Il trigono della vescica urinaria (UBT) è un'area limitata attraverso la quale penetrano nella vescica la maggior parte dei vasi e fibre e in cui le fibre nervose e neuroni intramurali sono più concentrati. Mediante l’utilizzo combinato di un tracciante retrogrado(FB) e dell’immunoistochimica sono stati valutati il fenotipo e l’area del soma dei neuroni dei gangli spinali (DRG), dei neuroni post-gangliari, il fenotipo dei gangli della catena simpatica (STG) e i gangli mesenterici caudali (CMG) innervanti l’UBT. - Caratterizzazione dei neuroni dei DRG con: peptide correlato al gene della calcitonina (CGRP)(30±3%, 29±3%, rispettivamente), sostanza P(SP)(26±8%, 27±12%), ossido nitrico sintasi neuronale (nNOS)(21±4%; 26±7%), neurofilamento 200kDa (NF200)(75±14%, 81±7% ) , transient receptor potential vanilloid1 (TRPV1)(48±13%, 43±6%) e isolectina-B4-positivi (IB4) (56±6%;43±10%). I neuroni sensoriali, distribuiti da L2 a Ca1 (DRG), hanno presentato una localizzazione segmentale, mostrando maggior densità nei DRG L4-L5 e S2-S4. I neuroni sensoriali lombari sono risultati significativamente più grandi di quelle sacrali (1.112±624μm2 vs716±421μm2). Complessivamente, questi dati indicano che le vie lombari e sacrali probabilmente svolgono ruoli diversi nella trasmissione sensitiva del trigono della vescica urinaria. -I neuroni FB+ della STG e dei CMG sono risultati immunoreattivi per la tirosina idrossilasi (TH)(66±10,1%, 53±8,2%, rispettivamente), la dopamina beta-idrossilasi (DβH)(62±6,2%, 52±6,2%), neuropeptideY (NPY)(59±8%; 66±7%), CGRP(24±3%, 22±3%), SP(22±2%; 38±8%), polipeptide intestinale vasoattivo (VIP)(19±2%; 35±4%), nNOS(15±2%; 33±8%), trasportatore vescicolare dell'acetilcolina (VAChT)(15±2%; 35±5%), leu-encefalina (LENK)(14±7%; 26±9%), e somatostatina (SOM)(12±3%;32±7%).Il numero medio di neuroni FB+ (1845,1±259,3) era nella STG in L1-S3, con i pirenofori più piccoli (465,6±82.7μm2). Un gran numero (4287,5±1450,6) di neuroni FB+ di piccole dimensioni (476,1±103,9μm2) sono stati localizzati lungo il margine dei CMG. Il maggior numero (4793,3±1990,8) di neuroni FB + è stato osservato nel plesso pelvico, dove i neuroni marcati erano raggruppati in micro-gangli e con pirenoforo ancora più piccolo (374,9±85,4 μm2).

In der vorliegenden Arbeit wurden die durch Training induzierten motorischen Gedächtnisleistungen der Taufliege Drosophila melanogaster beim Überklettern von acht symmetrisch verteilten Lücken auf einem rotierenden Ring untersucht. Durch den auf sie einwirkenden optischen Fluss der vorbeiziehenden äußeren Umgebung wurden die Fliegen angeregt, diesem optomotorischen Reiz entgegenzuwirken und die Lücken laufend zu überqueren. Durch Training verbessert und langfristig gelernt wird die kompensatorische Lückenüberquerung X+ gegen die Rotation. In der aus diesem Training erhaltenen Lernkurve war eine überdurchschnittlich hohe Leistungsverbesserung nach einem einzigen Trainingslauf mit einem zeitlichen Bestand von ca. 40 Minuten abzulesen, um danach vom motorischen Gedächtnisspeicher trainierter Fliegen nicht mehr abgerufen werden zu können. Nach einer Ruhephase von einem bis mehreren Tagen wurden die Fliegen auf mögliche Langzeitlernleistungen untersucht und diese für verschiedene Intervalle nachgewiesen. Sowohl die Leistungsverbesserung während des Trainings, als auch der Lerneffekt nach 24h bleiben in mutanten rutabaga2080 sowie rut1 Fliegen aus. Betroffen ist das Gen der Adenylylzyklase I, ein Schlüsselprotein der cAMP-Signalkaskade, die u.a. im olfaktorischen und visuellen Lernen gebraucht wird. Damit ergab sich die Möglichkeit die motorischen Gedächtnisformen durch partielle Rettung zu kartieren. Die motorische Gedächtniskonsolidierung ist schlafabhängig. Wie sich herausstellte, benötigen WTB Fliegen nur eine Dunkelphase von 10h zwischen einem ersten Trainingslauf und einem Testlauf um signifikante Leistungssteigerungen zu erzielen. In weiterführenden Versuchen wurden die Fliegen nachts sowie tagsüber mit einer LED-Lampe oder in einer Dunkelkammer, mit einem Kreisschüttler oder einer Laborwippe depriviert, mit dem Ergebnis, dass nur jene Fliegen ihre Leistung signifikant gegenüber einem ersten Trainingslauf verbessern konnten, welche entweder ausschließlich der Dunkelheit ausgesetzt waren oder welchen die Möglichkeit gegeben wurde, ein Gedächtnis zunächst in einer natürlichen Schlafphase zu konsolidieren (21Uhr bis 7Uhr MEZ). In weiteren Experimenten wurden die experimentellen Bedingungen entweder während des Trainings oder des Tests auf eine Fliege und damit verbunden auf eine erst durch das Training mögliche motorische Gedächtniskonsolidierung einwirken zu können, untersucht. Dazu wurden die Experimentparameter Lückenweite, Rotationsrichtung des Lückenringes, Geschwindigkeit des Lückenringes sowie die Verteilung der acht Lücken auf dem Ring (symmetrisch, asymmetrisch) im Training oder beim Gedächtnisabruf im Testlauf verändert. Aus den Ergebnissen kann geschlussfolgert werden, dass die Lückenweite langzeitkonsolidiert wird, die Rotationsrichtung kurzzeitig abgespeichert wird und die Drehgeschwindigkeit motivierend auf die Fliegen wirkt. Die symmetrische Verteilung der Lücken auf dem Ring dient der Langzeitkonsolidierung und ist als Trainingseingang von hoher Wichtigkeit. Mit Hilfe verschiedener Paradigmen konnten die Leistungsverbesserungen der Fliegen bei Abruf eines Kurz- bzw. Langzeitgedächtnisses hochauflösend betrachtet werden (Transfer). Die Konzentration, mit der eine WTB Fliege eine motorische Aufgabe - die Überquerung von Lücken entgegengesetzt der Rotationsrichtung - durchführt, konnte mit Hilfe von Distraktoreizen bestimmt werden. Wie sich herausstellte, haben Distraktoren einen Einfluss auf die Erfolgsquote einer Überquerung, d.h. mit zunehmender Distraktionsstärke nahm die Wahrscheinlichkeit einer Lückenüberquerung ab. Die Ablenkungsreize wirkten sich weiterhin auf die Vermessung einer Lücke aus, in dem entweder "peering"-artigen Bewegungen im Training durchgeführt wurden oder je nach Reizstärke ausschließlich nur jene Lücken vermessen wurden, welche auch überquert werden sollten.

La studio dell’Anatomia umana presenta una varietà di sfaccettature, che sono alla base della reale comprensione del corpo umano; ovvero la vera anatomia non è quella rappresentata nei testi ma quella che appare durante la dissezione o nelle più sofisticate analisi di immagine. Lo scopo di questa tesi è stato quello di rivisitare alcune situazioni vascolari che possono andare incontro a variazioni e cercare di comprendere, anche con l’aiuto della bibliografia, se tali variazioni possono essere causa o epifenomeni di patologie a carico delle arterie affette dalle variazioni stesse o di territori da esse dipendenti per l’afflusso sanguigno. E’ stata condotta una analisi su preparati cadaverici in particolare in tre distretti: a) addome e tripode celiaco/mesenterica superiore; b) circolo cerebrale; d) orco aortico.

Recentemente, vari ricercatori si sono concentrati sulle proprietà funzionali dei muscoli, sulla meccanica e sulla cinematica legata al volo, ma senza la possibilità di consultare un’adeguata letteratura poiché assente o incompleta. Si è quindi ritenuto utile studiare la miologia della regione della spalla e del braccio di specie non in precedenza esaminate e che mostrassero differenti stili di volo, al fine di creare una base di informazioni per futuri studi funzionali. Tutte le specie prese in esame sono membri della famiglia degli Accipitridae e nello specifico si sono selezionati: il Falco pecchiaiolo (Pernis apivorus Linnaeus, 1758), l’Astore comune (Accipiter gentilis Linnaeus, 1758) e lo Sparviero eurasiatico (Accipiter nisus Linnaeus, 1758). Questi animali attuano stili di volo differenti e sono stati pertanto scelti per capire se diverse performance di volo potessero indurre una differenziazione dell’apparato muscolare. La comparazione dei dati raccolti durante questo studio con quelli presenti in letteratura in altre specie aviarie ha evidenziato che, mentre alcuni muscoli non sembrano andare incontro a grandi modificazioni nei rapaci, altri invece presentano una grande eterogeneità. In particolare, grazie alle caratteristiche dei diversi muscoli, si è potuto raggruppare questi ultimi in base a possibili funzioni comuni: a) muscoli che concorrono a stabilizzare l’ala rispetto al tronco e che assorbono le forze generate durante il volo; b) muscoli con possibile ruolo nel controllo della rotazione dell’omero; c) muscoli con possibile funzione propriocettiva. Il presente studio ha inoltre evidenziato, nei muscoli esaminati, alcune similitudini e molteplici peculiarità anatomiche non precedentemente segnalate in letteratura. Si ritiene che questo studio macroscopico possa rappresentare un punto di partenza per futuri studi microscopici o elettrofisiologici, a dimostrazione dell’importanza della dissezione quale primo strumento di indagine.

The study of optic flow on postural control may explain how self-motion perception contributes to postural stability in young males and females and how such function changes in the old falls risk population. Study I: The aim was to examine the optic flow effect on postural control in young people (n=24), using stabilometry and surface-electromyography. Subjects viewed expansion and contraction optic flow stimuli which were presented full field, in the foveral or in the peripheral visual field. Results showed that optic flow stimulation causes an asymmetry in postural balance and a different lateralization of postural control in men and women. Gender differences evoked by optic flow were found both in the muscle activity and in the prevalent direction of oscillation. The COP spatial variability was reduced during the view of peripheral stimuli which evoked a clustered prevalent direction of oscillation, while foveal and random stimuli induced non-distributed directions. Study II was aimed at investigating the age-related mechanisms of postural stability during the view of optic flow stimuli in young (n=17) and old (n=19) people, using stabilometry and kinematic. Results showed that old people showed a greater effort to maintain posture during the view of optic flow stimuli than the young. Elderly seems to use the head stabilization on trunk strategy. Visual stimuli evoke an excitatory input on postural muscles, but the stimulus structure produces different postural effects. Peripheral optic flow stabilizes postural sway, while random and foveal stimuli provoke larger sway variability similar to those evoked in baseline. Postural control uses different mechanisms within each leg to produce the appropriate postural response to interact with extrapersonal environment. Ageing reduce the effortlessness to stabilize posture during optic flow, suggesting a neuronal processing decline associated with difficulty integrating multi-sensory information of self-motion perception and increasing risk of falls.

Movement analysis carried out in laboratory settings is a powerful, but costly solution since it requires dedicated instrumentation, space and personnel. Recently, new technologies such as the magnetic and inertial measurement units (MIMU) are becoming widely accepted as tools for the assessment of human motion in clinical and research settings. They are relatively easy-to-use and potentially suitable for estimating gait kinematic features, including spatio-temporal parameters. The objective of this thesis regards the development and testing in clinical contexts of robust MIMUs based methods for assessing gait spatio-temporal parameters applicable across a number of different pathological gait patterns. First, considering the need of a solution the least obtrusive as possible, the validity of the single unit based approach was explored. A comparative evaluation of the performance of various methods reported in the literature for estimating gait temporal parameters using a single unit attached to the trunk first in normal gait and then in different pathological gait conditions was performed. Then, the second part of the research headed towards the development of new methods for estimating gait spatio-temporal parameters using shank worn MIMUs on different pathological subjects groups. In addition to the conventional gait parameters, new methods for estimating the changes of the direction of progression were explored. Finally, a new hardware solution and relevant methodology for estimating inter-feet distance during walking was proposed. Results of the technical validation of the proposed methods at different walking speeds and along different paths against a gold standard were reported and showed that the use of two MIMUs attached to the lower limbs associated with a robust method guarantee a much higher accuracy in determining gait spatio-temporal parameters. In conclusion, the proposed methods could be reliably applied to various abnormal gaits obtaining in some cases a comparable level of accuracy with respect to normal gait.

ZusammenfassungrnAlle bekannten Exemplare der Amphibien-Gruppe Microsauria des mitteleuropäischen Rotliegend wurden untersucht sowie – für die kladistische Untersuchung – ihre nächsten Verwandten. Im Saar-Nahe-Becken sind vier Arten aus zwei Microsauria-Gattungen nachzuweisen: Batropetes niederkirchensis n. sp., Batropetes palatinus n. sp., Batropetes appelensis n. sp. und Altenglanerpeton schroederi n. sp.. Aus dem Döhlen-Becken bei Dresden stammt Batropetes fritschi.rnDie Rekonstruktionen legen nahe, dass die in Seeablagerungen gefundenen Batropetes-Arten kleine, terrestrisch lebende Microsaurier waren, die sich von Insekten oder deren Larven ernährten. Ihre mittlere Gesamtlänge beträgt fast 8 cm, ihr Rumpf ist auffallend kurz mit je nach Art 17 oder 19 Präsakralwirbeln. Sie besitzen große von Wülsten umgebene Augenöffnungen, ein großes Pinealforamen, drei längs angeordnete Grübchen auf den Frontalia, dreispitzige Zähne, einen zahnlosen Gaumen und vier kurze kräftige Extremitäten. Becken- und Schultergürtel sind komplett verknöchert, außer den bei Batropetes appelensis und Batropetes fritschi knorpeligen Coracoid-Anteilen. Die Batropetes-Arten unterscheiden sich bezüglich Form, Proportionen bzw. Verknöcherungsgrad einiger Knochen im cranialen und postcranialen Skelett. rnDie kladistischen Untersuchungen ergeben für die Arten der monophyletische Gattung Batropetes, dass Batropetes appelensis den übrigen Arten gegenübersteht und Batropetes palatinus mit Batropetes fritschi eine Schwestergruppe zu Batropetes niederkirchensis bildet. Mit den etwas jüngeren texanischen Gattungen Carrolla und Quasicaecilia bildet Batropetes die monophyletische Familie Brachystelechidae. rnAltenglanerpeton schroederi, nur durch ein schlecht erhaltenes Exemplar überliefert, besitzt einen massiv gebauten Schädel, kaum länger als breit mit annähernd dreieckigem Umriss, ohne Anzeichen für Seitenlinien, mit kleiner rundlicher Orbita, sehr breiter Interorbitalregion und bis weit vor die Orbita reichendem Jugale. Sein langer Rumpf umfasst mindestens 30 lepospondyle Präsakralwirbel. Wegen der unvollkommenen Entwicklung der Gliedmaßen kann diese Art nicht auf vier Beinen gelaufen sein und lebte möglicherweise in der Laubschicht wühlend oder in Sümpfen. Altenglanerpeton ist keiner bestimmten Microsauria-Familie zuzuordnen und steht den Pantylidae, Gymnarthridae, Goniorhynchidae und Ostodolepidae am nächsten, wobei zu letzteren die meisten morphologischen Ähnlichkeiten bestehen.rnAußerdem sind mit isolierten Einzelknochen, insbesondere Wirbeln, Microsauria und andere Gruppen lepospondyler Amphibien ohne Bestimmung der Gattung und Art aus wenigen stratigraphischen Niveaus des Saar-Nahe-Beckens überliefert. rn

Negli ultimi anni va sempre più affermandosi l’idea che nei test a fatica utilizzati in ambito industriale per testare la resistenza di numerosi prodotti, riprodurre profili vibratori con distribuzioni gaussiane non sia sufficientemente realistico. Nell’indagine sperimentale riportata in questo trattato vengono confrontati gli effetti generati da sollecitazioni leptocurtiche ottenute da misurazioni reali, con profili vibratori gaussiani a parità di RMS e forma della PSD (Power Spectral Density) verificando la validità della “Papoulis rule”. A partire da questi profili vibratori si è effettuata una progettazione ad hoc dalla quale sono stati ricavati dei provini piatti in lega di alluminio a cui è collegata una massa ausiliaria. Quest’ultimi montati a sbalzo su uno shaker elettrodinamico, sono caratterizzati da una variazione di sezione che localizza la sezione critica in prossimità dell’incastro. I provini sono stati inoltre caratterizzati attraverso prove a trazione e test accelerati a fatica, ricavandone la caratteristica a trazione del materiale ed il diagramma di Wohler. In seguito alla descrizione di tali prove viene riportata un’analisi dei provini sollecitati da due profili vibratori, uno gaussiano e uno ad elevato valore di kurtosis, monitorando tramite l’impiego di accelerometri i valori dell’eccitazione e la risposta. Vengono inoltre verificati i valori delle deformazioni dovute alle sollecitazioni imposte collocando due estensimetri in corrispondenza della sezione critica di due provini (uno per ogni tipologia di input).

Il presente lavoro di tesi si pone l’obiettivo di studiare come un Cubesat 1U reagisce se sollecitato con vibrazioni di una certa banda di frequenze, corrispondenti a quelle generate durante il lancio del vettore VEGA, e cercare di sviluppare un modello virtuale che in futuro permetta di simulare in maniera più realistica il comportamento delle successive strutture che saranno progettate. Il campo dei nanosatelliti è in rapida espansione negli ultimi anni, bassi costi di produzione e lancio abbinati a tempi rapidi di sviluppo e progettazione li rendono un ottimo strumento per aziende di piccole dimensioni e università per aquisire esperienza nel campo delle tecnologie spaziali e permettere la formazione degli studenti. Grazie a questi fattori il corso di Ingegneria Aerospaziale di Forlì ha intrapreso la progettazione di un Cubesat che una volta ultimato potrà essere lanciato in orbita grazie al contributo dell’ESA, che si presta a fornire lanci gratuiti alle università europee. Questo progetto porta avanti il lavoro compiuto durante la fase di tirocinio in cui abbiamo cercato di progettare una struttura semplice e realizzabile completamente nei laboratori dell’università, che permettesse anche successivamente a studenti e dottorandi di cimentarsi nello studio e nella progettazione di un nanosatellite. Il mio compito in particolare consiste nell’esecuzione di una serie di prove sperimentali con uno shaker presente nei laboratori della Scuola e, raccogliendo, analizzando e interpretando la risposta della struttura, si cercherà di modificare il modello CAD realizzato finora (lavoro compiuto in un'altra tesi di laurea) in modo che rispecchi più fedelmente il comportamento di quello reale e fornire quindi una base ai futuri lavori in questo ambito. La struttura, ispirata a quelle già presenti sul mercato, è stata progettata utilizzando Solidworks un software CAD, successivamente è stata fatta un analisi tramite un altro software FEM, realizzato e poi testato mediante uno shaker presso i laboratori del corso di studi in ingegneria aerospaziale di Forlì. Per poter fare ciò si è anche realizzata quella che è stata usata come interfaccia per connettere il Cubesat allo shaker, infatti data la sua proprietà di esercitare spostamenti solo lungo l’asse verticale si è reso necessario trovare un modo per sollecitare il nanosatellite lungo tutti e tre gli assi per poter evidenziare i modi flessionali della struttura. I dati ottenuti riguardano le accelerazioni rilevate in vari punti del Cubesat e del supporto da parte di accelerometri piezometrici che sono stati analizzati per risalire al tipo di comportamento con cui ha risposto la struttura a vibrazioni di varia frequenza e intensità.

PURPOSE: To determine the reproducibility and validity of video screen measurement (VSM) of sagittal plane joint angles during gait. METHODS: 17 children with spastic cerebral palsy walked on a 10m walkway. Videos were recorded and 3d-instrumented gait analysis was performed. Two investigators measured six sagittal joint/segment angles (shank, ankle, knee, hip, pelvis, and trunk) using a custom-made software package. The intra- and interrater reproducibility were expressed by the intraclass correlation coefficient (ICC), standard error of measurements (SEM) and smallest detectable difference (SDD). The agreement between VSM and 3d joint angles was illustrated by Bland-Altman plots and limits of agreement (LoA). RESULTS: Regarding the intrarater reproducibility of VSM, the ICC ranged from 0.99 (shank) to 0.58 (trunk), the SEM from 0.81 degrees (shank) to 5.97 degrees (trunk) and the SDD from 1.80 degrees (shank) to 16.55 degrees (trunk). Regarding the interrater reproducibility, the ICC ranged from 0.99 (shank) to 0.48 (trunk), the SEM from 0.70 degrees (shank) to 6.78 degrees (trunk) and the SDD from 1.95 degrees (shank) to 18.8 degrees (trunk). The LoA between VSM and 3d data ranged from 0.4+/-13.4 degrees (knee extension stance) to 12.0+/-14.6 degrees (ankle dorsiflexion swing). CONCLUSION: When performed by the same observer, VSM mostly allows the detection of relevant changes after an intervention. However, VSM angles differ from 3d-IGA and do not reflect the real sagittal joint position, probably due to the additional movements in the other planes.

Purpose: To report an angiographic investigation of midterm atherosclerotic disease progression in below-the-knee (BTK) arteries of claudicants. Methods: Angiograms were performed in 58 consecutive claudicants (35 men; mean age 68.3±8.7 years) with endovascular treatment of femoropopliteal arteries in 58 limbs after a mean follow-up of 3.6±1.2 years. Angiograms were reviewed in consensus by 2 experienced readers blinded to clinical data. Progression of atherosclerosis in 4 BTK arterial segments (tibioperoneal trunk, anterior and posterior tibial arteries, and peroneal artery) was assessed according to the Bollinger score. The composite per calf Bollinger score represented the average of the 4 BTK arterial segment scores. The association of the Bollinger score with cardiovascular risk factors and gender was scrutinized. Results: A statistically significant increase in atherosclerotic burden was observed for the mean composite per calf Bollinger score (5.7±8.3 increase, 95% CI 3.5 to 7.9, p<0.0001), as well as for each single arterial segment analyzed. In multivariate linear regression analysis, diabetes mellitus was associated with a more pronounced progression of atherosclerotic burden in crural arteries (β: 5.6, p=0.035, 95% CI 0.398 to 10.806). Conclusion: Progression of infrapopliteal atherosclerotic lesions is common in claudicants during midterm follow-up. Presence of diabetes mellitus was confirmed as a major risk factor for more pronounced atherosclerotic BTK disease progression.

OBJECTIVE: To compare the proportion and rate of healing, pain, and quality of life of low-strength medical compression stockings (MCS) with traditional bandages applied for the treatment of recalcitrant venous leg ulcers. METHODS: A single-center, randomized, open-label study was performed with consecutive patients. Sigvaris prototype MCS providing 15 mm Hg-25 mm Hg at the ankle were compared with multi-layer short-stretch bandages. In both groups, pads were placed above incompetent perforating veins in the ulcer area. The initial static pressure between the dressing-covered ulcer and the pad was 29 mm Hg and 49 mm Hg with MCS and bandages, respectively. Dynamic pressure measurements showed no difference. Compression was maintained day and night and changed every week. The primary endpoint was healing within 90 days. Secondary endpoints were healing within 180 days, time to healing, pain (weekly Likert scales), and monthly quality of life (ChronIc Venous Insufficiency Quality of Life [CIVIQ] questionnaire). RESULTS: Of 74 patients screened, 60 fulfilled the selection criteria and 55 completed the study; 28 in the MCS and 27 in the bandage group. Ulcers were recurrent (48%), long lasting (mean, 27 months), and large (mean, 13 cm2). All but one patient had deep venous reflux and/or incompetent perforating veins in addition to trunk varices. Characteristics of patients and ulcers were evenly distributed (exception: more edema in the MCS group; P = .019). Healing within 90 days was observed in 36% with MCS and in 48% with bandages (P = .350). Healing within 180 days was documented in 50% with MCS and in 67% with bandages (P = .210). Time to healing was identical. Pain scored 44 and 46 initially (on a scale in which 100 referred to maximum and 0 to no pain) and decreased within the first week to 20 and 28 in the MCS and bandage groups, respectively (P < .001 vs .010). Quality of life showed no difference between the treatment groups. In both groups, pain at 90 days had decreased by half, independent of completion of healing. Physical, social, and psychic impairment improved significantly in patients with healed ulcers only. CONCLUSION: Our study illustrates the difficulty of bringing large and long-standing venous ulcers to heal. The effect of compression with MCS was not different from that of compression with bandages. Both treatments alleviated pain promptly. Quality of life was improved only in patients whose ulcers had healed.

Lung recruitment maneuvers (RMs), used to reopen atelectatic lung units and to improve oxygenation during mechanical ventilation, may result in hemodynamic impairment. We hypothesize that pulmonary arterial hypertension aggravates the consequences of RMs in the splanchnic circulation. Twelve anesthetized pigs underwent laparotomy and prolonged postoperative ventilation. Systemic, regional, and organ blood flows were monitored. After 6 h (= baseline), a recruitment maneuver was performed with sustained inflation of the lungs. Thereafter, the pigs were randomly assigned to group C (control, n = 6) or group E with endotoxin-induced pulmonary arterial hypertension (n = 6). Endotoxemia resulted in a normotensive and hyperdynamic state and a deterioration of the oxygenation index by 33%. The RM was then repeated in both groups. Pulmonary artery pressure increased during lipopolysaccharide infusion from 17 ± 2 mmHg (mean ± SD) to 31 ± 10 mmHg and remained unchanged in controls (P < 0.05). During endotoxemia, RM decreased aortic pulse pressure from 37 ± 14 mmHg to 27 ± 13 mmHg (mean ± SD, P = 0.024). The blood flows of the renal artery, hepatic artery, celiac trunk, superior mesenteric artery, and portal vein decreased to 71% ± 21%, 69% ± 20%, 76% ± 16%, 79% ± 18%, and 81% ± 12%, respectively, of baseline flows before RM (P < 0.05 all). Organ perfusion of kidney cortex, kidney medulla, liver, and jejunal mucosa in group E decreased to 65% ± 19%, 77% ± 13%, 66% ± 26%, and 71% ± 12%, respectively, of baseline flows (P < 0.05 all). The corresponding recovery to at least 90% of baseline regional blood flow and organ perfusion lasted 1 to 5 min. Importantly, the decreases in regional blood flows and organ perfusion and the time to recovery of these flows did not differ from the controls. In conclusion, lipopolysaccharide-induced pulmonary arterial hypertension does not aggravate the RM-induced significant but short-lasting decreases in systemic, regional, and organ blood flows.

Protective patient equipment for CT examinations is not routinely provided. The aim of this study was to determine whether, and if so what, specific protective equipment is beneficial during CT scans. The absorbed organ doses and the effective doses for thorax, abdomen/pelvis and brain CT investigation with and without the use of protective patient equipment have been determined and compared. All measurements were carried out on modern multislice CT scanner using an anthropomorphic phantom and thermoluminescence dosemeters. The measurements show that protective equipment reduces the dose within the scattered beam area. The highest organ dose reduction was found in organs that protrude from the trunk like the testes or the female breasts that can largely be covered by the protective equipment. The most reduction of the effective dose was found in the male abdomen/pelvis examination (0.32 mSv), followed by the brain (0.11 mSv) and the thorax (0.06 mSv). It is concluded that the use of protective equipment can reduce the applied dose to the patient.

A 55-year-old woman was referred because of diffuse pruritic erythematous lesions and an ischemic process of the third finger of her right hand. She was known to have anaemia secondary to hypermenorrhea. She presented six months before admission with a cutaneous infiltration on the left cubital cavity after a paravenous leakage of intravenous iron substitution. She then reported a progressive pruritic erythematous swelling of her left arm and lower extremities and trunk. Skin biopsy of a lesion on the right leg revealed a fibrillar, small-vessel vasculitis containing many eosinophils.Two months later she reported Raynaud symptoms in both hands, with a persistent violaceous coloration of the skin and cold sensation of her third digit of the right hand. A round 1.5 cm well-delimited swelling on the medial site of the left elbow was noted. The third digit of her right hand was cold and of violet colour. Eosinophilia (19 % of total leucocytes) was present. Doppler-duplex arterial examination of the upper extremities showed an occlusion of the cubital artery down to the palmar arcade on the right arm. Selective angiography of the right subclavian and brachial arteries showed diffuse alteration of the blood flow in the cubital artery and hand, with fine collateral circulation in the carpal region. Neither secondary causes of hypereosinophilia nor a myeloproliferative process was found. Considering the skin biopsy results and having excluded other causes of eosinophilia, we assumed the diagnosis of an eosinophilic vasculitis. Treatment with tacrolimus and high dose steroids was started, the latter tapered within 12 months and then stopped, but a dramatic flare-up of the vasculitis with Raynaud phenomenon occurred. A new immunosuppressive approach with steroids and methotrexate was then introduced. This case of aggressive eosinophilic vasculitis is difficult to classify into the usual forms of vasculitis and constitutes a therapeutic challenge given the resistance to current immunosuppressive regimens.
