An analytical methodology for the simultaneous determination of seven pharmaceuticals and two metabolites belonging to the non-steroidal anti-inflammatory drugs (NSAIDs) and analgesics therapeutic groups was developed based on off-line solid-phase extraction and ultra-high performance liquid chromatography coupled to tandem mass spectrometry (SPE–UHPLC–MS/MS). Extraction conditions were optimized taking into account parameters like sorbent material, sample volume and sample pH. Method detection limits (MDLs) ranging from 0.02 to 8.18 ng/L were obtained. This methodology was successfully applied to the determination of the selected pharmaceuticals in seawater samples of Atlantic Ocean in the Northern Portuguese coast. All the pharmaceuticals have been detected in the seawater samples, with pharmaceuticals like ibuprofen, acetaminophen, ketoprofen and the metabolite hydroxyibuprofen being the most frequently detected at concentrations that can reach some hundreds of ng/L.

Considering the scarcity of defined antigens, actually useful and reliable for use in the field studies, we propose an alternative method for selection of cDNA clones with potential use in the diagnosis of schistosomiasis. Human antibodies specific to a protein fraction of 31/32 kDa (Sm31/32), dissociated from immune complexes, are used for screening of clones from an adult worm cDNA library. Partial sequencing of five clones, selected through this strategy, showed to be related to Schistosoma mansoni: two were identified as homologous to heat shock protein 70, one to glutathione S-transferase, one to homeodomain protein, and one to a previously described EST (expressed sequence tag) of S. mansoni. This last clone was the most consistently reactive during the screening process with the anti-Sm31/32 antibodies dissociated from the immune complexes. The complete sequence of this clone was obtained and the translation data yielded only one ORF (open reading frame) that code for a protein with 57 amino acids. Based on this amino acid sequence two peptides were chemically synthesized and evaluated separately against a pool of serum samples from schistosomiasis patients and non-schistosomiasis individuals. Both peptides showed strong reactivity only against the positive pool, suggesting that these peptides may be useful as antigens for the diagnosis of schistosomiasis mansoni.

A isoimunização RhD durante a gravidez tem graves repercussões fetais e neonatais. Apesar da imunoprofilaxia com imunoglobulina anti-D ter diminuído drasticamente a mortalidade e morbilidade perinatais, continuam a existir casos de isoimunização que se devem a uma administração inadequada. No âmbito de um projecto de padronização da administração de imunoglobulina anti-D, foram levados a cabo um inquérito aos obstetras da MAC e uma análise retrospectiva de processos de mulheres RhD negativas. Neste artigo são apresentados os resultados destes dois trabalhos. É também apresentada uma proposta de protocolo de imunoprofilaxia.

The aim of this study was to assess the antioxidant and anti-schistosomal activities of the garlic extract (AGE) and Nigella sativa oil (NSO) on normal and Schistosoma mansoni-infected mice. AGE (125 mg kg-1, i.p.) and NSO (0.2 mg kg-1, i.p.) were administrated separately or in combination for successive 28 days, starting from the 1st day post infection (pi). All mice were sacrificed at weeks 7 pi. Hematological and biochemical parameters including liver and kidney functions were measured to assess the progress of anemia, and the possibility of the tissue damage. Serum total protein level, albumin, globulin and cholesterol were also determined. Malondialdehyde (MDA) and glutathione (GSH) levels were determined in the liver tissues as biomarkers for oxidative and reducing status, respectively. The possible effect of the treatment regimens on Schistosoma worms was evaluated by recording percentage of the recovered worms, tissue egg and oogram pattern. Result showed that, protection with AGE and NSO prevented most of the hematological and biochemical changes and markedly improved the antioxidant capacity of schistosomiasis mice compared to the infected-untreated ones. In addition, remarkable reduction in worms, tissue eggs and alteration in oogram pattern were recorded in all the treated groups. The antioxidant and antischistosomal action of AGE and NSO was greatly diverse according to treatment regimens. These data point to these compounds as promising agents to complement schistosomiasis specific treatment.

Depression is associated with decreased serotonin metabolism and functioning in the central nervous system, evidenced by both animal models of depression and clinical patient studies. Depression is also accompanied by decreased hippocampal neurogenesis in diverse animal models. Neurogenesis is mainly defined in dentate gyrus of hippocampus as well as subventricular zone. Moreover, hypothalamus, amygdala, olfactory tubercle, and piriform cortex are reported with evidences of adult neurogenesis. Physical exercise is found to modulate adult neurogenesis significantly, and results in mood improvement. The cellular mechanism such as adult neurogenesis upregulation was considered as one major mood regulator following exercise. The recent advances in molecular mechanisms underlying exercise-regulated neurogenesis have widen our understanding in brain plasticity in physiological and pathological conditions, and therefore better management of different psychiatric disorders.

Introdução e Objectivo: A presença de anticorpos antifosfolípidos (AAF) está associada a perdas embrionárias e fetais e a outras complicações obstétricas como pré-eclâmpsia, restrição de crescimento intra-uterino e parto pré-termo. Os doentes com lúpus eritematoso sistémico (LES) com frequência possuem AAF e síndroma antifosfolípidica (SAAF) secundária. O objectivo deste estudo é avaliar comparativamente o prognóstico da gravidez em mulheres com LES na presença e na ausência de AAF. Material e Métodos:Avaliação retrospectiva de 136 gestações em mulheres com diagnóstico prévio de LES cujo seguimento foi realizado na nossa instituição entre 1993 e 2007. As grávidas com e sem AAF foram consideradas separadamente. Dados relativos a idade materna, história obstétrica passada, actividade do LES no início da gravidez, existência de nefropatia, evolução da gravidez actual, idade gestacional no parto, tipo de parto, peso do recém-nascido e a existência de complicações hipertensivas na gravidez foram analisados. Resultados: 28% (38) das grávidas com LES apresentavam AAF. Deste grupo, 28,9% tinham história de perdas fetais do segundo e/ou terceiro trimestre, em oposição a 6,12% nas doentes sem AAF(p<0,05). A avaliação da actual gravidez demonstrou uma taxa de insucesso na gravidez significativamente mais elevada na população com AAF(8,1% versus 2,1%, p<0,05). Não se verificaram diferenças significativas no peso médio do recém-nascido e na taxa de restrição intra-uterina em função da presença de AAF. A idade gestacional media no parto foi de 36,9 semanas em ambos os grupos com uma taxa idêntica de parto pré-termo. O parto ocorreu por cesariana em 47% das doentes com AAF e em 44,1% das doentes sem AAF. Conclusões:Os antecedentes obstétricos de perda fetal do 2º e 3º trimestre e a probabilidade de aborto espontâneo são mais frequentes em grávidas com LES e AAF. Nos restantes parâmetros analisados não se verificaram diferenças significativas em função da presença de AAF.

Toxoplasmosis is an usually asymptomatic worldwide disseminated infection. In its congenital presentation it may lead to abortion or fetal malformations. Antenatal evaluation is considered of paramount importance to identify seronegative women and allow for prophylaxis. Recent improvements in sensitivity of IgM tests has made IgM detection an extremely protracted acute phase marker, and IgG avidity evaluation test became necessary. Observation has shown that a correlation can be established between IgM levels and avidity percentages, suggesting that frequently the avidity test may not be necessary. In this study we analyzed Toxoplasma gondii IgM levels of 202 samples and their IgG avidity percentages, in order to define specific levels whose IgM quantification could by itself define serodiagnosis and therefore make the avidity evaluation unnecessary. We showed that for IgM levels bellow 2.0 and above 6.0 serodiagnosis of toxoplasmosis could be established without need of IgG avidity test. IgM levels between these two parameters are associated with varying avidity indexes highlighting the importance of its evaluation as a means to confirm toxoplasmosis. Following this demonstration it was possible to avoid the avidity test for 75% of the cases, to reduce the turnaround time and to reduce costs.

Nos doentes com infecção pelo vírus da imunodeficiência humana (VIH) o citomegalovírus torna-se um agente de doença importante quando existe imunossupressão avançada. O seu papel como agente de doença pulmonar neste contexto tem sido amplamente debatido. Nos doentes com pneumocistose, a presença do citomegalovírus no pulmão não parece conferir pior prognóstico, excepto nos que recebem terapêutica adjuvante com corticóides. Os autores apresentam dois casos de doentes com infecção VIH e imunossupressão avançada, admitidos na unidade de cuidados intensivos por insuficiência respiratória. Em ambos houve isolamento de Pneumocystis jirovecii no lavado broncoalveolar. Apesar da terapêutica instituída ambos vieram a falecer. A biópsia pulmonar post mortem mostrou, nos dois casos, a presença de Pneumocystis e inclusões por citomegalovírus. Perante este achado tecem-se algumas considerações sobre o papel do citomegalovírus como agente de pneumonia na SIDA e sobre o seu significado como co-infectante na pneumocistose, sobretudo nos casos de falência terapêutica.

BACKGROUND: Prospective testing for posttransplant circulating anti-HLA antibodies seems to be a critical noninvasive tool, but confirmatory data are lacking. MATERIALS AND METHODS: Over the last 3 years, peritubular capillary (PTC) C4d deposition was prospectively sought by an immunofluorescence technique applied to frozen tissue in biopsies obtained for allograft dysfunction. Screening for circulating anti-HLA class I/II alloantibodies (AlloAb) by the flow cytometric test was performed simultaneously. RESULTS: We evaluated 132 sets of biopsies and simultaneous serum samples. PTC C4d deposition was demonstrated in 15.9% (21/132) of biopsies. Circulating anti-HLA I/II AlloAb were detected in 25% (33/132) of serum samples. Employing receiver-operator characteristic (ROC) curves for all C4d-positive biopsies, screening for AlloAb showed a global specificity of 82% and sensitivity of 61.9%. When this analysis was restricted to biopsies obtained in the first month posttransplantation, the sensitivity increased to 81.8%, but the specificity decreased to 76.9%. After the first month posttransplantation, we observed sensitivity of 40.0% and a specificity of 86.4%. In the first month posttransplantation, all patients with a diagnosis of acute antibody-mediated rejection displayed circulating anti-HLA class I/II, but not always at the same time as the C4d-positive biopsy. CONCLUSIONS: In the first month posttransplantation, prospective monitoring of anti-HLA antibodies may be useful. The high sensitivity allows the identification of patients at risk, affording an earlier diagnosis of antibody-mediated rejection. After the first month, the test can be used to evaluate allograft dysfunction episodes, since positivity is highly suggestive of an antibody-mediated process.

The aim of this study was to determine the prevalence of anti-T. gondii total and IgM antibodies in women of childbearing age. One hundred serum samples of women were studied with age range from 11 to 45 years old. Samples were chosen by random. The determination of total antibodies was carried out through the indirect hemagglutination technique and IgM antibodies by ELISA's technique. The statistical analysis was carried out through the Chi square and the Spearman correlation tests. The theoretical estimated incidence of congenital toxoplasmosis was calculated, according to the annual increment of antibody prevalence among the age groups. The overall prevalence of toxoplasmosis was 33%, while only six individuals (18.2%) were positive to IgM. The highest prevalence was observed in the 11-35 year-old age group. The theoretical estimated incidence was 1.5 for 100 pregnancies in women of 21-25 year-old group; it decreased until 0.1% in the 41-45 year-old age group. The findings show a high prevalence of toxoplasmosis in this community with a high infection risk in women of the studied age group and the high cat population observed, suggesting that the transmission way by contaminated soils may play a main role in the spreading of toxoplasmosis in this community.

The present study reports the production of the rabbit anti-Leishmania (L.) chagasi hyperimmune serum, the standardization of the immunohistochemistry (IHC) technique and the evaluation of its employment in cutaneous leishmaniasis (CL) lesions diagnosed by Leishmania sp. culture isolation. Thirty fragments of active CL lesions were examined as well as 10 fragments of cutaneous mycosis lesions as control group. IHC proved more sensitive in detecting amastigotes than conventional hematoxylin-eosin (HE) stained slides: the former was positive in 24 (80%) biopsies whereas the latter, in 16 (53%) (p = 0.028). The reaction stained different fungus species causing cutaneous mycosis. Besides, positive reaction was noticed in mononuclear and endothelial cells. Nevertheless, this finding was present in the control group biopsies. It is concluded that IHC showed good sensitivity in detecting amastigotes.

During myocardial ischemia and reperfusion both purines and pyrimidines are released into the extracellular milieu, thus creating a signaling wave that propagates to neighboring cells via membrane-bound P2 purinoceptors activation. Cardiac fibroblasts (CF) are important players in heart remodeling, electrophysiological changes and hemodynamic alterations following myocardial infarction. Here, we investigated the role UTP on calcium signaling and proliferation of CF cultured from ventricles of adult rats. Co-expression of discoidin domain receptor 2 and -smooth muscle actin indicate that cultured CF are activated myofibroblasts. Intracellular calcium ([Ca2+]i) signals were monitored in cells loaded with Fluo-4 NW. CF proliferation was evaluated by the MTT assay. UTP and the selective P2Y4 agonist, MRS4062, caused a fast desensitizing [Ca2+]i rise originated from thapsigargin-sensitive internal stores, which partially declined to a plateau providing the existence of Ca2+ in the extracellular fluid. The biphasic [Ca2+]i response to UTP was attenuated respectively by P2Y4 blockers, like reactive blue-2 and suramin, and by the P2Y11 antagonist, NF340. UTP and the P2Y2 receptor agonist MRS2768 increased, whereas the selective P2Y11 agonist NF546 decreased, CF growth; MRS4062 was ineffective. Blockage of the P2Y11receptor or its coupling to adenylate cyclase boosted UTP-induced CF proliferation. Confocal microscopy and Western blot analysis confirmed the presence of P2Y2, P2Y4 and P2Y11 receptors. Data indicate that besides P2Y4 and P2Y2 receptors which are responsible for UTP-induced [Ca2+]i transients and growth of CF, respectively, synchronous activation of the previously unrecognized P2Y11 receptor may represent an important target for anti-fibrotic intervention in cardiac remodeling.

Introdução: A coreia, isolada ou associada a outros distúrbios do movimento, pode serforma de apresentação de diversas patologias. A história clínica e o exame objectivo sugerem o diagnóstico e orientam a investigação da generalidade das situações de coreia, sendo a sua etiologia identificada na maioria dos casos. Relato de caso: Adolescente do sexo feminino, 14 anos, residente em Angola, com movimentos involuntários dos membros e face, interpretados como coreia e discinésia oro-facial, e hipomobilidade do membro superior direito com cinco dias de evolução associada a labilidade emocional. Sem história sugestiva de doença estreptocócica, infeccções recentes ou exposição a fármacos. Avaliação laboratorial, incluindo função tiroideia, ceruloplasmina sérica, exame citoquímico e imunoelectroforese do líquor, sem alterações relevantes. Pesquisa de tóxicos negativa. Ressonância magnética crânio-encefálica e electroencefalograma normais. Evidência ecocardiográfica de insuficiência mitral ligeira sem aspectos sugestivos de cardite reumática. Exame bacteriológico do exsudado faríngeo negativo e doseamento de anticorpos anti-estreptococo negativo. Apesar destes resultados realizou penicilina benzatínica. Do restante estudo infeccioso destaca-se serologia compatível com infecção a Borrelia burgdorferi sem envolvimento neurológico. O doseamento de anticorpos anti-N-methyl-D-aspartate receptor (ac anti-NMDAR) foi positivo no soro. Iniciou tratamento sintomático com carbamazepina e haloperidol com resolução das queixas. A segunda amostra para pesquisa de ac anti-NMDAR no líquor e sangue foi negativa. Conclusões: O facto de não haver confirmação de doença estreptocócica prévia, não nos permite assumir a coreia de Sydenham, causa mais frequente de coreia em pediatria. Como a restante investigação não foi conclusiva deverá esta doente ser seguida a longo prazo; talvez a evolução nos venha o dar o diagnóstico final.

Introdução: A síndrome de Stevens-Johnson é uma doença rara com mortalidade de 1 a 5% e morbilidade significativa. Ocorre na sequência de uma reacção de hipersensibilidade imuno-mediada com susceptibilidade individual associada a factores genéticos. Pode ser desencadeada por agentes infecciosos, mas na maior parte dos casos o factor desencadeante é a exposição a fármacos. Caso clínico: Criança de 3 anos, previamente saudável, internada por febre alta, exantema papulovesicular generalizado com predomínio no tronco, dorso e face, enantema e hiperémia conjuntival. Posteriormente verificou-se coalescência das lesões cutâneas com evolução para necrose e descamação. Tinha adicionalmente erosões da mucosa oral, estomatite, edema e eritema dos lábios, períneo e balanite. Fotofobia, hiperémia conjuntival, edema palpebral, exsudado ocular sem sinéquias e córnea sem lesões. Duas semanas antes tinha sido medicado pela primeira vez com ibuprofeno e na admissão hospitalar realizou uma nova administração. Nega ingestão de outros fármacos. PCR para vírus do grupo herpes nas lesões, exames culturais negativos e serologias para Mycoplasma pneumoniae, Borrelia burgdoferi, vírus da hepatite B, Epstein-Barr e citomegalovírus negativos. TASO e anti-DNaseB sem alterações. IFI para vírus respiratórios negativa. Posteriormente identificou-se enterovírus por PCR nas fezes de que se aguarda cultura viral. Foi interrompida a administração de ibuprofeno e realizada terapêutica de suporte com fluidoterapia endovenosa, nutrição parentérica, analgesia sistémica e tópica. Manteve febre durante 10 dias, registando-se regressão progressiva da sintomatologia com melhoria das lesões ao fim de 3 semanas. Programou-se seguimento para rastreio de complicações cutâneo-mucosas e oftalmológicas e estudo de alergias medicamentosas. Comentários: O diagnóstico da síndrome de Stevens-Johnson é clínico e, em caso de dúvida, histológico, suportado por história de exposição a fármacos ou intercorrência infecciosa. A ingestão de ibuprofeno pela primeira vez com agravamento após a reexposição ao fármaco leva-nos a suspeitar ser esta a etiologia mais provável. Contudo, a identificação de enterovírus não permite excluir este agente como interveniente na doença.

The present study investigated if hepatitis B virus (HBV) mutants circulate in the southwestern region of the State of Paraná, Brazil, by analyzing samples from children who received immunoprophylaxis but were born to HBV carrier mothers. Samples from 25 children were screened for HBV serum markers and for HBV DNA by PCR. Only one sample was positive for HBsAg, anti-HBs and HBV DNA, although the child had been vaccinated. Analysis of the S gene sequence of this sample showed the presence of a proline at position 105, a serine at position 114, three threonines at positions 115, 116 and 140, and a glutamine at position 129. The presence of these amino acids, except for serine at position 114, has been related to monoclonal or polyclonal therapy with anti-HBs after liver transplantation, whereas the presence of threonine at position 116 has been described in immunized children from Singapore. This finding demonstrates the possible circulation of HBV strains resistant to hepatitis B immunoprophylaxis in southwestern Paraná, Brazil. The genotype of the sample was identified as genotype D, which is frequently found in the region studied. Since 36% of the children had received incomplete or no immunoprophylaxis, more extensive follow-up of children born to HBsAg-positive mothers is needed.
