China ya no es solo la principal fábrica del mundo, con el consiguiente nivel de consumo de recursos naturales (energía y materiales) y de emisiones de contaminantes, sino que se ha convertido en uno de los principales acreedores mundiales, y como tal, en garante del crecimiento de la mayoría de las economías occidentales. La insostenibilidad del modelo de crecimiento chino, basado en una copia de los países occidentales, se hace más evidente desde la crisis de oferta de petróleo que sacudió al mundo en julio de 2008, con los precios en máximos históricos, y que vino seguida de la crisis financiera en la que todavía nos encontramos. El presente artículo muestra la evolución del consumo de energía en China en los últimos 25 años y presenta algunas posibles causas del mismo, para concluir que es necesario repensar el modelo actual, que incluye las propuestas de salida de la crisis en China y en Occidente. Estas propuestas reiteran en los mismos errores del pasado inmediato, lo que nos puede conducir a una nueva crisis de oferta y a otra financiera, cuyos efectos distributivos a nivel mundial serán más graves que los actuales, a pesar de que puedan implicar una mejora ambiental.

An alternative model for the geodynamic evolution of Southeast Asia is proposed and inserted in a modern plate tectonic model. The reconstruction methodology is based on dynamic plate boundaries, constrained by data such as spreading rates and subduction velocities; in this way it differs from classical continental drift models proposed so far. The different interpretations about the location of the Palaeotethys suture in Thailand are revised, the Tertiary Mae Yuam fault is seen as the emplacement of the suture. East of the suture we identify an Indochina derived terrane for which we keep the name Shan-Thai, formerly used to identify the Cimmerian block present in Southeast Asia, now called Sibumasu. This nomenclatural choice was made on the basis of the geographic location of the terrane (Eastern Shan States in Burma and Central Thailand) and in order not to introduce new confusing terminology. The closure of the Eastern Palaeotethys is related to a southward subduction of the ocean, that triggered the Eastern Neotethys to open as a back-arc, due to the presence of Late Carboniferous-Early Permian arc magmatism in Mergui (Burma) and in the Lhasa block (South Tibet), and to the absence of arc magmatism of the same age East of the suture. In order to explain the presence of Carboniferous-Early Permian and Permo-Triassic volcanic arcs in Cambodia, Upper Triassic magmatism in Eastern Vietnam and Lower Permian-Middle Permian arc volcanites in Western Sumatra, we introduce the Orang Laut terranes concept. These terranes were detached from Indochina and South China during back-arc opening of the Poko-Song Ma system, due to the westward subduction of the Palaeopacific. This also explains the location of the Cathaysian West Sumatra block to the West of the Cimmerian Sibumasu block.

Identification of populations of Bulinus nasutus and B. globosus from East Africa is unreliable using characters of the shell. In this paper, a molecular method of identification is presented for each species based on DNA sequence variation within the mitochondrial cytochrome oxidase subunit I (COI) as detected by a novel multiplexed SNaPshotTM assay. In total, snails from 7 localities from coastal Kenya were typed using this assay and variation within shell morphology was compared to reference material from Zanzibar. Four locations were found to contain B. nasutus and 2 locations were found to contain B. globosus. A mixed population containing both B. nasutus and B. globosus was found at Kinango. Morphometric variation between samples was considerable and UPGMA cluster analysis failed to differentiate species. The multiplex SNaPshotTM assay is an important development for more precise methods of identification of B. africanus group snails. The assay could be further broadened for identification of other snail intermediate host species.

The control of schistosomiasis has been spectacularly successful in terms of controlling endemicity and severity of the disease during the last 50 years. It can be categorized into two stages. From 1955 through 1980, the transmission-control strategy had been widely and successfully carried out. By the end of 1980, the epidemic of schistosomiasis was successfully circumscribed in certain core regions including areas at the middle and low reaches of the Yangtze River and some mountainous areas in Sichuan and Yunnan provinces, where control of schistosomiasis had been demonstrated to be very difficult to be sustained. Therefore, since 1980, schistosomiasis control in China has been modified to employ a stepwise strategy, based on which morbidity control has been given priorities and if possible transmission control has been pursued. However, since snail-ridden areas remain unchanged so far, reinfections occur frequently. This necessitates a maintenance phase to consolidate the achievements in the control of schistosomiasis. In the mean time, we are challenged with some environmental, social and economical changes in terms of controlling schistosomiasis. Successfully controlling schistosomiasis in China is still a long-term task but will be achieved without doubt along with the economic development and the promotion of living and cultural standard of people.

The research examines general practitioners attitudes and how these effect the management of drug misusers within their practice. The methodology is quantitative in approach. The instrument used is a structured postal questionnaire. Participants include all general practitioners within the North East region of Ireland. Anonymity and confidentiality of all respondents are guaranteed. Data was collected over a six week period, under the following headings - attitudes and beliefs, factors influencing treatment, treatment options, training and demographics. Attitudes and beliefs towards drug users were measured using a five point Likert scale ranging from strongly agrees to strongly disagree. The data was analysed with the aid of a computer package, SPSS allowing descriptive statistics to be presented. Results indicate that the majority of respondents are male. There appears to be sympathy towards drug users and that treatment approaches should be holistic. However, there appears to be a major lack of confidence in treating and managing drug misusers. Patient, social and practice factors all influence the decision to the drug misuser. Treatment options are varied, ranging from methadone maintenance to referral for residential treatment. However, a number of respondents offer no treatment for drug misusers. General practitioners do not feel adequately trained in treating and/or managing this client group. Results indicate that improved communication, ongoing education and more research is needed in this area.This resource was contributed by The National Documentation Centre on Drug Use.

BACKGROUND: The incidence of and mortality from alcohol-related conditions, liver disease and hepatocellular cancer (HCC) are increasing in the UK. We compared mortality rates by country of birth to explore potential inequalities and inform clinical and preventive care. DESIGN: Analysis of mortality for people aged 20 years and over using the 2001 Census data and death data from 1999 and 2001-2003. SETTING: England and Wales. MAIN OUTCOME MEASURES: Standardized mortality ratios (SMRs) for alcohol-related deaths and HCC. RESULTS: Mortality from alcohol-related deaths (23 502 deaths) was particularly high for people born in Ireland (SMR for men [M]: 236, 95% confidence interval [CI]: 219-254; SMR for women [F]: 212, 95% CI: 191-235) and Scotland (SMR-M: 187, CI: 173-213; SMR-F 182, CI: 163-205) and men born in India (SMR-M: 161, CI: 144-181). Low alcohol-related mortality was found in women born in other countries and men born in Bangladesh, Middle East, West Africa, Pakistan, China and Hong Kong, and the West Indies. Similar mortality patterns were observed by country of birth for alcoholic liver disease and other liver diseases. Mortality from HCC (8266 deaths) was particularly high for people born in Bangladesh (SMR-M: 523, CI: 380-701; SMR-F: 319, CI: 146-605), China and Hong Kong (SMR-M: 492, CI: 168-667; SMR-F: 323, CI: 184-524), West Africa (SMR-M: 440, CI, 308-609; SMR-F: 319, CI: 165-557) and Pakistan (SMR-M: 216, CI: 113-287; SMR-F: 215, CI: 133-319). CONCLUSIONS: These findings show persistent differences in mortality by country of birth for both alcohol-related and HCC deaths and have important clinical and public health implications. Very large subsidence, with up to 20 km thick sediment layers, is observed in the East Barents Sea basin. Subsidence started in early Paleozoic, accelerated in Permo-Triassic times, finished during the middle Cretaceous, and was followed by moderate uplift in Cenozoic times. The observed gravity signal suggests that the East Barents Sea is at present in isostatic balance and indicates that a mass excess is required in the lithosphere to produce the observed large subsidence. Several origins have been proposed for the mass excess. We use 1-D thermokinematic modeling and 2-D isostatic density models of continental lithosphere to evaluate these competing hypotheses. The crustal density in 2-D thermokinematic models resulting from pressure-, temperature-, and composition-dependent phase change models is computed along transects crossing the East Barents Sea. The results indicate the following. (1) Extension can only explain the observed subsidence provided that a 10 km thick serpentinized mantle lens beneath the basin center is present. We conclude that this is unlikely given that this highly serpentinized layer should be formed below a sedimentary basin with more than 10 km of sediments and crust at least 10 km thick. (2) Phase changes in a compositionally homogeneous crust do not provide enough mass excess to explain the present-day basin geometry. (3) Phase change induced densification of a preexisting lower crustal gabbroic body, interpreted as a mafic magmatic underplate, can explain the basin geometry and observed gravity anomalies. The following model is proposed for the formation of the East Barents Sea basin: (1) Devonian rifting and extension related magmatism resulted in moderate thinning of the crust and a mafic underplate below the central basin area explaining initial late Paleozoic subsidence. (2) East-west shortening during the Permian and Triassic resulted in densification of the previously emplaced mafic underplated body and enhanced subsidence dramatically, explaining the present-day deep basin geometry.

Breastfeeding has important health benefits for both mother and child. Breastfed babies are less likely to report with gastric, respiratory and urinary tract infections and allergic diseases, while they are also less likely to become obese in later childhood. Improving breastfeeding initiation has become a national priority, and a national target has been set ̢?oto deliver an increase of two percentage points per annum in breastfeeding initiation rate, focusing especially on women from disadvantaged areas̢?. Despite improvements in data quality in previous years, it still remains difficult to construct an accurate and reliable picture of variations and trends in breastfeeding in the East Midlands. It is essential that nationally standardised data collection systems are put in place to enable effective and accurate monitoring and evaluation of breastfeeding status both at a local and national level.

These profiles highlight the causes of death that result in the largest reduction in life expectancy using England's mortality experience as the benchmark. They are part of wider programme of work aimed at supporting the regional Health Inequalities Task Group and local organisations in identifying key priorities for tackling health inequalities. This report by Trent Cancer Registry and EMPHO provides important information on variations in the incidence, mortality and survival for the four most common cancers diagnosed in people living in the East Midlands (lung, colorectal, breast and prostate). In particular, it highlights large differences in incidence and mortality rates between different parts of the Region and identifies significant gaps in survival after diagnosis between the most affluent and most deprived areas.
