Peptide hormones within the secretin-glucagon family are expressed in endocrine cells of the pancreas and gastrointestinal epithelium and in specialized neurons in the brain, and subserve multiple biological functions, including regulation of growth, nutrient intake, and transit within the gut, and digestion, energy absorption, and energy assimilation. Glucagon, glucagon-like peptide-1, glucagon-like peptide-2, glucose-dependent insulinotropic peptide, growth hormone-releasing hormone and secretin are structurally related peptides that exert their actions through unique members of a structurally related G protein-coupled receptor class 2 family. This review discusses advances in our understanding of how these peptides exert their biological activities, with a focus on the biological actions and structural features of the cognate receptors. The receptors have been named after their parent and only physiologically relevant ligand, in line with the recommendations of the International Union of Pharmacology Committee on Receptor Nomenclature and Drug Classification (NC-IUPHAR).

Diet composition, in particular fat intake, has been suggested to be a risk factor for obesity in humans. Several mechanisms may contribute to explain the impact of fat intake on fat gain. One factor may be the low thermogenesis induced by a mixed meal rich in fat. In a group of 11 girls (10.1 +/- 0.3 yr), 6 obese (body mass index, 25.6 +/- 0.6 kg/m(2)), and 5 nonobese (body mass index, 19 +/- 1.6 kg/m(2)), we tested the hypothesis that a mixed meal rich in fat can elicit energy saving compared with an isocaloric and isoproteic meal rich in carbohydrate. The postabsorptive resting energy expenditure and the thermic effect of a meal (TEM) after a low fat (LF; 20% fat, 68% carbohydrate, and 12% protein) or an isocaloric (2500 kJ or 600 Cal) and isoproteic high fat (HF; 48% fat, 40% carbohydrate, and 12% protein) meal were measured by indirect calorimetry. Each girl repeated the test with a different, randomly assigned menu (HF or LF) 1 week after the first test. TEM, expressed as a percentage of energy intake was significantly higher after a LF meal than after a HF meal (6.5 +/- 0.7% vs. 4.3 +/- 0.4%; P < 0.01). The postprandial respiratory quotient (RQ) was significantly higher after a LF meal than after a HF meal (0.86 +/- 0.013 vs. 0.83 +/- 0.014; P < 0.001). The HF low carbohydrate meal induced a significantly lower increase in carbohydrate oxidation than the LF meal (20.3 +/- 6.2 vs. 61.3 +/- 7.8 mg/min; P < 0.001). On the contrary, fat oxidation was significantly higher after a HF meal than after a LF meal (-1.3 +/- 2.4 vs. -15.1 +/- 3.6 mg/min; P < 0.01). However, the postprandial fat storage was 8-fold higher after a HF meal than after a LF meal (17.2 +/- 1.7 vs. 1.9 +/- 1.8 g; P < 0.001). These results suggest that a high fat meal is able to induce lower thermogenesis and a higher positive fat balance than an isocaloric and isoproteic low fat meal. Therefore, diet composition per se must be taken into account among the various risk factors that induce obesity in children.

Resíduos sólidos de curtume e CrCl3 foram aplicados em dois solos, Latossolo Roxo eutrófico (LRe) unidade Ribeirão Preto e Latossolo Vermelho-Amarelo (LVA) unidade Laranja Azeda, que se diferenciaram, dentre outros atributos, pelo teor de manganês facilmente redutível. Os resíduos utilizados foram lodo do efluente de caleiro com concentração 0,06 g kg-1 de crômio (LCL) e um lodo do decantador primário (LCR), contendo 17,4 g kg-1 de crômio, ambos na matéria seca, aplicados em doses correspondentes a 10, 20 e 30 Mg ha-1 e 19, 38 e 57 Mg ha-1 (base seca), respectivamente, de acordo com o teor de nitrogênio total de cada um. O CrCl3 foi aplicado nas doses de 330, 660 e 990 kg ha-1 de Cr, equivalentes às doses do metal aplicadas na forma de lodo (LCR). Realizou-se o experimento em vasos alocados em casa de vegetação (blocos ao acaso), que foram monitorados quanto à formação de Cr6+, aos 1, 6, 14, 28, 54 e 86 dias da instalação. Após o 56° dia de incubação, foi transplantada uma muda de alface (Lactuca sativa L.) para cada vaso, cultivada por um período de trinta dias. A oxidação do Cr3+ a Cr6+ foi verificada apenas para o LRe nos tratamentos que receberam doses crescentes de CrCl3. A formação de Cr6+ teve máximo entre 0,72 e 1,16% do Cr3+ aplicado, após um dia de incubação, decrescendo com o tempo, não sendo detectada a sua presença, para nenhuma das doses, após o 54° dia. A aplicação dos resíduos elevou a condutividade elétrica do extrato de saturação (2:1) de 1,40 a 5,07 Ds m-1 e a RAS de 3,05 a 14,12, afetando o desenvolvimento da alface e causando a morte das plantas nas doses mais altas, sendo tais efeitos mais pronunciados no LVA. A concentração de crômio na parte aérea das plantas aumentou, nem sempre de forma proporcional, com o aumento das doses aplicadas na forma de lodo ou sal, com efeito mais acentuado para o LVA do que para o LRe. A aplicação de resíduos de curtume no experimento, para ambos os solos, mostrou-se mais limitante pelo seu conteúdo de sais do que pela presença de crômio.

Tem sido recomendada a adição de superfosfato simples ou gesso agrícola nas pilhas de estercos e de outros resíduos orgânicos, com a finalidade de reduzir as perdas de amônia por volatilização durante a compostagem. Experimentos, contudo, têm mostrado resultados divergentes, quando avaliam a eficiência desses produtos e as doses em que devem ser aplicados. É provável que variações na acidez residual do gesso e do superfosfato simples possam explicar essas divergências. Para verificar essa hipótese, realizou-se um experimento, utilizando, como aditivos, o gesso agrícola, o superfosfato simples e o superfosfato triplo, cada um com dois valores de acidez residual (respectivamente, 0,13 e 0,20%; 7,02 e 2,36%; 2,38 e 1,86%). Em frascos de vidro de 1,6 litro de capacidade, foram colocados 10 g de substrato orgânico obtido por mistura de quantidades iguais, em massa, de estercos secos e frescos de galinha e de gado. Os aditivos foram misturados ao substrato em doses equivalentes a 0, 50, 100, 150 e 200 kg t-1. A quantidade de amônia perdida por volatilização foi determinada aos 7, 14, 21, 28, 35 e 42 dias, coletando-se o gás em solução de ácido sulfúrico encerrada em pequeno recipiente colocado no interior do frasco e titulando-se o ácido remanescente após o período de exposição à amônia, com solução de NaOH 0,01 mol L-1. As amostras de gesso agrícola estudadas não se mostraram eficientes em reduzir a volatilização de amônia da mistura de estercos. O superfosfato simples diminuiu a volatilização de NH3 em até 4,8 vezes, tendo sido a amostra com maior acidez residual a mais eficiente. O superfosfato triplo foi o aditivo mais eficiente no controle da volatilização de NH3, quando aplicado em doses baixas, igualando-se ao superfosfato simples na maior dose. Aparentemente, a ação dos aditivos deveu-se à formação de compostos amoniacais mais estáveis por meio de reações do gás NH3 com ácidos e, ou, fosfato monocálcico presentes naqueles produtos.

The identification of associations between interleukin-28B (IL-28B) variants and the spontaneous clearance of hepatitis C virus (HCV) raises the issues of causality and the net contribution of host genetics to the trait. To estimate more precisely the net effect of IL-28B genetic variation on HCV clearance, we optimized genotyping and compared the host contributions in multiple- and single-source cohorts to control for viral and demographic effects. The analysis included individuals with chronic or spontaneously cleared HCV infections from a multiple-source cohort (n = 389) and a single-source cohort (n = 71). We performed detailed genotyping in the coding region of IL-28B and searched for copy number variations to identify the genetic variant or haplotype carrying the strongest association with viral clearance. This analysis was used to compare the effects of IL-28B variation in the two cohorts. Haplotypes characterized by carriage of the major alleles at IL-28B single-nucleotide polymorphisms (SNPs) were highly overrepresented in individuals with spontaneous clearance versus those with chronic HCV infections (66.1% versus 38.6%, P = 6 × 10(-9) ). The odds ratios for clearance were 2.1 [95% confidence interval (CI) = 1.6-3.0] and 3.9 (95% CI = 1.5-10.2) in the multiple- and single-source cohorts, respectively. Protective haplotypes were in perfect linkage (r(2) = 1.0) with a nonsynonymous coding variant (rs8103142). Copy number variants were not detected. We identified IL-28B haplotypes highly predictive of spontaneous HCV clearance. The high linkage disequilibrium between IL-28B SNPs indicates that association studies need to be complemented by functional experiments to identify single causal variants. The point estimate for the genetic effect was higher in the single-source cohort, which was used to effectively control for viral diversity, sex, and coinfections and, therefore, offered a precise estimate of the net host genetic contribution.

CONTEXT: A passive knee-extension test has been shown to be a reliable method of assessing hamstring tightness, but this method does not take into account the potential effect of gravity on the tested leg. OBJECTIVE: To compare an original passive knee-extension test with 2 adapted methods including gravity's effect on the lower leg. DESIGN: Repeated measures. SETTING: Laboratory. PARTICIPANTS: 20 young track and field athletes (16.6 ± 1.6 y, 177.6 ± 9.2 cm, 75.9 ± 24.8 kg). INTERVENTION: Each subject was tested in a randomized order with 3 different methods: In the original one (M1), passive knee angle was measured with a standard force of 68.7 N (7 kg) applied proximal to the lateral malleolus. The second (M2) and third (M3) methods took into account the relative lower-leg weight (measured respectively by handheld dynamometer and anthropometrical table) to individualize the force applied to assess passive knee angle. MAIN OUTCOME MEASURES: Passive knee angles measured with video-analysis software. RESULTS: No difference in mean individualized applied force was found between M2 and M3, so the authors assessed passive knee angle only with M2. The mean knee angle was different between M1 and M2 (68.8 ± 12.4 vs 73.1 ± 10.6, P < .001). Knee angles in M1 and M2 were correlated (r = .93, P < .001). CONCLUSIONS: Differences in knee angle were found between the original passive knee-extension test and a method with gravity correction. M2 is an improved version of the original method (M1) since it minimizes the effect of gravity. Therefore, we recommend using it rather than M1.

O efeito da inoculação com fungos micorrízicos arbusculares (FMA), da adubação com fósforo e da esterilização do solo sobre o crescimento de mudas de maracujazeiro amarelo foi avaliado em experimento, realizado em casa de vegetação, inteiramente casualizado em fatorial de 2 × 3 × 4, com três repetições, utilizando potes de 1,6 dm³, sendo: dois tratamentos de solo (esterilizado - SE e não esterilizado - SNE), três tratamentos de P (4, 11 e 30 mg dm-3 de P) e quatro tratamentos de inoculação (Gigaspora albida, Scutellospora heterogama, inóculo misto de G. albida, G. margarita, Acaulospora longula e S. heterogama e o controle não inoculado). Mudas inoculadas e cultivadas em SE apresentaram aumento significativo na altura, diâmetro do caule, número de folhas e de gavinhas a partir de 50 dias da inoculação (d.a.i.), com exceção das inoculadas com S. heterogama, que mostraram respostas tardias (70 d.a.i.). Maiores índices de colonização radicular e densidade de esporos na rizosfera foram registrados em SE. A esterilização do solo afetou a concentração de P na parte aérea das mudas, apenas no tratamento-controle e inoculado com S. heterogama Em geral, a esterilização do solo, a adubação fosfatada e a inoculação com FMA favoreceram o crescimento das mudas de maracujazeiro, sendo registradas interações da inoculação micorrízica, esterilização do solo e adubação fosfatada.

A qualidade física de solos agrícolas pode ser afetada pelo sistema de manejo, sendo a magnitude das alterações dependente do tempo de uso do solo e das condições edafoclimáticas. Neste estudo, avaliou-se o efeito de longo prazo (21 anos) dos sistemas de preparo convencional (PC) e plantio direto (PD) sobre propriedades físicas da camada de 0-0,2 m de um Latossolo Bruno alumínico câmbico (629 g kg-1 de argila), em Guarapuava (PR). Em relação à área sob mata nativa, contígua ao experimento e tomada como referência, o cultivo do solo em PC resultou não só no aumento da densidade global (Ds), na resistência do solo à penetração (RP) e na temperatura do solo, mas também na diminuição do diâmetro médio geométrico dos agregados (DMG). A adoção do sistema PD promoveu uma melhoria nas propriedades físicas do solo em comparação ao PC, evidenciada pela diminuição de 9 % (de 1,08 para 0,99 Mg m-3) na Ds em subsuperfície (0,1-0,2 m), de 13 % (de 27,9 para 24,7 ºC) nas temperatura máximas (15 h), na camada de 0-0,05 m; e pelo aumento de 126 % (de 1,6 para 3,7 mm) no DMG dos agregados na superfície do solo (0-0,05 m), e de 26 % (de 0,38 m³ m-3 para 0,48 m³ m-3) no conteúdo de água volumétrica de 0-0,1 m. Por outro lado, o PC e o PD não se diferenciaram quanto aos seus efeitos na porosidade do solo (total, macro e micro), na condutividade hidráulica saturada, na resistência do solo à penetração e no grau de floculação de argila. O rendimento das culturas de soja (18 safras) e milho (4 safras) foi, respectivamente, 42 e 22 % superior em PD do que em PC, o que, possivelmente, reflete a melhoria na qualidade física do solo.

OBJECTIVES: Representative prevalence data of transmitted drug-resistant HIV-1 are essential to establish accurate guidelines addressing resistance testing and first-line treatments. METHODS: Systematic resistance testing was carried out in individuals in Switzerland with documented HIV-1 seroconversion during 1996-2005 and available samples with RNA > 1000 copies/ml obtained within 1 year of estimated seroconversion. Resistance interpretation used the Stanford list of mutations for surveillance of transmitted drug resistance and the French National Agency for AIDS Research algorithm. RESULTS: Viral sequences from 822 individuals were available. Risk groups were men having sex with men (42%), heterosexual contacts (32%) and intravenous drug users (20%); 30% were infected with non-B subtype viruses. Overall, prevalence of transmitted resistance was 7.7% [95% confidence interval (CI), 5.9-9.5] for any drug, 5.5% (95% CI, 3.9-7.1) for nucleoside reverse transcriptase inhibitors, 1.9% (95% CI, 1.0-2.8) for non-nucleoside reverse transcriptase inhibitors and 2.7% (95% CI, 1.6-3.8) for protease inhibitors. Dual- or triple-class resistance was observed in 2% (95% CI, 0.8-2.5). No significant trend in prevalence of transmitted resistance was observed over years. There were no differences according to ethnicity, risk groups or gender, but prevalence of transmitted resistance was highest among individuals infected with subtype B virus. CONCLUSIONS: The transmission rate of drug-resistant HIV-1 has been stable since 1996, with very rare transmission of dual- or triple-class resistance. These data suggest that transmission of drug resistance in the setting of easy access to antiretroviral treatment can remain stable and be kept at a low level.

Introduction. - Le TBS ou Score Trabéculaire Osseux (TBS, Med- Imaps, France) est un index d'architecture osseuse apportant des informations indépendantes de la densité minérale osseuse (DMO), et calculé par la quantification des variations locales des niveaux de gris à partir d'examen de densitométrie (DXA) lombaire. Dans des études antérieures prospectives et cas-témoins, cet index a été considéré comme associé aux fractures. Nous avons comparé les effets du ranélate de strontium (RanSr) et de l'alendronate (ALN) sur l'architecture vertébrale à l'aide du TBS, chez des femmes ostéoporotiques ménopausées. Patients et méthodes. - Une analyse post hoc a été réalisée sur des DXA (Hologic and GE Lunar Devices) de 79 des 189 femmes incluses dans une étude en double aveugle et double placebo et réparties de façon randomisée entre un groupe à 2 g/jour de RanSr et un groupe à 70 mg/semaine d'ALN pendant 2 ans. Les paramètres de TBS ont été évalués en aveugle par TBS iNsight (v1,9) au niveau vertébral après 12 et 24 mois de traitement. Nous avons appliqué les règles de l'ISCD (International Society for Clinical Densitometry) pour chaque exclusion de vertèbre, de façon indépendante respectivement pour la DMO et le TBS. Des doubles mesures ayant été réalisées initialement, la reproductibilité est exprimée en % CV. Résultats. - Les caractéristiques initiales (moyenne ± DS) étaient identiques entre les groupes en termes d'âge, 69,2 ± 4,4 ans ; d'IMC, 23,8 ± 4,4 kg/m2 ; de T-score L1-L4, - 2,9 ± 0,9 et de TBS, 1,230 ± 0,09. Comme prévu, le coefficient de détermination entre la DMO et le TBS au niveau du rachis était très basse r2 = 0,12. Les reproductibilités brutes étaient respectivement de 1,1 et 1,6 % pour la DMO et le TBS au niveau vertébral. Après 1 et 2 ans, la DMO en L1-L4 a augmenté de façon significative de respectivement 5,6 % et 9 % dans le groupe RanSr et de respectivement 5,2 % et 7,6 % dans le groupe ALN. De même, le TBS au niveau vertébral a augmenté respectivement de 2,3 % (p < 0,001) et de 3,1 % (p < 0,001) dans le groupe RanSr et de 0,5 % (NS) et de 1 % (NS) dans le groupe ALN avec une différence entre groupe significative en faveur du RanSr (p = 0,04 et p = 0,03). Il n'y avait aucune corrélation entre la différence de DMO et de TBS à 1 ou 2 ans. Les deux traitements étaient bien tolérés. Discussion. - Ces résultats sur le TBS confortent des études précédentes qui sont en faveur de l'effet bénéfique du RanSr sur l'architecture osseuse. Conclusion. - Le ranélate de strontium a des effets plus importants que l'alendronate sur le score trabéculaire osseux, indice d'architecture osseuse au niveau vertébral, chez les femmes ayant une ostéoporose post-ménopausique, après 2 ans de traitement.

Limbic encephalitis (LE) with waxing and waning neuropsychiatric manifestations including behavioral, personality, psychiatric, and memory changes can evolve over days to months. Many features of LE show remarkable overlap with the characteristics of mesial-temporal (limbic) status epilepticus (MTLSE or LSE). With LE, these prolonged impaired states are assumed not to be due to ongoing epileptic activity or MTLSE, because scalp EEGs usually show no epileptiform spike-wave activity; cycling behavioral and motor changes are attributed to LE; there may be little immediate improvement with antiepileptic drugs (AEDs); and of course, implanted electrodes are rarely used. Conversely, it is known that in pre-surgical patients with refractory limbic epilepsy, implanted electrodes have revealed limbic seizures that cannot be seen at the scalp. This paper assembles a chain of inferences to advance the proposition that refractory LE might represent LSE more often than is thought, and that implanted electrodes should be considered in some cases. We present two cases that suggest that LE was also LSE, one of which warranted implanted electrodes (case 1).

Object Recent years have been marked by efforts to improve the quality and safety of pedicle screw placement in spinal instrumentation. The aim of the present study is to compare the accuracy of the SpineAssist robot system with conventional fluoroscopy-guided pedicle screw placement. Methods Ninety-five patients suffering from degenerative disease and requiring elective lumbar instrumentation were included in the study. The robot cohort (Group I; 55 patients, 244 screws) consisted of an initial open robot-assisted subgroup (Subgroup IA; 17 patients, 83 screws) and a percutaneous cohort (Subgroup IB, 38 patients, 161 screws). In these groups, pedicle screws were placed under robotic guidance and lateral fluoroscopic control. In the fluoroscopy-guided cohort (Group II; 40 patients, 163 screws) screws were inserted using anatomical landmarks and lateral fluoroscopic guidance. The primary outcome measure was accuracy of screw placement on the Gertzbein-Robbins scale (Grade A to E and R [revised]). Secondary parameters were duration of surgery, blood loss, cumulative morphine, and length of stay. Results In the robot group (Group I), a perfect trajectory (A) was observed in 204 screws (83.6%). The remaining screws were graded B (n = 19 [7.8%]), C (n = 9 [3.7%]), D (n = 4 [1.6%]), E (n = 2 [0.8%]), and R (n = 6 [2.5%]). In the fluoroscopy-guided group (Group II), a completely intrapedicular course graded A was found in 79.8% (n = 130). The remaining screws were graded B (n = 12 [7.4%]), C (n = 10 [6.1%]), D (n = 6 [3.7%]), and E (n = 5 [3.1%]). The comparison of "clinically acceptable" (that is, A and B screws) was neither different between groups (I vs II [p = 0.19]) nor subgroups (Subgroup IA vs IB [p = 0.81]; Subgroup IA vs Group II [p = 0.53]; Subgroup IB vs Group II [p = 0.20]). Blood loss was lower in the robot-assisted group than in the fluoroscopy-guided group, while duration of surgery, length of stay, and cumulative morphine dose were not statistically different. Conclusions Robot-guided pedicle screw placement is a safe and useful tool for assisting spine surgeons in degenerative spine cases. Nonetheless, technical difficulties remain and fluoroscopy backup is advocated.
