En la Conferencia Regional Latinoamericana y del Caribe sobre Población y Desarrollo (México, D.F., 29 de abril al 4 de mayo de 1993);, preparatoria de la Conferencia Internacional sobre la Población y el Desarrollo (CIPD); (El Cairo, septiembre de 1994);, se aprobó por unanimidad una declaración denominada "Consenso Latinoamericano y del Caribe sobre Población y Desarrollo", en la que se expresa la posición conjunta de los países de la región ante los temas de población y desarrollo. Los objetivos y metas de este Consenso configuran la visión y aspiraciones de la región con respecto a las relaciones entre población, desarrollo y medio ambiente, así como al avance de los derechos humanos y de la lucha contra la desigualdad en las condiciones de vida que se da entre grupos sociales, grupos étnicos, zonas geográficas y hombres y mujeres. Con la premisa de que un mayor apoyo mutuo entre los países de la región les permitiría avanzar de una manera más sólida hacia el logro de esos objetivos y metas, en la Conferencia Regional los países decidieron promover la adopción de un plan de acción regional. Por lo tanto, los propósitos generales de ese plan debían apuntar precisamente a facilitar la integración y el intercambio de experiencias nacionales en la formulación y aplicación de políticas y programas de población, tanto del pasado como de la actualidad. Con el objetivo de asegurar una adecuada implementación, seguimiento y revisión del Plan de Acción Regional, los países acordaron, en el vigésimo quinto período de sesiones de la CEPAL, entregar dicha responsabilidad al Comité Especial sobre Población y Desarrollo, que se reunirá, por primera vez, en oportunidad del vigésimo sexto período de sesiones en San José, Costa Rica.

Background: Acute respiratory distress syndrome (ARDS) is associated with high in-hospital mortality. Alveolar recruitment followed by ventilation at optimal titrated PEEP may reduce ventilator-induced lung injury and improve oxygenation in patients with ARDS, but the effects on mortality and other clinical outcomes remain unknown. This article reports the rationale, study design, and analysis plan of the Alveolar Recruitment for ARDS Trial (ART). Methods/Design: ART is a pragmatic, multicenter, randomized (concealed), controlled trial, which aims to determine if maximum stepwise alveolar recruitment associated with PEEP titration is able to increase 28-day survival in patients with ARDS compared to conventional treatment (ARDSNet strategy). We will enroll adult patients with ARDS of less than 72 h duration. The intervention group will receive an alveolar recruitment maneuver, with stepwise increases of PEEP achieving 45 cmH(2)O and peak pressure of 60 cmH2O, followed by ventilation with optimal PEEP titrated according to the static compliance of the respiratory system. In the control group, mechanical ventilation will follow a conventional protocol (ARDSNet). In both groups, we will use controlled volume mode with low tidal volumes (4 to 6 mL/kg of predicted body weight) and targeting plateau pressure <= 30 cmH2O. The primary outcome is 28-day survival, and the secondary outcomes are: length of ICU stay; length of hospital stay; pneumothorax requiring chest tube during first 7 days; barotrauma during first 7 days; mechanical ventilation-free days from days 1 to 28; ICU, in-hospital, and 6-month survival. ART is an event-guided trial planned to last until 520 events (deaths within 28 days) are observed. These events allow detection of a hazard ratio of 0.75, with 90% power and two-tailed type I error of 5%. All analysis will follow the intention-to-treat principle. Discussion: If the ART strategy with maximum recruitment and PEEP titration improves 28-day survival, this will represent a notable advance to the care of ARDS patients. Conversely, if the ART strategy is similar or inferior to the current evidence-based strategy (ARDSNet), this should also change current practice as many institutions routinely employ recruitment maneuvers and set PEEP levels according to some titration method.

The municipality of San Juan La Laguna, Guatemala is home to approximately 5,200 people and located on the western side of the Lake Atitlán caldera. Steep slopes surround all but the eastern side of San Juan. The Lake Atitlán watershed is susceptible to many natural hazards, but most predictable are the landslides that can occur annually with each rainy season, especially during high-intensity events. Hurricane Stan hit Guatemala in October 2005; the resulting flooding and landslides devastated the Atitlán region. Locations of landslide and non-landslide points were obtained from field observations and orthophotos taken following Hurricane Stan. This study used data from multiple attributes, at every landslide and non-landslide point, and applied different multivariate analyses to optimize a model for landslides prediction during high-intensity precipitation events like Hurricane Stan. The attributes considered in this study are: geology, geomorphology, distance to faults and streams, land use, slope, aspect, curvature, plan curvature, profile curvature and topographic wetness index. The attributes were pre-evaluated for their ability to predict landslides using four different attribute evaluators, all available in the open source data mining software Weka: filtered subset, information gain, gain ratio and chi-squared. Three multivariate algorithms (decision tree J48, logistic regression and BayesNet) were optimized for landslide prediction using different attributes. The following statistical parameters were used to evaluate model accuracy: precision, recall, F measure and area under the receiver operating characteristic (ROC) curve. The algorithm BayesNet yielded the most accurate model and was used to build a probability map of landslide initiation points. The probability map developed in this study was also compared to the results of a bivariate landslide susceptibility analysis conducted for the watershed, encompassing Lake Atitlán and San Juan. Landslides from Tropical Storm Agatha 2010 were used to independently validate this study’s multivariate model and the bivariate model. The ultimate aim of this study is to share the methodology and results with municipal contacts from the author's time as a U.S. Peace Corps volunteer, to facilitate more effective future landslide hazard planning and mitigation.

This study is a secondary analysis of a survey developed by Dr. Jimmy Perkins and administered by San Antonio/Bexar County Metropolitan Health District. The survey was developed subsequent to the implementation of the city smoking ordinance effective January 1, 2004. The survey had a multi-purpose plan to establish the number of restaurants having smoke free status prior to and following the ordinance, determine compliance as it relates to a necessary smoking section and proper signage, and expose the rationale for restaurants to become smoke free. The data resulting from the survey was presented to the San Antonio/Bexar County Metropolitan Health District. The summary presented the types of establishments surveyed, smoking status of the establishment, reasons for the establishment becoming smoke free, compliance with smoking sections, compliance with signage requirements, awareness of ordinance, and chain status of the establishment. ^ The results of this study display the relationships among the variables previously mentioned. The following relationships have been examined and the outcomes have determined whether each is significant. After careful analysis, knowledge translates into compliance with signage regulations, which then translate into ordinance compliance. Size does matter as it relates to an establishment's number of employees and seating capacity. The smaller the establishment the more likely the establishment is to have become smoke free before the ordinance went into effect. Restaurants, rather than fast food establishments most commonly cited their reason for becoming smoke free was to comply with the ordinance and only ten percent of restaurants gave policy as the main reason for becoming smoke free. ^ This study is important for public health because the negative health effects of environmental tobacco smoke (ETS) are still an overwhelming problem in the United States (3). ETS is a Known Human Group A Carcinogen (5). The Environmental Protection Agency (EPA) has estimated that around 3,000 non-smoking Americans die every year from lung cancer caused by ETS (6). This information illustrates the importance of providing smoke free establishments, especially to non-smoking patrons. ^
