Post-absorptive glucose lowering (PALG) is observed in individuals with glucose intolerance and in healthy individuals. We report a prevalence of about 23% among healthy Asian Indians. Individuals with PALG are characterized by leaner phenotype, low body fat percentage, increased insulin sensitivity and higher fasting glucose levels. (C) 2014 Elsevier Ireland Ltd. Objective: The aim of this study is to validate the applicability of the PolyVinyliDene Fluoride (PVDF) nasal sensor to assess the nasal airflow, in healthy subjects and patients with nasal obstruction and to correlate the results with the score of Visual Analogue Scale (VAS). Methods: PVDF nasal sensor and VAS measurements were carried out in 50 subjects (25-healthy subjects and 25 patients). The VAS score of nasal obstruction and peak-to-peak amplitude (Vp-p) of nasal cycle measured by PVDF nasal sensors were analyzed for right nostril (RN) and left nostril (LN) in both the groups. Spearman's rho correlation was calculated. The relationship between PVDF nasal sensor measurements and severity of nasal obstruction (VAS score) were assessed by ANOVA. Results: In healthy group, the measurement of nasal airflow by PVDF nasal sensor for RN and LN were found to be 51.14 +/- 5.87% and 48.85 +/- 5.87%, respectively. In patient group, PVDF nasal sensor indicated lesser nasal airflow in the blocked nostrils (RN: 23.33 +/- 10.54% and LN: 32.24 +/- 11.54%). Moderate correlation was observed in healthy group (r = 0.710, p < 0.001 for RN and r = 0.651, p < 0.001 for LN), and moderate to strong correlation in patient group (r = 0.751, p < 0.01 for RN and r = 0.885, p < 0.0001 for LN). Conclusion: PVDF nasal sensor method is a newly developed technique for measuring the nasal airflow. Moderate to strong correlation was observed between PVDF nasal sensor data and VAS scores for nasal obstruction. In our present study, PVDF nasal sensor technique successfully differentiated between healthy subjects and patients with nasal obstruction. Additionally, it can also assess severity of nasal obstruction in comparison with VAS. Thus, we propose that the PVDF nasal sensor technique could be used as a new diagnostic method to evaluate nasal obstruction in routine clinical practice. (C) 2015 Elsevier Inc. All rights reserved.

Resumen: Las células madre tienen dos características naturales únicas, que son su capacidad de división celular indefinida, y de regeneración y reparación de tejidos. Esto genera un gran interés en las áreas de investigación y tratamiento de enfermedades que aún no tienen cura y son motivadoras de una importante fuente de esperanza para los pacientes. Surge, entonces, el dilema ético de definir cuáles serán los recursos para obtenerlas, procesarlas y determinar la seguridad y eficacia de su uso en la aplicación clínica. Un enfoque personalista ontológico considerará la protección de todos los participantes en los procesos de investigación: los donantes de células, los participantes de la investigación y los pacientes que serán beneficiarios de estos tratamientos, de manera de proteger a todos los involucrados sin exponer a riesgos a unos para salvar a otros.

Background: While pain is frequently associated with unipolar depression, few studies have investigated the link between pain and bipolar depression. In the present study we estimated the prevalence and characteristics of pain among patients with bipolar depression treated by psychiatrists in their regular clinical practice. The study was designed to identify factors associated with the manifestation of pain in these patients.- Methods:Patients diagnosed with bipolar disorder (n=121) were selected to participate in a cross-sectional study in which DSM-IV-TR criteria were employed to identify depressive episodes. The patients were asked to describe any pain experienced during the study, and in the 6 weeks beforehand, by means of a Visual Analogical Scale (VAS).- Results: Over half of the bipolar depressed patients (51.2%, 95% CI: 41.9%–60.6%), and 2/3 of the female experienced concomitant pain. The pain was of moderate to severe intensity and prolonged duration, and it occurred at multiple sites, significantly limiting the patient’s everyday activities. The most important factors associated with the presence of pain were older age, sleep disorders and delayed diagnosis of bipolar disorder.- Conclusions: Chronic pain is common in bipolar depressed patients, and it is related to sleep disorders and delayed diagnosis of their disorder. More attention should be paid to study the presence of pain in bipolar depressed patients, in order to achieve more accurate diagnoses and to provide better treatment options.

[ES]Los cateterismos venosos periféricos son los dispositivos que con mayor frecuencia se emplean en el acceso vascular para pacientes que se encuentran ingresados en unidades hospitalarias. Estos catéteres son reemplazados sistemáticamente cada tres o cuatro días para tratar de prevenir la flebitis como refleja la guía de Center for Disease control and prevention (CDC) y por extensión, nuestro sistema de salud de Osakidetza. Sin embargo, la evidencia que apoya esta práctica no está del todo cimentada.El objetivo de esta revisión bibliográfica es evaluar la efectividad de esta práctica clínica tan integrada en la vida diaria de los profesionales de enfermería mediante la evaluación de la evidencia científica existente hasta el momento.Se realizo una búsqueda exhaustiva en diferentes bases de datos electrónicas desde Octubre de 2012 hasta Abril del año 2013. Se descargaron los textos completos de aquellos artículos que pudiesen ser potencialmente útiles en el estudio y se analizaron bajo los criterios de inclusión y selección. Los siete artículos seleccionados como válidos no demuestran que sea necesario sustituir de forma sistemática el catéter venoso periférico así como lo defiende la CDC. Debido a ello, se podría abolir esta práctica clínica que reduciría significativamente el dolor y las molestias que sufren los pacientes día a día, el tiempo que el personal de enfermería dedica en este tipo de prácticas, además de todo el coste sanitario que ello con lleva.

Esse estudo aborda a perversão, tema de extrema relevância política e clínica no âmbito psicanalítico, que se revela como um dos problemas cruciais da psicanálise. Sua importância política imprime-se quando ressalta, com Lacan, que a política da psicanálise é a política da falta-a-ser correlata à ética do desejo e quando propõe que aceitar a diversidade do gozo, com suas múltiplas modalidades, e levar o sujeito a desvelar e a confrontar-se com o seu mais-de-gozar é uma indicação ética que deve orientar a política e a prática do psicanalista. A pesquisa, elaborada a partir dos fundamentos teórico-clínicos de Freud e Lacan, destaca o movimento lógico que delimita a perversão na obra freudiana e verifica uma convergência nas teses de ambos os autores, no que se refere à diferenciação entre a perversidade e a perversão como estrutura clínica. A construção de uma série de casos clínicos e o estudo da vida e da obra de cinco famosos escritores Marquês de Sade, Sacher-Masoch, André Gide, Jean Genet, Yukio Mishima vêm ilustrar que as práticas de gozo perverso não determinam a estrutura perversa. O matema da fantasia sadiana, forjado por Lacan, é tomado para demonstrar a Verleugnung freudiana, o modo que os sujeitos perversos encontram para lidar com castração da mãe/mulher. As fórmulas quânticas da sexuação, formuladas por Lacan, são utilizadas para evidenciar que a diferença entre a neurose e a perversão se explicita na estratégia de gozo que o sujeito utiliza na relação com o seu parceiro. A pesquisa, que se iniciou bibliográfica e se desenvolveu de cunho teórico-clínico, desvela que a clínica da perversão muito pode ensinar aos psicanalistas sobre os quatro conceitos fundamentais da psicanálise, sobre a relação entre o fetiche, a máscara e o semblante, sobre a sublimação, sobre as estratégias de gozo, em particular, o masoquismo. Esse estudo convoca a comunidade analítica ao debate, uma vez que enuncia, através da apresentação de casos de perversão, os impasses da clínica, além de presentificar o real da clínica na condução do tratamento de um caso de perversão.

As lesões impalpáveis da mama que muitas das vezes são assintomáticas, podem corresponder à um estágio de progressão de câncer difícil de ser detectado, durante os exames de rotina de palpação da mulher. O único método possível para a descoberta dessas lesões é através dos exames de imagem da mama, de modo geral, através da mamografia, que geralmente ocorre após os 45 anos. Devido a esses fatores, lesões impalpáveis, são frequentemente, descobertas apenas quando o estágio de desenvolvimento da doença já está avançado e as intervenções terapêuticas são menos reparadoras. Com a finalidade de iniciar a caracterização de tumores impalpáveis iniciais, objetivamos analisar o perfil genético (mutação) e epigenético (metilação de região promotora) de regiões do DNA relacionadas ao gene supressor tumoral TP53, provenientes de biópsias de mulheres residentes do Estado do Rio de Janeiro. Neste trabalho, foram investigadas 34 amostras de tecido de tumor de mama, por sequenciamento de DNA, nos exons de 5 a 8 do gene TP53. Nesta região, não foi encontrada nenhuma mutação. Este resultado pode estar relacionado ao tipo inicial de lesão, de acordo com os dados radiológicos das lesões de categorias 3 e 4 da escala BIRADS. Para verificar o estado de metilação da região promotora do gene TP53, analisamos 30 pares de amostras (sangue e tumor) de pacientes com suspeita de câncer de mama, pela técnica MSP-PCR. Nenhuma amostra tumoral apresentou alteração no estado de metilação na região promotora do gene TP53, quando comparada à amostra normal. Um motivo possível para a disparidade de resultados em relação à outros trabalhos pode ter sido a utilização da técnica. A caracterização das lesões impalpáveis apenas foi iniciada neste trabalho, no qual pudemos constatar que a mutação em TP53 pode ser um evento mais tardio. Portanto, a lesão mamária, em suas diferentes formas, continuará a ser o assunto investigado por nosso grupo, ampliando o número de amostras e alcançando melhor conexão da conduta e dos métodos clínicos já existentes, com as novas possibilidades de diagnóstico via marcadores moleculares em tumores e fluidos biológicos

Background: Neonatal trials remain difficult to conduct for several reasons: in particular the need for study sites to have an existing infrastructure in place, with trained investigators and validated quality procedures to ensure good clinical, laboratory practices and a respect for high ethical standards. The objective of this work was to identify the major criteria considered necessary for selecting neonatal intensive care units that are able to perform drug evaluations competently. Methodology and Main Findings: This Delphi process was conducted with an international multidisciplinary panel of 25 experts from 13 countries, selected to be part of two committees (a scientific committee and an expert committee), in order to validate criteria required to perform drug evaluation in neonates. Eighty six items were initially selected and classified under 7 headings: "NICUs description - Level of care'' (21), "Ability to perform drug trials: NICU organization and processes (15), "Research Experience'' (12), "Scientific competencies and area of expertise'' (8), "Quality Management'' (16), "Training and educational capacity'' (8) and "Public involvement'' (6). Sixty-one items were retained and headings were rearranged after the first round, 34 were selected after the second round. A third round was required to validate 13 additional items. The final set includes 47 items divided under 5 headings. Conclusion: A set of 47 relevant criteria will help to NICUs that want to implement, conduct or participate in drug trials within a neonatal network identify important issues to be aware of. Bipolar disorder (BD) and alcohol use disorders (AUDs) are usually comorbid, and both have been associated with significant neurocognitive impairment. Patients with the BD-AUD comorbidity (dual diagnosis) may have more severe neurocognitive deficits than those with a single diagnosis, but there is paucity of research in this area. To explore this hypothesis more thoroughly, we carried out a systematic literature review through January 2015. Eight studies have examined the effect of AUDs on the neurocognitive functioning of BD patients. Most studies found that BD patients with current or past history of comorbid AUDs show more severe impairments, especially in verbal memory and executive cognition, than their non-dual counterparts. Greater neurocognitive dysfunction is another facet of this severe comorbid presentation. Implications for clinical practice and research are discussed. Specifically, the application of holistic approaches, such as clinical staging and systems biology, may open new avenues of discoveries related to the BD-AUD comorbidity.
