There have been many studies pertaining to the management of herpetic meningoencephalitis (HME), but the majority of them have focussed on virologically unconfirmed cases or included only small sample sizes. We have conducted a multicentre study aimed at providing management strategies for HME. Overall, 501 adult patients with PCR-proven HME were included retrospectively from 35 referral centres in 10 countries; 496 patients were found to be eligible for the analysis. Cerebrospinal fluid (CSF) analysis using a PCR assay yielded herpes simplex virus (HSV)-1 DNA in 351 patients (70.8%), HSV-2 DNA in 83 patients (16.7%) and undefined HSV DNA type in 62 patients (12.5%). A total of 379 patients (76.4%) had at least one of the specified characteristics of encephalitis, and we placed these patients into the encephalitis presentation group. The remaining 117 patients (23.6%) had none of these findings, and these patients were placed in the nonencephalitis presentation group. Abnormalities suggestive of encephalitis were detected in magnetic resonance imaging (MRI) in 83.9% of the patients and in electroencephalography (EEG) in 91.0% of patients in the encephalitis presentation group. In the nonencephalitis presentation group, MRI and EEG data were suggestive of encephalitis in 33.3 and 61.9% of patients, respectively. However, the concomitant use of MRI and EEG indicated encephalitis in 96.3 and 87.5% of the cases with and without encephalitic clinical presentation, respectively. Considering the subtle nature of HME, CSF HSV PCR, EEG and MRI data should be collected for all patients with a central nervous system infection.

The western corn rootworm (WCR) is a major pest of maize that is well adapted to most crop management strategies. Breeding for tolerance is a promising alternative to combat WCR, but is currently constrained by a lack of physiological understanding and phenotyping tools. We developed dynamic precision phenotyping approaches using carbon-11 with positron emission tomography, root autoradiography and radiometabolite flux analysis to understand maize tolerance to WCR. Our results reveal that WCR attack induces specific patterns of lateral root growth which are associated with a shift in auxin biosynthesis from indole-3-pyruvic acid to indole-3-acetonitrile. WCR attack also increases transport of newly synthesized amino acids to the roots, including the accumulation of glutamine. Finally, the regrowth zones of WCR attacked roots show an increase in glutamine turnover which strongly correlates with the induction of indole-3-acetonitrile-dependent auxin biosynthesis. In summary, our findings identify local changes in the auxin flux network as a promising marker for induced WCR tolerance.

O objetivo dessa dissertação é analisar como as ideias sobre a juventude, representadas pelos discursos oriundos de diferentes materiais empíricos – documentos governamentais e de organizações da sociedade civil e entrevistas não-estruturadas – e a forma pela qual os jovens são reconhecidos como sujeitos de direitos influenciaram o processo de formação das políticas de juventude no Brasil e institucionalização do tema em âmbito federal, assim como a escolha pelas estratégias administrativas que constituem as principais formas de ação e de gestão governamental no tema. Para isso, adotamos como ponto de partida a reflexão sobre o uso da retórica, por meio de metáforas e narrativas na formação e trajetória das políticas, a existência de coalizões discursivas em seu desenvolvimento e a utilização de frames (enquadramentos interpretativos) no exame dos pontos de inflexão das ações para a juventude no Brasil. Em seguida, identificamos as principais ideias sobre as juventudes presentes no campo e analisamos como elas alimentaram o reconhecimento dos jovens enquanto segmento social diferenciado dos demais em função da sua “condição juvenil” e enquanto sujeitos de direitos integrais. Com base nessas concepções, examinamos a criação e a definição as principais funções da Secretaria Nacional de Juventude, a partir de um debate sobre o conceito de transversalidade, e a formulação e trajetória do Programa Nacional de Inclusão de Jovens (PROJOVEM), adentrando o debate sobre a intersetorialidade nas políticas públicas. A principal contribuição dessa pesquisa foi relacionar como as ideias e discursos influem não apenas na formação de políticas, mas também como as construções sociais que emergem do campo, balizadas pelo reconhecimento dos sujeitos de direitos, também impactam nas estratégias de gestão. Conclui-se, ainda, que apesar da crescente importância atribuída à gestão transversal e à articulação intersetorial na gestão das políticas de juventude no Brasil, são as próprias concepções que orientam sua escolha como estratégias administrativas das quais decorrem os principais limites para a sua operacionalização cotidiana pela administração pública brasileira em nível federal.

Dans ce mémoire, intitulé « Les dépotoirs dans le schème d’établissement iroquoien: exemple du site Mailhot-Curran», je m’attèle à définir les modes de rejets des déchets dans le contexte d’un schème d’établissement sédentaire semi-permanent au Sylvicole supérieur. Les dépotoirs des sites archéologiques ne semblent pas obtenir toute l’attention analytique qui leur est due et l’étude des modes de rejets a longtemps été négligée. Pourtant, les amas de déchets sont récurrents sur les sites du Nord-Est américain et ce depuis le Sylvicole moyen. L’étude approfondie de l’organisation d’un site iroquoien se doit de passer par la compréhension des modes d’élimination des déchets, puisque la gestion des rejets est généralement le reflet de la structure du site et du type de peuplement. Les dépotoirs peuvent fournir des informations précieuses sur les zones d’activité, la fonction du site ou encore la durée d’occupation. Le village de Mailhot-Curran témoigne de plusieurs concentrations d’objets. Je souhaite établir si ces concentrations de vestiges peuvent être qualifiées de dépotoirs et si oui, peut-on en distinguer différents types? D’autre part, je m’intéresse à l’aspect social qui encadre la mise en place de dépotoirs. Bien que les dépotoirs soient des indices associés aux premières traces de sédentarité saisonnière, leur présence est précisément liée au phénomène de sédentarisation annuelle. Il s’agit également de définir le geste culturel, l’assainissement d’un village étant un schéma culturel très répandu. De ce geste de rejet découle forcément le choix d’un emplacement, ainsi que le concept de «flux de déplacement» des déchets. Il s’agit de déterminer le lien entretenu entre les aires d’activités et les aires de dépôts primaires et secondaires. Pour ce faire, il est primordial d’évaluer la densité, l’hétérogénéité et le contenu des dépotoirs. Suite à la définition de l’ensemble de ces concepts appliqués au site Mailhot-Curran, je souhaite, à travers une étude comparative, répondre à la question suivante : Les dépotoirs peuvent-ils refléter une pratique culturelle commune aux villages iroquoiens de la région de Saint-Anicet?

One of the key environmental concerns about shrimp farming is the discharge of waters with high levels of nutrients and suspended solids into adjacent waterways. In this paper we synthesize the results of our multidisciplinary research linking ecological processes in intensive shrimp ponds with their downstream impacts in tidal, mangrove-lined creeks. The incorporation of process measurements and bioindicators, in addition to water quality measurements, improved our understanding of the effect of shrimp farm discharges on the ecological health of the receiving water bodies. Changes in water quality parameters were an oversimplification of the ecological effects of water discharges, and use of key measures including primary production rates, phytoplankton responses to nutrients, community shifts in zooplankton and delta(15)N ratios in marine plants have the potential to provide more integrated and robust measures. Ultimately, reduction in nutrient discharges is most likely to ensure the future sustainability of the industry. (C) 2003 Elsevier Ltd. All rights reserved.

The diversity, frequency, and scale of human impacts on coral reefs are increasing to the extent that reefs are threatened globally. Projected increases in carbon dioxide and temperature over the next 50 years exceed the conditions under which coral reefs have flourished over the past half-million years. However, reefs will change rather than disappear entirely, with some species already showing far greater tolerance to climate change and coral bleaching than others. International integration of management strategies that support reef resilience need to be vigorously implemented, and complemented by strong policy decisions to reduce the rate of global warming.

Conservation of genetic resources is a recognised necessity for the long term maintenance of evolutionary potential. Effective assessment and implementation Strategies are required to permit rapid evaluation and protection of resources. Here we use information from the chloroplast, total genome and quantitative characters assayed across wide-ranging populations to assess genetic resources in a Neotropical tree, Cedrela odorata. A major differentiation identified for organelle, total genomic and quantitative variation was found to coincide with an environmental gradient across Costa Rica. However, a major evolutionary divergence between the Yucatan region and Honduras/Nicaragua identified within the chloroplast genome was not differentiated using quantitative characters. Based on these and other results, a three-tiered conservation genetic prioritisation process is recommended. In order of importance, and where information is available, conservation units should be defined using quantitative (expressed genes), nuclear (genetic connectivity) and organellar (evolutionary) measures. Where possible, information from range wide and local scale studies should be combined and emphasis should be placed on coincidental disjunctions for two or more measures. However, if only rapid assessments of diversity are possible, then assessment of organelle variation provides the most cautious assessment of genetic resources, at least for C. odorata, and can be used to propose initial conservation units. When considering effective implementation of genetic resource management strategies a final tier should be considered, that of landuse/geopolitical divisions. (C) 2004 Elsevier B.V. In patients hospitalised with acute coronary syndromes (ACS) and congestive heart failure (CHF), evidence suggests opportunities for improving in-hospital and after hospital care, patient self-care, and hospital-community integration. A multidisciplinary quality improvement program was designed and instigated in Brisbane in October 2000 involving 250 clinicians at three teaching hospitals, 1080 general practitioners (GPs) from five Divisions of General Practice, 1594 patients with ACS and 904 patients with CHF. Quality improvement interventions were implemented over 17 months after a 6-month baseline period and included: clinical decision support (clinical practice guidelines, reminders, checklists, clinical pathways); educational interventions (seminars, academic detailing); regular performance feedback; patient self-management strategies; and hospital-community integration (discharge referral summaries; community pharmacist liaison; patient prompts to attend GPs). Using a before-after study design to assess program impact, significantly more program patients compared with historical controls received: ACS: Angiotensin-converting enzyme (ACE) inhibitors and lipid-lowering agents at discharge, aspirin and beta-blockers at 3 months after discharge, inpatient cardiac counselling, and referral to outpatient cardiac rehabilitation. CHF. Assessment for reversible precipitants, use of prophylaxis for deep-venous thrombosis, beta-blockers at discharge, ACE inhibitors at 6 months after discharge, imaging of left ventricular function, and optimal management of blood pressure levels. Risk-adjusted mortality rates at 6 and 12 months decreased, respectively, from 9.8% to 7.4% (P=0.06) and from 13.4% to 10.1% (P= 0.06) for patients with ACS and from 22.8% to 15.2% (P < 0.001) and from 32.8% to 22.4% (P= 0.005) for patients with CHF. Quality improvement programs that feature multifaceted interventions across the continuum of care can change clinical culture, optimise care and improve clinical outcomes.

Cervical cancer is caused by infection with a range of high risk oncogenic human papillomavirus (HPV) types, and it is now accepted that >99% of cervical cancer is initiated by HPV infection. The estimated lifetime risk of cervical cancer is nevertheless relatively low (less than I in 20 for most community based studies). Although sensitivity and specificity of the available diagnostic techniques are suboptimal, Screening for persistent HPV infection is effective in reducing the incidence of cervical cancer. Infection can be detected by molecular techniques or by cytological examination of exfoliated cervical cells. Persistent infection is the single best predictor of risk of cervical cancer.(1) The latest findings of HPV and cervical cancer research need to be widely disseminated to the scientific and medical societies that are updating screening and management protocols, public health professionals, and to women and clinicians. This report reviews current evidence, clinical implications and directions for further research in the prevention, control and management of cervical cancer. We report the conclusions of the Experts' Meeting at the EUROGIN 2003 conference. (C) 2003 Wiley-Liss, Inc.
