A dopplervelocimetria colorida é uma modalidade do exame ultra-sonográfico que se expandiu rapidamente em todas as áreas da medicina, em virtude da sua riqueza de informações. A dopplervelocimetria colorida das artérias orbitais tem sido empregada no auxílio diagnóstico de doenças oftalmológicas, bem como para o estudo de doenças específicas como a pré-eclâmpsia. O objetivo deste estudo é descrever a anatomia normal da região orbital e detalhar a técnica de exame dopplervelocimétrico colorido, tendo em vista ser este um exame acessível e reprodutível. Acreditamos que surgirão novas aplicações deste método, que deverá ser inserido na prática diária do radiologista, visando à melhoria da qualidade do diagnóstico e seguimento das doenças oculares e sistêmicas.

Triangular facets, related drainage basins, alluvial fans and subtle scarps reveal the recent activity of the normal Amer fault (NE Spain). The 1427 earthquakes which reached epicentral intensities ranging between VI1 and VI11 have been attributed to the Amer fault. However, the geomorphologic and geologic characteristics of this fault (30 km length) suggest that it might be capable of producing larger earthquakes than those occurred during de 15th century.

La présente recherche traite des défis posés à l'action publique territoriale par la transition énergétique, transition désormais érigée au rang de priorité par les autorités françaises et suisses, comme plus globalement européennes. Elle prend pour cela appui sur une analyse des démarches de planification énergétique territoriale menées entre 2007 et 2014 sur le territoire franco-valdo-genevois (agglomération du « Grand-Genève »). Considérées comme des laboratoires d'expérimentation de la territorialisation des politiques énergétiques, ces démarches sont ici examinées selon une perspective institutionnaliste et pragmatiste visant à mettre lumière les éléments qui interviennent dans la délimitation du champ des possibles en matière d'action publique énergétique et territoriale. Ce positionnement découle des évolutions observées sur le territoire franco-valdo-genevois durant la période d'étude (chapitre 1). Il découle plus précisément du constat de récurrence de certains points de blocage rencontrés aussi bien dans les démarches de planification énergétique elles-mêmes que dans les travaux méthodologiques qui ont pu être réalisés parallèlement à ces démarches, dans le but d'en affiner les outils techniques et organisationnels de mise en oeuvre. Ainsi, le point de départ de la présente recherche est le constat selon lequel on peine tout autant à construire des solutions énergétiques appropriables et réalisables par les acteurs des territoires concernés qu'à reconfigurer les outils de production de ces solutions. De ce constat découle l'intérêt porté aux cadres institutionnels qui régissent ces planifications énergétiques territoriales. Définis comme l'ensemble des repères - formels et informels - qui rendent possibles en même temps qu'ils contraignent les interactions territorialisées entre les acteurs, ces cadres institutionnels sont placés au coeur de la grille de (re)lecture des expériences de planification énergétique territoriale établie au chapitre 2 de la thèse. En référence aux concepts institutionnalistes et pragmatistes sur lesquels elle prend appui, cette grille conduit à appréhender ces expériences comme autant d'enquêtes contribuant, à travers le travail de mobilisation et construction de représentations territoriales auquel elles donnent lieu, à l'équipement sociocognitif d'un champ d'intervention territorial spécifique. Partant de l'hypothèse selon laquelle les potentialités comme les limites associées à l'équipement sociocognitif de ce champ orientent les possibilités d'action collective, la réflexion consiste en une application de cette grille à une trentaine d'expériences de planification énergétique territoriale. Cette application s'effectue en deux temps, correspondant à deux niveaux de lecture de ces démarches. Le premier porte sur les dispositifs organisationnels et les modalités d'interactions entre les cultures d'action qu'elles réunissent (chapitre 3). Le second se concentre davantage sur les supports cognitifs (représentations territoriales) autour desquels se structurent ces interactions (chapitre 4). Présentés dans le dernier chapitre de la thèse (chapitre 5), les enseignements tirés de ce travail de réexamen des démarches franco-valdo-genevoises de planification énergétique territoriale sont de deux ordres. Ils portent d'abord sur les caractéristiques des cadres institutionnels existants, la manière dont ils orientent ces démarches et délimitent les évolutions possibles dans les modes d'action collective et plus particulièrement d'action publique qui y sont associés. Mais ils portent aussi sur les potentiels de changement associés à ces démarches, et sur les pistes envisageables pour mieux valoriser es potentiels, dont l'activation passe par des évolutions profondes des systèmes institutionnels en place. -- In France as in Switzerland, local authorities stand out as leading players of energy transition, a transition that requires an important renewal of public intervention instruments. It is the stakes and the conditions of such a renewal that the present work aims to examine, based on the experiments of territorial energy planning led on the franco-valdo-genevan cross-border territory. Conceived as initiatives of relocation of the energy supply system, these energy planning initiatives are examined through an institutionalist and pragmatic « reading template ». This « reading template » consists of seeing these energy planning initiatives as pragmatist inquiries aiming, through a collective work of cognitive equipment of the territorial franco-valdo-genevan field of intervention, at the reconstruction of the means of coordination between people about their material, organizational and political territory. It opens towards a double reading of the energy planning initiatives. The first one concentrates on the organizational dimension of these inquiries - i.e. on the cultures of action which they gather and the modalities of interaction between them - whereas the second focuses on the cognitive substance which represents the medium of the interactions. This double reading provides insights at various levels. The first one concerns the (cognitive) territorial field of intervention that these energy-planning experiments contribute to draw. A field which, although better and better characterized in its technical dimensions, remains at the same time limited and " deformed " so that it values more the fossil energy systems, from which we want to release ourselves, than the renewable ones, which we would like to replace them with. The second level of teaching concerns the processes of production of territorial knowledge (PPTK) which presides over the demarcation and « equipment » of the territorial field of intervention. Examined through the institutional norms and the culture of action at stake in them, this PPTK turns out to create a sociocognitive "cross-border" area, the kind of area that could shelter the desired reconfigurations...on the condition that they are beforehand correctly "equipped", in cognitive and also in organizational terms. The determining factor for the quality of this equipment is concentrated in the third category of teaching. Starting with the opportunities created by these energy planning experiments concerning the renewal of public intervention instruments, these elements also allow us to take a new look at the urban area project under construction in this cross-border territory, a project th t shows itself closely linked to the energy experiments through a common challenge of territorialisation.

OBJECTIVE: To identify and quantify sources of variability in scores on the speech, spatial, and qualities of hearing scale (SSQ) and its short forms among normal-hearing and hearing-impaired subjects using a French-language version of the SSQ. DESIGN: Multi-regression analyses of SSQ scores were performed using age, gender, years of education, hearing loss, and hearing-loss asymmetry as predictors. Similar analyses were performed for each subscale (Speech, Spatial, and Qualities), for several SSQ short forms, and for differences in subscale scores. STUDY SAMPLE: One hundred normal-hearing subjects (NHS) and 230 hearing-impaired subjects (HIS). RESULTS: Hearing loss in the better ear and hearing-loss asymmetry were the two main predictors of scores on the overall SSQ, the three main subscales, and the SSQ short forms. The greatest difference between the NHS and HIS was observed for the Speech subscale, and the NHS showed scores well below the maximum of 10. An age effect was observed mostly on the Speech subscale items, and the number of years of education had a significant influence on several Spatial and Qualities subscale items. CONCLUSION: Strong similarities between SSQ scores obtained across different populations and languages, and between SSQ and short forms, underline their potential international use.

OBJETIVO: Obter valores da angulação média dos hilos renais em relação ao plano horizontal, traçado sobre músculos eretores da espinha direito e esquerdo, considerando como parâmetro de referência para as medidas das angulações o centro da coluna vertebral lombar. MATERIAIS E MÉTODOS: Foram analisados 250 exames de tomografia computadorizada de abdomes considerados normais de indivíduos de ambos os sexos, sendo 128 masculinos (idade média de 52,45 ± 17,42 anos) e 122 femininos (idade média de 54,39 ± 18,27 anos), totalizando 500 hilos renais estudados. A angulação média de cada hilo renal em relação ao plano horizontal foi obtida, sendo considerados sempre os ângulos agudos. RESULTADOS: O estudo comparativo entre os sexos mostrou que não houve diferença estatisticamente significante em relação aos ângulos agudos dos hilos renais. A análise estatística mostrou, com intervalo de confiança de 95%, para média do ângulo direito os limites de 40,40° e 44,54° e para o hilo renal esquerdo os limites de 39,91° e 43,23°. CONCLUSÃO: Os hilos renais, independentemente do sexo, apresentam angulações semelhantes. Valores angulares maiores terão anomalia de hiper-rotação ou hiper-rotação exagerada e valores menores terão anomalia de rotação incompleta ou rotação invertida.

BACKGROUND: Pancreatic stone protein (PSP) has been identified as a promising sepsis marker in adults, children and neonates. However, data on population-based reference values are lacking. This study aimed to establish age-specific reference values for PSP. METHODS: PSP was determined using a specific ELISA. PSP serum concentrations were determined in 372 healthy subjects including 217 neonates, 94 infants and children up to 16 years, and 61 adults. The adjacent categories method was used to determine which age categories had significantly different PSP concentrations. RESULTS: PSP circulating levels were not gender-dependent and ranged from 1.0 to 99.4 ng/ml with a median of 9.2 ng/ml. PSP increased significantly between the age categories, from a median of 2.6 ng/ml in very preterm newborns, to 6.3 ng/ml in term newborns, to 16.1 ng/ml in older children (p < 0.001). PSP levels were higher on postnatal day three compared to levels measured immediately post delivery (p < 0.001). Paired umbilical artery and umbilical vein samples were strongly correlated (p < 0.001). Simultaneously obtained capillary heel-prick versus venous samples showed a good level of agreement for PSP (Rho 0.89, bias 19 %). CONCLUSIONS: This study provides age-specific normal values that may be used to define cut-offs for future trials on PSP. We demonstrate an age-dependent increase of PSP from birth to childhood.

Renal disorders are an emerging problem in HIV-infected patients. We performed a cross-sectional study of the first 1000 HIV-infected patients attended at our HIV unit who agreed to participate. We determined the frequency of renal alterations and its related risk factors. Summary statistics and logistic regression were applied. The study sample comprised 970 patients with complete data. Most were white (94%) and men (76%). Median (IQR) age was 48 (42–53) years. Hypertension was diagnosed in 19%, dyslipidemia in 27%, and diabetes mellitus in 3%. According to the Chronic Kidney Disease Epidemiology Collaboration (CKD EPI) equation, 29 patients (3%) had an eGFR < 60 ml/min/1.73m2; 18 of them (62%) presented altered albumin/creatinine and protein/creatinine (UPC or UAC) ratios. Of the patients with eGFR> 60mL/min, it was present in 293 (30%), 38 of whom (7.1%) had UPC> 300mg/g. Increased risk of renal abnormalities was correlated with hypertension (OR, 1.821 [95%CI, 1.292;2.564]; p = 0.001), age (OR, 1.015 [95%CI, 1.001;1.030], per one year; p = 0.040), and use of tenofovir disoproxil fumarate (TDF) plus protease inhibitor (PI), (OR, 1.401 [95%CI, 1.078;1.821]; p = 0.012). Current CD4 cell count was a protective factor (OR, 0.9995 [95%CI, 0.9991;0.9999], per one cell; p = 0.035). A considerable proportion of patients presented altered UPC or UAC ratios, despite having an eGFR > 60mL/min. CD4 cell count was a protective factor; age, hypertension, and use of TDF plus PIs were risk factors for renal abnormalities. Based on our results, screen of renal abnormalities should be considered in all HIV-infected patients to detect these alterations early.

There is no agreement about the distinction between pathological, excessive and normal gaming. The present study compared two classifications for defining pathological gaming: the polythetic format (gamers who met at least half of the criteria) and monothetic format (gamers who met all criteria). Associations with mental, health and social issues were examined to assess differences between subgroups of gamers. A representative sample of 5,663 young Swiss men filled in a questionnaire as part of the ongoing Cohort Study on Substance Use Risk Factors (C-SURF). Game use was assessed with the Game Addiction Scale. Mental, social and physical factors (depression, anxiety, aggressiveness, physical and mental health, social and health consequences), gambling and substance use (illicit drug use, alcohol dependence and problematic cannabis use) were also assessed. The results indicated that monothetic gamers shared problems with polythetic gamers, but were even more inclined to mental health issues (depression, anxiety, and aggressiveness) and were more vulnerable to other dependencies like substance use, alcohol dependence or gambling. A second analysis using Latent Class Analysis confirmed the distinction between monothetic and polythetic gamers. These findings support the use of a monothetic format to diagnose pathological gaming and to differentiate it from excessive gaming.
