, MEHFMRI distinguished different pancreatic tissues and evaluated the relative abundance of islets in the pancreata of normoglycemic mice. MEHFMRI also detected a significant decrease in the numerical and volume density of islets in STZ-injected mice. However, in the latter measurements the loss of beta-cells was undervalued under the conditions tested. The experiments on the externalized pancreata confirmed that MEHFMRI could visualize native individual islets in living, anesthetized mice.CONCLUSIONS-Data show that MEHFMRI quantitatively visualizes individual islets in the intact mouse pancreas, both ex vivo and in vivo. INTRODUCTION: EORTC trial 22991 was designed to evaluate the addition of concomitant and adjuvant short-term hormonal treatments to curative radiotherapy in terms of disease-free survival for patients with intermediate risk localized prostate cancer. In order to assess the compliance to the 3D conformal radiotherapy protocol guidelines, all participating centres were requested to participate in a dummy run procedure. An individual case review was performed for the largest recruiting centres as well. MATERIALS AND METHODS: CT-data of an eligible prostate cancer patient were sent to 30 centres including a description of the clinical case. The investigator was requested to delineate the volumes of interest and to perform treatment planning according to the protocol. Thereafter, the investigators of the 12 most actively recruiting centres were requested to provide data on five randomly selected patients for an individual case review. RESULTS: Volume delineation varied significantly between investigators. Dose constraints for organs at risk (rectum, bladder, hips) were difficult to meet. In the individual case review, no major protocol deviations were observed, but a number of dose reporting problems were documented for centres using IMRT. CONCLUSIONS: Overall, results of this quality assurance program were satisfactory. The efficacy of the combination of a dummy run procedure with an individual case review is confirmed in this study, as none of the evaluated patient files harboured a major protocol deviation. Quality assurance remains a very important tool in radiotherapy to increase the reliability of the trial results. Special attention should be given when designing quality assurance programs for more complex irradiation techniques.

Novel cancer vaccines are capableto efficiently induce and boost humantumor antigen specific T-cells. However,the properties of these CD8T-cells are only partially characterized.For in depth investigation ofT-cells following Melan-A/MART-1peptide vaccination in melanoma patients,we conducted a detailed prospectivestudy at the single cell level.We first sorted individual human naiveand effector CD8 T-cells from peripheralblood by flow cytometry, andtested a modified RT-PCR protocolincluding a global amplification ofexpressed mRNAs to obtain sufficientcDNAfromsingle cells.We successfullydetected the expression ofseveral specific genes of interest evendown to 106-fold dilution (equivalentto 10-5 cell). We then analyzed tumor-specific effector memory (EM)CD8T-cell subpopulations ex vivo, assingle cells from vaccinated melanomapatients. To elucidate the hallmarksof effective immunity the genesignatures were defined by a panel ofgenes related to effector functions(e.g. IFN-, granzyme B, perforin),and individual clonotypes were identifiedaccording to the expression ofdistinct T-cell receptors (TCR). Usingthis novel single cell analysis approach,we observed that T-cell differentiationis clonotype dependent,with a progressive restriction in TCRBV clonotype diversity from EMCD28pos to EMCD28neg subsets. However,the effector function gene imprintingis clonotype-independent,but dependent on differentiation,since it correlates with the subset oforigin (EMCD28pos or EMCD28neg). We also conducted a detailedcomparative analysis after vaccinationwith natural vs. analog Melan-Apeptide. We found that the peptideused for vaccination determines thefunctional outcome of individualT-cell clonotypes, with native peptideinducing more potent effector functions.Yet, selective clonotypic expansionwith differentiation was preservedregardless of the peptide usedfor vaccination. In summary, the exvivo single cell RT-PCR approach ishighly sensitive and efficient, andrepresents a reliable and powerfultool to refine our current view of molecularprocesses taking place duringT-cell differentiation.

In contemporary society, religious signification and secular systems mix and influence each other. Holistic conceptions of a world in which man is integrated harmoniously with nature meet representations of a world run by an immanent God. On the market of the various systems, the individual goes from one system to another, following his immediate needs and expectations without necessarily leaving any marks in a meaningful long term system. This article presents the first results of an ongoing research in Switzerland on contemporary religion focusing on (new) paths of socialization of modern that individuals and the various (non-) belief systems that they simultaneously develop
