50 years ago, the introduction of penicillin, followed by many other antibacterial agents, represented an often underestimated medical revolution. Indeed, until that time, bacterial infections were the prime cause of mortality, especially in children and elderly patients. The discovery of numerous new substances and their development on an industrial scale gave us the illusion that bacterial infections were all but vanquished. However, the widespread and sometimes uncontrolled use of these agents has led to the selection of bacteria resistant to practically all available antibiotics. Bacteria utilize three main resistance strategies: (1) modification of their permeability, (2) modification of target, and (3) modification of the antibiotic. Bacteria modify their permeability either by becoming impermeable to antibiotics, or by actively excreting the drug accumulated in the cell. As an alternative, they can modify the structure of the antibiotic's molecular target--usually an essential metabolic enzyme of the bacterium--and thus escape the drug's toxic effect. Lastly, they can produce enzymes capable of modifying and directly inactivating antibiotics. In addition, bacteria have evolved extremely efficient genetic transfer systems capable of exchanging and accumulating resistance genes. Some pathogens, such as methicillin-resistant Staphylococcus aureus and multiresistant Mycobacterium tuberculosis, have become resistant to almost all available antibiotics and there are only one or two substances still active against such organisms. Antibiotics are very precious drugs which must be administered to patients who need them. On the other hand, the development of resistance must be kept under control by a better comprehension of its mechanisms and modes of transmission and by abiding by the fundamental rules of anti-infectious chemotherapy, i.e.: (1) choose the most efficient antibiotic according to clinical and local epidemiological data, (2) target the bacteria according to the microbiological data at hand, and (3) administer the antibiotic in an adequate dose which will leave the pathogen no chance to develop resistance.

Em 137 amostras de alimentos de diferentes origens (animal e vegetal) foi investigada a ocorrência de sorotipos de Escherichia coli mais comumente descritos como produtores de enterotoxinas. A análise sorológica dos antígenos somáticos, de envoltórios e flagelares nas 265 culturas isoladas, resultou na identificação de 34 amostras distribuídas em doze sorotipos e oriundas de 24 produtos de origem animal. Outros aspectos foram analisados, visando associá-los como possíveis marcadores epidemiológicos. Assim, na biotipificação, verificou-se o perfil das 34 amostras diante da melibiose, rafinose, sacarose, salicina e sorbitol, obtendo-se a caracterização de 11 biotipos. Todavia, a acentuada heterogeneidade de biotipos distribuídos pelos sorotipos, não permitiu um relacionamento de tipos soro-fermentativos com as fontes de isolamento. Os demais testes, representados pela atividade hemolítica e a capacidade hemaglutinante, pouco acrescentaram para o problema da diferenciação de fenótipos ou na caracterização de marcadores epidemiológicos.

Enteropathogenic E. coli (EPEC) infection of Hep-2 cells preoceeds through bacterial attachment to cell surface and internalization of adhered bacteria. EPEC attachment is a prerequisite for cell infection and is mediated by adhesins that recognize carbohydrate-containing receptors on cell membrane. Such endocytosis-inducer adhesins (EIA) also promote EPEC binding to infant enterocytes, suggesting that EIA may have an important role on EPEC gastroenteritis.

Over the past twenty years, many authors have reported evidence of the immunoprotective capacity of ribosomes isolated from bacteria, fungi and parasites. Since 1971 we have explored the protective capacity of ribosomes isolated from a large variety of microorganisms responsible for human and animal diseases. More recently, using monoclonal antibodies raised against ribosomes and then selected for their ability to confer passive immunity to mice, we have studied the mechanism of the protection induced by ribosomes. These studies, in parallel with the development of a technology for the large scale production of ribosomes, have allowed us to achieve a new regard for ribosomal vaccines for use in human. The general concept of ribosomal vaccines in presented and examples of two such vaccines are described with data on the specific protection that they induce in mice against experimental infections with Klebsiella peneumoniae, Streptococcus pneumoniae, S. pyogenes and Haemophilus influenzae for the first one, and against Candida albicans type A and type B for the second one. Because of their high immunogenicity and their innocuity these vaccines represent a decisive improvement over classical microbial vaccines.

Estudamos 59 Escherichia coli uropatogênicas (ECUP) obtidas de pacientes com infecção urinária e 30 E. coli originárias das fezes de indivíduos normais. Cada amostra originou-se de um paciente ou controle. Verificamos que 44% e 3,3% respectivamente eram hemolíticas em meio sólido segundo a origem. Apenas 15% das ECUP hemolíticas produziram alfa-hemolisina, isoladamente ou em associação com ß-hemolisina. A alfa-hemolisina correspondeu a 92% das amostras com atividade hemolítica. Não encontramos correlação entre títulos de alfa-hemolisina e o sítio de origem das ECUP (infecção alta ou baixa). Em 71% das ECUP e 30% das E. coli fecais detectamos a produção de citotoxina com ação citocida para linhagens celulares epitelióides como Vero, He-La e Hep-2 e pouco ativa para fibroblastos de embrião de galinha. A produção desta citotoxina não apresenta correlação com a síntese de hemolisinas. Não verificamos associação entre títulos citotóxicos e origem das ECUP. Certas características biológicas desta citotoxina como a resposta morfológica que determina nas células, o aumento dos títulos citotóxicos com o tempo, sua atividade citocida irreversível e sua termolabilidade sugerem analogia com a Verotoxina (VT) de E. coli. As células afetadas pela citoxina inicialmente mostram aspecto estrelado, tornam-se arredondadas e finalmente desprendem-se do seu suporte. É sugerido que a produção de citotoxina por E. coli aderidas às mucosas do trato urinário possa contribuir para a agressão ao uroepitélio.

OBJECTIVES: In patients with septic shock, circulating monocytes become refractory to stimulation with microbial products. Whether this hyporesponsive state is induced by infection or is related to shock is unknown. To address this question, we measured TNF alpha production by monocytes or by whole blood obtained from healthy volunteers (controls), from patients with septic shock, from patients with severe infection (bacterial pneumonia) without shock, and from patients with cardiogenic shock without infection. MEASUREMENTS: The numbers of circulating monocytes, of CD14+ monocytes, and the expression of monocyte CD14 and the LPS receptor, were assessed by flow cytometry. Monocytes or whole blood were stimulated with lipopolysaccharide endotoxin (LPS), heat-killed Escherichia coli or Staphylococcus aureus, and TNF alpha production was measured by bioassay. RESULTS: The number of circulating monocytes, of CD14+ monocytes, and the monocyte CD14 expression were significantly lower in patients with septic shock than in controls, in patients with bacterial pneumonia or in those with cardiogenic shock (p < 0.001). Monocytes or whole blood of patients with septic shock exhibited a profound deficiency of TNF alpha production in response to all stimuli (p < 0.05 compared to controls). Whole blood of patients with cardiogenic shock also exhibited this defect (p < 0.05 compared to controls), although to a lesser extent, despite normal monocyte counts and normal CD14 expression. CONCLUSIONS: Unlike patients with bacterial pneumonia, patients with septic or cardiogenic shock display profoundly defective TNF alpha production in response to a broad range of infectious stimuli. Thus, down-regulation of cytokine production appears to occur in patients with systemic, but not localised, albeit severe, infections and also in patients with non-infectious circulatory failure. Whilst depletion of monocytes and reduced monocyte CD14 expression are likely to be critical components of the hyporesponsiveness observed in patients with septic shock, other as yet unidentified factors are at work in this group and in patients with cardiogenic shock.

Candidaemia is the fourth most common cause of bloodstream infection, with a high mortality rate of up to 40%. Identification of host genetic factors that confer susceptibility to candidaemia may aid in designing adjunctive immunotherapeutic strategies. Here we hypothesize that variation in immune genes may predispose to candidaemia. We analyse 118,989 single-nucleotide polymorphisms (SNPs) across 186 loci known to be associated with immune-mediated diseases in the largest candidaemia cohort to date of 217 patients of European ancestry and a group of 11,920 controls. We validate the significant associations by comparison with a disease-matched control group. We observe significant association between candidaemia and SNPs in the CD58 (P = 1.97 × 10(-11); odds ratio (OR) = 4.68), LCE4A-C1orf68 (P = 1.98 × 10(-10); OR = 4.25) and TAGAP (P = 1.84 × 10(-8); OR = 2.96) loci. Individuals carrying two or more risk alleles have an increased risk for candidaemia of 19.4-fold compared with individuals carrying no risk allele. We identify three novel genetic risk factors for candidaemia, which we subsequently validate for their role in antifungal host defence.

Rapport de synthèse : Le Magnet Tracking System (MTS) est une technique peu invasive d'investigation de la motilité de l'entier du tube digestif. Elle repose sur le suivi de la progression d'un aimant par des senseurs externes en temps réel et dans les 3 dimensions. Dans cette étude, le MTS a été utilisé pour étudier les caractéristiques de propulsion propres aux différents segments coliques ainsi que pour comparer le transit de l'aimant permanent du MTS à celui de marqueurs radioopaques habituellement utilisés. Dix hommes et 10 femmes ayant un transit gastro-intestinal régulier ont ingéré simultanément un aimant de MTS et une capsule contenant 10 marqueurs radio-opaques, à 20h00. Les enregistrements se sont ensuite déroulés sur 2 matinées successives de 5 heures. L'analyse des données brutes recueillies a permis de réaliser une projection spatio-temporelle de la trajectoire de l'aimant dans le tube digestif ainsi qu'une description précise de l'origine, de la direction, de l'amplitude et de la vitesse des mouvements coliques. Des radiographies d'abdomen ont permis de comparer les positions respectives des marqueurs radio-opaques et de l'aimant du MTS. Durant 90% du temps d'enregistrement, l'aimant était immobile ou présentait des mouvements alternatifs de faible amplitude. Le reste des enregistrements consiste en activité propulsive dont 20% représentent des déplacements rétrogrades et une description très précise de 34 mouvements de masses. L'analyse des déplacements démontre une distribution bimodales des vitesses voisine de 1.5 et 50 cm /min, ce tant en direction orale que caudale. Deux tiers des distances parcourues le sont à vitesse rapide. L'analyse segmentaire confirme une progression horaire absolue supérieure dans le côlon gauche que droit. L'analyse détaillée par segment colique, reposant sur la description des déplacements enregistrés correspond aux rôles reconnus des différents segments, notamment de aire de stockage et de conditionnement du côlon ascendant ou de transit du côlon descendant. La comparaison des 2 sexes démontre un nombre plus important de mouvements, particulièrement de mouvements de masse chez l'homme. Les radiographies montrent une bonne corrélation entre la position de l'aimant et celle des marqueurs radio-opaques. Le MTS permet ainsi une description précise des caractéristiques propulsives des différents segments coliques, notamment par l'analyse détaillée des progressions à vitesses lente et rapide et leurs directions. Des distinctions peuvent également être notées en fonction du sexe. Ces investigations offrent de nouvelles perspectives pour l'étude des troubles de la motilité digestive.

P fimbriae are proteinaceous appendages on the surface of Escherichia coli bacteria that mediate adherence to uroepithelial cells. E. coli that express P fimbriae account for the majority of ascending urinary tract infections in women with normal urinary tracts. The hypothesis that P fimbriae on uropathic E. coli attach to renal epithelia and may regulate the immune response to establish infection was investigated. The polymeric Ig receptor (pIgR), produced by renal epithelia, transports IgA into the urinary space. Kidney pIgR and urine IgA levels were analyzed in a mouse model of ascending pyelonephritis, using E. coli with (P+) and without (P-) P fimbriae, to determine whether P(+) E. coli regulate epithelial pIgR expression and IgA transport into the urine. (P+) E. coli establish infection and persist to a greater amount than P(-) E. coli. P(+)-infected mice downregulate pIgR mRNA and protein levels compared with P(-)-infected or PBS controls at &gt; or =48 h. The decrease in pIgR was associated with decreased urinary IgA levels in the P(+)-infected group at 48 h. pIgR mRNA and protein also decline in P(+) E. coli-infected LPS-hyporesponsive mice. These studies identify a novel virulence mechanism of E. coli that express P fimbriae. It is proposed that P fimbriae decrease pIgR expression in the kidney and consequently decrease IgA transport into the urinary space. This may explain, in part, how E. coli that bear P fimbriae exploit the immune system of human hosts to establish ascending pyelonephritis.

Social deprivation can have negative effects on the lives of social animals, including humans, yet little is known about the mechanisms by which social withdrawal affects animal health. Here we show that in the carpenter ant Camponotus fellah, socially isolated workers have a greatly reduced life span relative to ants kept in groups of ten individuals. By using a new tracking system, we found that social isolation resulted in important behavioral changes and greatly increased locomotor activity. The higher activity of single ants and their increased propensity to leave the nest to move along the walls suggested that the increased mortality of isolated ants might stem from an imbalance of energy income and expenditure. This view was supported by the finding that while isolated ants ingested the same amount of food as grouped ants, they retained food in the crop, hence preventing its use as an energy source. Moreover, the difference in life span between single and grouped individuals vanished when ants were not fed. This study thus underlines the role of social interactions as key regulators of energy balance, which ultimately affects aging and health in a highly social organism.
