Hepatitis B virus (HBV) and hepatitis C virus (HCV) infections account for a substantial proportion of liver diseases worldwide. The aim of this study was to determine the prevalence of HBV and HCV serological markers among children and adolescents and verify the epidemiology of the HBV infection over than a decade of the introduction of vaccination program. Serologic markers to HBsAg, total anti-HBc and anti-HCV had been tested in 393 samples. The seropositivity for HBsAg was 0.76% and for total anti-HBc was 1.02%. Copositivity between HBsAg and total anti-HBc was verified in 0.76% of the analyzed samples. There was no seropositivity for anti-HCV marker. The seroprevalence of HBV infection markers among children and adolescents in the southern Brazilian region is high compared to that reported in other countries. Preventive measures, such as educational activities in addition to the universal childhood HBV vaccination, should be initiated in order to reduce the morbimortality and the economic burden associated with the disease.

Some bat species have adapted to the expanding human population by acquiring the ability to roost in urban buildings, increasing the exposure risk for people and domestic animals, and consequently, the likelihood of transmitting rabies. Three dead bats were found in the yard of a house in an urban area of Jundiaí city in the state of São Paulo in southeast Brazil. Two of the three bats tested positive for rabies, using Fluorescent Antibody and Mouse Inoculation techniques. A large colony of Eptesicus furinalis was found in the house's attic, and of the 119 bats captured, four more tested positive for rabies. The objectives of this study were to report the rabies diagnosis, characterize the isolated virus antigenically and genetically, and study the epidemiology of the colony.

INTRODUCTION: Community-associated methicillin-resistant Staphylococcus aureus (MRSA) has emerged as a pathogen in individuals without traditional risk factors. MATERIAL AND METHODS: MRSA nasal carriage was assessed in individuals consulting at a Primary Health Unit in Brazil. RESULTS: A total of 336 individuals were included: 136 were tested only for MRSA and 200 for any S. aureus. No MRSA was found among the 336 individuals and 23 (11.5%) of 200 were colonized by S. aureus. DISCUSSION: Low prevalence rates have been found in non-hospitalized individuals, but MRSA surveillance should be encouraged to monitor clinical and molecular epidemiology of CA- MRSA.

Until 1999 the endemic cases of Sylvatic Yellow Fever were located in the states of northern, midwestern and pre-Amazon regions. Since then, the disease progressively expanded its territory of occurrence, cases being registered beyond the traditional boundaries of endemism. The São Paulo State is considered to be part of this context, since after decades without registration of autochthonous cases of the disease, it reported, in 2000 and 2008-2009, epizootic occurrence in non-human primates and 30 cases in humans. Facts like these, added to the increase in incidences of serious adverse effects resulting from the Yellow Fever vaccination, have highlighted the importance of defining priority municipalities for vaccination against the disease in the state. Two groups of municipalities, some affected and some non-affected by YF, were compared for environmental variables related to the eco-epidemiology of the disease according to literature. The Multiple Correspondence Analysis (MCA) was used to pinpoint the factor able to differentiate the two groups of municipalities and define the levels of risk. The southeast region of the São Paulo State was considered to be the area with a higher number of municipalities classified as high risk and should be considered a priority for the application of prevention measures against Yellow Fever.

Asymptomatic Plasmodium infection is a new challenge for public health in the American region. The polymerase chain reaction (PCR) is the best method for diagnosing subpatent parasitemias. In endemic areas, blood collection is hampered by geographical distances and deficient transport and storage conditions of the samples. Because DNA extraction from blood collected on filter paper is an efficient method for molecular studies in high parasitemic individuals, we investigated whether the technique could be an alternative for Plasmodium diagnosis among asymptomatic and pauciparasitemic subjects. In this report we compared three different methods (Chelex®-saponin, methanol and TRIS-EDTA) of DNA extraction from blood collected on filter paper from asymptomatic Plasmodium-infected individuals. Polymerase chain reaction assays for detection of Plasmodium species showed the best results when the Chelex®-saponin method was used. Even though the sensitivity of detection was approximately 66% and 31% for P. falciparum and P. vivax, respectively, this method did not show the effectiveness in DNA extraction required for molecular diagnosis of Plasmodium. The development of better methods for extracting DNA from blood collected on filter paper is important for the diagnosis of subpatent malarial infections in remote areas and would contribute to establishing the epidemiology of this form of infection.

Candidemia remains a major cause of morbidity and mortality in the health care environment. The epidemiology of Candida infection is changing, mainly in relation to the number of episodes caused by species C. non-albicans. The overall objective of this study was to evaluate the frequency of yeasts of the genus Candida, in a four-year period, isolated from blood of pediatric patients hospitalized in a public hospital of the city of São Paulo, Brazil. In this period, yeasts from blood of 104 patients were isolated and, the identified species of Candida by phenotypic and genotypic methods were: C. albicans (39/104), C. tropicalis (25/104), C. parapsilosis (23/104), Pichia anomala (6/104), C. guilliermondii (5/104), C. krusei (3/104), C. glabrata (2/104) and C. pararugosa (1/104). During the period of the study, a higher frequency of isolates of C. non-albicans (63.55%) (p = 0.0286) was verified. In this study we verified the increase of the non-albicans species throughout the years (mainly in 2009 and 2010). Thus, considering the peculiarities presented by Candida species, a correct identification of species is recommended to lead to a faster diagnosis and an efficient treatment.

This study describes the development and application of a new PCR assay for the specific detection of pathogenic leptospires and its comparison with a previously reported PCR protocol. New primers were designed for PCR optimization and evaluation in artificially-infected paraffin-embedded tissues. PCR was then applied to post-mortem, paraffin-embedded samples, followed by amplicon sequencing. The PCR was more efficient than the reported protocol, allowing the amplification of expected DNA fragment from the artificially infected samples and from 44% of the post-mortem samples. The sequences of PCR amplicons from different patients showed >99% homology with pathogenic leptospires DNA sequences. The applicability of a highly sensitive and specific tool to screen histological specimens for the detection of pathogenic Leptospira spp. would facilitate a better assessment of the prevalence and epidemiology of leptospirosis, which constitutes a health problem in many countries.

SUMMARY In this study, Leishmaniaspecies were identified by Polymerase Chain Reaction (PCR). The epidemiology of patients suspected of having American Cutaneous Leishmaniasis in the municipality of Assis Brasil, Acre State, located in the Brazil/Peru/Bolivia triborder was also investigated. By PCR, the DNA of Leishmaniawas detected in 100% of the cases (37 samples) and a PCR-Restriction Fragment Length Polymorphism (RFLP) of the hsp 70gene identified the species in 32 samples: Leishmania (Viannia) braziliensis (65.6%) , L. (V.) shawi (28.1%) , L. (V.) guyanensis (3.1%) and mixed infection L. (V.) guyanensis and L. (Leishmania) amazonensis (3.1%)This is the first report of L. (V.) shawiand L. (L.) amazonensis in Acre. The two predominant species were found in patients living in urban and rural areas. Most cases were found in males living in rural areas for at least three years and involved in rural work. This suggests, in most cases, a possible transmission of the disease from a rural/forest source, although some patients had not engaged in activities associated with permanence in forestall areas, which indicate a possible sandflies adaptation to the periurban setting.

The role of rodents in the epidemiology of toxoplasmosis was investigated inLondrina, Paraná State, Brazil. One hundred and eighty-one Rattus rattus and one Mus musculus were caught in 37 places. Blood and tissues were collected and submitted to the indirect fluorescence antibody test (IFAT) and the bioassay. Serum samples from 61 contacting dogs were also collected. Sixteen rats (8.8%) were positive for Toxoplasma gondii, but just two of them were positive by serology and bioassay test. Antibodies were found in nine (4.9%) rats. Tissues of nine rats bioassayed were positive and four isolates were obtained. Restriction fragment length polymorphism (RFLP) analysis was performed using 12 markers (SAG1, SAG2, SAG2-alt, C22-8, C29-2, L358, PK1, BTUB, GRA6, SAG3, Apico, CS3). Genotyping revealed that the four strains isolated from this study have been isolated before in cats and chickens from Brazil. None of the isolates was identified like clonal archetypal T-types I, II, and III. The rats presented lower serologic Toxoplasma gondii prevalence (8.8%) compared to contacting dogs (70.5%).

RESUMO - O aumento da prevalência da diabetes e a baixa adesão ao seu tratamento estão associados a um mau controlo metabólico, desenvolvimento de complicações, aumento dos custos económicos e ineficiência do sistema de saúde. De acordo com o modelo de auto-regulação, o estudo das representações da doença permite predizer os comportamentos dos doentes face a esta patologia. O objectivo do estudo é verificar a existência de relação entre a forma como os diabéticos tipo 2 percepcionam a sua doença, de acordo com o modelo de auto-regulação de Leventhal e colaboradores, e a adesão às actividades de auto-cuidado da diabetes. Consiste num estudo observacional, descritivo, analítico, do tipo transversal, a desenvolver em Cuidados de Saúde Primários. A amostra é constituída por 339 indivíduos diabéticos, de ambos os sexos, com idade igual ou superior a 20 anos e com diagnóstico de diabetes tipo 2 há mais de 6 meses. Trata-se de uma amostra probabilística, aleatória simples, seleccionada numa Unidade de Saúde Familiar (USF). A recolha de dados é realizada através do preenchimento de uma ficha de caracterização socio-demográfica e clínica, a partir do processo clínico electrónico, e através da aplicação de dois questionários de auto-preenchimento. Os resultados são analisados através do Statistical Program for Social Sciences (SPSS) – versão 17.0.

Glaucoma is a multifactorial condition under serious influence of many risk factors. The role of diabetes mellitus (DM) in glaucoma etiology or progression remains inconclusive. Although, the diabetic patients have different healing mechanism comparing to the general population and it has a possible-negative role on surgical outcomes. This review article attempts to analyze the association of both diseases, glaucoma and DM, before and after the surgery. The epidemiological studies, based mainly in population prevalence analyzes, have shown opposite outcomes in time and even in the most recent articles also the association remains inconclusive. On the contrary, the experimental models based on animal induced chronic hyperglycemia have shown an important association of both diseases, explained by common neurodegenerative mechanisms. Diabetic patients have a different wound healing process in the eye viz-a-viz other organs. The healing process is more and it results in lower surgical survival time, higher intraocular pressure (IOP) levels and, therefore, these patients usually need more medication to lower the IOP. Both randomized and nonrandomized retrospective and experimental molecular studies have shown the association between DM and glaucoma. Further studies are needed to get better explanations about outcomes on more recent surgical procedures and with the exponential use of antifibrotics. RESUMO - Actualmente, a diabetes mellitus é a doença crónica que representa o maior desafio aos sistemas de saúde, e é um importante problema de saúde pública, tanto no que concerne ao crescente aumento do número de pessoas afectadas pela doença, incapacidade, morbilidade e mortalidade prematura, como nos custos envolvidos no controlo e tratamento das suas complicações. Nesse sentido, e consciente do grande peso económico e epidemiológico que a diabetes mellitus acarreta, este trabalho versa sobre: (1) identificar a prevalência da diabetes mellitus, em 2010, na região do Alto Alentejo; (2) conhecer as evidências sobre as intervenções e cuidados prestados aos diabéticos desta região; e (3) analisar os custos directos com o internamento hospitalar, cujo diagnóstico principal foi a diabetes, no ano 2010, nesta região. Através deste estudo, constatamos que a diabetes, na região do Alto Alentejo, no universo dos diabéticos identificados (n=8.767), representou uma prevalência de 7,7 diabéticos por 100 habitantes. Quanto ao género, a prevalência é superior no sexo feminino (8,1%), em relação ao sexo masculino (7,2%). A média da idade foi de 67,5 anos, e verificou-se que a diabetes aumenta em grande escala com a idade, encontrando-se uma taxa de prevalência superior em idades acima dos 60 anos. Referente ao seguimento dos cuidados de saúde prestados pela ULS aos doentes com diabetes mellitus, fica muito aquém do que as Guidelines definem, para os cuidados prestados a estes doentes, tanto em termos de seguimento da própria doença, como na prevenção das suas complicações. Em relação ao internamento hospitalar, em 2010, registaram-se 255 episódios cujo diagnóstico principal foi a diabetes mellitus (250.xx), correspondendo estes casos a um total de 761.990€. A demora média de internamento foi de 10 dias (11,5 para o sexo masculino e 8,5 para o sexo feminino) e, a média das idades situa-se nos 65,5 anos.

RESUMO- Introdução: A obesidade e a Síndrome Metabólica (SM) são atualmente um importante problema de saúde pública, com prevalências crescentes, que se acompanham também por aumento da prevalência de Diabetes Mellitus (DM).Estudos prévios demonstram associação destas entidades com o aumento de risco de eventos cardiovasculares, em particular a DM. A SM tem sido uma entidade muito debatida nos últimos anos, com aparecimento de diversas definições, contribuindo para resultados díspares no que diz respeito à influência da SM nas doenças cardiovasculares. Também têm sido descritas variações étnicas e regionais. Para além de alguns estudos epidemiológicos na população geral, a informação relativamente à sua influência na presença de doença cardiovascular é desconhecida em Portugal, em particular em populações com suspeita de doença coronária. Objetivos - Esclarecimento de questões relacionadas com a prevalência de SM e a sua influência na evolução de doença ateroclerótica arterial por avaliação de uma população com suspeita de doença coronária. População e Métodos - Estudo observacional, transversal, com inclusão prospetiva de indivíduos admitidos letivamente para realização de angiografia coronária por suspeita de doença coronária, tendo sido também efetuadas análises laboratoriais e ecografia carotidea para avaliação da espessura intima-média carotidea (EIMc) e da presença de placas carotídeas. Efetuou-se avaliação dos parâmetros demográficos, antropométricos, determinação do perfil lipídico, glicémia e insulinémia. Os exames angiográficos foram analisados por análise quantitativa semi-automática. Foram excluídos indivíduos com antecedentes conhecidos de doença cardíaca. Resultados - Incluíram-se 300 doentes, com idade média de 64 ± 9 anos, 59% do género masculino. A prevalência de SM de acordo com a definição da AHA/NHLBI foi 48,4% (ajustada para idade e género da população portuguesa) e a prevalência de DM foi 14,8% (ajustada). A concordância global das três definições mais recentes de SM foi de apenas 43%. A prevalência de SM aumenta com a idade e é também mais elevada no género feminino. O componente mais frequente foi a hipertensão arterial, seguido pela obesidade abdominal, a elevação da glicémia e por fim as alterações dos triglicéridos e do colesterol HDL. Por outro lado, a presença de doença coronária significativa (lesões ≥50%) ocorreu em apenas 51,3% dos doentes, sendo ainda mais baixa no género feminino. Demonstrou-se também uma baixa capacidade preditiva para doença coronária dos testes não invasivos clássicos, em particular no género feminino. A prevalência de doença coronária significativa foi idêntica nos indivíduos com SM comparativamente com indivíduos sem alterações metabólicas (46,3% vs. 48,2%, respectivamente), sendo mais elevado nos diabéticos (65,2%). Os fatores predizentes independentes de doença coronária significativa foram a idade, o género masculino, a elevação da glicémia e dos triglicéridos. Pelo contrário, o Índice de Massa Corporal (IMC) mostrou uma associação protetora relativamente à presença de doença coronária. A SM não é fator predizente de doença coronária. Relativamente às dimensões dos vasos coronários, o IMC correlaciona-se positivamente e a glicémia / DM correlacionam-se negativamente. A EIMc aumenta com o aumento da idade e no género masculino. A EIMc foi intermédia nos doentes com SM (0,88 ± 0,31 mm) comparativamente com os doentes diabéticos (0,97 ± 0,34 mm) e os indivíduos “Normais” (0,85 ± 0,34 mm). Os fatores predizentes independentes de EIMc foram a idade, o género masculino, o colesterol HDL e a insulinémia. A EIMc permite predizer com uma acuidade moderada a presença de doença coronária significativa (AUC 0,638), em particular no género feminino, sendo um fator predizente independente de presença de doença coronária (OR 2,35, IC 95% 1,04-5,33. p=0,04). Apesar de não se correlacionar com o número de vasos coronários com doença, correlacionou-se com a gravidade da doença (pelo score de Gensini). A insulinémia e o índice HOMA aumentam diretamente com a idade e com o IMC, sendo contudo sobreponíveis em ambos os géneros. Os fatores predizentes de índice HOMA (resistência à insulina) foram o IMC, bem como os restantes componentes de SM, estando o índice HOMA relacionado com a presença de SM e o número dos seus componentes presentes. O limiar para resistência à insulina foi de 2,66 e para SM foi 2,41. Ao contrário das restantes definições de SM, a definição da AHA/NHLBI não é predizente da presença de DM no género masculino. A associação da resistência à insulina com doença coronária foi limiar (OR 1,13, IC 95% 1,00-1,28, p=0,045). Conclusões - Numa população com suspeita de doença coronária, a prevalência de SM é muito elevada (superior a 50%), sendo a prevalência de DM de 23%. Também a obesidade e o excesso de peso foram extremamente prevalentes nesta população. A concordância entre definições de SM é baixa. A hipertensão arterial e a obesidade abdominal são os componentes mais frequentes de SM, sendo menos prevalentes as alterações lipídicas. Pelo contrário, a presença de doença coronária significativa foi muito baixa, em particular nas mulheres. A SM não se associou à presença de doença coronária significativa, estando esta mais dependente das alterações do metabolismo glicídico e dos triglicéridos, bem como de outros fatores de risco não modificáveis, nomeadamente a idade e o género. A EIMc da carótida comum e a presença de placas carotídeas é mais elevada nos indivíduos diabéticos, estando também ligeiramente aumentada nos doentes com SM, sendo os fatores predizentes de EIMc apenas a idade, o género, a hiperinsulinémia bem como os níveis baixos de colesterol HDL. A utilização da avaliação da EIMc na estratificação de risco pré-angiografia coronária, poderá ser útil no género feminino. A hiperinsulinémia e o índice HOMA (índice de resistência à insulina), estão relacionados com o IMC e consequentemente com a presença de obesidade, embora também se correlacione de forma independente com os outros componentes de SM. A resistência à insulina associou-se à presença de SM. Relativamente à capacidade preditiva da coexistência com DM, verificou-se associação com a definição da NCEP-ATP III e da IDF, contudo, a definição da AHA/NHLBI só foi predizente de DMnas mulheres. -------------ABSTRACT - Introduction: Obesity and Metabolic Syndrome (MS) are a major public health problem, with increasing prevalence, that follows the increase in diabetes prevalence. Previous studies showed an association of both entities with increased cardiovascular risk, particularly diabetes. MS has been debated in the last few years, with several definitions and different results when analysed the influence of MS on cardiovascular diseases. There are also some regional and ethnical variations. Beyond general population epidemiological studies, information about the influence on cardiovascular disease in Portugal is unknown, particularly in patients with suspected coronary disease. Objectives- To clarify several questions regarding the prevalence of MS and the influence in arterial atherosclerotic disease by evaluation of a population with suspected coronary artery disease. Population and Methods- Observational, cross-sectional study with prospective inclusion of individuals admitted electively for coronary angiography with suspicion of coronary artery disease. All individuals also performed laboratorial evaluation and carotid ultrasound to evaluate carotid intima-media thickness (cIMT) and carotid plaques. We also evaluated demographic, anthropometric parameters, lipid profile, blood glucose and blood insulin. Angiographic data was obtained by semi-automated quantitation. Individuals with previously known cardiac history were excluded from the study. Results- We included 300 individuals with a mean age of 64 ± 9 years, 59% males. MS prevalence according to AHA/NHLBI definition was 48.4% (adjusted for age and gender of the Portuguese population) and the adjusted prevalence of diabetes was 14.8%. Global agreement between the more recent three definitions of MS was only 43%. MS prevalence increases with age and is also higher in women. The most frequent components were hypertension and abdominal obesity, followed by elevated glucose and triglicerides and low HDL-cholesterol. Significant coronary artery disease (stenosis ≥50%) was present in only 51.3% of patients, being lower in females. Non-invasive tests also had a low predictive capacity, particularly in females. The prevalence of significant coronary disease was identical in patients with MS compared with normal metabolism individuals (46.3% vs. 48.2%, respectively), being higher in diabetics (65.2%). Independent predictive factors for coronary disease were age, male gender, high blood glucose and triglycerides. On the contrary, Body Mass Index (BMI) was a protective factor for coronary disease. MS wasn’t a predictor of coronary disease. BMI showed a positive correlation with coronary vessel diameter and glucose /diabetes had a negative correlation. CIMT increased with age and was higher in males. CIMT was intermediate in patients with MS (0.88 ± 0.31 mm) when compared to diabetic patients (0.97 ± 0.34 mm) and “Normal” individuals (0.85 ± 0.34 mm). Independent predictors for cIMT were age, male gender, HDL-cholesterol and insulin. CIMT had a moderate predictive accuracy for coronary disease (AUC 0,638), particularly in females and is an independent predictor of the presence of significant coronary disease (OR 2.35, 95% CI 1.04-5.33. p=0.04). Although it did not correlate with the number of diseased coronary arteries, it correlated with coronary disease severity by the Gensini score. Insulin and HOMA index increase directly with age and BMI, but were identical in both genders. Predictive factors for HOMA index (insulin resistance) were BMI as well as the other MS components. HOMA index is related to MS and the number of its components. The cut-off for insulin resistance was 2.66 and for MS 2.41. Unlike other MS definitions, AHA/NHLBI definition is not a predictor of diabetes in males. There was a borderline association between insulin resistance and coronary disease (OR 1.13, 95% CI 1.00-1.28, p=0.045). Conclusions - In a population of patients with suspected coronary disease, MS prevalence is extremely high (above 50%) with a diabetes prevalence of 23%. Also obesity and overweight are very prevalent in this population. Global agreement between MS definitions is however low. Hypertension and abdominal obesity are the most frequent components, with a lower prevalence of lipid abnormalities. Coronary disease prevalence was low, particularly in women. MS wasn’t associated with coronary disease. Coronary disease was related to glucose and triglycerides, as well as with other non-modifiable factors such as age and gender. CIMT and carotid plaques are increased in diabetic patients, and also slightly elevated in patients with MS, but cIMT independent predictors were age, male gender, insulin and HDLcholesterol. CIMT can be useful in risk stratification before coronary angiography particularly in women. Elevated insulin and HOMA index (an insulin resistance index) are related with BMI and consequently with obesity, and it was also correlated with other MS components. Insulin resistance was associated with MS. The presence of diabetes was associated with the presence of MS by NCEP-ATP III and IDF definitions; however, AHA/NHLBI definition was only predictive of diabetes in females.

Ticks were collected from vegetation and animals at monthly intervals during one year (1993-1994) in an endemic area of Brazilian spotted fever in the County of Pedreira, State of São Paulo. Six species of ticks were identified Amblyomma cajennense, Amblyomma cooperi, Amblyomma triste, Anocentor nitens, Rhipicephalus sanguineus and Boophilus microplus. Only the first species was sufficiently numerous to permit a quantitative study with seasonal activity, although the distribution and source of capture of other species were observed and are reported. This information is correlated with the epidemiology of tick-borne rickettsiosis.

The prevalence and age distribution of Epstein-Barr virus infection varies in different populations and there is little information about the epidemiology of this infection in Brazil. We studied the prevalence of EBV antibodies in a sample of 283 children and adolescents between 1 and 21 years old. The sample was taken from two neighborhoods in Vitória (capital city of Espirito Santo, Brazil). The São Pedro (SP) neighborhood represented an area with lower socioeconomic status and the Praias (P) neighborhood represented an area with higher SES. Anti-VCA (Virus Capsid Antigen) antibodies were detected by ELISA and anti-EBNA (Epstein-Barr Nuclear Antigen) antibodies were detected by an anti-complement immunofluorescence method, both using commercial kits. The results showed an overall prevalence rates of anti-VCA and anti-EBNA of 71% and 54% respectively. The prevalence for both anti-EBV antibodies was higher and probably the infection occurred earlier in the SP neighborhood. Among the various socioeconomic factors studied only low family income and maternal education level were significantly correlated with a higher frequency of positive serology for anti-VCA. These results demonstrate that there is a high prevalence of EBV antibodies in children and adolescents living in Vitória, that occurs more frequently at a younger age in children from families with low socioeconomic status. In addition, the results demonstrate an intermediate age distribution pattern between those reported in developed and underdeveloped countries.
