The main feature of the so called multiproblem families is the persistence along time of a set of problems in various areas of the individual’s functioning in several family members.This research study aims: a) To identify and characterise the major health problems faced by the members of these families; b) To explore the perceived relevance of these problems; c) To explore the perceived effectiveness of health care interventions received by respondents; d) To explore the level of control perceived over these problems.

A systematic study for the production of porous poly(vinylidene fluoride-trifluoroethylene), P(VDF-TrFE), films using solvent evaporation and non-solvent induced phase separation techniques is presented. Processing parameters such as copolymer volume fraction, solvent, preset exposure time to air before immersion, and non-solvent and temperature of the coagulation bath were varied and the corresponding sample morphology, hydrophobicity, thermal and mechanical properties were determined. Film morphologies including homogeneous pore distributions, micropores, microvoids, spherulites and non-porous films were obtained. The morphology variations strongly influence sample hydrophobicity and mechanical properties. All samples crystallize in the electroactive β-phase with a degree of crystallinity around 30 %.

Buildings are one of the major consumers of energy in Europe. This makes them an important target when aiming to reduce the energy consumptions and carbon emissions. The majority of the European building stock has already some decades and so it needs renovation in order to keep its functionality. Taking advantage of these interventions, the energy performance of the buildings may also be improved. In Portugal the renovation techniques, both regarding energy efficiency measures as well as measures for the use of renewable energy sources, are normally planned at the building scale. It is important to explore the possibility of having large scale interventions, has it has been done in other countries, namely at neighbourhood scale with district energy system in order to optimize the results in terms of costs and environmental impact.

Purpose: Higher myopic refractive errors are associated with serious ocular complications that can put visual function at risk. There is respective interest in slowing and if possible stopping myopia progression before it reaches a level associated with increased risk of secondary pathology. The purpose of this report was to review our understanding of the rationale(s) and success of contact lenses (CLs) used to reduce myopia progression. Methods: A review commenced by searching the PubMed database. The inclusion criteria stipulated publications of clinical trials evaluating the efficacy of CLs in regulating myopia progression based on the primary endpoint of changes in axial length measurements and published in peerreviewed journals. Other publications from conference proceedings or patents were exceptionally considered when no peer-review articles were available. Results: The mechanisms that presently support myopia regulation with CLs are based on the change of relative peripheral defocus and changing the foveal image quality signal to potentially interfere with the accommodative system. Ten clinical trials addressing myopia regulation with CLs were reviewed, including corneal refractive therapy (orthokeratology), peripheral gradient lenses, and bifocal (dual-focus) and multifocal lenses. Conclusions: CLs were reported to be well accepted, consistent, and safe methods to address myopia regulation in children. Corneal refractive therapy (orthokeratology) is so far the method with the largest demonstrated efficacy in myopia regulation across different ethnic groups. However, factors such as patient convenience, the degree of initial myopia, and non-CL treatments may also be considered. The combination of different strategies (i.e., central defocus, peripheral defocus, spectral filters, pharmaceutical delivery, and active lens-borne illumination) in a single device will present further testable hypotheses exploring how different mechanisms can reinforce or compete with each other to improve or reduce myopia regulation with CLs.

OBJETIVO: A religião tem um importante papel na vida de indivíduos encarcerados. A saúde mental e a possibilidade de reabilitação parecem ser favorecidas através da religião. Entretanto poucos estudos no Brasil abordaram a saúde mental em população carcerária feminina e a relacionaram com a religiosidade. O objetivo deste estudo é verificar o perfil de saúde mental e a relação entre religião, religiosidade e saúde mental numa amostra de mulheres encarceradas em São Paulo. MÉTODOS: Foram entrevistadas 358 mulheres, detentas da Penitenciária Feminina da Capital (São Paulo-SP). Foi aplicado um questionário amplo que abrangeu dados sociodemográficos e culturais (incluindo a religiosidade), assim como itens sobre saúde mental e perfil criminal. A avaliação psicopatológica foi feita com o General Health Questionnaire (GHQ-12). Análises estatísticas comparando as variáveis com o GHQ-12 foram realizadas. Foi utilizada também a análise de regressão logística multivariada para respostas dicotômicas para as variáveis que possivelmente interagiram com a saúde mental. RESULTADOS: A idade média da amostra foi 30,7 anos ± 8,5. A prevalência de possível caso psiquiátrico foi 26,6%. CONCLUSÕES: Uma maior religiosidade pessoal associou-se a menor freqüência de possível transtorno mental. Não foram encontradas associações entre tipo de crime e saúde mental.

OBJECTIVE: Brazil is the country with the largest community of Japanese descendants in the world, from a migration movement that started in 1908. However, more recently (1988), a movement in the opposite direction began. Many of these descendants went to Japan for work purposes and suffered mental distress. Some of them sought treatment in Japan, while others returned to Brazil to seek treatment. The aim of the present study was to compare the sociodemographic profile and diagnoses of Japanese Brazilian psychiatric outpatients in Japan (remaining group) and in Brazil (returning group). METHOD: All consecutive Japanese Brazilian outpatients who received care from the psychiatric units in Japan and Brazil from April 1997 to April 2000 were compared. The diagnoses were based on ICD-10 and were made by psychiatrists. Sociodemographic data and diagnoses in Brazil and Japan were compared by means of the Chi-Squared Test. RESULTS: The individuals who returned to Brazil were mostly male and unmarried, had lived alone in Japan, had stayed there for short periods and were classified in the schizophrenia group. The individuals who remained in Japan were mostly female and married, were living with family or friends, had stayed there for long periods and were classified in the anxiety group. Logistic regression showed that the most significant factors associated with the returning group were that they had lived alone and stayed for short periods (OR = 0.93 and 40.21, respectively). CONCLUSION: We conclude that living with a family and having a network of friends is very important for mental health in the context evaluated.

OBJETIVO: Avaliar as relações entre as dimensões da vida sociocultural, como rede social de apoio e religião, saúde mental e qualidade de vida (QV) em idosos de um Programa de Saúde da Família (PSF). MÉTODOS: Avaliou-se uma amostra de 82 idosos (> 60 anos), cadastrados em um PSF. Foram descritos o perfil de saúde física e mental, a capacidade funcional e a qualidade de vida, assim como fatores demográficos, socioeconômicos e culturais associados a essas dimensões. Foram utilizados os seguintes instrumentos: Inventário Sociodemográfico, Clínico e de Religiosidade, MINI, WHOQOL-bref e o Índice de Barthel. RESULTADOS: Dos 82 idosos, 47 (57%) eram mulheres e 35 (43%) homens, porquanto 42 (51%) com de idade de 60 a 69 anos e 40 (49%) com idade de 70 anos ou mais. Por meio de modelos multivariados, identificou-se que a presença de transtornos mentais associa-se a morar só ou com apenas uma pessoa. Apresentaram uma pior QV os idosos que não recebem aposentadoria e que fizeram uso de benzodiazepínicos no último ano. A religião revelou-se dimensão importante associada à QV; em todos os domínios da WHOQOL-bref os idosos membros de igrejas evangélicas apresentaram piores escores. CONCLUSÕES: O presente estudo evidencia que idosos com menores escolaridade e renda tendem a ter pior QV e saúde.

OBJECTIVE: Common mental disorders (CMD) have a high impact on interpersonal relationships and quality of life and are potential underlying causes for the development of more serious disorders. Medical students have been indicated as a risk population for the development of CMD. The aim of this study was to determine the frequency of CMD in undergraduate medical students and to identify related factors. METHODS: A cross-sectional study was performed in a sample population of medical students. CMD was identified according to the 20-item Self-Report Questionnaire. RESULTS: Two hundred and twenty-three students completed the questionnaire. The overall prevalence of CMD was 29.6% and its presence was independently associated with sleep disorders, not owning a car, not working and sedentary lifestyle. CONCLUSIONS: These findings indicate a high prevalence of CMD in the sample studied and are important for supporting actions to prevent mental disorders in future doctors and for reflecting on the curricula currently in use in medical schools.

OBJETIVO: O estudo objetivou caracterizar os idosos institucionalizados residentes em três instituições de longa permanência para idosos, quanto a sua capacidade funcional e seu estado mental, e verificar se existe correlação entre ambas. MÉTODOS: Para isso foram utilizados três instrumentos: um formulário de identificação, o Mini-Exame do Estado Mental (MEEM) e o Índice de Barthel. Foram utilizados como tratamento estatístico a Correlação de Person e o Teste da ANOVA. RESULTADOS: A população avaliada consistiu de 115 idosos com idades entre 62 e 104 anos, sendo 40,66% do sexo feminino e 59,13% do sexo masculino. São alfabetizados 49,56% e analfabetos 50,43%, quanto ao estado civil predominaram os solteiros (46,65%) e viúvos (21,8%). Quanto ao estado mental e funcional, houve alta taxa de idosos apresentando déficit cognitivo (76,72%) e de idosos independentes funcionalmente (75,65%). CONCLUSÕES: De acordo com os resultados observa-se que existe correlação significativa entre o Índice de Barthel e o MEEM (r = 0,441; p < 0,000), e também que as variáveis sexo e idade não influenciam nos resultados destes instrumentos. Existe relação estatística significativa (p < 0,0001) entre escolaridade e estado mental e/ou funcional do indivíduo.

OBJETIVO: Os níveis de satisfação e sobrecarga de profissionais em serviços de saúde mental afetam tanto a sua saúde e o bem-estar quanto a qualidade do cuidado prestado aos pacientes. Para avaliar estas dimensões, é necessário ter acesso a escalas de medida válidas e fidedignas. Este artigo visou descrever um estudo da validade de construto de duas escalas de avaliação da satisfação (SATIS-BR) e da sobrecarga (IMPACTO-BR) de profissionais em serviços de saúde mental. MÉTODOS: As escalas foram aplicadas a uma amostra de 172 profissionais, que trabalhavam em seis serviços de psiquiatria de uma cidade de porte médio de São Paulo. A validade de construto foi avaliada a partir do teste da hipótese de que deve haver uma correlação significativa entre dois construtos teoricamente relacionados. Para isso, foram feitas análises estatísticas correlacionais de Spearman entre as escalas de satisfação e sobrecarga, assim como uma comparação de dois subgrupos de sujeitos, com e sem indicativo de estresse, com relação aos seus graus de satisfação e sobrecarga no trabalho. RESULTADOS: Obteve-se correlação significativa negativa esperada entre os escores de satisfação e sobrecarga (r = -0,38). Obteve-se ainda a diferença significativa entre dois grupos de sujeitos, com e sem estresse, com relação ao grau de satisfação e sobrecarga no trabalho, sujeitos com estresse apresentando níveis mais baixos de satisfação no trabalho e grau mais elevado de sobrecarga. CONCLUSÕES: Conclui-se que as escalas analisadas apresentam validade de construto e são adequadas para monitorar a qualidade de serviços de saúde mental. A escala de sobrecarga constituiu um preditor do estresse apresentado pelos trabalhadores em saúde mental.

Cartilage tissue is a complex nonlinear, viscoelastic, anisotropic, and multiphasic material with a very low coefficient of friction, which allows to withstand millions of cycles of joint loading over decades of wear. Upon damage, cartilage tissue has a low self-reparative capacity due to the lack of neural connections, vascularization, and a latent pool of stem/chondroprogenitor cells. Therefore, the healing of articular cartilage defects remains a significant clinical challenge, affecting millions of people worldwide. A plethora of biomaterials have been proposed to fabricate devices for cartilage regeneration, assuming a wide range of forms and structures, such as sponges, hydrogels, capsules, fibers, and microparticles. In common, the fabricated devices were designed taking in consideration that to fully achieve the regeneration of functional cartilage it is mandatory a well-orchestrated interplay of biomechanical properties, unique hierarchical structures, extracellular matrix (ECM), and bioactive factors. In fact, the main challenge in cartilage tissue engineering is to design an engineered device able to mimic the highly organized zonal architecture of articular cartilage, specifically its spatiomechanical properties and ECM composition, while inducing chondrogenesis, either by the proliferation of chondrocytes or by stimulating the chondrogenic differentiation  of stem/chondro-progenitor cells. In this chapter we present the recent advances in the development of innovative and complex biomaterials that fulfill the required structural key elements for cartilage regeneration. In particular, multiphasic, multiscale, multilayered, and hierarchical strategies composed by single or multiple biomaterials combined in a welldefined structure will be addressed. Those strategies include biomimetic scaffolds mimicking the structure of articular cartilage or engineered scaffolds as models of research to fully understand the biological mechanisms that influence the regeneration of cartilage tissue.

In the last few years, many reports have been describing promising biocompatible and biodegradable materials that can mimic in a certain extent the multidimensional hierarchical structure of bone, while are also capable of releasing bioactive agents or drugs in a controlled manner. Despite these great advances, new developments in the design and fabrication technologies are required to address the need to engineer suitable biomimetic materials in order tune cells functions, i.e. enhance cell-biomaterial interactions, and promote cell adhesion, proliferation, and differentiation ability. Scaffolds, hydrogels, fibres and composite materials are the most commonly used as biomimetics for bone tissue engineering. Dynamic systems such as bioreactors have also been attracting great deal of attention as it allows developing a wide range of novel in vitro strategies for the homogeneous coating of scaffolds and prosthesis with ceramics, and production of biomimetic constructs, prior its implantation in the body. Herein, it is overviewed the biomimetic strategies for bone tissue engineering, recent developments and future trends. Conventional and more recent processing methodologies are also described.
