WOMAC 20, 50, 70 response levels in patients treated with Rofecoxib for osteoarthritis

Aim of study: The goal of this post-hoc analysis was to examine the difference between treatment groups when varying the target response level from at least a 20% improvement from baseline, to at least 50% and 70% improvements in Phase III studies of rofecoxib in patients with osteoarthritis. Methods: The analysis focused on results from two 6-week, placebo-controlled, ibuprofen-comparator, Phase III osteoarthritis studies. These studies employed a flare design requiring a minimum level of symptoms at entry following discontinuation of prior analgesics. Two definitions of ‘‘patient improved’’ from baseline were used: (1) WOMAC-P: a reduction in the WOMAC pain score and (2) WOMAC-PFS: a reduction in the WOMAC pain score and either a reduction in the WOMAC stiffness or function score. The improvement target was increased from 20% to 50% to 70%, relative to baseline, to investigate how the increase affects the ability to detect the differences between treatment groups. Analyses were conducted on the average and last of all measurements collected during a 6-week treatment period. Results: In the ibuprofen-comparator studies, 1545 patients were randomized to placebo, rofecoxib 12.5 mg once daily, rofecoxib 25 mg once daily, and ibuprofen 800 mg three times daily in a 1:3:3:3 ratio. The percentages of patients who met the improvement targets decrease as the target increases from 20% to 50% to 70%. There were meaningful differences between the active treatment and placebo that were inversely related to the improvement target. For example, there was a 31 (P ! 0.001), 21 (P ! 0.001), and 12 (P ! 0.001) percentage-point difference between rofecoxib 25 mg and placebo for the 20%, 50%, and 70% targets for WOMAC-P. For WOMAC-PFS, the differences between rofecoxib 25 mg and placebo were 33 (P ! 0.001), 18 (P ! 0.001), and 9 (P ! 0.01) percentage points for the 20%, 50%, and 70% improvement targets. Conclusions: Meaningful differences between active treatments and placebo were detected at all three response levels associated with the WOMAC-P and WOMAC-PFS endpoints. The differences between groups were more dramatic at the 20% and 50% response levels. The WOMAC (20,50,70)-P and WOMAC (20,50,70)-PFS endpoints further confirm, at an individual patient level, the clinical benefit of rofecoxib in the treatment of osteoarthritis that was previously reported as a difference in means.

Le PS: cher disparu de la rubrique nécrologique?

Le pass´e simple (PS) franc¸ais, en tant que pass´e perfectif, semble fait pour l’article n´ecrologique qui relate les points forts d’une existence dans l’ordre chronologique. Cependant, des linguistes ont affirm´e au cours du 20`eme si`ecle que le PS ´etait mourant. Il vaut donc la peine de voir si le PS est utilis´e dans les articles n´ecrologiques et si son usage d´ecline dans le temps. Cette ´etude se penche sur un corpus synchronique (Le Monde, Le Figaro, L’Humanit´e, La Croix, Le Parisien) et un corpus diachronique (Le Monde, Le Figaro). Elle s’int´eresse `a l’´evolution des formes et des fonctions du PS au cours du temps et dans les diff´erents journaux. Elle ´evalue ´egalement la capacit´e des pass´e compos´e, imparfait, pr´esent, plus-que-parfait et futur simple `a jouer un r ˆole narratif dans l’article n´ecrologique.

Soliton mode locking fiber laser with an all-fiber polarization interference filter

An erbium doped fiber ring laser achieving soliton mode locking by the use of an intra-cavity all-fiber polarization interference filter (AFPIF) has been demonstrated. To incorporate an AFPIF with relative narrow transmission bandwidth, the laser has produced clean soliton pulses of 1.2 ps duration at a repetition rate of 14.98 MHz with a polarization extinction ratio up to 25.7 dB. Moreover, we have demonstrated that the operating wavelength of the mode locking laser can be tuned over 20 nm range from 1545 to 1565 nm by thermally tuning the AFPIF cavity. © 2012 Optical Society of America.

Exclusive contracts with vertically differentiated products

In this paper we develop a simple model to analyze the effects of exclusive contracts in vertically integrated markets where both the upstream and the downstream market are characterized as oligopolies and manufacturers produce vertically differentiated products. We find that firms prefer to deal exclusively with retailers. If the extent of consumers' heterogeneity is small, manufacturers offer exclusive contracts unilaterally. On the other hand, if consumers' valuations differ significantly both manufacturers engage in exclusive contracting.

Measuring Benefits of Technology Spillovers from Foreign Direct Investment in Central and Eastern Europe

In this paper, we construct a composite indicator to estimate the potential of four Central and Eastern European countries (the Czech Republic, Hungary, Poland and Slovakia) to benefit from productivity spillovers from foreign direct investment (FDI) in the manufacturing sector. Such transfers of technology are one of the main benefits of FDI for the host country, and should also be one of the main determinants of FDI incentives offered to investing multinationals by governments, but they are difficult to assess ex ante. For our composite index, we use six components to proxy the main channels and determinants of these spillovers. We have tried several weighting and aggregation methods, and we consider our results robust. According to the analysis of our results, between 2003 and 2007 all four countries were able to increase their potential to benefit from such spillovers, although there are large differences between them. The Czech Republic clearly has the most potential to benefit from productivity spillovers, while Poland has the least. The relative positions of Hungary and Slovakia depend to some extent on the exact weighting and aggregation method of the individual components of the index, but the differences are not large. These conclusions have important implications both for the investment strategies of multinationals and government FDI policies.
