BACKGROUND: Connexin37 (Cx37) and Cx40 are crucial for endothelial cell-cell communication and homeostasis. Both connexins interact with endothelial nitric oxide synthase (eNOS). The exact contribution of these interactions to the regulation of vascular tone is unknown. RESULTS: Cx37 and Cx40 were expressed in close proximity to eNOS at cell-cell interfaces of mouse aortic endothelial cells. Absence of Cx37 did not affect expression of Cx40 and a 50 % reduction of Cx40 in Cx40(+/-) aortas did not affect the expression of Cx37. However, absence of Cx40 was associated with reduced expression of Cx37. Basal NO release and the sensitivity for ACh were decreased in Cx37(-/-) and Cx40(-/-) aortas but not in Cx40(+/-) aortas. Moreover, ACh-induced release of constricting cyclooxygenase products was present in WT, Cx40(-/-) and Cx40(+/-) aortas but not in Cx37(-/-) aortas. Finally, agonist-induced NO-dependent relaxations and the sensitivity for exogenous NO were not affected by genotype. CONCLUSIONS: Cx37 is more markedly involved in basal NO release, release of cyclooxygenase products and the regulation of the sensitivity for ACh as compared to Cx40.

A proposta deste trabalho foi estudar, prospectivamente, por ressonância magnética (RM), pacientes com diagnóstico de tetralogia de Fallot e avaliar a validade da RM como exame pré e pós-operatório nesses pacientes. Foram estudados, através de seqüências spin-eco e angiografia por RM (angio-RM) com utilização de gadolínio, 20 pacientes com idades entre 1 e 29 anos. Onze pacientes apresentavam a forma clássica da tetralogia de Fallot e nove, a forma extrema (tetralogia de Fallot com atresia pulmonar). O estudo permitiu a avaliação adequada da aorta, possibilitando a análise quanto à presença ou não de dilatação e quanto à posição do arco aórtico em todos os casos. Houve também uma excelente visualização das artérias pulmonares principal, direita e esquerda, possibilitando a avaliação qualitativa quanto à presença de dilatação, hipoplasia, estenose, ou quanto à ausência do vaso. Os resultados obtidos indicam que a RM, incluindo técnicas de angio-RM com meio de contraste, é um método de grande utilidade no estudo pré e pós-operatório de pacientes com tetralogia de Fallot, porque permite a obtenção de informações anatômicas importantes e complementares à ecocardiografia, podendo ser considerada uma alternativa ao cateterismo cardíaco, principalmente na avaliação da anatomia vascular pulmonar.

O objetivo é apresentar uma técnica recente para a avaliação de obstruções arteriais dos membros inferiores por ressonância magnética em um único momento e com a utilização de dose dupla do meio de contraste paramagnético administrado de forma lenta através de bomba injetora. O método baseia-se em um "software" e "hardware" denominado Mobitrak, disponível nos aparelhos de alto campo de ressonância da Philips, que permite a avaliação de uma grande extensão vascular a partir de aquisições de alta resolução, segmentares e contínuas. A seqüência utilizada é um gradiente eco (FFE) que permite a programação de três segmentos simultaneamente, com pequena sobreposição nas intersecções desses segmentos. Essa seqüência dinâmica é obtida em duas fases, uma previamente ao contraste e outra durante a injeção lenta deste, com subtração do sinal dos tecidos adjacentes e reconstruções em 3D. O método apresenta vários benefícios, como: melhor visualização dos segmentos tibiofibulares, estudo de toda a aorta e o membro inferior em uma única visita do paciente, com baixo volume de meio de contraste.

Neste trabalho é descrito um caso de uma paciente de 49 anos de idade que apresentava infecções pulmonares de repetição. Na telerradiografia de tórax observou-se massa no segmento basal posterior do pulmão direito, que na angiotomografia e nas reconstruções em 3D notou-se a presença de um suprimento sanguíneo proveniente da artéria aorta descendente. No exame da peça cirúrgica ratificou-se a presença de seqüestro pulmonar intralobar com cavidade preenchida por muco.

É relatado um caso de uma paciente do sexo feminino, 40 anos de idade, com queixas de disfagia e dor torácica retroesternal há três anos causadas pela presença de artéria subclávia esquerda aberrante retroesofágica com origem em uma dilatação aneurismática (divertículo de Kommerell). O arco aórtico e a aorta torácica descendente estão localizados à direita. O diagnóstico foi estabelecido por meio dos exames de esofagograma, tomografia computadorizada e angiorressonância magnética. A paciente apresentou melhora importante com o tratamento clínico e, atualmente, segue em acompanhamento ambulatorial há um ano.

OBJECTIVES: To determine inter-session and intra/inter-individual variations of the attenuations of aortic blood/myocardium with MDCT in the context of calcium scoring. To evaluate whether these variations are dependent on patients' characteristics. METHODS: Fifty-four volunteers were evaluated with calcium scoring non-enhanced CT. We measured attenuations (inter-individual variation) and standard deviations (SD, intra-individual variation) of the blood in the ascending aorta and of the myocardium of left ventricle. Every volunteer was examined twice to study the inter-session variation. The fat pad thickness at the sternum and noise (SD of air) were measured too. These values were correlated with the measured aortic/ventricular attenuations and their SDs (Pearson). Historically fixed thresholds (90 and 130 HU) were tested against different models based on attenuations of blood/ventricle. RESULTS: The mean attenuation was 46 HU (range, 17-84 HU) with mean SD 23 HU for the blood, and 39 HU (10-82 HU) with mean SD 18 HU for the myocardium. The attenuation/SD of the blood were significantly higher than those of the myocardium (p < 0.01). The inter-session variation was not significant. There was a poor correlation between SD of aortic blood/ventricle with fat thickness/noise. Based on existing models, 90 HU threshold offers a confidence interval of approximately 95% and 130 HU more than 99%. CONCLUSIONS: Historical thresholds offer high confidence intervals for exclusion of aortic blood/myocardium and by the way for detecting calcifications. Nevertheless, considering the large variations of blood/myocardium CT values and the influence of patient's characteristics, a better approach might be an adaptive threshold.

Les importants progrès dans la qualité et la résolution des images obtenues par «absorptiométrie biphotonique à rayons X» ou DXA ont amélioré certaines modalités existantes et favorisé le développement de nouvelles fonctions permettant d'affiner de manière significative la prise en charge de nos patients dans diverses pathologies. On peut par exemple améliorer la prédiction du risque fracturaire par l'analyse indirecte de la micro et de la macroarchitecture osseuse, rechercher les marqueurs de pathologies associées (recherche de fractures vertébrales ou de fractures fémorales atypiques), ou évaluer le statut métabolique par la mesure de la composition corporelle. Avec les appareils DXA les plus performants, on pourra bientôt déterminer l'âge osseux, estimer le risque cardiovasculaire (par la mesure de la calcification de l'aorte abdominale), ou prédire la progression de l'arthrose articulaire et son évolution après la prise en charge chirurgicale dans la routine clinique. The significant progress on the quality and resolution of the images obtained by "Dual X-ray Absorptiometry" or DXA has permitted on one hand to improve some existing features and on the other to develop new ones, significantly refining the care of our patients in various pathologies. For example, by improving the prediction of fracture risk by indirect analysis of micro- and macro-architecture of the bone, by looking for markers of associated bone diseases (research vertebral fractures or atypical femoral fractures), or by assessing the metabolic status by the measurement of body composition. With the best performing DXA devices we will soon be able, in clinical routine, to determine bone age, to estimate cardiovascular risk (by measuring the calcification of the abdominal aorta) or to predict the progression of joint osteoarthritis and its evolution after surgical management.

OBJETIVO: Descrever as indicações, os principais diagnósticos e os achados de imagem nas angiografias por ressonância magnética das artérias renais. MATERIAIS E MÉTODOS: Estudo retrospectivo, no período de 6/12/2001 a 11/3/2004, num total de 56 exames, totalizando 111 artérias renais estudadas. Os exames foram realizados em um equipamento de 1,5 tesla, segundo o protocolo do Serviço. RESULTADOS: Foi demonstrado que 55,4% (n = 31) pacientes eram masculinos e 44,6% (n = 25), femininos. O paciente mais novo tinha 12 anos e o mais velho, 88 anos. De um total de 25 diferentes indicações, a hipertensão arterial sistêmica com 26,7% (n = 15) foi a principal, seguida de dor abdominal e/ou lombar com 12,5% (n = 7), aneurisma da aorta abdominal com 10,7% (n = 6), estenose da artéria renal com 8,9% (n = 5), e outros. No que se refere às imagens, 43 (76,7%) exames tiveram algum tipo de alteração e 13 (23,2%) foram normais. Dentre os que tinham alterações, a maioria se deu no calibre, e dentre elas, as irregularidades parietais, aneurismas e estenoses foram as mais comuns. Na artéria renal direita as alterações mais comuns foram as irregularidades parietais com 17,87% (n = 10), e na artéria renal esquerda foram as estenoses com 25,45% (n = 14). CONCLUSÃO: A angio-RM mostrou-se excelente método no estudo das artérias renais, devido à sua sensibilidade e capacidade multiplanar para avaliar as estruturas vasculares.

Case: A 11 yo girl with Marfan syndrome was referred to cardiac MR (CMR) to measure the size of her thoracic aorta. She had a typical phenotype with arachnodactyly, abnormally long arms, and was tall and slim (156 cm, 28 kg, body mass index 11,5 kg/m2). She complained of no symptoms. Cardiac auscultation revealed a prominent mid-systolic click and an end-systolic murmur at the apex. A recent echocardiogram showed a moderately dilated left ventricle with normal function and a mitral valve prolapse with moderate mitral valve regurgitation. CMR showed a dilatation of the aortic root (38 mm, Z-score 8.9) and a severe prolapse of the mitral valve with regurgitation. The ventricular cavity was moderately dilated (116 ml/m2) and its contraction was hyperdynamic (stroke volume (SV): 97 ml; LVEF 72%, with the LV volumes measured by modified Simpson method from the apex to the mitral annulus). In this patient however, the mitral prolapse was characterized by a severe backward movement of the valve toward the left atrium (LA) in systole and the dyskinetic movement of the atrioventricular plane caused a ventricularisation of a part of the LA in systole (Figure). This resulted in a significant reduction of LVEF: more than ¼ of the apparent SV was displaced backwards into the ventricularized LA volume, reducing the effective LVEF to 51% (effective SV 69ml). Moreover, by flow measurement, the SV across the ascending aorta was 30 ml (cardiac index 2.0 l/min/m2) allowing the calculation of a regurgitant fraction across the mitral valve of 56%, which was diagnostic for a severe mitral valve insufficiency. Conclusion: This case illustrates the phenomenon of a ventricularisation of the LA where the severe prolapse gives the illusion of a higher attachement of the mitral leaflets within the atrial wall. Besides the severe mitral regurgitation, this paradoxical backwards movement of the valve causes an intraventricular unloading during systole reducing the apparent LVEF of 72% to an effective LVEF of only 51%. In addition, forward flow fraction is only 22% after accounting for the regurgitant volume, as well. This combined involvement of the mitral valve could explain the discrepancy between a low output state and an apparently hyperdynamic LV contraction. Due to its ability to precisely measure flows and volumes, CMR is particularly suited to detect this phenomenon and to quantify its impact on the LV pump function.

OBJETIVO: O objetivo deste estudo é avaliar a variabilidade interobservador do método ultrassonográfico para medida da gordura subcutânea, visceral e perirrenal por meio de técnica padronizada. MATERIAIS E MÉTODOS: Foram avaliados 50 pacientes entre novembro de 2006 e janeiro de 2007. A medida da espessura subcutânea foi realizada com transdutor linear de 7,5 MHz posicionado transversalmente a 1 cm acima da cicatriz umbilical. Para a gordura visceral foi utilizado transdutor de 3,5 MHz posicionado 1 cm acima da cicatriz umbilical, considerando-se a medida entre a face interna do músculo reto abdominal e a parede posterior da aorta na linha média do abdome. A gordura perirrenal foi medida no terço médio do rim direito, com transdutor posicionado na linha axilar média. RESULTADOS: A reprodutibilidade interobservador foi analisada por meio do teste t de Student, com significância de 95%. Não houve diferença significativa entre as médias das medidas das gorduras subcutânea, visceral e perirrenal, com p = 0,7141, 0,7286 e 0,6368, respectivamente. As médias encontradas, com seus respectivos desvios-padrão, foram: 2,64 ± 1,37 para a espessura subcutânea, 6,84 ± 2,38 para a espessura visceral e 4,89 ± 2,6 para a espessura perirrenal. CONCLUSÃO: A ultrassonografia apresentou boa reprodutibilidade interobservador para avaliação da gordura abdominal por meio das medidas das espessuras subcutânea, visceral e perirrenal.

Off-pump coronary bypass grafting may decrease the rate of stroke, due to minimal aortic manipulation. For venous grafts, clampless hemostasis when performing the proximal anastomosis can be achieved using the Heartstring device. We describe a technique using a single device to suture two veins to one aortotomy. This technique requires less space and could be advantageous in very short, small, and calcified aortas. In to our experience, this technique is rapid, simple, easy to reproduce, and cost-saving.

The present work was undertaken to investigate, in young healthy volunteers, the relationships between the forward propagation times of arterial pressure waves and the timing of reflected waves observable on the aortic pulse, in the course of rapid changes in body position. 20 young healthy subjects, 10 men, and 10 women, were examined on a tilt table at two different tilt angles, -10° (Head-down) and + 45° (Head-up). In each position, carotid-femoral (Tcf) and carotid-tibial forward propagation times (Tct) were measured with the Complior device. In each position also, the central aortic pressure pulse was recorded with radial tonometry, using the SphygmoCor device and a generalized transfer function, so as to evaluate the timing of reflected waves reaching the aorta in systole (onset of systolic reflected wave, sT1r) and diastole (mean transit time of diastolic reflected wave, dMTT). The position shift from Head-up to Head-down caused a massive increase in both Tct (women from 130 ± 10 to 185 ± 18 msec P < 0.001, men from 136 ± 9 to 204 ± 18 msec P < 0.001) and dMTT (women from 364 ± 35 to 499 ± 33 msec P < 0.001, men from 406 ± 22 to 553 ± 21 msec P < 0.001). Mixed model regression showed that the changes in Tct and dMTT observed between Head-up and Head-down were tightly coupled (regression coefficient 2.1, 95% confidence interval 1.9-2.3, P < 0.001). These results strongly suggest that the diastolic waves observed on central aortic pulses reconstructed from radial tonometric correspond at least in part to reflections generated in the lower limbs.

OBJETIVO: Relatar uma série de casos de endoleaks, com descrição da classificação vigente. MATERIAIS E MÉTODOS: Realizou-se um estudo retrospectivo dos endoleaks diagnosticados em nossa instituição, entre 2005 e 2009. Foram incluídos 20 casos, utilizados para ilustrar os diferentes tipos de endoleaks. RESULTADOS: Setenta por cento dos pacientes eram do sexo masculino. A idade variou entre 43 e 91 anos, média de 76,3 anos. Treze casos foram observados na aorta abdominal infrarrenal, quatro na aorta torácica, dois nas artérias ilíacas e um no território carotídeo. A ultrassonografia foi o método utilizado para o diagnóstico em 3 casos e a tomografia computadorizada, nos outros 17 casos. Classificação: tipo I, 60%; tipo II, 25%; tipo III, 15%. Não foram observados os demais tipos nesta série. CONCLUSÃO: O diagnóstico precoce e a correta classificação são fundamentais para o manejo adequado dos casos de endoleaks, tornando o conhecimento de seus subtipos conceito fundamental na formação do médico especialista em radiologia e diagnóstico por imagem e para o cirurgião vascular.

OBJECTIVE: To evaluate a comprehensive MRI protocol that investigates for cancer, vascular disease, and degenerative/inflammatory disease from the head to the pelvis in less than 40 minutes on a new generation 48-channel 3T system. MATERIALS AND METHODS: All MR studies were performed on a 48-channel 3T MR scanner. A 20-channel head/neck coil, two 18-channel body arrays, and a 32-channel spine array were employed. A total of 4 healthy individuals were studied. The designed protocol included a combination of single-shot T2-weighted sequences, T1-weighted 3D gradient-echo pre- and post-gadolinium. All images were retrospectively evaluated by two radiologists independently for overall image quality. RESULTS: The image quality for cancer was rated as excellent in the liver, pancreas, kidneys, lungs, pelvic organs, and brain, and rated as fair in the colon and breast. For vascular diseases ratings were excellent in the aorta, major branch vessel origins, inferior vena cava, portal and hepatic veins, rated as good in pulmonary arteries, and as poor in the coronary arteries. For degenerative/inflammatory diseases ratings were excellent in the brain, liver and pancreas. The inter-observer agreement was excellent. CONCLUSION: A comprehensive and time efficient screening for important categories of disease processes may be achieved with high quality imaging in a new generation 48-channel 3T system.

Objective To evaluate the diagnostic capacity of abdominal computed tomography in the assessment of hepatic steatosis using the portal phase with a simplified calculation method as compared with the non-contrast-enhanced phase. Materials and Methods In the present study, 150 patients were retrospectively evaluated by means of non-contrast-enhanced and contrast-enhanced computed tomography. One hundred patients had hepatic steatosis and 50 were control subjects. For the diagnosis of hepatic steatosis in the portal phase, the authors considered a result of < 104 HU calculated by the formula [L - 0.3 × (0.75 × P + 0.25 × A)] / 0.7, where L, P and A represent the attenuation of the liver, of the main portal vein and abdominal aorta, respectively. Sensitivity, specificity, positive and negative predictive values were calculated, using non-contrast-enhanced computed tomography as the reference standard. Results The simplified calculation method with portal phase for the diagnosis of hepatic steatosis showed 100% sensitivity, 36% specificity, negative predictive value of 100% and positive predictive value of 75.8%. The rate of false positive results was 64%. False negative results were not observed. Conclusion The portal phase presents an excellent sensitivity in the diagnosis of hepatic steatosis, as compared with the non-contrast-enhanced phase of abdominal computed tomography. However, the method has low specificity.
