Giant aortic aneurysms (transverse diameter greater than 10.0 cm) are rare and open surgery is often the treatment of choice. We report an infrarenal saccular giant aortic aneurysm (measuring 25 cm in transverse diameter), which was treated with endovascular repair, with immediate technical success. No similar report of a giant infrarenal aortic aneurysm treated with an endovascular technique was found in the literature. High-risk patients could possibly benefit from the endovascular technique. Nevertheless, patient survival remains strongly influenced by comorbidities.

Embora haja consenso sobre a validade e facilidade do uso da antropometria na classificação da obesidade abdominal, ainda não existe consenso sobre a nomenclatura da medida e os sítios anatômicos para sua obtenção. Desta forma, o objetivo deste estudo foi analisar de forma crítica o uso da circunferência da cintura ou abdominal em adultos para aferição da obesidade abdominal. Foi realizada uma revisão sistemática a partir de busca em bases de dados indexadas MEDLINE (PubMed) e Scientific Eletronic Library Online (SciELO). Foram revisados artigos publicados no período compreendido entre 2003 e 2009. Quanto à nomenclatura, apesar dos National Institutes Health tratarem circunferência da cintura e abdominal como sinônimos, os pesquisadores internacionais, tem preferido o uso do termo circunferência da cintura. Quanto ao sítio anatômico para a medida, falta ainda consenso entre os pesquisadores e orientação da International Diabetes Federation (IDF). Considerando que os atuais pontos de corte para as populações de origem européia foram elaborados a partir dos sítios anatômicos estabelecidos pela OMS 1989, torna-se razoável a adoção desta referência. Sobre os pontos de corte, a proposta da IDF representa resultado de um consenso internacional e avalizado pela Sociedade Brasileira de Cardiologia.

Endovascular aneurysm repair (EVAR) is already considered the first choice treatment for abdominal aortic aneurysms (AAA). Several different strategies have been used to address limitations to arterial access caused by unfavorable iliac artery anatomy. The aim of this report is to illustrate the advantages and limitations of each option and present the results of using the internal endoconduit technique and the difficulties involved.

Background: Abdominal obesity is an important cardiovascular risk factor. Therefore, identifying the best method for measuring waist circumference (WC) is a priority. Objective: To evaluate the eight methods of measuring WC in patients with acute coronary syndrome (ACS) as a predictor of cardiovascular complications during hospitalization. Methods: Prospective study of patients with ACS. The measurement of WC was performed by eight known methods: midpoint between the last rib and the iliac crest (1), point of minimum circumference (2); immediately above the iliac crest (3), umbilicus (4), one inch above the umbilicus (5), one centimeter above the umbilicus (6), smallest rib and (7) the point of greatest circumference around the waist (8). Complications included: angina, arrhythmia, heart failure, cardiogenic shock, hypotension, pericarditis and death. Logistic regression tests were used for predictive factors. Results: A total of 55 patients were evaluated. During the hospitalization period, which corresponded on average to seven days, 37 (67%) patients had complications, with the exception of death, which was not observed in any of the cases. Of these complications, the only one that was associated with WC was angina, and with every cm of WC increase, the risk for angina increased from 7.5 to 9.9%, depending on the measurement site. It is noteworthy the fact that there was no difference between the different methods of measuring WC as a predictor of angina. Conclusion: The eight methods of measuring WC are also predictors of recurrent angina after acute coronary syndromes. Key words: Evaluation; Acute Coronary Syndrome; Abdominal Circumference

In veterinary medicine, the same clinician usually follows an animal from birth to old age, whereas in human medicine different specialists are required. It is therefore essential that veterinarians know the normal imaging, biochemical, hematological and physical exam values for younglings, which differ significantly from those of adults of the same species. Abdominal radiographies of young animals are poorly defined due to differences in tissue constitution in relation to adults. These same factors, however, substantially improve sonographic images of the region. Nonetheless, the interpretation of B-mode sonographic findings in puppies is difficult due to the lack of information about the normal parameters in this phase of life. The objective of this study was to compile information about peculiarities of abdominal B-mode ultrasonography in puppies and kittens.

The diameters and areas of portal vein, caudal vena cava and abdominal aorta are useful measurements in dogs. These values can be easily measured by ultrasonographic exam, and variations of normality can be an important indicator of hepatic or extra-hepatic alterations. This study aimed to measure the diameter and areas of portal vein, caudal vena cava and abdominal aorta inhealthy dogs, with normal corporal score, divided in groups according to the body weight, and assess whether the data are influenced by animal weight. Thirty dogs were examined and divided into three groups (Group A: ≤ 10 kg Group B: from 10.1 to 20.0 kg; Group C: ≥ 20.1 kg). To measure thediameters and areas of portal vein, caudal vena cava and abdominal aorta, the animal was kept in left lateral decubitus position and the transducer was placed on the right lateral abdominal wall, at approximately the 10th or 11th intercostal space, in the porta hepatis region. The diameters and areas of the portal vein, caudal vena cava and abdominal aorta were significantly lower for dogs in Group A with respect to other groups and the dogs from Groups B and C had similar results with each other. The diameters and areas of the portal vein, caudal vena cava and abdominal aorta may vary with the animal size, and reference values must be specific for small, medium and large dogs. The elimination of all external incisions is an important step in reducing the invasiveness of surgical procedures. Natural Orifice Translumenal Endoscopic Surgery (NOTES) is an incision-less surgery and provides explicit benefits such as reducing patient trauma and shortening recovery time. However, technological difficulties impede the widespread utilization of the NOTES method. A novel robotic tool has been developed, which makes NOTES procedures feasible by using multiple interchangeable tool tips. The robotic tool has the capability of entering the body cavity through an orifice or a single incision using a flexible articulated positioning mechanism and once inserted is not constrained by incisions, allowing for visualization and manipulations throughout the cavity. Multiple interchangeable tool tips of the robotic device initially consist of three end effectors: a grasper, scissors, and an atraumatic Babcock clamp. The tool changer is capable of selecting and switching between the three tools depending on the surgical task using a miniature mechanism driven by micro-motors. The robotic tool is remotely controlled through a joystick and computer interface. In this thesis, the following aspects of this robotic tool will be detailed. The first-generation robot is designed as a conceptual model for implementing a novel mechanism of switching, advancing, and controlling the tool tips using two micro-motors. It is believed that this mechanism achieves a reduction in cumbersome instrument exchanges and can reduce overall procedure time and the risk of inadvertent tissue trauma during exchanges with a natural orifice approach. Also, placing actuators directly at the surgical site enables the robot to generate sufficient force to operate effectively. Mounting the multifunctional robot on the distal end of an articulating tube provides freedom from restriction on the robot kinematics and helps solve some of the difficulties otherwise faced during surgery using NOTES or related approaches. The second-generation multifunctional robot is then introduced in which the overall size is reduced and two arms provide 2 additional degrees of freedom, resulting in feasibility of insertion through the esophagus and increased dexterity. Improvements are necessary in future iterations of the multifunctional robot; however, the work presented is a proof of concept for NOTES robots capable of abdominal surgical interventions.

Gymnotus tiquie, new species, is described from the Rio Tiquie, a tributary of the Uaupes (Vaupes) in the upper Negro basin, Amazonas, Brazil. The new species was collected in non-floodplain (terra firme) streams, where it occurs sympatrically and syntopically with two geographically widespread congeners, the type species of the genus, G. carapo, and G. coropinae. The new species is diagnosed by a unique combination of morphometric, meristic, and osteological traits, and by a characteristic color pattern in which the dark oblique pigment bands, diverse in shape and design, are divided into band-pairs along the length of the body, in which the band-pairs are often recurved (dorsally concave), more variable, and often reticulated in the abdominal region, and in which the pale inter-bands meet at the dorsal midline along most of the length of the body. Gymnotus tiquie is a member of the G. pantherinus species group, with which it shares the presence of one (vs. two) pore in the dorsolateral portion of the preopercle (except in G. pantanal and G. anguillaris), needle-shaped (vs. conical or arrowhead-shaped) teeth on the dentary and premaxilla, and a slender body (BD 5.6-10.6% HL vs. deep 8.7-13.5%, except G. chaviro, G. curupira, G. varzea, G. chimarrao, G. maculosus, G. henni, and G. inaequilabiatus that also have a slender body). Gymnotus tiquie is most similar in overall appearance to G. cataniapo of the upper Orinoco. These two species share three unique features within the G. pantherinus group: dark band-pairs with wavy irregular margins along the length of the body, a long body cavity with 45 or more pre-caudal vertebrae, and a darkly pigmented membrane in the caudal region of the anal fin.
