This research investigates the relationship between elevated trace elements in soils, stream sediments and stream water and the prevalence of Chronic Kidney Disease (CKD). The study uses a collaboration of datasets provided from the UK Renal Registry Report (UKRR) on patients with renal diseases requiring treatment including Renal Replacement Therapy (RRT), the soil geochemical dataset for Northern Ireland provided by the Tellus Survey, Geological Survey of Northern Ireland (GSNI) and the bioaccessibility of Potentially Toxic Elements (PTEs) from soil samples which were obtained from the Unified Barge Method (UBM). The relationship between these factors derives from the UKRR report which highlights incidence rates of renal impaired patients showing regional variation with cases of unknown aetiology. Studies suggest a potential cause of the large variation and uncertain aetiology is associated with underlying environmental factors such as the oral bioaccessibility of trace elements in the gastrointestinal tract.
As previous research indicates that long term exposure is related to environmental factors, Northern Ireland is ideally placed for this research as people traditionally live in the same location for long periods of time. Exploratory data analysis and multivariate analyses are used to examine the soil, stream sediments and stream water geochemistry data for a range of key elements including arsenic, lead, cadmium and mercury identified from a review of previous renal disease literature. The spatial prevalence of patients with long term CKD is analysed on an area basis. Further work includes cluster analysis to detect areas of low or high incidences of CKD that are significantly correlated in space, Geographical Weighted Regression (GWR) and Poisson kriging to examine locally varying relationship between elevated concentrations of PTEs and the prevalence of CKD.

Solid organ transplant recipients have elevated cancer risks, owing in part to pharmacologic immunosuppression. However, little is known about risks for hematologic malignancies of myeloid origin. We linked the US Scientific Registry of Transplant Recipients with 15 population-based cancer registries to ascertain cancer occurrence among 207 859 solid organ transplants (1987–2009). Solid organ transplant recipients had a significantly elevated risk for myeloid neoplasms, with standardized incidence ratios (SIRs) of 4.6 (95% confidence interval 3.8–5.6; N=101) for myelodysplastic syndromes (MDS), 2.7 (2.2–3.2; N=125) for acute myeloid leukemia (AML), 2.3 (1.6–3.2; N=36) for chronic myeloid leukemia and 7.2 (5.4–9.3; N=57) for polycythemia vera. SIRs were highest among younger individuals and varied by time since transplantation and organ type (Poisson regression P<0.05 for all comparisons). Azathioprine for initial maintenance immunosuppression increased risk for MDS (P=0.0002) and AML (2–5 years after transplantation, P=0.0163). Overall survival following AML/MDS among transplant recipients was inferior to that of similar patients reported to US cancer registries (log-rank P<0.0001). Our novel finding of increased risks for specific myeloid neoplasms after solid organ transplantation supports a role for immune dysfunction in myeloid neoplasm etiology. The increased risks and inferior survival should heighten clinician awareness of myeloid neoplasms during follow-up of transplant recipients.

Background: Hirschsprung's disease is a congenital gut motility disorder, characterised by the absence of the enteric ganglion cells along the distal gut. The aim of this study was to describe the epidemiology of Hirschsprung's disease, including additional congenital anomalies, total prevalence, trends, and association with maternal age. Methods: Cases of Hirschsprung's disease delivered during 1980 to 2009 notified to 31 European Surveillance of Congenital Anomaly registers formed the population-based case-series. Prevalence rates and 95% confidence intervals were calculated as the number of cases per 10,000 births. Multilevel Poisson regression was performed to investigate trends in prevalence, geographical variation and the association with maternal age. Results: There were 1,322 cases of Hirschsprung's disease among 12,146,210 births. The total prevalence was 1.09 (95% confidence interval, 1.03–1.15) per 10,000 births and there was a small but significant increase in prevalence over time (relative risk = 1.01; 95% credible interval, 1.00–1.02; p = 0.004). There was evidence of geographical heterogeneity in prevalence (p < 0.001). Excluding 146 (11.0%) cases with chromosomal anomalies or genetic syndromes, there were 1,176 cases (prevalence = 0.97; 95% confidence interval, 0.91–1.03 per 10,000 births), of which 137 (11.6%) had major structural anomalies. There was no evidence of a significant increased risk of Hirschsprung's disease in cases born to women aged ≥35 years compared with those aged 25 to 29 (relative risk = 1.09; 95% credible interval, 0.91–1.31; p = 0.355). Conclusion: This large population-based study found evidence of a small increasing trend in Hirschsprung's disease and differences in prevalence by geographic location. There was also no evidence of an association with maternal age.
T- and L-type Ca2+ currents in freshly dispersed smooth muscle cells from the human proximal urethra

The purpose of the present study was to characterise Ca2+ currents in smooth muscle cells isolated from biopsy samples taken from the proximal urethra of patients undergoing surgery for bladder or prostate cancer. Cells were studied at 37 degreesC using the amphotericin B perforated-patch configuration of the patch-clamp technique. Currents were recorded using Cs+-rich pipette solutions to block K+ currents. Two components of current, with electrophysiological and pharmacological properties typical of T- and L-type Ca2+ currents, were present in these cells. When steady-state inactivation curves for the L current were fitted with a Boltzmann equation, this yielded a V-1/2 of -45 +/- 5 mV. In contrast, the T current inactivated with a V-1/2 of -80 +/- 3 mV. The L currents were reduced in a concentration-dependent manner by nifedipine (ED50 = 159 +/- 54 nm) and Ni2+ (ED50 = 65 +/- 16 muM) but were enhanced when external Ca2+ was substituted with Ba2+. The T current was little affected by TTX, reduction in external Na+, application of nifedipine at concentrations below 300 nm or substitution of external Ca2+ with Ba2+, but was reduced by Ni2+ with an ED50 of 6 +/- 1 mum. When cells were stepped from -100 to -30 mV in Ca2+-free conditions, small inward currents could be detected. These were enhanced 40-fold in divalent-cation-free solution and blocked in a concentration-dependent manner by Mg2+ with an ED50 of 32 +/- 16 mum. These data support the idea that human urethral myocytes possess currents with electrophysiological and pharmacological properties typical of T- and L-type Ca2+ currents.

Background: This survey aimed to record the dietary habits and oral health behaviours of patients undergoing methadone maintenance therapy at a Scottish drug rehabilitation centre.The objectives were to obtain descriptive data for each of the participants on items including dietary habits, oral hygiene practices and dental health. The study also aimed to explore explanatory relationships between dietary habits, oral hygiene practices and dental health (DMFT) in methadone users.
Methods: A cross – sectional descriptive study using survey methodology was conducted of consecutive adult patients undergoing methadone maintenance therapy at a non-residential drug rehabilitation centre in Dundee, Scotland. A self-completion retrospective questionnaire was distributed to 66 consecutive patients.
Results: A response rate of 74.2% was achieved. Participants reported low daily intakes of fresh fruit and vegetables with diets high in fatty foods. Respondents reported regular snacking between meals and consumption of large amounts of sugared carbonated drinks. Oral hygiene practices were poorly adhered to and a high level of dental disease was observed amongst participants. Poisson regression analysis revealed that the amount of alcohol consumed per day (p=0.02), the length of time taking methadone (p=0.002) the amount of sugar added to hot drinks (p<0.0001) and regular dental attendance (p=0.0001) were all independently associated with poor dental health.
Conclusions: Dietary habits and adherence to oral hygiene practices amongst this group of patients were very poor. This study suggests that these behaviours were contributing to the high levels of dental disease observed in this group.

Differential equations are often directly solvable by analytical means only in their one dimensional version. Partial differential equations are generally not solvable by analytical means in two and three dimensions, with the exception of few special cases. In all other cases, numerical approximation methods need to be utilized. One of the most popular methods is the finite element method. The main areas of focus, here, are the Poisson heat equation and the plate bending equation. The purpose of this paper is to provide a quick walkthrough of the various approaches that the authors followed in pursuit of creating optimal solvers, accelerated with the use of graphical processing units, and comparing them in terms of accuracy and time efficiency with existing or self-made non-accelerated solvers.

Based on models with calibrated parameters for infection, case fatality rates, and vaccine efficacy, basic childhood vaccinations have been estimated to be highly cost effective. We estimate the association of vaccination with mortality directly from survey data. Using 149 cross-sectional Demographic and Health Surveys, we determine the relationship between vaccination coverage and under five mortality at the survey cluster level. Our data include approximately one million children in 68,490 clusters in 62 countries. We consider the childhood measles, Bacille Calmette-Guérin (BCG), Diphtheria-Pertussis-Tetanus (DPT), Polio, and maternal tetanus vaccinations. Using modified Poisson regression to estimate the relative risk of child mortality in each cluster, we also adjust for selection bias caused by the vaccination status of dead children not being reported. Childhood vaccination, and in particular measles and tetanus vaccination, is associated with substantial reductions in childhood mortality. We estimate that children in clusters with complete vaccination coverage have relative risk of mortality 0.73 (95% Confidence Interval: 0.68, 0.77) that of children in a cluster with no vaccination. While widely used, basic vaccines still have coverage rates well below 100% in many countries, and our results emphasize the effectiveness of increasing their coverage rates in order to reduce child mortality.

To evaluate the performance of the co-channel transmission based communication, we propose a new metric for area spectral efficiency (ASE) of interference limited ad-hoc network by assuming that the nodes are randomly distributed according to a Poisson point processes (PPP). We introduce a utility function, U = ASE/delay and derive the optimal ALOHA transmission probability p and the SIR threshold τ that jointly maximize the ASE and minimize the local delay. Finally, numerical results have been conducted to confirm that the joint optimization based on the U metric achieves a significant performance gain compared to conventional systems.

This letter investigates the uplink spectral efficiency (SE) of a two-tier cellular network, where massive multiple-input multiple-output macro base stations are overlaid with dense small cells. Macro user equipments (MUEs) and small cells with single user equipment uniformly scattered are modeled as two independent homogeneous Poisson point processes. By applying stochastic geometry, we analyze the SE of the multiuser uplink at a macro base station that employs a zero-forcing detector and we obtain a novel lower bound as well as its approximation. According to the simple and near-exact analytical expression, we observe that the ideal way to improve the SE is by increasing the MUE density and the base station antennas synchronously rather than increasing them individually. Furthermore, a large value of path loss exponent has a positive effect on the SE due to the reduced aggregated interference.

The Richardson–Lucy algorithm is one of the most important in image deconvolution. However, a drawback is its slow convergence. A significant acceleration was obtained using the technique proposed by Biggs and Andrews (BA), which is implemented in the deconvlucy function of the image processing MATLAB toolbox. The BA method was developed heuristically with no proof of convergence. In this paper, we introduce the heavy-ball (H-B) method for Poisson data optimization and extend it to a scaled H-B method, which includes the BA method as a special case. The method has a proof of the convergence rateof O(K^2), where k is the number of iterations. We demonstrate the superior convergence performance, by a speedup factor off ive, of the scaled H-B method on both synthetic and real 3D images.

A análise das séries temporais de valores inteiros tornou-se, nos últimos anos, uma área de investigação importante, não só devido à sua aplicação a dados de contagem provenientes de diversos campos da ciência, mas também pelo facto de ser uma área pouco explorada, em contraste com a análise séries temporais de valores contínuos. Uma classe que tem obtido especial relevo é a dos modelos baseados no operador binomial thinning, da qual se destaca o modelo auto-regressivo de valores inteiros de ordem p. Esta classe é muito vasta, pelo que este trabalho tem como objectivo dar um contributo para a análise estatística de processos de contagem que lhe pertencem. Esta análise é realizada do ponto de vista da predição de acontecimentos, aos quais estão associados mecanismos de alarme, e também da introdução de novos modelos que se baseiam no referido operador. Em muitos fenómenos descritos por processos estocásticos a implementação de um sistema de alarmes pode ser fundamental para prever a ocorrência de um acontecimento futuro. Neste trabalho abordam-se, nas perspectivas clássica e bayesiana, os sistemas de alarme óptimos para processos de contagem, cujos parâmetros dependem de covariáveis de interesse e que variam no tempo, mais concretamente para o modelo auto-regressivo de valores inteiros não negativos com coeficientes estocásticos, DSINAR(1). A introdução de novos modelos que pertencem à classe dos modelos baseados no operador binomial thinning é feita quando se propõem os modelos PINAR(1)T e o modelo SETINAR(2;1). O modelo PINAR(1)T tem estrutura periódica, cujas inovações são uma sucessão periódica de variáveis aleatórias independentes com distribuição de Poisson, o qual foi estudado com detalhe ao nível das suas propriedades probabilísticas, métodos de estimação e previsão. O modelo SETINAR(2;1) é um processo auto-regressivo de valores inteiros, definido por limiares auto-induzidos e cujas inovações formam uma sucessão de variáveis independentes e identicamente distribuídas com distribuição de Poisson. Para este modelo estudam-se as suas propriedades probabilísticas e métodos para estimar os seus parâmetros. Para cada modelo introduzido, foram realizados estudos de simulação para comparar os métodos de estimação que foram usados.

Nesta tese relatam-se estudos de fotoluminescência de nanopartículas de óxidos e fosfatos dopados com iões trivalentes de lantanídeos, respectivamente, nanobastonetes de (Gd,Eu)2O3 e (Gd,Yb,Er)2O3 e nanocristais de (Gd,Yb,Tb)PO4, demonstrando-se também aplicações destes materiais em revestimentos inteligentes, sensores de temperatura e bioimagem. Estuda-se a transferência de energia entre os sítios de Eu3+ C2 e S6 dos nanobastonetes Gd2O3. A contribuição dos mecanismos de transferência de energia entre sítios para o tempo de subida 5D0(C2) é descartada a favor da relaxação directa 5D1(C2) 5D0(C2) (i.e., transferência de energia entre níveis). O maior tempo de decaimento do nível 5D0(C2) nos nanobastonetes, relativamente ao valor medido para o mesmo material na forma de microcristais, é atribuído, quer à existência de espaços livres entre nanobastonetes próximos (factor de enchimento ou fracção volúmica), quer à variação do índice de refracção efectivo do meio em torno dos iões Eu3+. A dispersão de nanobastonetes de (Gd,Eu)2O3 em três resinas epoxi comerciais através da cura por UV permite obter nanocompósitos epoxi- (Gd,Eu)2O3. Relatam-se estudos cinéticos e das propriedades térmicas e de fotoluminescência destes nanocompósitos. Estes, preservam as típicas propriedades de emissão do Eu3+, mostrando o potencial do método de cura por UV para obter revistimentos inteligentes e fotoactivos. Considera-se um avanço significativo a realização de uma nanoplataforma óptica, incorporando aquecedor e termómetro e capaz de medir uma ampla gama de temperaturas (300-2000 K) à escala nano, baseada em nanobastonetes de (Gd,Yb,Er)2O3 (termómetros) cuja superfície se encontra revestida com nanopartículas de ouro. A temperature local é calculada usando, quer a distribuição de Boltzmann (300-1050 K) do rácio de intensidades da conversão ascendente 2H11=2!4I15=2/4S3=2!4I15=2, quer a lei de Planck (1200-2000 K) para uma emissão de luz branca atribuída à radiação do corpo negro. Finalmente, estudam-se as propriedades de fotoluminescência correspondentes às conversões ascendente e descendente de energia em nanocristais de (Gd,Yb,Tb)PO4 sintetizados por via hidrotérmica. A relaxividade (ressonância magnética) do 1H destes materiais são investigadas, tendo em vista possíveis aplicações em imagem bimodal (luminescência e ressonância magnética nuclear).

A modelação e análise de séries temporais de valores inteiros têm sido alvo de grande investigação e desenvolvimento nos últimos anos, com aplicações várias em diversas áreas da ciência. Nesta tese a atenção centrar-se-á no estudo na classe de modelos basedos no operador thinning binomial. Tendo como base o operador thinning binomial, esta tese focou-se na construção e estudo de modelos SETINAR(2; p(1); p(2)) e PSETINAR(2; 1; 1)T , modelos autorregressivos de valores inteiros com limiares autoinduzidos e dois regimes, admitindo que as inovações formam uma sucessão de variáveis independentes com distribuição de Poisson. Relativamente ao primeiro modelo analisado, o modelo SETINAR(2; p(1); p(2)), além do estudo das suas propriedades probabilísticas e de métodos, clássicos e bayesianos, para estimar os parâmetros, analisou-se a questão da seleção das ordens, no caso de elas serem desconhecidas. Com este objetivo consideraram-se algoritmos de Monte Carlo via cadeias de Markov, em particular o algoritmo Reversible Jump, abordando-se também o problema da seleção de modelos, usando metodologias clássica e bayesiana. Complementou-se a análise através de um estudo de simulação e uma aplicação a dois conjuntos de dados reais. O modelo PSETINAR(2; 1; 1)T proposto, é também um modelo autorregressivo com limiares autoinduzidos e dois regimes, de ordem unitária em cada um deles, mas apresentando uma estrutura periódica. Estudaram-se as suas propriedades probabilísticas, analisaram-se os problemas de inferência e predição de futuras observações e realizaram-se estudos de simulação.

The Asymmetric Power Arch representation for the volatility was introduced by Ding et al.(1993) in order to account for asymmetric responses in the volatility in the analysis of continuous-valued financial time series like, for instance, the log-return series of foreign exchange rates, stock indices or share prices. As reported by Brannas and Quoreshi (2010), asymmetric responses in volatility are also observed in time series of counts such as the number of intra-day transactions in stocks. In this work, an asymmetric power autoregressive conditional Poisson model is introduced for the analysis of time series of counts exhibiting asymmetric overdispersion. Basic probabilistic and statistical properties are summarized and parameter estimation is discussed. A simulation study is presented to illustrate the proposed model. Finally, an empirical application to a set of data concerning the daily number of stock transactions is also presented to attest for its practical applicability in data analysis.
