A população “sem-abrigo” e todos os problemas envolventes, como a pobreza e a exclusão social apresentam uma complexidade cada vez mais inquietante e preocupante na nossa sociedade. O fenómeno multifacetado dos sem-abrigo constitui um grave problema social. A pertinência deste estudo prende-se com a escassez de investigação relativa à população sem-abrigo e perceber melhor a sua complexidade, nomeadamente no que respeita à caracterização psicológica dos indivíduos sem-abrigo. Neste sentido, o presente estudo tem como objectivo conhecer melhor esta população através da recolha de informação relativa aos seus receios, valores, prazeres, percepção do motivo da sua situação, consumo de substâncias psicotrópicas, contacto com a família e amigos. Paralelamente pretendemos avaliar os sentimentos de vergonha (em função de experiências negativas precoces), de paranóia e emoções negativas (depressão, ansiedade e stress) nesta população específica e analisar a relação entre estas variáveis. A amostra é constituída por 56 sujeitos na condição de sem-abrigo, com uma média de idade de 40,86 e de 7 anos de escolaridade, sendo a maioria (n=49) do sexo masculino. O protocolo de avaliação, para além do questionário psicossocial desenvolvido para o efeito, incluía ainda escalas para medir a Depressão, Ansiedade e Stress (DASS-21), Vergonha Externa (OAS), Vergonha Interna (ISS) e Paranóia (GPS). Os dados do nosso estudo permitiram pôr em evidência algumas das dificuldades mais significativas dos sem-abrigo, nomeadamente, as rupturas dos laços familiares, o desemprego e a toxicodependência entre outros. Constatámos que os sem-abrigo da nossa amostra apresentam valores médios mais elevados de paranóia, vergonha externa e interna, quando comparados com os valores da população normal. Verifica-se ainda que quando mais escolarizada é a nossa amostra, menor sintomatologia depressiva e relacionada com o stress apresenta. Encontrámos correlações significativas positivas e moderadas entre as variáveis em estudo. Dada a escassez de investigação em indivíduos sem-abrigo, e apesar das limitações inerentes a este estudo, esperamos ter dado um contributo para alargar o conhecimento deste complexo fenómeno, nomeadamente no que respeita à caracterização psicológica destes indivíduos. / The homeless population and all the related issues such as poverty and social exclusion are a complex and increasingly worrying concern of our society. The multifaceted phenomenon of homelessness is a serious social problem. The relevance of this study relates to the scarcity of research on the homeless population and to better understand its complexity, particularly the psychological characteristics of the homeless individuals. As such, this study aims to better understand this population by collecting information on their fears, values, pleasures, perception of the reason for their situation, consumption of psychotropic substances, contact with family and friends. Simultaneously we intend to evaluate the feelings of shame (due to early negative experiences), paranoia and negative emotions (depression, anxiety and stress) in this specific population as well as analyse the relationship between these variables. The study sample is based on 56 individuals in the condition of homelessness, mostly male (n = 49), with an average age of 40.86 and 7 years of schooling. In addition to the psychological questionnaire developed for this purpose, the evaluation protocol also included scales to measure Depression, Anxiety and Stress (DASS-21), External Shame (OAS), Internal Shame (ISS) and Paranoia (GPS). The data in our study allowed to highlight some of the most significant problems of homelessness, including disruption of family ties, unemployment, drug abuse and others. We learnt that the homeless in our sample have a higher average number of paranoia, internal and external shame, when compared to the normal population. We have also found that the more educated our sample is, the less are the depressive symptoms and stress-related features. Among the studied variables, we have found significant positive and moderate correlations. Given the scarcity of research on homeless individuals and despite the limitations of this study, we hope to have contributed to increase the knowledge on this complex phenomenon, especially concerning the psychological characterization of these individuals.

The discussions concerning the absence of a management model appropriate to the peculiarities of third sector organizations have not been impeditive to their emphasized expansion in the last decades. In the attempt of understanding this phenomenon from the perspective of those who manage social organizations, this work based on the theory of social representations to understand the notion that organization managers of the third sector - based in Fortaleza CE - have of the part that they play and how this notion influences the direction of their activities. Social representations of managers of four different categories of non-governmental organizations have been investigated, each category composed of two unities. The categories researched were: social integration through art and education, prevention and treatment of alcohol and drug abuse, children s health assistance and community action. By using Doise s Societal Approach, the role of social managers translated in intraindividual, interindividual and situational processes of their actions, has been analysed within the social representations, focusing on beliefs, values, symbols and stories that give meaning to the existence of non-governmental organizations. Analysis and discussion of data displayed the existence of diversity in the understanding of managers within their practice, in other words, the management profile is also its own manager s. The branch where an organization acts is also preponderant in the shaping of a management style. It could be deduced, from to the organizations researched, that professional formation and the manager s social insertion mainly, are determinative factors in the outlining of a management model of its own. It was concluded that, due to heterogeneity of interests and action segments, there is no systematic process for social management among organizations. Management styles are supported by their director s own perception of achievement, who model organizations according to their contingencies

Introdução: com o aumento da toxicodependência, que engloba um problema mundial, também cresce o número de doentes com suas devidas consequências, que são patologias sistémicas, englobando a saúde oral, que é uma das partes do corpo que mais sofre com a toxicodependência. Objetivos: Esta tese tem como objetivo a revisão da literatura sobre “toxicodependência” e “medicina dentária”, correlacionando estes temas com patologias e complicações na cavidade oral, nomeadamente, bruxismo, desgaste dental, xerostomia, halitose, cárie rampante e doença periodontal. Metodologia: Foi realizada uma revisão bibliográfica com base em artigos publicados em revistas e teses disponíveis em bibliotecas online, nos últimos dez anos. As palavraschaves usadas foram, assim como a conjugação entre elas e seus devidos idiomas: toxicodependência, desintoxicação, abstinência, cannabis, álcool, metanfetamina, heroína, opiáceos, tabaco, nicotina, metadona, erosão dentária, xerostomia, cáries, bruxismo, entre outras. Conclusão: Preconiza-se que haja uma maior atenção e conhecimento por parte do médico dentista sobre esta condição, bem como das complicações associadas, uma vez que é um dos principais profissionais de saúde a ter o contacto e a oportunidade de tratar os pacientes sintomáticos, como também os que estão em fase de recuperação e reinserção social.

Background and aims: The prevalence of anti-HCV and HBsAg in Portugal has been shown to be elevated in high-risk groups, such as intravenous drug-users and incarcerated individuals. However, in the general population, prevalence remains largely unknown. The aims of this study were to estimate the prevalence of anti-HCV and HBsAg in the general Portuguese population and identify associated risk factors. Materials and methods: We carried out a nationwide, population-based cross-sectional study of adults resident in mainland Portugal. Serology for HBsAg, anti-HBc, anti-HBs, and anti-HCV was performed. Anti-HCV-positive individuals were tested for HCV RNA by PCR. Results: Of 1685 participants, 50.6% were men, mean age 50.2±18.3 years. In terms of hepatitis C, the prevalence of anti-HCV was 0.54% [95% confidence interval (CI): 0.2–0.9] and 0.12% (95% CI: 0.0–0.3) were viremic, with peak prevalence among individuals 35–64 years of age (0.8%), men (0.8%), and individuals from Lisbon and Tagus Valley region (1.9%). In terms of hepatitis B, the estimated prevalence of HBsAg was 1.45% (95% CI: 0.9–2.0). A higher prevalence was found in individuals who were 35–64 years old (2.2%), in men (2.5%), and in the Northern region (2.6%). The presence of positive serological markers of hepatitis C virus and hepatitis B virus infection did not correlate with elevated aminotransferases, race, place of birth, and alcohol consumption. Conclusion: These results suggest a low endemicity for both hepatitis B and hepatitis C in the general population, in contrast to a very high prevalence in risk groups, thus suggesting that targeted screening to high-risk groups may be more cost-effective than general population screening.

Background: Tracheal intubation is extremely distressing, painful, and may influence heart rate and blood pressure. Sedatives, analgesics, and muscle relaxants are not commonly used for intubation in neonates. Objectives: This study aimed to evaluate the effects of lidocaine spray as a non-intravenous drug before neonatal intubation on blood pressure, heart rate, oxygen saturation and time of intubation. Patients and Methods: In a randomized, controlled study each neonate was randomly assigned to one of the two study groups by staffs who were not involved in the infant's care. The allocation concealment was kept in an opaque sealed envelope, and the investigators, the patient care team, and the assessors were blinded to the treatment allocation. The selected setting was NICU unit of a teaching hospital in Ilam city, Iran and participants were 60 neonates with indication of tracheal intubation with gestational age >30 weeks. Patients in the treatment group received lidocaine spray and the placebo group received spray of normal saline prior to intubation. Main outcome measurements were the mean rates of blood pressure, heart rate, oxygen saturation, intubation time and lidocaine side effects were measured before and after intubation. Results: Totally 60 newborns including 31 boys and 29 girls were entered into the study (drug group n = 30; placebo group n = 30). Boy/girl ratio in treatment and placebo groups were 1.3 and 0.88, respectively. Mean age ± SD of participants was 34.1 ± 24.8 hours (treatment: 35.3 ± 25.7; placebo: 32.9 ± 24.3; P < 0.0001). Mean weight ± SD of neonates was 2012.5 ± 969 g. Application of lidocaine spray caused a significant reduction of mean intubation time among treatment group compared with placebo group (treatment: 15.03 ± 2.2 seconds; placebo: 18.3 ± 2.3 seconds; P < 0.0001). Mean blood pressure, heart rate and oxygen saturation rate, among neonates in treatment group was reduced after intubation compared with their relevant figures before intubation; however, their differences were not statistically significant except for mean oxygen saturation rate that was reduced significantly in placebo group. No side effects were observed during study. Conclusions: Though the current study revealed some promising results in the application of lidocaine spray during neonatal intubation without any considerable side effects; however, the current investigation could only be considered as a pilot study for further attempts in different locations with higher sample sizes and in different situations.

Objective: The burden of sexually transmitted infections (STIs) rests with young people, yet in Ireland there has been very little research into this population. The purpose of this study was to determine the incidence rate and establish risk factors that predict STI occurrence among adolescents in Ireland. Design: Routine diagnostic, demographic and behavioural data from first-time visits to three screening centres in the southwest of Ireland were obtained. Univariate and multivariable logistic regression models were used to assess risk factors that predict STI occurrence among adolescents. Results: A total of 2784 first-time patients, aged 13–19 years, received 3475 diagnoses between January 1999 and September 2009; 1168 (42%) of adolescents had notifiable STIs. The incidence rate of STIs is 225/100 000 person-years. Univariate analysis identified eligible risk factors (p<0.2) for inclusion in the multivariable model. Multivariable logistic regression showed the dominant risk factors for STI diagnosis to be: males who sometimes [odds ratio (OR) 2.02] or never (OR 1.83) use condoms; and females 18–19 years (OR 2.26) and 16–18 years (OR 1.8), with 2 (OR 1.33) or 3+ (OR 1.56) partners in the last 12 months, who are non-intravenous drug users (OR 0.72), are most likely to receive a positive STI diagnosis. Conclusions: STI diagnosis has become increasingly common in Ireland. The proportion of notifications among those aged under 20 years is increasing. These data illustrate the significance of age, condom use and number of sexual partners as risk factors for STI diagnosis. Furthermore, providing data for the first time, we report on the high incidence rate of STIs among adolescents in Ireland. The high levels of risk-taking behaviour and STI acquisition are highlighted and suggest that there is a need for an integrated public health approach to combat this phenomenon in the adolescent population.

Liquid chromatography coupled with mass spectrometry is one of the most powerful tools in the toxicologist’s arsenal to detect a wide variety of compounds from many different matrices. However, the huge number of potentially abused substances and new substances especially designed as intoxicants poses a problem in a forensic toxicology setting. Most methods are targeted and designed to cover a very specific drug or group of drugs while many other substances remain undetected. High resolution mass spectrometry, more specifically time-of-flight mass spectrometry, represents an extremely powerful tool in analysing a multitude of compounds not only simultaneously but also retroactively. The data obtained through the time-of-flight instrument contains all compounds made available from sample extraction and chromatography, which can be processed at a later time with an improved library to detect previously unrecognised compounds without having to analyse the respective sample again. The aim of this project was to determine the utility and limitations of time-of-flight mass spectrometry as a general and easily expandable screening method. The resolution of time-of-flight mass spectrometry allows for the separation of compounds with the same nominal mass but distinct exact masses without the need to separate them chromatographically. To simulate the wide variety of potentially encountered drugs in such a general screening method, seven drugs (morphine, cocaine, zolpidem, diazepam, amphetamine, MDEA and THC) were chosen to represent this variety in terms of mass, properties and functional groups. Consequently, several liquid-liquid and solid phase extractions were applied to urine samples to determine the most general suitable and unspecific extraction. Chromatography was optimised by investigating the parameters pH, concentration, organic solvent and gradient of the mobile phase to improve data obtained by the time-of-flight instrument. The resulting method was validated as a qualitative confirmation/identification method. Data processing was automated using the software TargetAnalysis, which provides excellent analyte recognition according to retention time, exact mass and isotope pattern. The recognition of isotope patterns allows excellent recognition of analytes even in interference rich mass spectra and proved to be a good positive indicator. Finally, the validated method was applied to samples received from the A& E Department of Glasgow Royal Infirmary in suspected drug abuse cases and samples received from the Scottish Prison Service, which we received from their own prevalence study targeting drugs of abuse in the prison population. The obtained data was processed with a library established in the course of this work.

La reducción de la demanda, específicamente por medios alternativos al sistema penal tradicional, ha sido uno de los componentes más discutidos en los análisis que abordan la lucha contra las drogas. Este artículo se propone determinar el papel de la Organización de Estados Americanos en el establecimiento de los Tribunales de Tratamiento de Drogas en Estados Unidos como mecanismo de reducción del daño en el periodo comprendido entre 1989 y 2013. Se sugiere que la organización internacional, a través de la CICAD, ha jugado un papel de promoción, y evaluación de los Tribunales de Tratamiento de Drogas en Estados Unidos, generando presión entre sus Estados miembro, dada la necesidad de instaurar alternativas legales de reducción del daño. Sin embargo, se presentan deficiencias en los mecanismos de evaluación, ya que las indicaciones realizadas no tienen un carácter obligatorio y por tanto las determinaciones de la CICAD no necesariamente son adoptadas.
