Sociolinguists have documented the substrate influence of various languages on the formation of dialects in numerous ethnic-regional setting throughout the United States. This literature shows that while phonological and grammatical influences from other languages may be instantiated as durable dialect features, lexical phenomena often fade over time as ethnolinguistic communities assimilate with contiguous dialect groups. In preliminary investigations of emerging Miami Latino English, we have observed that lexical forms based on Spanish lexical forms are not only ubiquitous among the speech of the first generation Cuban Americans but also of the second. Examples, observed in field work, casual observation, and studied formally in an experimental context include the following: “get down from the car,” which derives from the Spanish equivalent, bajar del carro instead of “get out of the car”. The translation task administered to thirty-one participants showed a variety lexical phenomena are still maintained at equal or higher frequencies.

This Licentiate Thesis is devoted to the presentation and discussion of some new contributions in applied mathematics directed towards scientific computing in sports engineering. It considers inverse problems of biomechanical simulations with rigid body musculoskeletal systems especially in cross-country skiing. This is a contrast to the main research on cross-country skiing biomechanics, which is based mainly on experimental testing alone. The thesis consists of an introduction and five papers. The introduction motivates the context of the papers and puts them into a more general framework. Two papers (D and E) consider studies of real questions in cross-country skiing, which are modelled and simulated. The results give some interesting indications, concerning these challenging questions, which can be used as a basis for further research. However, the measurements are not accurate enough to give the final answers. Paper C is a simulation study which is more extensive than paper D and E, and is compared to electromyography measurements in the literature. Validation in biomechanical simulations is difficult and reducing mathematical errors is one way of reaching closer to more realistic results. Paper A examines well-posedness for forward dynamics with full muscle dynamics. Moreover, paper B is a technical report which describes the problem formulation and mathematical models and simulation from paper A in more detail. Our new modelling together with the simulations enable new possibilities. This is similar to simulations of applications in other engineering fields, and need in the same way be handled with care in order to achieve reliable results. The results in this thesis indicate that it can be very useful to use mathematical modelling and numerical simulations when describing cross-country skiing biomechanics. Hence, this thesis contributes to the possibility of beginning to use and develop such modelling and simulation techniques also in this context.

Background: In the field of Plastic Reconstructive Surgery the development of new innovative matrices for skin repair is in urgent need. The ideal biomaterial should promote attachment, proliferation and growth of cells. Additionally, it should degrade in an appropriate time period without releasing harmful substances, but not exert a pathological immune response. Spider dragline silk from Nephila spp meets these demands to a large extent. Methodology/Principal Findings: Native spider dragline silk, harvested directly out of Nephila spp spiders, was woven on steel frames. Constructs were sterilized and seeded with fibroblasts. After two weeks of cultivating single fibroblasts, keratinocytes were added to generate a bilayered skin model, consisting of dermis and epidermis equivalents. For the next three weeks, constructs in co-culture were lifted on an originally designed setup for air/liquid interface cultivation. After the culturing period, constructs were embedded in paraffin with an especially developed program for spidersilk to avoid supercontraction. Paraffin cross-sections were stained in Haematoxylin & Eosin (H&E) for microscopic analyses. Conclusion/Significance: Native spider dragline silk woven on steel frames provides a suitable matrix for 3 dimensional skin cell culturing. Both fibroblasts and keratinocytes cell lines adhere to the spider silk fibres and proliferate. Guided by the spider silk fibres, they sprout into the meshes and reach confluence in at most one week. A well-balanced, bilayered cocultivation in two continuously separated strata can be achieved by serum reduction, changing the medium conditions and the cultivation period at the air/liquid interphase. Therefore spider silk appears to be a promising biomaterial for the enhancement of skin regeneration.

The effect of a pre-shipment hypochlorite treatment on botrytis incidence was evaluated in a large number of rose cultivars and under different long-term storage conditions. Application parameters, stability and sources of hypochlorite were investigated. Irrespective of the type of packaging and shipment conditions, roses that received a pre-shipment treatment with 100 to 150 mg/L hypochlorite showed a significantly decreased botrytis incidence compared to non-hypochlorite treated roses. The hypochlorite treatment generally was more effective than a comparable treatment with commercial fungicides. Dipping the flower heads for approximately one second in a hypochlorite solution was more effective than spraying the heads. In few cases minor hypochlorite-induced damage on the petal tips was observed at higher concentrations (>200 mg/L). Apart from the effect on botrytis incidence, the treatment resulted in reduced water loss that may have an additional beneficial effect on the eventual flower quality. It is concluded that, apart from other obvious measures to reduce botrytis incidence (prevention of high humidity at the flower heads) a pre-shipment floral dip in 100 to 150 mg/L hypochlorite from commercial household bleach is an easy and cost effective way to reduce botrytis incidence following long term storage/transportation of roses. © 2015, International Society for Horticultural Science. New psychoactive substances (NPSs) have appeared on the recreational drug market at an unprecedented rate in recent years. Many are not new drugs but failed products of the pharmaceutical industry. The speed and variety of drugs entering the market poses a new complex challenge for the forensic toxicology community. The detection of these substances in biological matrices can be difficult as the exact compounds of interest may not be known. Many NPS are sold under the same brand name and therefore users themselves may not know what substances they have ingested. The majority of analytical methods for the detection of NPSs tend to focus on a specific class of compounds rather than a wide variety. In response to this, a robust and sensitive method was developed for the analysis of various NPS by solid phase extraction (SPE) with gas chromatography mass spectrometry (GCMS). Sample preparation and derivatisation were optimised testing a range of SPE cartridges and derivatising agents, as well as derivatisation incubation time and temperature. The final gas chromatography mass spectrometry method was validated in accordance with SWGTOX 2013 guidelines over a wide concentration range for both blood and urine for 23 and 25 analytes respectively. This included the validation of 8 NBOMe compounds in blood and 10 NBOMe compounds in urine. This GC-MS method was then applied to 8 authentic samples with concentrations compared to those originally identified by NMS laboratories. The rapid influx of NPSs has resulted in the re-analysis of samples and thus, the stability of these substances is crucial information. The stability of mephedrone was investigated, examining the effect that storage temperatures and preservatives had on analyte stability daily for 1 week and then weekly for 10 weeks. Several laboratories identified NPSs use through the cross-reactivity of these substances with existing screening protocols such as ELISA. The application of Immunalysis ketamine, methamphetamine and amphetamine ELISA kits for the detection of NPS was evaluated. The aim of this work was to determine if any cross-reactivity from NPS substances was observed, and to determine whether these existing kits would identify NPS use within biological samples. The cross- reactivity of methoxetamine, 3-MeO-PCE and 3-MeO-PCP for different commercially point of care test (POCT) was also assessed for urine. One of the newest groups of compounds to appear on the NPS market is the NBOMe series. These drugs pose a serious threat to public health due to their high potency, with fatalities already reported in the literature. These compounds are falsely marketed as LSD which increases the chance of adverse effects due to the potency differences between these 2 substances. A liquid chromatography tandem mass spectrometry (LC-MS/MS) method was validated in accordance with SWGTOX 2013 guidelines for the detection for 25B, 25C and 25I-NBOMe in urine and hair. Long-Evans rats were administered 25B-, 25C- and 25I-NBOMe at doses ranging from 30-300 µg/kg over a period of 10 days. Tail flick tests were then carried out on the rats in order to determine whether any analgesic effects were observed as a result of dosing. Rats were also shaved prior to their first dose and reshaved after the 10-day period. Hair was separated by colour (black and white) and analysed using the validated LC-MS/MS method, assessing the impact hair colour has on the incorporation of these drugs. Urine was collected from the rats, analysed using the validated LC-MS/MS method and screened for potential metabolites using both LC-MS/MS and quadrupole time of flight (QToF) instrumentation.

A new type of space debris was recently discovered by Schildknecht in near -geosynchronous orbit (GEO). These objects were later identified as exhibiting properties associated with High Area-to-Mass ratio (HAMR) objects. According to their brightness magnitudes (light curve), high rotation rates and composition properties (albedo, amount of specular and diffuse reflection, colour, etc), it is thought that these objects are multilayer insulation (MLI). Observations have shown that this debris type is very sensitive to environmental disturbances, particularly solar radiation pressure, due to the fact that their shapes are easily deformed leading to changes in the Area-to-Mass ratio (AMR) over time. This thesis proposes a simple effective flexible model of the thin, deformable membrane with two different methods. Firstly, this debris is modelled with Finite Element Analysis (FEA) by using Bernoulli-Euler theory called “Bernoulli model”. The Bernoulli model is constructed with beam elements consisting 2 nodes and each node has six degrees of freedom (DoF). The mass of membrane is distributed in beam elements. Secondly, the debris based on multibody dynamics theory call “Multibody model” is modelled as a series of lump masses, connected through flexible joints, representing the flexibility of the membrane itself. The mass of the membrane, albeit low, is taken into account with lump masses in the joints. The dynamic equations for the masses, including the constraints defined by the connecting rigid rod, are derived using fundamental Newtonian mechanics. The physical properties of both flexible models required by the models (membrane density, reflectivity, composition, etc.), are assumed to be those of multilayer insulation. Both flexible membrane models are then propagated together with classical orbital and attitude equations of motion near GEO region to predict the orbital evolution under the perturbations of solar radiation pressure, Earth’s gravity field, luni-solar gravitational fields and self-shadowing effect. These results are then compared to two rigid body models (cannonball and flat rigid plate). In this investigation, when comparing with a rigid model, the evolutions of orbital elements of the flexible models indicate the difference of inclination and secular eccentricity evolutions, rapid irregular attitude motion and unstable cross-section area due to a deformation over time. Then, the Monte Carlo simulations by varying initial attitude dynamics and deformed angle are investigated and compared with rigid models over 100 days. As the results of the simulations, the different initial conditions provide unique orbital motions, which is significantly different in term of orbital motions of both rigid models. Furthermore, this thesis presents a methodology to determine the material dynamic properties of thin membranes and validates the deformation of the multibody model with real MLI materials. Experiments are performed in a high vacuum chamber (10-4 mbar) replicating space environment. A thin membrane is hinged at one end but free at the other. The free motion experiment, the first experiment, is a free vibration test to determine the damping coefficient and natural frequency of the thin membrane. In this test, the membrane is allowed to fall freely in the chamber with the motion tracked and captured through high velocity video frames. A Kalman filter technique is implemented in the tracking algorithm to reduce noise and increase the tracking accuracy of the oscillating motion. The forced motion experiment, the last test, is performed to determine the deformation characteristics of the object. A high power spotlight (500-2000W) is used to illuminate the MLI and the displacements are measured by means of a high resolution laser sensor. Finite Element Analysis (FEA) and multibody dynamics of the experimental setups are used for the validation of the flexible model by comparing with the experimental results of displacements and natural frequencies.

Poliisin rakenneuudistus Pora III ja Puolustusvoimauudistus 2011–2015 ovat olleet viime vuosien esimerkkejä julkisen sektorin työn tehostamisen vaatimuksista. Sekä Puolustusvoimilla että poliisilla, kahdella turvallisuusalan viranomaistaholla, on lakisääteiset tehtävänsä, jotka tulee työn tehostamisen vaatimuksista huolimatta suorittaa. Turvallisuusala yksityistyy vauhdilla, ja keskustelua käydään siitä, mitä tehtäviä viranomainen hoitaa itse, mitä annetaan kaupallisen toimijan tai järjestöjen hoidettavaksi ja mistä kansalainen vastaa itse jatkossa. Resurssi- ja tehostamisvaatimuksia mietittäessä nousee esille upseereiden kohdalla koko maan puolustus ja turvaaminen. Kyetäänkö tämän tehtävän täyttämiseen mahdollisessa sotatilanteessa enää nykyisellä tai mahdollisesti vähenevällä resursoinnilla? Poliisitoimen osalta vasteajat eri puolilla Suomea puhuttavat, samoin se, miten tehtäviä priorisoidaan hoidettavaksi. Saavatko kansalaiset enää perusoikeuksiinsa kuuluvaa arjen turvallisuutta, jonka vielä tänä päivänä katsotaan kuuluvan valtion perustehtäviin? Viranomaisten pitäisi tutkimukseen valittujen aineistojen sekä lakien perusteella hoitaa tehtävänsä laadukkaasti ja tasa-arvoisesti kaikkialla Suomessa. Nykyiset sisäistä ja ulkoista turvallisuutta käsittelevät asiakirjat, esimerkiksi strategiat, puhuvat laajasta turvallisuuskäsityksestä, sisäisen ja ulkoisen turvallisuuden rajojen hämärtymisestä globalisoitumisen seurauksena ja lisääntyvästä poikkihallinnollisesta yhteistyöstä toimintaa ohjaavana ajattelumallina. Viranomaisyhteisyötä tulisi lisätä osana normaalia toimintaa, samoin yhteistyötä järjestöjen, elinkeinoelämän ja jokaisen kansalaisen kanssa. Valtioneuvoston tulevaisuusselonteossa (2013) peräänkuulutetaan uusia, innovatiivisia tapoja hoitaa sekä valtion että kuntien tehtäviä. Yhteisen toiminnan kohteen eli laajan turvallisuuskäsityksen viitekehyksessä on mahdollisuus pohtia uudenlaista turvallisuusalan viranomaisyhteistyötä, eli etsiä perusteluja upseeri- ja poliisiprofession syvemmälle yhteistyölle – yhteiskehittelylle. Tutkimukseni tavoitteena on herätellä keskustelua siitä, onko yhteisen toiminnan kohteen löytymiselle edellytyksiä. Nähtävissä on, että valtiolle kuuluvia toimintoja tehostetaan jatkossakin. Yksi järkevä tapa tehostamisessa on löytää töiden rajapintoja ja yhdistää resurssit näiden osalta. Jotta toiminta olisi tehokasta, sen pitää olla osa jokapäiväistä toimintaa eikä perustua vain muutamiin yhteistoimintaharjoituksiin tai jo tapahtuneiden poikkeustilanteiden hoitoon. Suurin osa kriisiajan toiminnasta perustuu normaaliolojen toimintaan, jolloin sen lähtökohdat voisivat olla yhteisessä työssä ja alkaa jo koulutuksesta, mikä nostetaan tässä työssä yhtenä mahdollisuutena esille. Koulutuksellinen yhteistyö ja liikkuvuus ovat eurooppalaisen tutkintojen viitekehyksen perusteella mahdollista myös kahden eri hallinnonalan koulutuksessa. Tällaista koulutuksen tehostamista haetaan tällä hetkellä muualla yhteiskunnassa. Mutta halutaanko omasta toiminnasta ja tehtävistä luopua edes osittain ja tehdä yhteistyötä mahdollisesti oman työn hallinnan, vallan tai resurssien menettämisen pelossa? Vai onko kyse vain siitä, että yhteistyön syventämiselle ei ole nähty kovinkaan suurta tarvetta tai hyötyä eikä yhteistyö näytä tuovan mitään uutta ammattikuntien osaamiseen? Tutkimuksen aineisto koostuu valtionhallinnon aineistoista, kuten strategioista, mietinnöistä ja raporteista niin sisäisen kuin ulkoisen turvallisuuden alalta. Aineistona käytetään myös upseeri- (n=71) ja poliisipäällystöopiskelijoille (n=65) suunnattua kyselyä ja kirjoitelmaa tulevaisuuden turvallisuusasiantuntijuudesta vuonna 2030. Lisäksi opiskelijavastauksista tehtyä analyysia syvennetään molempien korkeakoulujen (Maanpuolustuskorkeakoulu ja Poliisiammattikorkeakoulu) rehtoreiden sekä molempien hallinnonalojen (puolustusministeriö sekä sisäministeriö) kansliapäälliköiden haastatteluilla. Aineistojen avulla pyritään herättelemään ajatuksia siitä, voisiko yhteisiä töitä löytyä yhteistyön pohjaksi. Tarkoituksena on perustella, miksi yhteistyötä kannattaa tehdä ja ikään kuin vastata etukäteen vastaväitteisiin, miksi sitä ei voitaisi tehdä. Strategioiden yhteistyön tahtotilaa verrataan muihin strategioiden toimenpide ehdotuksiin ja sitä kautta vielä kyselyaineistoon. Opiskelijakyselyllä haetaan näkemyksiä tulevaisuuden turvallisuusasiantuntijuudesta ja mahdollisesta yhteistyöstä sekä sen painopisteistä. Muilla asiantuntijahaastatteluilla haetaan korkeakoulujen sekä ministeriön tason näkemyksiä opiskelijoiden mielipiteisiin. Opiskelijakyselyn avulla on haluttu selvittää sitä, mitä jo työelämässä olleet mutta vaihteeksi opiskelevat sotatieteiden maisteriopiskelijat Maanpuolustuskorkeakoulussa ja poliisin päällystötutkinnon opiskelijat Poliisiammattikorkeakoulussa ajattelevat turvallisuusalan ja -asiantuntijuuden muutoksesta. Minkälaisena he näkevät oman tulevan työnsä ja yhteistyökentän muiden viranomaisten kanssa? Selvää opiskelijavastausten mukaan on se, että turvallisuus halutaan pitää jatkossakin viranomaisen vastuulla ja välttää viimeiseen asti yksityisen sektorin liiallista vastuuta enempää kuin on pakko. Yhteistyötä halutaan edelleen lisätä, ja erityisesti tämä koskee viranomaisten välistä yhteistyötä. Tutkimus on tietoisesti rajattu koskemaan kahta turvallisuusalan viranomaistoimijaa, ammattikorkeakoulutuksen käyneitä poliiseja ja Puolustusvoimien Maanpuolustuskorkeakoulussa opiskelevia upseereita, joiden tehtävistä ja koulutuksesta on löydettävissä yhteisiä rajapintoja ja yhteistyön alueita. Kiinnostus syventyä valittuun kahteen ammattialaan johtuu myös siitä, että usein esimerkiksi sisäasianhallinnon strategioissa Puolustusvoimat jätetään ulkopuolelle varsinkin normaaliolojen yhteistyötä tarkasteltaessa tai vain yksittäisen maininnan asteelle. Sama huomio on havaittavissa puolustushallinnon strategioista. Tämä nousee esille erityisesti alueellista yhteistyötä tai viranomaisyhteistyötä pohdittaessa. Silti sekä sisäministeriön että puolustusministeriön hallinnonalan strategiat ym. perustuvat laajaan turvallisuuskäsitykseen, ja usein eri ammattikuntia analysoitaessa puhutaan tehtävistä, joita tekevät useat ammattikunnat ja professiot. Puhutaan niin sanotuista harmaista alueista. Ministeriöiden tahtotilassa ja toiminnassa on tutkimukseni mukaan nähtävissä ristiriita. Koulutuksen osalta yhteistyön lisääminen on mahdollista etenkin nyt, kun Poliisiammattikorkeakoulussa peruskoulutus on muuttunut ammattikorkeakoulutasoiseksi ja näin tämän ammattikunnan professioasema koulutuksen näkökulmasta on vahvistunut entisestään. Käsittelenkin tutkimuksessani kahta professiota professiotutkimuksen perinteisiä kriteereitä käyttäen, eli rinnastaessani näitä kahta ammattia. Rinnastettavuus koulujen kesken on tullut mahdolliseksi sekä tutkintojen että osaamisen tarkastelun näkökulmasta. Tämän myötä myös molempia korkeakouluja hyödyttävää yhteistyötä olisi mahdollista miettiä osana muutakin hallinnon tehostamista ja rauhan ajan viranomaistoimintaa.

Background: Ear related diseases are commonly seen in clinics worldwide especially among children. They are associated with significant morbidity and frequent hospital visits. Limited data exists regarding the burden of ear disease among Nigerian children. Objective: To determine the prevalence of ear-related problems among children presenting at the Paediatric and Otorhinolaryngology clinics of the University of Nigeria Teaching Hospital, Enugu. Materials and methods: This was a cross-sectional study conducted at the Paediatric and Otorhinolaryngology Clinics of the University of Nigeria Teaching Hospital Enugu. All children aged 0 and 17 years presenting between 1st June and 31st August 2006 with ear-related problems were enrolled consecutively into the study. Data analysis was by SPSS version 11. Results: Three thousand and twenty-one children were seen during the study period. Out of these, 248 children (8.2%) presented with ear-related problems. Chronic otitis media (30.5%), acute otitis media (29.9%), cerumen auris (11.3%), otitis externa(10.1%), hearing impairment (7.3%) and foreign body in the ear (5.7%) were the most commonly diagnosed ear-related problems. Conclusion: Ear-related problems among children presenting at the UNTH Enugu were not uncommon. However, otitis media was the most commonly diagnosed ailment affecting the ears in children.

Introduction Tuberculosis (TB) is caused by Mycobacterium tuberculosis and is transmitted mainly through aerosolization of infected sputum which puts laboratory workers at risk in spite of the laboratory workers’ risk of infection being at 3 to 9 times higher than the general public. Laboratory safety should therefore be prioritized and optimized to provide sufficient safety to laboratory workers. Objective To assess the safety for the laboratory workers in TB primary microscopy centres in Blantyre urban. Methodology TB primary microscopy centers in Blantyre urban were assessed in aspects of equipment availability, facility layout, and work practice, using a standardized WHO/AFRO ISO 15189 checklist for the developing countries which sets the minimum safety score at ≥80%. Each center was graded according to the score it earned upon assessment. Results Only one (1) microscopy center out nine (9) reached the minimum safety requirement. Four (4) centers were awarded 1 star level, four (4) centers were awarded 2 star level and only one (1) center was awarded 3 star level. Conclusion In Blantyre urban, 89% of the Tuberculosis microscopy centers are failing to provide the minimum safety to the laboratory workers. Government and other stake holders should be committed in addressing the safety challenges of TB microscopy centres in the country to ensure safety for the laboratory workers. Recommendations It is recommended that the study be conducted at the regional or national level for both public and private laboratories in order to have a general picture of safety in Tb microscopy centres possibly across the country.

Like many Americans, African American women often view marriage as an important life goal; however, it is likely that many also view it as one that is out of reach (Dixon, 2009; Wallace, 2014). The present study sought to address this issue by developing a new measure to assess African American women’s experiences in this regard in hopes of shedding light on how it impacts their dating and mating experiences. A total of 251 heterosexual African American women who were either enrolled in college or who had completed some form of postsecondary education completed an online survey assessing a variety of topics including their attitudes and emotions regarding a shortage of marriageable men, their standards to marry, and their willingness to date interracially. Results from this study established preliminary construct validity for a new scale assessing African American women’s attitudes and emotions about the shortage of “marriageable” African American men.

Background and Aim: The prevalence of alcohol use has increased globally. Out-of-school youth are a vulnerable group who might have missed opportunities for learning healthy behaviours in a formal school environment. The purpose of this study was to determine the risk perception, pattern of use, and correlates of alcohol use among out-of-school youth in Lagos, Nigeria. Methods: A cross-sectional study was conducted among 380 out-of-school youth in motor parks in Lagos State, Nigeria, using interviewer administered questionnaires. Results: The lifetime prevalence of alcohol use was 61.1%, while 55.5% were current drinkers. Beer (57.3%) was the most consumed type of alcohol, followed by distilled spirits (29.8%). Using the CAGE scoring system, more than half (57.8%) of the current drinkers had a drinking problem. Almost three quarters (70.1%) had experienced at least one episode of alcohol intoxication within the past month. A considerable number of current drinkers (63.5%) desired to reduce their alcohol intake or stop drinking, while 45.5% had made unsuccessful attempts to do so within the past year. Only 28.9% had received assistance to quit or reduce their drinking and of these less than half (39.3%) received assistance from a professional or healthcare worker. Males were more likely to be current drinkers and to have experienced episodes of alcohol intoxication. Parental and peer drinking were associated with alcohol use but not with intoxication. Conclusions: It is important to design specific programmes to reduce alcohol use among out-of-school youth in these settings.

Objective: To analyze pharmaceutical interventions that have been carried out with the support of an automated system for validation of treatments vs. the traditional method without computer support. Method: The automated program, ALTOMEDICAMENTOS® version 0, has 925 052 data with information regarding approximately 20 000 medicines, analyzing doses, administration routes, number of days with such a treatment, dosing in renal and liver failure, interactions control, similar drugs, and enteral medicines. During eight days, in four different hospitals (high complexity with over 1 000 beds, 400-bed intermediate, geriatric and monographic), the same patients and treatments were analyzed using both systems. Results: 3,490 patients were analyzed, with 42 155 different treatments. 238 interventions were performed using the traditional system (interventions 0.56% / possible interventions) vs. 580 (1.38%) with the automated one. Very significant pharmaceutical interventions were 0.14% vs. 0.46%; significant was 0.38% vs. 0.90%; non-significant was 0.05% vs. 0.01%, respectively. If both systems are simultaneously used, interventions are performed in 1.85% vs. 0.56% with just the traditional system. Using only the traditional model, 30.5% of the possible interventions are detected, whereas without manual review and only the automated one, 84% of the possible interventions are detected. Conclusions: The automated system increases pharmaceutical interventions between 2.43 to 3.64 times. According to the results of this study the traditional validation system needs to be revised relying on automated systems. The automated program works correctly in different hospitals.

Background: Despite improvements in child health, malnutrition still remains one of the main public health challenges in Iran. Objectives: The aim of this study was to compare under nutrition among under-five children with regard to ethnicity in rural area in north Iran. Patients and Methods: In three cross-sectional studies 7575 subjects in three time-periods including 2339 children in 1998, 2749 in 2004, and 2487 in 2013, were evaluated. All under-five-children in 20 out of 118 villages were chosen by random sampling and assessed. Under nutrition was defined as underweight, stunting and wasting lower than -2 SD (Z < -2 SD). Results: Generally, stunting was declined 17.1% and underweight and wasting were increased 0.9% and 1%, respectively during 15 years (1998 - 2013). Underweight increased 0.5% in Fars-natives and 3.2% in Turkmans and it was decreased 0.9% in Sistanis. Statistical difference in Turkman children among the three stages of the study was significant (P = 0.001). Stunting has decreased 28.7% in Fars-natives and 35.1% in Sistanis, it was increased 9.3% in Turkman group. Statistical differences among three stages in inter-ethnic groups were significant (P = 0.001 for all). Compared the group with good economic status, the odds ratio was 1.831 in poor economic group (P = 0.001). The risk of under nutrition in Sistanis was 1.754 times more than in Fars-natives (P = 0.001). Conclusions: Under nutrition remains one of the main health problems in under-five-year children in north Iran being more common in Sistani children. Stunting in Sistani children deeply decreased while in Turkman children slightly increased during the 15-year period study. Poor economic status is a risk factor for under nutrition in this area.

Over 2 million Anterior Cruciate Ligament (ACL) injuries occur annually worldwide resulting in considerable economic and health burdens (e.g., suffering, surgery, loss of function, risk for re-injury, and osteoarthritis). Current screening methods are effective but they generally rely on expensive and time-consuming biomechanical movement analysis, and thus are impractical solutions. In this dissertation, I report on a series of studies that begins to investigate one potentially efficient alternative to biomechanical screening, namely skilled observational risk assessment (e.g., having experts estimate risk based on observations of athletes movements). Specifically, in Study 1 I discovered that ACL injury risk can be accurately and reliably estimated with nearly instantaneous visual inspection when observed by skilled and knowledgeable professionals. Modern psychometric optimization techniques were then used to develop a robust and efficient 5-item test of ACL injury risk prediction skill—i.e., the ACL Injury-Risk-Estimation Quiz or ACL-IQ. Study 2 cross-validated the results from Study 1 in a larger representative sample of both skilled (Exercise Science/Sports Medicine) and un-skilled (General Population) groups. In accord with research on human expertise, quantitative structural and process modeling of risk estimation indicated that superior performance was largely mediated by specific strategies and skills (e.g., ignoring irrelevant information), independent of domain general cognitive abilities (e.g., metal rotation, general decision skill). These cognitive models suggest that ACL-IQ is a trainable skill, providing a foundation for future research and applications in training, decision support, and ultimately clinical screening investigations. Overall, I present the first evidence that observational ACL injury risk prediction is possible including a robust technology for fast, accurate and reliable measurement—i.e., the ACL-IQ. Discussion focuses on applications and outreach including a web platform that was developed to house the test, provide a repository for further data collection, and increase public and professional awareness and outreach (www.ACL-IQ.org). Future directions and general applications of the skilled movement analysis approach are also discussed.
