Este estudo teve como objetivos verificar a adesão de portadores de transtorno afetivo bipolar (TAB) à terapêutica medicamentosa e identificar possíveis causas de adesão e não adesão ao medicamento de acordo com o perfil farmacoterapêutico. Trata-se de estudo transversal, descritivo, realizado em Núcleo de Saúde Mental de um município do interior paulista. Participaram do estudo 101 pacientes com TAB. Para coleta dos dados, utilizou-se a entrevista estruturada e o teste de Morisky-Green e, para a análise dos mesmos, o programa Statistical Package for the Social Science. Os resultados mostraram que a maioria (63%) dos sujeitos investigados não adere ao medicamento. Apesar de não ter ocorrido diferenças significativas entre o grupo de aderentes e não aderentes, para as variáveis investigadas, foi possível verificar a utilização de polifarmacoterapia e regimes terapêuticos complexos no tratamento do TAB. Permanece como desafio a implementação de estratégias que possam melhorar, na prática, a adesão de pacientes ao tratamento medicamentoso.

We study optimal public rationing of an indivisible good and private sector price responses. Consumers differ in their wealth and costs of provisions. Due to a limited budget, some consumers must be rationed. Public rationing determines the characteristics of consumers who seek supply from the private sector, where a firm sets prices based on consumers' cost information and in response to the rationing rule. We consider two information regimes. In the first, the public supplier rations consumers according to their wealth information. In equilibrium, the public supplier must ration both rich and poor consumers. Supplying all poor consumers would leave only rich consumers in the private market, and the firm would react by setting a high price. Rationing some poor consumers is optimal, and implements price reduction in the private market. In the second information regime, the public supplier rations consumers according to consumers' wealth and cost information. In equilibrium, consumers are allocated the good if and only if their costs are below a threshold. Wealth information is not used. Rationing based on cost results in higher equilibrium total consumer surplus than rationing based on wealth. [Authors]

In Finland, European Union membership and economic globalisation have changed the position of regions from closed territorial systems to nodes of open international networks. The increasing complexity of cities as globalised knowledge centres and functionally specialised and diversified rural areas, and on the other hand growing disparities between prosperous urban cores and lagging peripheral areas are also essential features in contemporary regional development. These trends have produced new needs to promote mutual dialogue between cities and the countryside in western market economies. Urban-rural interaction is an idea which was developed in the late 1990s within regional policy to pull together these new challenges to regional development and handle cities and the countryside as a whole. The aim of my study is to conceptualise the idea of urban-rural interaction, explain the phenomenon theoretically, clarify past and present urban and rural development and analyse regional policies from the interaction angle. The ultimate purpose is to illustrate the existence and nature of particular interaction policy in a globalising society. The general method is discourse analysis, which I use in three cases: Central Finland, South-Ostrobothnia and South-West Finland. Theoretically I have a two-dimensional approach. On the first hand I use World-System theory to explain how the global economy is moulding urban and rural structures at the regional level. On the other hand I use regime theory to explain local political actions and practises between cities and the countryside under the overlapping pressures deriving from reformulated regional structures and policies.
Adaptation to globalisation in Finland has been carried out by strengthening urban centres. The stress in regional policy has been in urban development. The development of the countryside has mostly been implemented by a separate rural policy. At the end of the 1990s and early 2000s Finnish cities have actually shown themselves to be competitive in global markets. The drawbacks of the new growth centre policy have been the sparse network of prosperous cities and their weak spreading effects, which have hindered comprehensive regional development. Tensions between urban and rural areas have also deepened. In this situation the interaction policy is used as a way of balancing development and moderate conflicts within the regions. From this point of view urban-rural interaction can be seen as a way of tackling the challenges of globalisation.
On the other hand the results emphasise that actors involved in regional development still believe, although the hegemonic discourse is on urban policy, that there are opportunities to stimulate progress in the countryside as well. In the situation where regional authorities control development resources, rural development can be successful only if rural actors manage to establish fruitful relationships with their urban partners. This is also the weakness of the programme-based regional policy. If rural municipalities or other actors are for any reason incapable of building development regimes with cities, the offers of interaction policy will be useless.
The problem of the interaction policy is that the focus and methods of it have so far been rather underdeveloped. In order to improve the efficiency of the interaction policy, further research should concentrate on the social processes which define the position of cities and the countryside as partners of interaction, and practises which promote or prohibit the possibilities of developing the interaction policy. The efforts to define different contents of urban-rural interaction or promote interaction projects should not have such an important role in the future as they have had so far. Instead, the focus of interaction policy should be on questions such as how to manage the political tensions between town and country and how to create a positive atmosphere for regional policy where the needs of urban and rural development are promoted equally.

This article presents a formal model of policy decision-making in an institutional framework of separation of powers in which the main actors are pivotal political parties with voting discipline. The basic model previously developed from pivotal politics theory for the analysis of the United States lawmaking is here modified to account for policy outcomes and institutional performances in other presidential regimes, especially in Latin America. Legislators' party indiscipline at voting and multi-partism appear as favorable conditions to reduce the size of the equilibrium set containing collectively inefficient outcomes, while a two-party system with strong party discipline is most prone to produce 'gridlock', that is, stability of socially inefficient policies. The article provides a framework for analysis which can induce significant revisions of empirical data, especially regarding the effects of situations of (newly defined) unified and divided government, different decision rules, the number of parties and their discipline. These implications should be testable and may inspire future analytical and empirical work.

This paper studies oligopolistic competition in off-patent pharmaceutical markets using a vertical product differentiation model. This model can explain the observation that countries with stronger regulations have smaller generic market shares. It can also explain the differences in observed regulatory regimes. Stronger regulation may be due to a higher proportion of production that is done by foreign firms. Finally, a closely related model can account for the observed increase in prices by patent owners after entry of generic producers.

Transcriptome analysis is a powerful tool for unveiling the distribution and magnitude of genetic incompatibilities between hybridizing taxa. The nature of such incompatibilities is closely associated with the evolutionary histories of the parental species and may differ across tissues and between the sexes. In eusocial insects, the presence of castes that experience divergent selection regimes may result in additional distinct patterns of caste-specific hybrid incompatibilities. We analysed levels of expression of >14 000 genes in two life stages of each caste in the fire ants Solenopsis invicta and Solenopsis richteri and in their hybrids. We found strong contributions of both developmental stage and caste to gene expression patterns. In contrast, variability in expression was only weakly associated with taxonomic identity, with hybrid scores falling between those of the two parental species. Hybrid incompatibilities were surprisingly modest, with only 32 genes being mis-expressed, indicating low levels of disruption in gene regulation in hybrids; males and workers each mis-expressed at least seven times as many genes as queens. Interestingly, homologues of many of the mis-expressed genes have been implicated in behavioural variation in Drosophila melanogaster. General expression profiles of hybrids consistently were more similar to those of S. richteri than S. invicta, presumably because S. richteri trans-regulatory elements tend to be dominant and/or because there is an overall bias in the genetic composition of the hybrids towards S. richteri. Altogether, our results suggest that selection acting on each caste may contribute differently to interspecific divergence and speciation in this group of ants.

Les Pays-Bas et la Suisse sont souvent décrits comme ayant des relations entre travail et capital similaires, marquées par des négociations et le compromis. La conviction est courante dans les deux pays qu'il y a une tendance constante au consensus et au compromis dans l'histoire nationale. Cette thèse livre une comparaison historique du développement des relations de classe dans la 1ers moitié du 20eme siècle. Une attention particulière est donnée aux confrontations sévères et partiellement violentes entre les classes qui ont eu lieu pendant et juste après la Première Guerre Mondiale. Dans la crise des années trente, les deux pays ont longtemps maintenu l'attachement à une politique de déflation, ce qui a mené à une nouvelle accentuation des tensions sociales. Dans ce contexte, loin d'être prêts à faire des concessions au mouvement ouvrier, d'importantes composantes du patronat néerlandais et suisse sympathisaient avec un corporatisme autoritaire comme il était prôné par les mouvements et régimes fascistes. Des alliances entre le mouvement ouvrier socialiste et des fractions importantes des classes moyennes étaient capables d'arrêter cette dérive autoritaire, sans avoir assez de force pour imposer leurs propres alternatives. C'est seulement après l'affaiblissement de la droite dure entre la deuxième moitié des années trente et l'immédiat après-guerre que des premiers fondements pour un compromis de classe stable ont été établis. Généralement, l'approche comparée de la thèse permet de mieux saisir les histoires nationales dans leur contexte européen et global et permet donc de dépasser une vue de l'histoire nationale comme «cas spécial».¦Die Beziehungen zwischen Arbeit und Kapital in Niederlande und die Schweiz werden oft als âhnlich und stark von Verhandlungen und von Kompromissen geprâgt beschrieben. In beiden Lândern ist die Auffassung weitverbreitet, dass es in der nationalen Geschichte eine konstante Tendenz zum Konsens und zum Kompromiss gebe. Diese Dissertation bietet einen historischen Vergleich der Entwicklung der Klassenbeziehungen in der ersten Hâlfte des 20. Jahrhunderts. Thematisiert werden insbesondere die ernsthaften und teilweise gewalttàtigen Zusammenstôsse zwischen den Klassen, die sich wàhrend und unmittelbar nach dem Ersten Weltkrieg ereigneten. In der Krise der Dreissigerjahre hielten beide Staaten lange an der Deflationspolitik fest, was erneut zu einer Verschârfung der sozialen Spannungen fïïhrte. Vor diesem Hintergrund lehnten weite Teile der schweizerischen und niederlândischen Grossunternehmer Konzessionen gegenuber der Arbeiterbewegung ab. Vielmehr sympathisierten sie mit einem autoritàren Korporatismus, wie er von den faschistischen Bewegungen und Regimes vertreten wurde. Allianzen zwischen der sozialistischen Arbeiterbewegung und Teilen der Mittelschichten gelang es zwar, diese autoritàren Tendenzen aufzuhalten, nicht aber ihre eigenen Alteraativkonzepte durchzusetzen. Erst nach der Schwâchung der radikalen Rechten in der Zeit zwischen der zweiten Hàlfte der 1930er Jahre und der unmittelbaren Nachkriegszeit nach dem Zweiten Weltkrieg fand sich eine¦Grundlage fur einen stabilen Klassenkompromiss. Generell ermôglicht es der vergleichende Ansatz der Dissertation, die nationale Geschichte in ihrem europâischen und globalen Kontext zu sehen und so eine Sicht auf die Geschichte eines einzelnen Landes als ,,Sonderfall" zu uberwinden.

Host-pathogen interactions are a major evolutionary force promoting local adaptation. Genes of the major histocompatibility complex (MHC) represent unique candidates to investigate evolutionary processes driving local adaptation to parasite communities. The present study aimed at identifying the relative roles of neutral and adaptive processes driving the evolution of MHC class IIB (MHCIIB) genes in natural populations of European minnows (Phoxinus phoxinus). To this end, we isolated and genotyped exon 2 of two MHCIIB gene duplicates (DAB1 and DAB3) and 1665 amplified fragment length polymorphism (AFLP) markers in nine populations, and characterized local bacterial communities by 16S rDNA barcoding using 454 amplicon sequencing. Both MHCIIB loci exhibited signs of historical balancing selection. Whereas genetic differentiation exceeded that of neutral markers at both loci, the populations' genetic diversities were positively correlated with local pathogen diversities only at DAB3. Overall, our results suggest pathogen-mediated local adaptation in European minnows at both MHCIIB loci. While at DAB1 selection appears to favor different alleles among populations, this is only partially the case in DAB3, which appears to be locally adapted to pathogen communities in terms of genetic diversity. These results provide new insights into the importance of host-pathogen interactions in driving local adaptation in the European minnow, and highlight that the importance of adaptive processes driving MHCIIB gene evolution may differ among duplicates within species, presumably as a consequence of alternative selective regimes or different genomic context.

O tema desenvolvido neste trabalho é o sequestro do navio de passageiros português Santa Maria, desencadeado em Janeiro de 1961 no mar das Caraíbas, acção a que foi dado o nome de “Operação Dulcineia”. Esta acção foi perpetrada por um grupo de revolucionários e activistas políticos portugueses e espanhóis, chefiados pelo ex-capitão do exército português Henrique Galvão, pelo espanhol de origem galega José Velo Mosquera, e pelo ex-comandante da marinha de guerra espanhola, também de origem galega, José Fernandéz Vásquez. Teve como principal objectivo desencadear uma acção armada de carácter revolucionário, para chamar a atenção da opinião pública internacional para os regimes ditatoriais vigentes nos países ibéricos, procurando criar uma corrente de convergência política capaz de influenciar ou mesmo provocar a queda dos regimes salazarista e franquista, bem como a libertação dos territórios coloniais portugueses e espanhóis, em África. O desenvolvimento deste trabalho visa conseguir uma melhor compreensão das causas sociais e políticas que estiveram na génese do desencadear da acção, bem como procurar explicitar melhor a envolvência dos “actores” nessa acção armada, acção que se constituiu como o primeiro acto de “pirataria” dos tempos modernos, perpetrado por razões exclusivamente de natureza política. Este facto foi inédito pelo modo como se desenrolou, bem como pela motivação que lhe esteve associada. Os principais métodos utilizados na recolha da informação foram: a pesquisa bibliográfica e de outra documentação avulsa existente em Bibliotecas e Arquivos Históricos militares e civis, contactos com intervenientes na acção, consulta de literatura sobre o assunto. Julga-se que, com este trabalho, fica uma ideia mais clara, concisa e precisa, das razões que motivaram o desencadear da “Operação Dulcineia”, um melhor conhecimento dos seus intervenientes, bem como do modo como se processou a acção revolucionária e as suas consequências.

Résumé en français La thèse de doctorat porte, de manière générale, sur le rôle des acteurs administratifs dans la mise en oeuvre de l'action publique. En particulier, dans le contexte de l'implémentation de la politique suisse d'assurance-chômage, la recherche se focalise sur l'étude de la bureaucratie de guichet, à savoir un type spécifique d'administrations et d'agents publics situés aux premières lignes de l'action étatique et qui sont en contact direct et quotidien avec les usagers. La thèse a pour objectif principal d'obtenir une compréhension détaillée des impacts concrets dont sont porteurs deux types de réformes majeures du secteur public : les réformes de Nouvelle Gestion Publique (NGP) et les nouvelles technologies informatiques. Le questionnement central de la recherche consiste en une étude approfondie des effets de ces réformes sur les bureaucraties de guichet, et en particulier sur deux aspects centraux les concernant : d'une part, le niveau de pouvoir discrétionnaire que les agents publics de base disposent dans l'application de la loi fédérale sur l'assurance-chômage, autrement dit leur marge de manoeuvre ; d'autre part, les manières au travers desquelles ces mêmes agents sont appelés à rendre des comptes quant à leurs actions et leurs décisions, à savoir l'enjeu plus large de la redevabilité publique des acteurs administratifs. Ces enjeux ont été analysés au niveau empirique dans le contexte organisationnel d'une caisse publique cantonale de chômage ayant expérimenté les réformes évoquées ci-dessus. L'organisation choisie a été investiguée au travers d'une étude de cas ethnographique approfondie (observation directe du travail quotidien des agents, entretiens semi-directifs, analyse de documents) pendant une période de six mois environ entre 2008 et 2009. L'analyse empirique fournit quatre résultats : a) de manière générale, les taxateurs de la caisse de chômage disposent d'un faible niveau de pouvoir discrétionnaire ; b) le degré de pouvoir discrétionnaire varie selon le type de tâche ; c) les agents sur le terrain rendent des comptes auprès d'une multiplicité d'acteurs, sur une variété d'aspects de leur travail et au travers de différents mécanismes de contrôle ; d) les outils de NGP et les nouvelles technologies informatiques ont peu d'impact sur l'étendue du pouvoir discrétionnaire des agents mais contribuent à influencer le type de redevabilité publique pratiquée à ce niveau. Summary in English This PhD dissertation deals with the role of public administration in policy implementation. In the Swiss context of unemployment insurance policy, it focuses on street-level bureaucracy, a specific type of public organisations and agents located at the frontline of public action, that is to say low-level civil servants who are in direct, daily and face-to-face contact with citizens. The dissertation aims at a deep understanding of what are the concrete impacts of two main important changes touching public sector organizations : New Public Management reforms (NPM) and Information and Communication technologies (ICT). The main research question consists in assessing the impacts of those reforms on two central issues regarding street-level bureaucrats : on the one hand, the effective degree of discretion frontline agents do have in implementing the federal law on unemployment insurance ; on the other hand, the ways through which these bureaucrats are held accountable about their action and decisions, i.e. accountability regimes at the street-level. These issues have been empirically addressed in the organisational context of a cantonal Unemployment Insurance Funds having experienced the above mentioned reforms. The organisation has been investigated through an in-depth ethnographic case-study (direct observation of daily work, semi-structured interwiews, documentary analysis) in 2008 and 2009 for approximately six months. The empirical analysis indicates that a) in general, street-level agents do exert low degree of policy discretion; b) the level of discretion is variable from one specific task to another ; c) frontline workers are held accountable to many actors, on various aspects of their work and through different mechanisms of control ; d) NPM and ICT instruments have few impact on the issue of policy discretion but more on the type of street-level accountability which is concretely practised at this level.

A gestão da economia é uma actividade inerente a todos os países, uma vez que a escassez de recursos é uma realidade inquestionável. Cabo Verde elegeu a estabilidade de preços como um dos principais pilares da sua política económica e, neste contexto identificou a Paridade Cambial ao Euro como um meio privilegiado para a sua consecução. Neste âmbito, assinou o Acordo de Cooperação Cambial (ACC) em 1998 com Portugal como forma de credibilizar a Paridade Cambial com o Euro. Este trabalho, embora de cariz académico, tem por objectivo estudar as vantagens da Paridade Cambial para a Economia Cabo-verdiana. É composto por quatro capítulos e um glossário. Para sua realização foram consultados livros, Decretos-Lei, sítios, relatórios e boletins do Banco de Cabo Verde (BCV). O Capítulo I descreve os principais regimes cambiais existentes, apresentando as suas características, suas vantagens e desvantagens. O Capítulo II apresenta os critérios e indicadores, normalmente utilizados pelos países na escolha do regime cambial. O Capítulo III fala dos regimes cambiais em Cabo Verde, mostra que até se chegar ao regime cambial actual o país já passou por várias experiências cambiais. O Capítulo IV descreve o ACC, começando por apresentar as suas características e os seus objectivos, analisa as vantagens e os eventuais custos da Paridade Cambial CVE/Euro e por fim fala da credibilidade do regime cambial de Cabo Verde.

PURPOSE OF REVIEW: To review the current experience with angiogenesis inhibitors in the treatment of gliomas. RECENT FINDINGS: Antiangiogenic therapy has recently reached the clinic with the approval of bevacizumab for recurrent glioblastomas. A number of promising antiangiogenic and vasculature-modifying agents are under investigation for newly diagnosed and recurrent malignant gliomas. A recurrence under ongoing or after antiangiogenic therapy is often characterized by a more aggressive and, in particular, invasive phenotype. SUMMARY: Despite impressively high radiological response rates in patients with recurrent malignant glioma, the duration of response is usually short-lived, and the observed effect to a large extent may be due to normalization of the disrupted blood-brain barrier and less due to a direct antitumor effect. Overall survival remains poor. Induction of invasive phenotypes and escape with proangiogenic alternative pathways are contributing to resistance. Investigation of combination regimes targeting several pathways will determine the possibilities to overcome the resistance to antiangiogenic therapy in malignant gliomas. This article summarizes the results of recent clinical trials in this field, points towards mechanisms of resistance arising under angiogenesis inhibition and discusses the challenges for the future.

Embora cada vez mais assumida como um imperativo institucional pelo mundo todo, em Cabo Verde, as experiencias de participação das comunidades locais na gestão da biodiversidade e recursos naturais são até hoje, muito fracas. No entanto, embora legalmente protegidas, ano após ano as populações de tartarugas marinhas vem-se cada vez mais reduzidas, com os respectivos habitats degradados. Num contexto em que importantes recursos da pesca artesanal se encontram em franca diminuição, ou já não são comercialmente viáveis,as comunidades piscatórias enfrentam cada dia condições adversas face a globalização do desenvolvimento económico. Consequentemente, a introdução de sistemas de gestão de recursos costeiros, mais inclusivos, eficazes e eficientes e que reforcem a participação das comunidades, constituem um imperativo. Dessa forma, a conservação das tartarugas marinhas na região de Barlavento, mais propriamente nas ilhas à Noroeste, foi concebida para promover o envolvimento de comunidades costeiras na preservação dessas espécies e de outros recursos marinhos ameaçados, tudo isto em prol do desenvolvimento rural sustentável e redução da pobreza. Os resultados indicam que a abordagem é eficaz, traduzindo-se em 1) Redução efectiva das capturas e consumo local de tartarugas marinhas; 2) Identificação local das espécies, sua distribuição e, identificação das ameaças que enfrentam. Assim, a população de fêmeas da Caretta caretta em São Nicolau, foi identificada como uma das mais abundantes do arquipélago. 3) Trabalhos relevantes no reforço da consciência ambiental da população no geral. As comunidades piscatórias têm livremente apoiado não só na redução da captura de tartarugas mas também tem estado envolvidas em acções de vigilância e fiscalização de praias, prevenindo mesmo a extracção de areia em zonas de postura. Internamente, têm promovido luta contra práticas irresponsáveis de pesca, tais como o uso de dinamite na pesca de pequenos pelágicos. 4) Adopção de instrumentos integradores promovendo a participação interinstitucional e multidisciplinar e, particularmente, a mobilização de recursos, criando espaços para o envolvimento directo das comunidades na concepção e planificação de acções e planos locais de conservação. 5) Investigação participativa, apropriadamente articulada com a comunicação social e fiscalização como responsabilidade compartilhada, enquanto escola eficaz de aprendizagem interactiva de formas alternativas de gestão e utilização sustentável de recursos naturais costeiros. Os desafios resumem na 1) necessidade de um contexto político e legal que oriente e regulamente a implementação de iniciativas de conservação de tartarugas marinhas baseadas na comunidade, em regimes de gestão concertada (co-gestão); 2) no engajamento e apropriação efectiva, assim como a devida articulação institucional e, 3) sustentabilidade económica das acções.

O trabalho de investigação ora apresentado, intitulado “Ntóni Denti d’Oro – O Rei do Batuque” teve para além do cumprimento das obrigações curriculares para a obtenção do grau de licenciatura em Estudos Cabo-verdianos e Portugueses pelo Instituto Superior de Educação (ISE) uma faceta motivadora bastante grande, porque permite-nos aprofundar conhecimentos teóricos adquiridos ao longo do curso na disciplina de Cultura Caboverdiana. Segundo alguns documentos consultados, a cultura pode ser entendida como um conjunto de instituições e de obras que constituem a herança social de uma comunidade, através da qual se exprime a maneira colectiva de pensar e de sentir e assegurar a integração do indivíduo numa colectividade. A par da tradição oral, o Batuque constitui desde os primórdios da ocupação e povoamento da Ilha de Santiago, um dos meios privilegiados de expressão deste povo. Por meio desse género, os Santiaguenses cantam e dançam as suas alegrias, criticam e aconselham, desafiam e resistem os regimes políticos e as vicissitudes da vida. Ntóni Denti d’Oro como sendo homem que se dedica ao Batuque, é o alvo sobre o qual iremos debruçar. Pretendemos analisar o contributo dado através desse género na afirmação da identidade cultural dos Santiaguenses, o papel desempenhado por ele na conservação e divulgação de uma das manifestações culturais mais antigas de Cabo Verde, transcrever e analisar as letras de algumas das cantigas mais conhecidas.

This paper studies how the strength of intellectual property rights (IPRs) affects investments in biological innovations when the value of an innovation is stochastically reduced to zero because of the evolution of pest resistance. We frame the problem as a research and development (R&D) investment game in a duopoly model of sequential innovation. We characterize the incentives to invest in R&D under two competing IPR regimes, which differ in their treatment of the follow-on innovations that become necessary because of pest adaptation. Depending on the magnitude of the R&D cost, ex ante firms might prefer an intellectual property regime with or without a “research exemption” provision. The study of the welfare function that also accounts for benefit spillovers to consumers—which is possible analytically under some parametric conditions, and numerically otherwise—shows that the ranking of the two IPR regimes depends critically on the extent of the R&D cost.
