Endocarditis prophylaxis following genitourinary or gastrointestinal procedures targets Enterococcus faecalis. Prophylaxis recommendations advocate oral amoxicillin (2 g in the United States and 3 g in the United Kingdom) in moderate-risk patients and intravenous amoxicillin (2 g) or vancomycin (1 g) plus gentamicin in high-risk patients. While ampicillin-resistant (or amoxicillin-resistant) E. faecalis is still rare, there is a concern that these regimens might fail against vancomycin-resistant and/or aminoglycoside-resistant isolates. The present study tested oral linezolid as an alternative. Rats with catheter-induced aortic vegetations were given prophylaxis simulating human pharmacokinetics of oral amoxicillin (2- to 3-g single dose), oral linezolid (600 mg, single or multiple oral doses every 12 h), or intravenous vancomycin (1-g single dose). Rats were then inoculated with the minimum inoculum infecting 90% of the animals (90% infective dose [ID(90)]) or with 10 times the ID(90) of the vancomycin-susceptible E. faecalis strain JH2-2 or the vancomycin-resistant (VanA phenotype) E. faecalis strain UCN41. Amoxicillin was also tested with two additional vancomycin-susceptible E. faecalis strains, 309 and 1209. Animals were sacrificed 3 days later. All the tested bacteria were susceptible to amoxicillin and gentamicin. Single-dose amoxicillin provided 100% protection against all four isolates at both the ID(90) and 10 times the ID(90). In contrast, linezolid required up to four consecutive doses to provide full protection against the vancomycin-resistant isolate. Vancomycin protected only against the vancomycin-susceptible strain. The high efficacy of single-dose oral amoxicillin suggests that this regimen could be used for prophylaxis in both moderate-risk and high-risk patients without additional aminoglycosides. Linezolid appears to be less reliable, at least against the vancomycin-resistant strain.

Este artigo tem como objetivos descrever os processos de desenvolvimento conceitual e metodológico utilizados na criação do instrumento Ferrans and Powers Quality of Life Index (QLI) e disponibilizar a versão genérica atual adaptada para o português, visando ampliar a disseminação do seu uso em diferentes contextos e amostras da população brasileira. O QLI avalia a qualidade de vida de acordo com os níveis de satisfação e de importância atribuídos a quatro dimensões: Saúde/funcionamento, Psicológico/espiritual, Socioeconômico e Família. Aplicações prévias da versão em português (Índice de Qualidade de Vida de Ferrans e Powers) têm demonstrado que o instrumento pode ser utilizado em nosso meio como uma medida válida e confiável de qualidade de vida.

O artigo visa a apresentar o processo de construção e validação do Índice de Qualidade de Vida de Ferrans & Powers - Versão Feridas (IQVFP-VF) para emprego em pessoas com feridas agudas e crônicas de diferentes etiologias. O estudo metodológico desenvolveu-se por meio de procedimentos teóricos, empíricos e analíticos. Os resultados indicaram que o instrumento possui consistência interna e estabilidade satisfatórias; confirmaram a validade de conteúdo, concorrente (com o item sua satisfação) e convergente (com o WHOQOL-breve), bem como sua capacidade de discriminar os indivíduos conforme o número e duração das feridas, intensidade de dor e idade. A análise fatorial confirmatória mostrou que o instrumento manteve-se razoavelmente ajustado ao modelo original. Em conclusão, pode-se considerar que o IQVFP-VF é válido e tem confiabilidade atestada nos aspectos mais importantes da Qualidade de Vida para a população com feridas - geral, saúde e aspectos psicológicos e espirituais.

There are suggestions of an inverse association between folate intake and serum folate levels and the risk of oral cavity and pharyngeal cancers (OPCs), but most studies are limited in sample size, with only few reporting information on the source of dietary folate. Our study aims to investigate the association between folate intake and the risk of OPC within the International Head and Neck Cancer Epidemiology (INHANCE) Consortium. We analyzed pooled individual-level data from ten case-control studies participating in the INHANCE consortium, including 5,127 cases and 13,249 controls. Odds ratios (ORs) and the corresponding 95% confidence intervals (CIs) were estimated for the associations between total folate intake (natural, fortification and supplementation) and natural folate only, and OPC risk. We found an inverse association between total folate intake and overall OPC risk (the adjusted OR for the highest vs. the lowest quintile was 0.65, 95% CI: 0.43-0.99), with a stronger association for oral cavity (OR = 0.57, 95% CI: 0.43-0.75). A similar inverse association, though somewhat weaker, was observed for folate intake from natural sources only in oral cavity cancer (OR = 0.64, 95% CI: 0.45-0.91). The highest OPC risk was observed in heavy alcohol drinkers with low folate intake as compared to never/light drinkers with high folate (OR = 4.05, 95% CI: 3.43-4.79); the attributable proportion (AP) owing to interaction was 11.1% (95% CI: 1.4-20.8%). Lastly, we reported an OR of 2.73 (95% CI:2.34-3.19) for those ever tobacco users with low folate intake, compared with nevere tobacco users and high folate intake (AP of interaction =10.6%, 95% CI: 0.41-20.8%). Our project of a large pool of case-control studies supports a protective effect of total folate intake on OPC risk.

BACKGROUND: In chronic kidney disease (CKD) patients, the intake of calcium-based phosphate binders is associated with a marked progression of coronary artery and aortic calcification, in contrast to patients receiving calcium-free phosphate binders. The aim of this study was to reexamine the role of calcium carbonate in vascular calcification and to analyse its effect on aortic calcification-related gene expression in chronic renal failure (CRF). METHODS: Mice deficient in apolipoprotein E underwent either sham operation or subtotal nephrectomy to create CRF. They were then randomly assigned to one of the three following groups: a control non-CRF group and a CRF group fed on standard diet, and a CRF group fed on calcium carbonate enriched diet, for a period of 8 weeks. Aortic atherosclerotic plaque and calcification were evaluated using quantitative morphologic image processing. Aortic gene and protein expression was examined using immunohistochemistry and Q-PCR methods. RESULTS: Calcium carbonate supplementation was effective in decreasing serum phosphorus but was associated with a higher serum calcium concentration. Compared with standard diet, calcium carbonate enriched diet unexpectedly induced a significant decrease of both plaque (p<0.05) and non-plaque-associated calcification surface (p<0.05) in CRF mice. It also increased osteopontin (OPN) protein expression in atherosclerotic lesion areas of aortic root. There was also a numerical increase in OPN and osteoprotegerin gene expression in total thoracic aorta but the difference did not reach the level of significance. Finally, calcium carbonate did not change the severity of atherosclerotic lesions. CONCLUSION: In this experimental model of CRF, calcium carbonate supplementation did not accelerate but instead decreased vascular calcification. If our observation can be extrapolated to humans, it appears to question the contention that calcium carbonate supplementation, at least when given in moderate amounts, necessarily enhances vascular calcification. It is also compatible with the hypothesis of a preponderant role of phosphorus over that of calcium in promoting vascular calcification in CRF.

Background: Fusarium onychomycoses are weakly responsive or unresponsive to standard onychomycosis treatments with oral terbinafine and itraconazole. Objective: To examine whether the use of terbinafine and itraconazole, which are highly effective in fighting Trichophyton onychomycoses, could be a cause of the high incidence of Fusarium nail infections. Methods: Polymerase chain reaction methods were used to detect both Fusarium spp. and Trichophyton spp. in nails of patients who had either received treatment previously or not. Results: No significant microbiological differences were found between treated and untreated patients. In 24 of 79 cases (30%), Fusarium spp. was detected in samples of patients having had no previous antifungal therapy and when Trichophyton spp. grew in culture. Conclusion: Oral terbinafine and itraconazole treatments do not appear to favor the establishment of Fusarium spp. in onychomycosis. © 2014 S. Karger AG, Basel.

The focus of this review is to highlight the need for improved communication between medical and dental professionals in order to deliver more effective care to patients. The need for communication is increasingly required to capitalise on recent advances in the biological sciences and in medicine for the management of patients with chronic diseases. Improvements in longevity have resulted in populations with increasing special oral-care needs, including those who have cancer of the head and neck, those who are immunocompromised due to HIV/AIDS, advanced age, residence in long-term care facilities or the presence of life-long conditions, and those who are receiving long-term prescription medications for chronic conditions (e.g., anti-hypertensives, anticoagulants, immunosuppressants, antidepressants). These medications can cause adverse reactions in the oral cavity, such as xerostomia and ulceration. Patients with xerostomia are at increased risk of tooth decay, periodontal disease and infection. The ideal management of such individuals should involve the collaborative efforts of physicians, nurses, dentists and dental hygienists, thus optimising treatment and minimising secondary complications deriving from the oral cavity.

Este estudo teve como objetivo propor e validar um índice para identificar famílias em situação de vulnerabilidade a incapacidades e dependência. Adaptou-se o Índice de Desenvolvimento da Família, acrescentando indicadores associados ao surgimento de incapacidade e dependência. Para validação aparente, utilizou-se a técnica Delphi e foram consultados cinco experts no assunto. Foi adotado nível de concordância de 80% entre os juízes. Após duas rodadas de avaliação, foram realizados ajustes quanto à forma e ao conteúdo do instrumento. Itens foram transferidos de componente, outros acrescentados e alguns, excluídos. O Índice resultante é composto por oito domínios, 38 componentes e 103 questões. A contribuição multiprofissional para a construção de um Índice que se propõe a captar a vulnerabilidade física e social das famílias resultou na primeira etapa para o desenvolvimento de uma ferramenta de diagnóstico e intervenção para profissionais de saúde que prestam assistência às famílias na Atenção Básica.

This study investigates the development of fluency in 30 advanced L2 learners of English over a period of 15 months. In order to measure fluency, several temporal variables and hesitation phenomena are analyzed and compared. Oral competence is assessed by means of an oral interview carried out by the learners. Data collection takes place at three different times: before (T1) and after (T2) a six-month period of FI (80 hours) in the home university, and after a three-month SA term (T3). The data is analyzed quantitatively. Developmental gains in fluency are measured for the whole period, adopting a view of complementarity between the two learning contexts. From these results, a group of high fluency speakers is identified. Correlations between fluency gains and individual and contextual variables are executed and a more qualitative analysis is performed for high fluency speakers' performance and behavior. Results show an overall development of students' oral fluency during a period of 15 months favored by the combination of a period of FI at home followed by a 3-months SA.

A alta do paciente da recuperação pós-anestésica (RPA) depende, dentre outros fatores, do retorno à normotermia e do escore alcançado pelo Índice de Aldrete e Kroulik. Sendo assim, o objetivo deste estudo foi verificar a relação entre o Índice de Aldrete e Kroulik e a temperatura corporal dos pacientes. O local de pesquisa foi o Hospital Universitário da Universidade de São Paulo. O cálculo amostral foi determinado por conveniência e foi constituído por 60 pacientes, entre 18 e 60 anos, submetidos à anestesia geral. Foram verificados a temperatura corporal na região timpânica e o Índice de Aldrete e Kroulik do paciente na recepção e alta da recuperação pós-anestésica. Os dados obtidos foram processados pelo pacote estatístico SPSS, com um nível de 5% de significância, e aplicaram-se o teste de Spearman e o teste de Wilcoxon. Conclui-se que não houve correlação significativa entre os dois parâmetros indicativos de alta.
