Purpose: To describe corneal graft survival and visual outcome after therapeutic penetrating keratoplasty in patients with Acanthamoeba keratitis (AK) that is unresponsive to clinical treatment. Methods: Retrospective study. Thirty-two patients with AK who underwent therapeutic penetrating keratoplasty (tPK) from August 1996 to August 2005 were included. Data relating to clinical features, visual acuity, surgical technique, graft survival and complications were collected. Graft survival was evaluated by the Kaplan-Meier method and comparisons were performed using the Log-rank test. Results: Most patients (62.5%) were female. Mean age [+/- standard deviation (SD)] was 35 (+/- 13) years (range 15-68 years). All patients were contact lens wearers. Eighteen patients (56%) presented paralytic mydriasis and glaucoma during the treatment. Thirteen patients (40%) developed glaucoma after surgery; eight of them (61%) required a second PK because of graft failure. Of the 32 keratoplasty eyes, 56.2% presented graft failure at any follow-up point. Forty-five per cent of graft failures occurred before the 12 month follow-up, so 55% remained clear in the first year after surgery. Twelve patients underwent a second PK; seven of them failed and 45% were clear at 1 year. Two patients presented graft recurrence of amoebic infection. There was no significant difference in graft survival when eyes with or without mydriasis were compared (P = 0.40). Eyes with glaucoma presented a significantly shorter graft survival (P = 0.01). Conclusion: Penetrating keratoplasty is a treatment option for eyes that are unresponsive to clinical treatment infections. However, graft survival is poor; postoperative glaucoma is frequent and is associated with shorter graft survival.

This paper analyzes empirically the effect of crude oil price change on the economic growth of Indian-Subcontinent (India, Pakistan and Bangladesh). We use a multivariate Vector Autoregressive analysis followed by Wald Granger causality test and Impulse Response Function (IRF). Wald Granger causality test results show that only India’s economic growth is significantly affected when crude oil price decreases. Impact of crude oil price increase is insignificantly negative for all three countries during first year. In second year, impact is negative but smaller than first year for India, negative but larger for Bangladesh and positive for Pakistan.

Objective To investigate if a home environment test battery can be used to measure effects of Parkinson’s disease (PD) treatment intervention and disease progression. Background Seventy-seven patients diagnosed with advanced PD were recruited in an open longitudinal 36-month study at 10 clinics in Sweden and Norway; 40 of them were treated with levodopa-carbidopa intestinal gel (LCIG) and 37 patients were candidates for switching from oral PD treatment to LCIG. They utilized a mobile device test battery, consisting of self-assessments of symptoms and objective measures of motor function through a set of fine motor tests (tapping and spiral drawings), in their homes. Both the LCIG-naïve and LCIG-non-naïve patients used the test battery four times per day during week-long test periods. Methods Assessments The LCIG-naïve patients used the test battery at baseline (before LCIG), month 0 (first visit; at least 3 months after intraduodenal LCIG), and thereafter quarterly for the first year and biannually for the second and third years. The LCIG-non-naïve patients used the test battery from the first visit, i.e. month 0. Out of the 77 patients, only 65 utilized the test battery; 35 were LCIG-non-naïve and 30 LCIG-naïve. In 20 of the LCIG-naïve patients, assessments with the test battery were available during oral treatment and at least one test period after having started infusion treatment. Three LCIG-naïve patients did not use the test battery at baseline but had at least one test period of assessments thereafter. Hence, n=23 in the LCIG-naïve group. In total, symptom assessments in the full sample (including both patient groups) were collected during 379 test periods and 10079 test occasions. For 369 of these test periods, clinical assessments including UPDRS and PDQ-39 were performed in afternoons at the start of the test periods. The repeated measurements of the test battery were processed and summarized into scores representing patients’ symptom severities over a test period, using statistical methods. Six conceptual dimensions were defined; four subjectively-reported: ‘walking’, ‘satisfied’, ‘dyskinesia’, and ‘off’ and two objectively-measured: ‘tapping’ and ‘spiral’. In addition, an ‘overall test score’ (OTS) was defined to represent the global health condition of the patient during a test period. Statistical methods Change in the test battery scores over time, that is at baseline and follow-up test periods, was assessed with linear mixed-effects models with patient ID as a random effect and test period as a fixed effect of interest. The within-patient variability of OTS was assessed using intra-class correlation coefficient (ICC), for the two patient groups. Correlations between clinical rating scores and test battery scores were assessed using Spearman’s rank correlations (rho). Results In LCIG-naïve patients, mean OTS compared to baseline was significantly improved from the first test period on LCIG treatment until month 24. However, there were no significant changes in mean OTS scores of LCIG-non-naïve patients, except for worse mean OTS at month 36 (p<0.01, n=16). The mean scores of all subjectively-reported dimensions improved significantly throughout the course of the study, except ‘walking’ at month 36 (p=0.41, n=4). However, there were no significant differences in mean scores of objectively-measured dimensions between baseline and other test periods, except improved ‘tapping’ at month 6 and month 36, and ‘spiral’ at month 3 (p<0.05). The LCIG-naïve patients had a higher within-subject variability in their OTS scores (ICC=0.67) compared to LCIG-non-naïve patients (ICC=0.71). The OTS correlated adequately with total UPDRS (rho=0.59) and total PDQ-39 (rho=0.59). Conclusions In this 3-year follow-up study of advanced PD patients treated with LCIG we found that it is possible to monitor PD progression over time using a home environment test battery. The significant improvements in the mean OTS scores indicate that the test battery is able to measure functional improvement with LCIG sustained over at least 24 months.

It outlines the political context, based on New Public Management (NPM) theory, according to what role the Schools Inspectorate is supposed to play in order to govern and control. Attention is also devoted, referring an on-going case study, to how inspections influence head teachers and their leadership in their everyday work.   Reports from the Schools inspectorate are public. This forces both politicians and head teachers to take measures. In this case, the head teachers perceived that the inspection reports confirmed what they already knew, but it also gave them an alibi and a tool to push their teachers to take part in everyday school development work. During the first year after the inspection the head teachers mainly strived to adjust formal deficiencies in local steering documents. However, some of the deviations reported from the Schools inspectorate are regarding pedagogical problems that are complicated and difficult to handle. As interventions in many cases will show up much later the results are, for example as increased goal fulfilment, in this case, still an open question. Nevertheless, it seems obvious that the Schools Inspectorate must be seen as a result of the governing philosophy that denotes New Public Management NPM).

Background. Nurses' research utilization (RU) as part of evidence-based practice is strongly emphasized in today's nursing education and clinical practice. The primary aim of RU is to provide high-quality nursing care to patients. Data on newly graduated nurses' RU are scarce, but a predominance of low use has been reported in recent studies. Factors associated with nurses' RU have previously been identified among individual and organizational/contextual factors, but there is a lack of knowledge about how these factors, including educational ones, interact with each other and with RU, particularly in nurses during the first years after graduation. The purpose of this study was therefore to identify factors that predict the probability for low RU among registered nurses two years after graduation. Methods. Data were collected as part of the LANE study (Longitudinal Analysis of Nursing Education), a Swedish national survey of nursing students and registered nurses. Data on nurses' instrumental, conceptual, and persuasive RU were collected two years after graduation (2007, n = 845), together with data on work contextual factors. Data on individual and educational factors were collected in the first year (2002) and last term of education (2004). Guided by an analytic schedule, bivariate analyses, followed by logistic regression modeling, were applied. Results. Of the variables associated with RU in the bivariate analyses, six were found to be significantly related to low RU in the final logistic regression model: work in the psychiatric setting, role ambiguity, sufficient staffing, low work challenge, being male, and low student activity. Conclusions. A number of factors associated with nurses' low extent of RU two years postgraduation were found, most of them potentially modifiable. These findings illustrate the multitude of factors related to low RU extent and take their interrelationships into account. This knowledge might serve as useful input in planning future studies aiming to improve nurses', specifically newly graduated nurses', RU.

Introduction: Based on randomised controlled trials, evidence exists that early supported discharge (ESD) from the hospital with continued rehabilitation at home has beneficial effects after stroke; however, the effects of ESD service in regular clinical practice have not been investigated. The purpose of the current study was to compare ESD service with conventional rehabilitation in terms of patient outcomes, caregiver burden at 3 and 12 months and the use and costs of healthcare during the first year after stroke. Material and methods: This study was a subgroup analysis of a longitudinal observational study of patients who received care in the stroke unit at Karolinska University Hospital in Sweden. Patients who met the inclusion criteria for ESD in previous experimental studies were included. The patients were referred to available rehabilitation services at discharge, and comparisons between those who received ESD service (the ESD group, n = 40) and those who received conventional rehabilitation (the NoESD group, n = 110) were performed with regard to independence in activities of daily living (ADL), the frequency of social activities, life satisfaction, and caregiver burden and the use and costs of healthcare during the first year after stroke. Results: At 3 and 12 months, no differences were observed with regard to patient outcomes; however, ESD was associated with a lower caregiver burden (p = 0.01) at 12 months. The initial length of stay (LOS) at the hospital was 8 days for the ESD group and 15 days for the NoESD group (p = 0.02). The median number of outpatient rehabilitation contacts was 20.5 for the ESD group (81% constituting ESD service) and 3 for the NoESD group (p<0.001). There was no difference between the groups with regard to overall healthcare costs. Conclusions: ESD service in usual clinical practice renders similar health benefits as conventional rehabilitation but a different pattern of resource use and with released capacity in acute stroke care.

Introdução – Grande parte da população de crianças portadoras de defeitos cardíacos congênitos torna-se criticamente doente durante o primeiro ano de vida e necessita tratamento cirúrgico. No entanto, durante cirurgias cardíacas, ocorre uma resposta inflamatória intensa desencadeada pelo período de circulação extracorpórea (CEC), o que provoca disfunção de órgãos e tecidos. Os neutrófilos assumem um papel importante e complexo neste período, envolvendo a ligação às células endoteliais, ativação e liberação de mediadores inflamatórios. Esse processo é iniciado e mantido através de moléculas de adesão específicas, como a molécula de adesão intercelular-1 (ICAM-1) e a molécula de adesão celular-vascular-1 (VCAM-1). Formas solúveis destas moléculas apresentam variações de seus níveis durante cirurgias cardíacas com uso de CEC. Há poucos dados na literatura relacionados ao comportamento destas moléculas após cirurgia cardíaca com uso de CEC em lactentes, estando seu significado no plasma ainda por ser decifrado. Objetivos – Mensurar os níveis plasmáticos das moléculas de adesão solúveis ICAM-1 e VCAM-1 em condições basais e após exposição ao circuito de CEC em lactentes submetidos à cirurgia cardíaca para correção de defeitos cardíacos congênitos. Comparar os níveis plasmáticos destas moléculas entre pacientes acianóticos e cianóticos. Métodos – Foram estudados 21 lactentes, durante o período de junho de 1998 a março de 1999, submetidos à cirurgia cardíaca com uso de CEC. Foram analisados os níveis plasmáticos da ICAM-1 e da VCAM-1 solúveis pelo método de ELISA, na indução anestésica, ao término da CEC e 8 e 26 horas após o término da CEC. Resultados – As patologias cardíacas congênitas mais comuns foram defeito do septo atrioventricular e tetralogia de Fallot. As médias de idade e de peso foram 6,6 meses e 5,8 Kg. As medianas dos tempos de CEC e de clampeamento de aorta foram, respectivamente, 87 e 53 minutos. Todos os lactentes utilizaram inotrópicos. As medianas dos tempos de intubação e de internação foram 72 horas e 21 dias. A taxa de mortalidade dos pacientes foi de 9,5%. Os níveis basais da ICAM-1 e da VCAM-1 foram significativamente mais elevados do que aqueles considerados normais (P<0,0001). Os níveis da ICAM-1 diminuíram significaticamente ao término da CEC (P<0,001), voltando a aumentar significativamente 8 horas após o término deste período (P<0,005), sem no entanto, alcançar os valores basais 26 horas depois. A VCAM-1 apresentou comportamento semelhante. No entanto, 26 horas após CEC houve diminuição significativa de seus níveis em relação ao valor identificado 8 horas após tal período (P<0,005). Não houve diferença estatisticamente significativa quanto aos valores das moléculas entre pacientes acianóticos e cianóticos (P>0,1). Não houve associação entre os valores das moléculas e as variáveis perioperatórias e os desfechos clínicos. Conclusões – Os níveis plasmáticos das moléculas de adesão solúveis ICAM-1 e VCAM-1 são aumentados em lactentes com cardiopatias congênitas acianóticas e cianóticas no seu estado basal. Os níveis plasmáticos de ICAM-1 e VCAM-1 solúveis variam após exposição ao circuito de CEC em lactentes submetidos à cirurgia cardíaca para correção de cardiopatias congênitas, apresentando um comportamento característico nestes pacientes. Não há diferenças no comportamento das moléculas entre pacientes acianóticos e cianóticos.

O presente estudo objetivou identificar as principais causas da mortalidade das micro e pequenas empresas brasileiras e como os instrumentos de planejamento e gerenciamento econômico-financeiros podem reduzir esta mortalidade. Foi realizada pesquisa bibliográfica sobre as micro e pequenas empresas (MPE) em que se identificaram: as definições, as principais características, a importância, as principais necessidades e dificuldades, a taxa de mortalidade e os fatores de mortalidade desse segmento de empresas. Dessa forma, verificou-se que as MPE têm grande importância na economia e sociedade brasileiras. A maioria expressiva (98%) das empresas brasileiras é classificada como MPE, sendo elas responsáveis por 43% da oferta de empregos e por quase a metade de toda a receita e valor da produção do país. O trabalho abordou pesquisas mostrando que as taxas médias de mortalidade das empresas brasileiras são significativas e preocupantes, podendo atingir índices próximos a 60% somente no primeiro ano de sua constituição, em alguns Estados do Brasil. Este fenômeno não pode ser atribuído a um fator isoladamente, mas a um conjunto de fatores ambientais, técnicos, administrativos e outros mais que, combinados, podem ampliar os riscos de fechamento das x empresas. No entanto, grande parte dos estudos aponta problemas de origem financeira como um dos principais fatores condicionantes da mortalidade das MPE. As necessidades mais prementes nas MPE, identificadas na pesquisa bibliográfica, foram: instrumentos de planejamento e gestão, treinamento especializado, capital de giro, crédito a taxas compatíveis com a rentabilidade e formalização dos processos. A pesquisa principal do trabalho é o estudo de caso abordando a experiência vivencial de implantação, planejamento, gerenciamento e controle do Restaurante Buongustaio em Brasília, nos seus 10 meses iniciais de funcionamento. Foram abordadas as principais dificuldades, problemas enfrentados e soluções envolvendo planejamento, gestão, recursos humanos, capital de giro e atendimento a clientes, dentre outros. o estudo de caso possibilitou a aferição de vários conceitos e conteúdos abordados na pesquisa bibliográfica e conclui-se que a utilização dos instrumentos de planejamento e gerenciamento econômico-financeiros, como: plano de negócios, análise econômico-financeira, gerenciamento do fluxo de caixa e de capital de giro, fornece importante contribuição para a sobrevivência e crescimento da micro e pequena empresa, reduzindo o seu risco de falência ou mortalidade prematura.

This work consists of three essays organized into chapters that seek to answer questions at first sight unrelated, but with one common denominator, which is the scarcity of public resources devoted to education, overall, especially in lower education. . The first chapter deals with the scarcity of resources devoted to education in a context of population aging. Two hypotheses were tested for Brazilian municipalities on the relationship between the aging of the population and educational expenditure. The first, already proven in the literature, is that there is an intergenerational conflict for resources and the increase of the share of elderly in the population reduces the educational expenditure. The second, proposed here for the first time, is that there should be reduction of competition for resources if there is a relationship of co-residence between young and old. The results indicated that an increase in the share of elderly reduces the educational expenditure per youth. But the results also illustrate that an increase in the share of elderly co-residing with youth (family arrangement more common in Latin American countries) raises the educational expenditure, which reflects a reduction of competition for resources between generations. The second chapter assesses the allocative efficiency of investments in Higher Education. Using the difference between first-year and last-year students’ scores from Enade aggregated by HEI as a product in the Stochastic Production Function, is possible to contribute with a new element in the literature aimed at estimating the production function of education. The results show that characteristics of institutions are the variables that best explain the performance of students, and that public institutions are more inefficient than the private ones. Finally, the third chapter presents evidence that the allocation of public resources in early childhood education is important for a better future school performance. In this chapter was calculated the effects of early childhood education on literacy scores of children attending the 2nd grade of elementary school. The results using OLS and propensity score matching show that students who started school at the ages to 5, 4, and 3 years had literacy scores between 12.22 and 19.54 points higher than the scores of those who began school at the ages 6 years or late. The results also suggest that the returns in terms of literacy scores diminish in relation to the number of years of early childhood education.

Um dos maiores desafios atuais do Brasil em relação ao seu sistema educacional é a melhora da qualidade da educação oferecida pela rede pública de ensino. Motivado por isso, o Instituto Unibanco criou o Projeto Jovem de Futuro. O projeto oferece às escolas públicas estaduais de ensino médio apoio técnico e financeiro para a implantação de um plano de melhoria de qualidade na dinâmica de funcionamento escolar, com o objetivo de aumentar o rendimento dos alunos e diminuir os índices de evasão escolar. Este trabalho faz uma avaliação do primeiro ano do projeto nos estado de Minas Gerais e Rio Grande do Sul. Os conceitos de avaliação estatística de programas sociais são utilizados na estimação do efeito médio do projeto sobre as notas dos alunos, dos efeitos heterogêneos e do efeito na dispersão das notas dos alunos. Os resultados encontrados indicam que o Projeto Jovem de Futuro teve grande impacto sobre a nota média dos alunos e contribuiu para a redução da desigualdade nas notas dos alunos das escolas participantes. A fim de entender o que gerou o aumento da proficiência dos alunos participantes do programa, também são feitas algumas estimações com o objetivo de captar qual o tipo de investimento que, em média, contribuiu mais para o desenvolvimento dos alunos. Neste aspecto não foi possível chegar a um resultado conclusivo.

Este trabalho tem por objetivo analisar se a capacidade de resposta de política econômica foi fator relevante para minimizar a severidade da crise financeira de 2008, no primeiro ano do episódio. A hipótese da pesquisa é que países com um maior espaço para políticas expansionistas – maiores taxas de juros maiores e melhores resultados do governo central – tenham registrado uma crise menos severa, tudo mais constante. Os resultados econométricos corroboram com a hipótese em relação à política monetária. No que diz respeito à política fiscal, o sinal dos parâmetros encontrado é oposto ao esperado, sinalizando que, possivelmente, mesmo países com bons resultados fiscais possam ter limitações a estímulos keynesianos em função da tolerância ao seu nível de endividamento. Entretanto, a interação entre o resultado do governo central e o endividamento está em linha com a hipótese da pesquisa, uma vez que uma melhor gestão tanto do fluxo fiscal, quanto do estoque da dívida no ano anterior ao evento mostrou-se relevante. A adição da variável de investment grade às especificações ressaltou uma crise mais severa nas economias desenvolvidas.
