Carotid artery dissection (CAD) is a major cause of stroke in those under age 45, accounting for around 20% of ischaemic events[1,2]. In the absence of known connective tissue disorders, most dissections are traumatic[2]. First-line management is comprised of antiplatelet or anticoagulation therapy, but many traumatic dissections progress despite this and carry the risk of long-term complications from embolism or stenosis[3]. We report a case of traumatic bilateral carotid dissection leading to progressive neurological symptoms and hypoperfusion on computed tomography perfusion (CTP), despite escalation in anticoagulation, which led to emergency carotid stenting.

Polymyositis (PM) is usually associated to other autoimmune or connective tissue diseases. The authors report the case of a 59-year-old man with pulmonary fibrosis, who presented with constitutional symptoms and gradually developed proximal muscle weakness, Raynaud phenomenon, and dysphagia. Besides creatine kinase (CK) elevation, he had positive anti-Polymyositis-Scleromyositis (PM-Scl) and anti-Sjögren's-syndrome A (SSA) antibodies. Nailfold capillaroscopy showed a scleroderma pattern and muscle biopsy revealed necrosis, regeneration of muscle fibers, and inflammatory infiltrate. Prednisolone was started, with great improvement. Taking into account the overlap features between PM and systemic sclerosis sine scleroderma, it is important to closely monitor the patient for signs of pulmonary and cardiac decompensation.

O nevo do tecido conjuntivo (NTC) é um hamartoma que pode estar associada a várias síndromes, como esclerose tube- rosa, síndrome de Buschke-Ollendorf ou síndrome de Proteus. Casos familiares de NTC têm sido relacionados com maior risco de patologia cardíaca. A apresentação clínica típica caracteriza-se pelo aparecimento de múltiplas placas cor de pele ou amareladas, confluentes no tronco e/ou nos membros, assintomáticas. São geralmente identificados na infância, sem predileção de género. O diagnóstico é confirmado por biopsia cutânea. Estudos adicionais podem ser necessários, de acordo com a presença de sinais ou sintomas sugestivos de uma doença subjacente.

Background: Recombinant human endostatin (Endostar) has been widely used to suppress angiogenesis in carcinoma patients. Hypertrophic scar (HS) tissue, much like a carcinoma, is often associated with angiogenesis. However, there have been few studies conducted on the effects of Endostar on HS or its mechanism. Objective: This paper investigated the effects Endostar on the HS of rabbit ears and studied the effects of Endostar on VEGF and TIMP-1 expression. Methods: Sixteen New Zealand white rabbits were used to establish HS models. Then, rabbit ears containing HS were randomly assigned to either the Endostar group or the control group. The changes of appearance and histology were evaluated using the naked eye, hematoxylin eosin staining, and a scar elevation index. The VEGF and TIMP-1 expressions were detected by immunohistochemical staining, RT-PCR, and western blot. Results: The thickness of the connective tissue in the Endostar group were thinner, the numbers of micro vessels and fibroblasts were fewer, and the collagen fibers were smoother. Moreover, the mRNA and protein expressions of VEGF and TIMP-1 in the Endostar group were significantly lower than those in the control group. Conclusion: The results suggested that Endostar reduced the formation of HS by down-regulation of VEGF and TIMP-1 expressions.

In most anatomical studies developed with mammals, the tongue is described as highly differentiated among different species. However, studies on the tongue of aquatic mammals are still limited as compared to those on terrestrial mammals. The aim of this study was to describe the tongue morphology of the Franciscana dolphin (Pontoporia blainvillei) using macroscopic observations, light, and scanning electron microscopy. Microscopically, the dorsal surface was covered by a keratinized stratified epithelium. Salivary gland acini were found on the middle and caudal third of the tongue. The dorsal surface was totally covered by filiform papillae with a connective tissue core and a connective tissue structure round in shape in the middle and caudal regions. Microsc. Res. Aim: To investigate the healing process following use of collagen sponges in the dental socket after extraction. Wound complications during the study were also evaluated. Methods: 32 cats were included in this study. IV administration of the combination of diazepam (0.22 mg/kg) and ketamine (10 mg/kg) was used to induce general anesthesia. Surgical extraction of both 3rd mandibular premolars was performed. The open dental sockets were divided in two groups. In Group A, the open dental socket on the left side was closed using 4-0 Monocryl in simple interrupted pattern. In Group B, the right dental socket was filled with lyophilized hydrolyzed collagen and the buccal and lingual flaps were sutured using 4-0 Monocryl and simple interrupted pattern. Meloxicam (0.2 mg/kg) was used to manage the post-extraction pain in all cats. Ampicilline 20 mg/kg was used as prophylaxis. The wounds were observed during the study to evaluate any signs of inflammation or dehiscence. Radiographs were taken to compare healing of the socket 3 weeks after the procedure. A 1 mm biopsy punch sample was taken from sockets in all cats for comparison of the healing in both groups. Results: Hemorrhage occurred only in the sockets of Group A. Remission of radiolucent area occurred in both groups. Mean score of inflammation was lower and mean scores of fibrotic reaction and fibroplasia were higher in Group B (p<0.05). Conclusions: Use of hemosponge in alveolar socket may accelerate fibroplasia and formation of the connective tissue and reduce inflammation after tooth extraction. Therefore, post-extraction use of the hemostatic agent in the dental socket is recommended.

Antecedentes: La esclerosis sistémica es una enfermedad crónica del tejido conectivo, de etiología desconocida; caracterizada por presentar vasculopatía, autoinmunidad y fibrosis. Genera importantes repercusiones socio sanitarias y hasta la actualidad no tiene cura. Objetivo: Determinar las características demográficas, clínicas e inmunológicas de la Esclerosis Sistémica en pacientes de los Hospitales Vicente Corral Moscoso y José Carrasco Arteaga. Metodología: Es un estudio cuantitativo, descriptivo y retrospectivo. El universo está constituido por los pacientes diagnosticados de esclerosis sistémica en ambos hospitales. La información se recopiló de las historias clínicas y se analizó en Microsoft Excel 2013 y SPSS 19. Se utilizaron variables cuantitativas y cualitativas, mostrándose a través de frecuencias y porcentajes. Se consideró un nivel de asociación estadística para p<0.05. Resultados: La esclerosis sistémica es más común en mujeres. La forma limitada es la predominante. Las manifestaciones clínicas más comunes son las cutáneas, seguida de las osteomusculares y el Síndrome de Raynaud. El grupo de 50-64 años es el que muestra mayor grado de afectación general; en los más jóvenes las afectaciones cutáneas y osteomusculares son las de mayor frecuencia. Los anticuerpos antinucleares resultaron los de mejor valor diagnóstico, siendo positivos en más del 70% de los pacientes. Los tratamientos más comunes se basan en controlar los síntomas, siendo el paracetamol el más utilizado (80,4%). Conclusiones: La Esclerosis Sistémica afecta más a mujeres que a hombres, el grupo etario más frecuente está entre los 25-64 años. La forma limitada predomina sobre la difusa. Sus principales manifestaciones clínicas son las cutáneas

Antecedentes: La miomatosis es la neoplasia benigna más común en ginecología, presentándose en 20-30% en mujeres en edad fértil, es indicación frecuente de histerectomía entre los 35 y 54 años y puede producir infertilidad, coexistir con un embarazo y causar complicaciones en el mismo. Son tumores de fibras musculares lisas y tejido conectivo con manifestaciones clínicas variables dependiendo del tamaño, posición y número, aunque la mayoría son asintomáticos, es un diagnóstico común en ginecología y generan importantes gastos en salud. Objetivo: Determinar la frecuencia y las características clínico-ecográficas de la miomatosis uterina en las pacientes atendidas en el Hospital Vicente Corral Moscoso, periodo 2010- 2014 Metodología: Se realizó un estudio descriptivo, retrospectivo. El universo de estudio estuvo conformado por las historias clínicas de pacientes hospitalizadas en el departamento de ginecología del Hospital Vicente Corral Moscoso, periodo 2010-2014, los datos fueron analizados en la base de datos Excel 2010 y SPSS 20, los resultados fueron presentados en cuadros y gráficos, para las variables cualitativas se utilizó medidas de frecuencia absolutas y relativas; y para las variables cuantitativas medidas de tendencia central y dispersión. Resultados: Se obtuvieron 303 casos, se observó que el mayor número de casos (90.1%) se presentó en mujeres que tenían una edad mayor a 35 años con una media de 43.31 años y DS de 6.96. Los miomas más frecuentes fueron los intramurales (50.8 %); El síntoma de consulta más frecuente fue el sangrado transvaginal (74.6%)

Mesenchymal stem cells (MSCs) have been used in cell replacement therapies for connective tissue damage, but also can stimulate wound healing through paracrine activity. In order to further understand the potential use of MSCs to treat dogs with neurological disorders, this study examined the paracrine action of adipose-derived canine MSCs on neuronal and endothelial cell models. The culture-expanded MSCs exhibited a MSC phenotype according to plastic adherence, cell morphology, CD profiling and differentiation potential along mesenchymal lineages. Treating the SH-SY5Y neuronal cell line with serum-free MSC culture-conditioned medium (MSC CM) significantly increased SH-SY5Y cell proliferation (P < 0.01), neurite outgrowth (P = 0.0055) and immunopositivity for the neuronal marker βIII-tubulin (P = 0.0002). Treatment of the EA.hy926 endothelial cell line with MSC CM significantly increased the rate of wound closure in endothelial cell scratch wound assays (P = 0.0409), which was associated with significantly increased endothelial cell proliferation (P < 0.05) and migration (P = 0.0001). Furthermore, canine MSC CM induced endothelial tubule formation in EA.hy926 cells in a soluble basement membrane matrix. Hence, this study has demonstrated that adipose-derived canine MSC CM stimulated neuronal and endothelial cells probably through the paracrine activity of MSC-secreted factors. This supports the use of canine MSC transplants or their secreted products in the clinical treatment of dogs with neurological disorders and provides some insight into possible mechanisms of action.

Introdução: Ao longo da última década a procura por um sorriso estético (inclui harmonia e continuidade de forma) transformou-se numa preocupação relevante na Medicina Dentária e em particular nos tratamentos periodontais. As recessões gengivais com as consequentes exposições radiculares e alteração morfológica dos tecidos periodontais, podem constituir um problema estético importante podendo trazer outros problemas associados. Objetivo: O objetivo deste trabalho é identificar qual a técnica cirúrgica mais vantajosa para recobrimento radicular (RRC, RRC com ETC e TUN) e saber em que situações uma poderá ser melhor escolha que a outra, sabendo que ambas são técnicas de alta fiabilidade. Materiais e métodos: Para o cumprimento do objetivo, foi desenvolvida uma pesquisa entre Junho e Setembro de 2016, de artigos em português e inglês, sem limites temporais, recorrendo às bases de dados electrónicas: PUBMED e Google Académico utilizando para o efeito as seguintes “palavras-chave”: “tunnel technique”, “microsurgery”, “recession coverage”, “connective tissue graft”, “coronally advanced flap”, “coronally advanced flap vs. tunnel technique”. Foram utilizados 40 artigos científicos e duas obras literárias (Clinical Periodontology and Implant Dentistry e Plastic-Esthetic Periodontal and Implant Surgery) para complementar o tema. Conclusão: Segundo a literatura publicada e consultada, os procedimentos mais eficazes são aqueles que utilizam enxertos de tecido conjuntivo para o aumento da espessura gengival. Sendo que comparando as duas técnicas Retalho de Reposicionamento Coronal e Técnica de Tunelização, a segunda leva vantagem em relação à primeira, uma vez que, necessitando de menos incisões trará aspetos positivos quanto à cicatrização pois permite maior aporte sanguíneo, além de haver uma preservar das papilas.

Introdução: A artrite reumatoide é uma doença inflamatória, crónica, de etiologia e patogénese ainda desconhecida, caraterizada pelo envolvimento preferencial da membrana sinovial das articulação, pelo desenvolvimento de erosões ósseas e progressiva destruição articular (Fonseca et al., 2002). A periodontite é uma doença infeciosa e crónica caraterizada por perda irreversível dos tecidos de suporte periodontais e de osso alveolar (Ziebolz et al., 2002). Liubomorova (1964) foi o primeiro autor a descrever os possíveis mecanismos inflamatórios comuns de ambas as doenças, assistindo-se, nos últimos anos, a um aumento de evidência científica relativa a uma possível associação entre a periodontite e a artrite reumatoide. Objetivo: A presente revisão de literatura teve como objetivo analisar a evidência científica disponível relativa a uma possível associação entre a periodontite e a artrite reumatoide, atentando-se em particular à prevalência e severidade da periodontite em pacientes com artrite reumatoide. Materiais e métodos: Foi realizada uma pesquisa bibliográfica na base de dados PubMed, priorizando-se revisões sistemáticas e meta-análises, estudos coorte e caso-controlo. Foram apenas incluídos estudos disponíveis em inglês e publicados entre 2010 a 2016. Resultados: Onze estudos compreenderam os critérios para integrar esta revisão. A grande maioria verificou que pacientes com artrite reumatoide são mais suscetíveis a manifestar periodontite e a apresentar piores parâmetros periodontais, quando comparados com indivíduos saudáveis.

Colorectal cancer (CRC) is the third most common cancer in the UK with 41,000 new cases diagnosed in 2011. Despite undergoing potentially curative resection, a significant amount of patients develop recurrence. Biomarkers that aid prognostication or identify patients who are suitable for adjuvant treatments are needed. The TNM staging system does a reasonably good job at offering prognostic information to the treating clinician, but it could be better and identifying methods of improving its accuracy are needed. Tumour progression is based on a complex relationship between tumour behaviour and the hosts’ inflammatory responses. Sustained tumour cell proliferation, evading growth suppressors, resisting apoptosis, replicative immortality, sustained angiogenesis, invasion & metastasis, avoiding immune destruction, deregulated cellular energetics, tumour promoting inflammation and genomic instability & mutation have been identified as hallmarks. These hallmarks are malignant behaviors are what makes the cell cancerous and the more extreme the behaviour the more aggressive the cancer the more likely the risk of a poor outcome. There are two primary genomic instability pathways: Microsatellite Instability (MSI) and Chromosomal Instability (CI) also referred to as Microsatellite Stability (MSS). Tumours arising by these pathways have a predilection for specific anatomical, histological and molecular biological features. It is possible that aberrant molecular expression of genes/proteins that promote malignant behaviors may also act as prognostic and predictive biomarkers, which may offer superior prognostic information to classical prognostic features. Cancer related inflammation has been described as a 7th hallmark of cancer. Despite the systemic inflammatory response (SIR) being associated with more aggressive malignant disease, infiltration by immune cells, particularly CD8+ lymphocytes, at the advancing edge of the tumour have been associated with improved outcome and tumour MSI. It remains unknown if the SIR is associated with tumour MSI and this requires further study. The mechanisms by which colorectal cancer cells locally invade through the bowel remain uncertain, but connective tissue degradation by matrix metalloproteinases (MMPs) such as MMP-9 have been implicated. MMP-9 has been found in the cancer cells, stromal cells and patient circulation. Although tumoural MMP-9 has been associated with poor survival, reports are conflicting and contain relatively small sample sizes. Furthermore, the influence of high serum MMP-9 on survival remains unknown. Src family kinases (SFKs) have been implicated in many adverse cancer cell behaviors. SFKs comprise 9 family members BLK, C-SRC, FGR, FYN, HCK, LCK, LYN, YES, YRK. C-SRC has been the most investigated of all SFKs, but the role of other SFKs in cellular behaviors and their prognostic value remains largely unknown. The development of Src inhibitors, such as Dasatinib, has identified SFKs as a potential therapeutic target for patients at higher risk of poor survival. Unfortunately, clinical trials so far have not been promising but this may reflect inadequate patient selection and SFKs may act as useful prognostic and predictive biomarkers. In chapter 3, the association between cancer related inflammation, tumour MSI, clinicopathological factors and survival was tested in two independent cohorts. A training cohort consisting of n=182 patients and a validation cohort of n=677 patients. MSI tumours were associated with a raised CRP (p=0.003). Hypoalbuminaemia was independently associated with poor overall survival in TNM stage II cancer (HR 3.04 (95% CI 1.44 – 6.43);p=0.004), poor recurrence free survival in TNM stage III cancer (HR 1.86 (95% 1.03 – 3.36);p=0.040) and poor overall survival in CI colorectal cancer (HR 1.49 (95% CI 1.06 – 2.10);p=0.022). Interestingly, MSI tumours were associated with poor overall survival in TNM stage III cancer (HR 2.20 (95% CI 1.10 – 4.37);p=0.025). In chapter 4, the role of MMP-9 in colorectal cancer progression and survival was examined. MMP-9 in the tissue was assessed using IHC and serum expression quantified using ELISA. Serum MMP-9 was associated with cancer cell expression (Spearman’s Correlation Coefficient (SCC) 0.393, p<0.001)) and stromal expression (SCC 0.319, p=0.002). Serum MMP-9 was associated with poor recurrence-free (HR 3.37 (95% CI 1.20 – 9.48);p=0.021) and overall survival (HR 3.16 (95% CI 1.22 – 8.15);p=0.018), but tumour MMP-9 was not survival or MSI status. In chapter 5, the role of SFK expression and activation in colorectal cancer progression and survival was studied. On PCR analysis, although LYN, C-SRC and YES were the most highly expressed, FGR and HCK had higher expression profiles as tumours progressed. Using IHC, raised cytoplasmic FAK (tyr 861) was independently associated with poor recurrence free survival in all cancers (HR 1.48 (95% CI 1.02 – 2.16);p=0.040) and CI cancers (HR 1.50 (95% CI 1.02 – 2.21);p=0.040). However, raised cytoplasmic HCK (HR 2.04 (95% CI 1.11 – 3.76);p=0.022) was independently associated with poor recurrence-free survival in TNM stage II cancers. T84 and HT29 cell lines were used to examine the cellular effects of Dasatinib. Cell viability was assessed using WST-1 assay and apoptosis assessed using an ELISA cell death detection assay. Dasatinib increased T84 tumour cell apoptosis in a dose dependent manner and resulted in reduced expression of nuclear (p=0.008) and cytoplasmic (p=0.016) FAK (tyr 861) expression and increased nuclear FGR expression (p=0.004). The results of this thesis confirm that colorectal cancer is a complex disease that represents several subtypes of cancer based on molecular biological behaviors. This thesis concentrated on features of the disease related to inflammation in terms of genetic and molecular characterisation. MSI cancers are closely associated with systemic inflammation but despite this observation, they retain their relatively improved survival. MMP-9 is a feature of tissue remodeling during inflammation and is also associated with degradation of connective tissue, advanced T-stage and poor outcome when measured in the serum. The lack of stromal quantification due to TMA use rather than full sections makes the value of tumoural MMP-9 immunoreactivity in the prognostication and its association with MSI unknown and requires further study. Finally, SFK activation was also associated with SIR, however, only cytoplasmic HCK was independently associated with poor survival in patients with TNM stage II disease, the group of patients where identifying a novel biomarker is most needed. There is still some way to go before these biomarkers are translated into clinical practice and future work needs to focus on obtaining a reliable and robust scientific technique with validation in an adequately powered independent cohort.
