O indicador Internações por Condições Sensíveis à Atenção Primária é adotado no Brasil para avaliação da atenção básica. Considerando a sua recente adoção, este estudo tem como objetivo apresentar o panorama dessas internações em um hospital do município de São Paulo. Foi realizado um estudo ecológico exploratório, tendo como fontes o Sistema de Informação Hospitalar do Sistema Único de Saúde e uma amostra de prontuários de pacientes internados neste hospital. Para análise, foi utilizada a estatística descritiva. As Internações por Condições Sensíveis à Atenção Primária seguem tendência de redução, sendo as pneumonias bacterianas as que mais internaram no período; maior frequência para a faixa etária de 65 anos de idade e mais, e para o sexo feminino. Internações por Condições Sensíveis, somente, não são suficientes para avaliação da atenção básica, mas permite avaliar a organização da rede de saúde, que deve assegurar continuidade do cuidado em busca do princípio da integralidade.

Background: Aggregatibacter actinomycetemcomitans serotypes are clearly associated with periodontitis or health, which suggests distinct strategies for survival within the host. Objective: We investigated the transcription profile of virulence-associated genes in A. actinomycetemcomitans serotype b (JP2 and SUNY 465) strains associated with disease and serotype a (ATCC 29523) strain associated with health. Design: Bacteria were co-cultured with immortalized gingival epithelial cells (OBA-9). The adhesion efficiency after 2 hours and the relative transcription of 13 genes were evaluated after 2 and 24 hours of interaction. Results: All strains were able to adhere to OBA-9, and this contact induced transcription of pgA for polysaccharide biosynthesis in all tested strains. Genes encoding virulence factors as Omp29, Omp100, leukotoxin, and CagE (apoptotic protein) were more transcribed by serotype b strains than by serotype a. ltxA and omp29, encoding the leukotoxin and the highly antigenic Omp29, were induced in serotype b by interaction with epithelial cells. Factors related to colonization (aae, flp, apaH, and pgA) and cdtB were upregulated in serotype a strain after prolonged interaction with OBA-9. Conclusion: Genes relevant for surface colonization and interaction with the immune system are regulated differently among the strains, which may help explaining their differences in association with disease.

INTRODUCTION: Choosing the antimicrobial agent for initial therapy of urinary tract infection (UTI) is usually empirical and should consider the prevalence of uropathogens in different age groups and gender. OBJECTIVE: To establish prevalence rates of uropathogens in community-acquired UTI in relation to age and gender. METHODS: Crosssectional study conducted in the emergency department (ED) of a general hospital, from January to December, 2010, in patients younger than 15 years old who had clinical suspicion of UTI and collected quantitative urine culture. UTI was defined as urine culture with growth of a single agent > 100.000 colony forming units (cfu)/mL in a midstream collection or > 50.000 cfu/mL in urethral catheterization. RESULTS: There were 63.464 visits to ED. 2577 urine cultures were obtained, of whom 291 were positive for UTI (prevalence = 11.3% of clinical suspicion and 0.46% of visits), 212 cases (72.8%) in females, median age = 2.6 years. The predominant uropathogen was E. coli (76.6%), followed by Proteus mirabilis (10.3%) and Staphylococcus saprophyticus (4.1%). Among infants < 3 months, prevalence rates of E. coli were significantly lower (50% vs 78.4%; OR = 0.276; p = 0.006). Higher prevalences of Staphylococcus saprophyticus occurred among patients > 10 years (24.4% vs 0.4%; OR = 79.265; p < 0.0001). Proteus mirabilis was significantly more prevalent in boys than girls (24.0% vs 5.2%; OR = 5.786; p < 0.001). CONCLUSIONS: E. coli was the most prevalent community-acquired uropathogen. Nevertheless, initial empiric antimicrobial treatment of UTI should consider the significant prevalence of other agents different from E. coli in infants < 3 months, the high prevalence of Staphylococcus saprophyticus in patients > 10 years and Proteus mirabilis in males.

INTRODUÇÃO Cerca de 20% das crianças menores de 36 meses trazidas ao setor de emergência por febre apresentam febre sem sinais localizatórios (FSSL). Nos últimos anos, vários estudos estabeleceram critérios para a identificação dos pacientes com risco de infecção bacteriana grave (IBG), além de demonstrarem a aplicabilidade da pesquisa de vírus respiratórios (PVR) na avaliação destes pacientes. Entretanto, não existem estudos abordando a identificação de vírus respiratórios em crianças menores de 36 meses com FSSL no nosso meio. OBJETIVOS Descrever a frequência dos vírus respiratórios na febre sem sinais localizatórios (FSSL) em crianças menores de 36 meses de idade CASUÍSTICA E MÉTODOS Estudo prospectivo e observacional de crianças menores de 36 meses no setor de emergência pediátrico, durante o período de abril de 2011 a abril de 2013, com diagnóstico de FSSL. Foram excluídas crianças portadoras de doenças de base que implicavam em alteração da imunidade e uso de antibiótico até 14 dias antes da consulta. Os pacientes foram avaliados laboratorialmente de acordo com o protocolo institucional para avaliação de FSSL. Além de hemograma completo, análise do sedimento urinário e culturas de sangue e urina, foi coletada amostra de secreção de nasofaringe para pesquisa de vírus respiratórios por imunoflourescência indireta. Resultados Foram estudados 232 crianças menores de 36 meses com diagnóstico de FSSL, sendo 53% do sexo masculino, com idade mediana de 10,7 meses (5,4 - 16,1). Em 57 (24,6%) destes pacientes, foi identificado vírus respiratórios, sendo o adenovírus (33 casos - 57,9%) o agente mais identificado, seguido do parainfluenza 3 (17,5%) e do influenza A (12,3%). CONCLUSÃO Em torno de 25% dos casos de FSSL do nosso meio foram identificados vírus respiratórios, mostrando que a PVR é uma ferramenta útil na avaliação do paciente com FSSL, possibilitando a redução do número de retornos hospitalares e uso de antibioticoterapia empírica.

Rickettsia rickettsii is an obligate intracellular tick-borne bacterium that causes Rocky Mountain Spotted Fever (RMSF), the most lethal spotted fever rickettsiosis. When an infected starving tick begins blood feeding from a vertebrate host, R. rickettsii is exposed to a temperature elevation and to components in the blood meal. These two environmental stimuli have been previously associated with the reactivation of rickettsial virulence in ticks, but the factors responsible for this phenotype conversion have not been completely elucidated. Using customized oligonucleotide microarrays and high-throughput microfluidic qRT-PCR, we analyzed the effects of a 10 degrees C temperature elevation and of a blood meal on the transcriptional profile of R. rickettsii infecting the tick Amblyomma aureolatum. This is the first study of the transcriptome of a bacterium in the genus Rickettsia infecting a natural tick vector. Although both stimuli significantly increased bacterial load, blood feeding had a greater effect, modulating five-fold more genes than the temperature upshift. Certain components of the Type IV Secretion System (T4SS) were up-regulated by blood feeding. This suggests that this important bacterial transport system may be utilized to secrete effectors during the tick vector's blood meal. Blood feeding also up-regulated the expression of antioxidant enzymes, which might correspond to an attempt by R. rickettsii to protect itself against the deleterious effects of free radicals produced by fed ticks. The modulated genes identified in this study, including those encoding hypothetical proteins, require further functional analysis and may have potential as future targets for vaccine development.

Ureaplasma diversum in veterinary studies is an undesirable microbe, which may cause infection in bulls and may result in seminal vesiculitis, balanopostitis, and alterations in spermatozoids, whereas in cows, it may cause placentitis, fetal alveolitis, abortion, and birth of weak calves. U. diversum is released through organic secretions, especially semen, preputial and vaginal mucus, conjunctival secretion, and milk. The aim of the present study was to develop a TaqMan probe, highly sensitive and specific quantitative PCR (qPCR) assay for the detection and quantification of U. diversum from genital swabs of bovines. Primers and probes specific to U. diversum 16S rRNA gene were designed. The specificity, detection limit, intra- and inter-assay variability of qPCR to detect this ureaplasma was compared with the results of the conventional PCR assay (cPCR). Swabs of vaginal mucus from 169 cows were tested. The qPCR assay detected as few as 10 copies of U. diversum and was 100-fold more sensitive than the cPCR. No cross-reactivity with other Mollicutes or eubacteria was observed. U. diversum was detected in 79 swabs (46.42%) by qPCR, while using cPCR it was detected in 42 (25%) samples. The difference in cPCR and qPCR ureaplasma detection between healthy and sick animals was not statistically significant. But the U. diversum load in samples from animals with genital disorders was higher than in healthy animals. The qPCR assay developed herein is highly sensitive and specific for the detection and quantification of U. diversum in vaginal bovine samples.

[ES]Las tortugas marinas, al igual que otros vertebrados, son susceptibles de padecer múltiples patologías, muchas de ellas desconocidas hasta épocas recientes por parte de la Ciencia Veterinaria. Algunas de estas patologías ocurren de forma natural, tanto en animales de vida libre como en tortugas mantenidas en cautividad. Pero otras son consecuencia de las actividades Se exponen los diferentes sistemas orgánicos, las principales enfermedades bacterianas, micóticas, víricas y parasitarias que afectan a las tortugas marinas, con especial atención a la fibropapilomatosis. Seguidamente se expondrán los cuadros lesionales consecutivos a la patología medioambiental derivada de actividades humanas: enmallamiento en artes de pesca, ingestión de anzuelos, traumatismos por colisión con embarcaciones, ingestión de restos antropogénicos, crudo y contaminantes químicos.

[EN]One of the main questions in microbial ecology is to understand what are the processes that structure and govern the species composition of communities. Answering to this question implies the need to detect bacterial species in environmental samples or at least to define ecologically meaningful “units of differentiation”. The dynamics and distribution of different broad taxonomic groups that constitute the bacterioplankton (at the phylum or class level) has been widely studied. The different large groups are known to follow different spatial distributions and seasonal cycles, but important variations also occur at lower levels (i.e. species or ecologically distinct populations). In this thesis we aimed to study some of these aspects for an important group of marine bacteria: the phylum Bacteroidetes.

Objetivo: Determinar etiología, manifestaciones clínicas, morbimortalidad y recursos diagnósticos y terapéuticos utilizados en pacientes internados con pancitopenia. Material y métodos: Estudio protocolizado, descriptivo y observacional de 14 meses. Criterios de inclusión: pacientes internados con pancitopenia definida por hemoglobina (Hb) <12 g/dL; plaquetas <150.000/ mL y leucocitos <3.800/mL. Los datos fueron analizados con Epi Info 6.04. Resultados: Se diagnosticaron 54 casos de pancitopenia. Prevalencia: 22/1.000 egresos. Edad media: 48,72 años (DS±20,64); 29,63% fueron > 65 años y 53,70% hombres. Permanencia media: 17,13 días (DS±13,22) vs 7,25 días (DS±5,4) del Servicio (p<0.0001). Charlson medio: 7,16 (DS±2,96) y APACHEII medio: 12 (DS±5,04). El 83,33% (45/54, IC95%70,71-92,08) de las pancitopenias fueron secundarias a compromiso medular, 22 casos (40,74%; IC95%27,57-54,97) postquimioterapia (15 en neoplasias oncohematológicas y 7 en sólidas), 11 (20,37%; IC95%10,63-33,53) por mieloptisis y 4 casos (7,41%; IC95%2,06-17,89) por megaloblastosis. El 16,66% (9/54; IC95%7,92- 29,29) fue secundaria a hiperesplenismo y el 16,66% asociadas a infecciones (3 casos por SIDA). Se realizó estudio de médula ósea en 19 casos (35,18%). El 96,29% (IC95%87,25-99,55) presentó comórbidas. El síndrome de respuesta inflamatoria sistémica (85,19%), síndrome anémico (77,8%) y púrpura (50%) fueron las manifestaciones clínicas más frecuentes. Presentó sepsis el 81,48% (IC95%68,57-90,75) y el 29,63% (IC95%17,98-46,31) hemorragias. El 81,48% tuvo infecciones; el 50% de origen nosocomial y el 65,91% clínicamente documentadas. El 34,09% (IC95%20,49-49,92) tuvo aislamiento microbiológico, con hemocultivos positivos en 29,55%. El 51,85% (IC95%37,84-65,66) desarrolló neutropenia febril (75% postquimioterapia). El 64,81% recibió hemoderivados y factores estimulantes de colonias (G-CSF) el 46,34% (IC95%32,62-60,39). La mortalidad fue mayor a la media global del Servicio (16,66 vs 8,65%)(p=0.07).- Conclusiones: Las pancitopenias en pacientes hospitalizados se caracterizaron por ser secundarias a compromiso medular, hiperesplenismo e infecciones, asociarse a permanencia prolongada, altos índices de comorbilidad, complicaciones infecciosas, y mayor mortalidad que la media global del Servicio.
